Placental abruption in association with advanced abdominal pregnancy. A case report.

BACKGROUND: Recognition of advanced abdominal pregnancy and care of the patient afflicted with it may present formidable challenges. Aside from the difficulty of diagnosing the problem and thereby delaying necessary intervention, management can be difficult at best, even when the condition is relatively uncomplicated. When it is compounded by a life-threatening complication, such as uncontrollable hemorrhage, it challenges the skills of the most experienced obstetrician and the resources of the best-equipped facility and its personnel. CASE: Partial placental separation was encountered at surgery; it progressed intraoperatively despite the care taken to avoid disturbing the placental implantation site. Severe hemorrhage was controlled by a combination of aortic compression, packing and use of large "liver" sutures incorporating the uterine wall for tamponade of the principal placental implantation site, on the mesentery. CONCLUSION: It is important to be prepared to deal with the complication of intense intraabdominal bleeding in the course of intraoperative management of abdominal pregnancy.

[Characteristics and recent trends in endemicity of human parasitic diseases in China].

An analysis of the nationwide survey of the distribution of human parasites in the light of the literature previously published, two major trends in the endemicity of parasitic diseases in China was observed. For one trend, the infection rate of E. histolytica, Fasciolopsis buski and soil-transmitted helminthasis etc. declined markedly. The second trend was that the prevalence of food-transmitted parasitic diseases (Trichinella spiralis, Clonorchis sinensis, Paragonimiasis, cysticercosis) and hydatidosis was increasing. The relationship between the social economic factors and the two trends were discussed.

Anti-inflammatory effects of tianrong acupoint on blood vessels of dura mater.

Although Tianrong acupoint (SI 17) is used for migraine headache, its mechanism of action remains obscure. The effects of Tianrong acupoint therapy on neurogenically-mediated plasma protein extravasation (125I-BSA) in rat dura mater induced by electrical stimulation of the right trigeminal ganglion was studied. When the unilateral trigeminal ganglion was stimulated (5 Hz, 1.2 mA, 5 msec for 5 min), the ratio of stimulated side/unstimulated side (cpm/mg) was 1.6663 +/- 0.0217. The plasma extravasation was blocked by different kinds of treatment in different degrees. The order of the ratio was as follow: Tianrong acupoint therapy (1.0917 +/- 0.0266) and Tianrong electrical needle therapy (1.1281 +/- 0.0227) < Tianrong acupoint injection with normal saline (1.325 +/- 0.0444) < Quchi acupoint (LI 11) injection with prednisolonum (1.5284 +/- 1.1624). The results indicated that Tianrong acupoint therapy could inhibit the neurongenic inflammation on the affected side.

Expression of MHC-I and MHC-II antigens in endomyocardial biopsies from patients with viral myocarditis and cardiomyopathy.

In situ expression of major histocompatibility complex (MHC) I and II antigens were examined in endomyocardial biopsy samples from 35 patients with viral myocarditis (VMC) and dilated cardiomyopathy (DCM). Increased expressions of these 2 antigens were observed on endocardia, capillary endothelia, dentritic and mononuclear cell membranes. The expression of sarcolemma was also found in specimens of VMC and DCM. Thus the abnormal expression of MHC is a new marker of autoimmune reactivity directed against cardiac structures.

[Improving effect of rhizoma Gastrodiae on learning and memory of senile rats].

After giving Rhizoma Gastrodiae to aging rats continuously for 3 months, the process of their learning and memory was observed through step-down test and the content of lipid peroxides (LPO) was determined. It has been found out that Rhizoma Gastrodiae can effectively improve the ability of learning and memmory of these rats and reduce the content of LPO. The result may indicate that it is by clearing away the free-radicals that Rhizoma Gastrodiae improves the brain function.

[Clinical and radiographic analysis of 56 cases of mural ameloblastoma].

Fifty-six cases of mural ameloblastoma were ananlyzed clinically and radiographically. They were transformed by ameloblastomous changes from keratocyst, occuring in 28.2% of ameloblastoma. The disease was often seen in patients of 10-29 years old with a mean of 21.8. Most of them were seen in mandible (53/56), especially in ramus (40/50). The radiographic appearance is divided into three subtypes: (1) ramus-mandibular body pattern, including dentigerous (31/56) and non-dentigerous (9/56); (2) sub-apical pattern (12/56); (3) inter-radicular pattern (4/56). The mainpoint of radiographic diagnosis is the appearance of unilocular or multilocular radiolucency of similar size, located in ramus-mandibular body or mandibular body region. The large lesions have a tendency to expand along the mandible. The roots of teeth are absorbed apparently, demonstrating serrated or amputated appearance.

[Status and strategy of parasitosis control in preventive health care in China].

According to the result of nationwide human parasitic disease distribution survey, the public significance of parasitosis control was set forth in terms of preventive health care and economic impact as well as parasitosis control and primary health care. A suggestion was made on the control strategy aiming at the parasitosis of priorities listed in the above survey.

[Diagnosis of meconium aspiration syndrome by fluorometry technique].

OBJECTIVE: To establish a new diagnostic method of neonatal meconium aspiration syndrome (MAS). METHODS: The fluorometry technique was used for examination of urine samples from three groups of neonates. The neonates were divided into 3 groups: Group I, MAS (n = 17); Group II, no MAS, but amniotic fluid with meconium stain (n = 24); Group III, normal control (n = 27). Urine fluorescence meconium index (UFMI) and urine meconium index (UMI) were calculated. RESULTS: UFMI values of group I were higher than that of group II and III. The sensitivity and specificity of UFMI were 100% and 97% respectivety. However, it were 51% and 48% respectivety in UMI. CONCLUSIONS: UFMI is an index for identification of Zn-coproporphycin, and it is a more sensitive index for diagnosis of MAS.

Recombinant soluble human thrombomodulin: a randomized, blinded assessment of prevention of venous thrombosis and effects on hemostatic parameters in a rat model.

UNLABELLED: Thrombomodulin is an endothelial surface receptor that binds thrombin and accelerates the activation of protein C. We compared the effects of a recombinant thrombomodulin analog (TME), recombinant hirudin (r-HIR), heparin sodium (HEP), and normal saline (Control) on thrombus formation, activated partial thromboplastin time (APTT), thrombin time (TT), platelet aggregation and tail transection bleeding time (BT) in a rat model of vena cava thrombosis. RESULTS: TME, r-HIR and HEP prevented venous thrombosis in this model in a dose-dependent manner. At the dose required to reduce vena cava thrombosis by 50% (ED50), TME did not prolong the APTT or TT as did HEP and r-HIR. Platelet aggregation in response to thrombin was not effected by TME but was inhibited by both r-HIR and HEP. BT did not differentiate the agents tested. CONCLUSION: TME inhibited venous thrombosis in a rat vena cava model with less effect on hemostatic variables than HEP or r-HIR.

Construction, expression, and properties of a recombinant chimeric human protein C with replacement of its growth factor-like domains by those of human coagulation factor IX.

The cDNA encoding a chimeric human protein C (PC), in which its epidermal growth factor-(EGF) like regions have been replaced with equivalent structures from human factor IX (fIX), was constructed and the gene product was expressed in human 293 cells. A molecular subpopulation of the recombinant chimeric protein (r-[PC/delta EGF-1,2/delta fIXEGF-1,2]) was purified that contained the full complement (9 residues/mol) of gamma-carboxyglutamic acid (Gla). After conversion by thrombin to its activated form (r-[APC/delta EGF-1,2/delta fIXEGF-1,2]), this latter enzyme was found to possess approximately 10% of the activity of wild-type recombinant APC (wtr-APC) in an APTT assay. In assay systems employing purified components, the activity of the mutant enzyme toward prothrombinase cofactor Va (fVa) and tenase cofactor VIII (fVIII) was approximately 30% and < 10%, respectively, of that of wtr-APC. The chimeric protein displayed full reactivity with a Ca(2+)-dependent monoclonal antibody to the Gla domain of PC, yielding a C50 for Ca2+ that was very similar to that obtained with wtr-PC (ca. 3.7 mM). Titrations of the dependency on Ca2+ of the intrinsic fluorescence of r-[PC/delta EGF-1,2/delta fIXEGF-1,2] allowed calculation of a C50 value of 0.34 mM, again very similar to that of wtr-PC. As with wtr-PC, Ca2+ inhibited the thrombin-catalyzed activation of r-[PC/delta EGF-1,2/delta fIXEGF-1,2] with aKi of 148 microM, as compared to a Ki of 125 microM for wtr-PC. At a saturating level of Ca2+, activation of r-[PC/delta EGF-1,2/delta fIXEGF-1,2/] by the thrombin/thrombomodulin (thrombin/TM) complex occurred at approximately 70% of the rate of that of wtr-PC.(ABSTRACT TRUNCATED AT 250 WORDS)

Lung cancer among tin miners in southeast China: silica exposure, silicosis, and cigarette smoking.

The Dachang Tin Mine is an industrial facility with high rates of lung cancer compared to the resident population in Guangxi province in southern China. Historically, exposure to silica dust was very high in the 1950s, falling in recent years to levels in keeping with international standards. Radon levels in the mine are low. We report the findings of a case-control study nested into a cohort study on miners working in Dachang. Cases of lung cancer among miners incident from 1973-1989 were obtained from local comprehensive medical records covering workers employed at the mine. These were matched approximately 3 to 1 with miners randomly chosen from the district surrounding the mine within the same birth decade. Matched odds ratios of 2.42 (95% confidence limit [CL] 1.3, 4.4) for underground employment, 3.52 (95% CL 1.7, 7.5) for smoking, and 2.04 (95%) CL 1.2, 3.7) for silicosis as determined on chest film were noted. Multifactor analysis of unconditional logistic regression showed that among the risk factors for excess mortality from lung cancer only the years spent drilling underground and the cumulative smoking index (product of daily cigarette consumption and number of years smoking) were independent contributors to risk and there was no interaction observed. The presence of silicosis did not contribute to predicting risk independently of the years spent underground.

Phenotype analysis of mononuclear cells in endomyocardial biopsy specimens from patients with viral myocarditis and dilated cardiomyopathy.

This study investigated the distribution and phenotype of mononuclear cells in endomyocardial biopsy (EMB) specimens from patients with viral myocarditis (VMC, 25 cases) and dilated cardiomyopathy (DCM, 10 cases). T lymphocytes, including Th/i cells in VMC and Tc/s cells in DCM, predominated in the infiltrating mononuclear cells in these specimens. The morphological relationship between infiltration of T lymphocytes and pathological changes of myocardia was also observed and neither macrophages nor B lymphocytes were found.

Effects of isoprenaline, phenylephrine on heart and influence of nifedipine on these effects.

In a perfused isovolumetrically contracting rat heart model, the effects of isoprenaline (IPN) and phenylephrine (PE) on myocardial contraction and relaxation were investigated, and the influence of nifedipine on these effects was studied. Both IPN and PE increased the myocardial contraction and improved its relaxation, but some differences existed. Nifedipine (10 nmol/L) substantially inhibited the PE-mediated inotropic effect, but in case of IPN-mediated inotropic ones, it did not. It was assumed that there may be various types of slow channels, one was activated by IPN, and the other, by PE.

The combined effect of high iron and zinc intake on copper status in rats.

The interactions between copper, zinc, and iron intake in rats were investigated with regard to copper status. Weanling male rats were fed purified diets containing two levels of each of the three elements in a 2(3) factorial design. The added amounts of copper, zinc, and iron in the diets were 5, 12, and 35 mg/kg feed or were 10 times as high. After feeding on the experimental diets for 4 wk, the rats were killed and copper concentrations in plasma and organs measured. Plasma copper concentration was lowered by high zinc and iron intakes but this was seen only in the rats fed the normal-copper instead of the high-copper diets. In essence, the effects of zinc and iron were additive. Neither in rats fed the normal-copper diets nor in those fed the high-copper diets did extra iron or zinc intake alter copper concentrations in liver, spleen, kidney, and tibia.

Development of a candidate vaccine against Ross River virus infection.

Ross River virus is a mosquito-borne alphavirus which causes several thousand cases of arthritis (epidemic polyarthritis) each year. In this study, binary ethylenimine (BEI) was used to destroy the infectivity of this virus without abolishing the antigenicity or immunogenicity of the virion. Mice immunized intramuscularly with BEI-inactivated virus, with or without Alhydrogel adjuvant, produced antibody which neutralized Ross River virus in vitro, and the mice also failed to develop viraemia when challenged intravenously with live virus. Serum neutralization and in vivo protection were greatest when BEI-inactivated virus was administered without adjuvant.

Myocutaneous flaps in genitourinary oncology.

Between January 1983 and September 1992, 32 myocutaneous flaps were fashioned in 28 patients for reconstruction following treatment of genitourinary malignancies or complex pelvic fistulas. Of the myocutaneous flaps 14 were used to obtain primary soft tissue coverage of large but otherwise uncomplicated wounds and 10 were used to cover previously irradiated resection sites. Four myocutaneous flaps were used to repair complex radiation-induced fistulas involving the bladder, vagina, urethra and rectum. Flaps were used to cover infected or nonhealing open wounds in 8 cases, 4 of which also had been previously irradiated. Myocutaneous flap donor sites were the tensor fascia lata in 11 cases, gracilis in 9, rectus abdominis in 10 and rectus femoris in 2. There was 1 major complication (flap loss) and 9 minor complications. There were no perioperative deaths. Myocutaneous flaps are an effective means of covering large groin, perineal and lower abdominal surgical defects after radical surgery.
