[Treatment of meningeal carcinomatosis--neocarzinostatin perfusion therapy in the CSF pathway].

The authors presented the results of perfusion chemotherapy in the CSF pathway with neocarzinostatin (NCS) for the treatment of meningeal carcinomatosis. In three cases of stomach cancer and one case of malignant lymphoma, cranial polyneuropathy, progressive hydrocephalus and positive cytology of the CSF were found during treatment of malignant tumors. NCS therapy was performed by means of perfusion from the spinal subarachnoid space to the ventricle utilizing Ommaya's reservoir, or simply by repeated lumbar punctures. In all of four cases the number of malignant cells in the CSF was significantly diminished, and temporary neurological improvement was observed until the patient's death due to advanced primary lesions. These results indicated that in cases of meningeal carcinomatosis, NCS perfusion therapy using the CSF pathway was a much more favorable method than treatment with other chemotherapeutic agents with less noticeable side effects.

Lung tumor imaging by positron emission tomography using C-11 L-methionine.

This paper described the first clinical study of lung tumor scanning by positron emission tomography (PET) using C-11-labeled L-methionine (11C-L-Met). Tumor images were clearly visualized by high contrast in eight lung cancer patients and also in a patient with a silicotic nodule. Quantitative evaluations of methionine uptake in tumor tissue and normal tissue by comparing differential uptake ratios suggested that the extent to which 11C-L-Met accumulates in a tumor is closely correlated to the tumor's viability such as benign or malignant, viable or necrotic. 11C-L-Met is considered to be an effective tumor marker for PET diagnosis which represents increased amino acid metabolism of tumors in the mediastinum and lung.

[Randomized control study of high-dose metoclopramide in the prevention of CDDP-induced emesis].

The effect of high-dose metoclopramide (2 mg/kg, 4 times every 2 hours) on the emesis of patients treated with CDDP (80 mg/m2) was examined by randomized control trial. The above metoclopramide regimen significantly suppressed the frequency of vomiting on the day of CDDP administration. The duration of nausea and anorexia after CDDP treatment was also shortened by high-dose metoclopramide administration.

Phospholipid (diacyl, alkylacyl, alkenylacyl) and fatty acyl chain composition in murine mastocytoma cells.

The phospholipids from murine mastocytoma FMA3 and P-815 clone cells were quantitatively analyzed, and the major glycerophospholipids were examined for their fatty acyl chain distribution. In these cells, the content of histamine was less than 1/100 of normal mouse mast cells, and FMA3 cells had 1.5-fold as much histamine content as P-815 cells. The predominant phospholipid species of both mastocytoma FMA3 and P-815 were choline-containing glycerophospholipids (48%) and ethanolamine-containing glycerophospholipids (29%). The remaining minor constituents were sphingomyelin (6%, 7%), phosphatidylinositol (7%, 5%), phosphatidylserine (2%, 5%), cardiolipin (4%, 3%), and phosphatidic acid (2%, 1% for FMA3 and P-815, respectively). The choline-containing glycerophospholipids consisted of high amounts of 1-O-alkyl-2-acyl type (31%, 25%) and 1,2-diacyl type (63%, 66%) and a smaller amount of 1-O-alk-1'-enyl-2-acyl type (7%, 8%). In contrast, ethanolamine-containing glycerophospholipids were characterized by high contents of 1-O-alk-1'-enyl-2-acyl type (36%, 31%) and 1,2-diacyl type (55%, 58%), and a lower level of 1-O-alkyl-2-acyl type (12% and 11% for FMA3 and P-815, respectively). Unlike choline-containing glycerophospholipids and sphingomyelin that were rich in palmitic acid, ethanolamine-containing glycerophospholipids, phosphatidylserine and phosphatidylinositol showed a high proportion of stearic acid in the overall fatty acid composition. The content of arachidonic acid was highest in phosphatidylinositol. Sphingomyelin had a large amount of long chain and polyunsaturated fatty acids. In both choline- and ethanolamine-containing glycerophospholipids, the predominant fatty acids in the sn-1-position were palmitic, stearic, and oleic acid.(ABSTRACT TRUNCATED AT 250 WORDS)

Recurrence of thymoma with appearance of myasthenia gravis 18 years after surgery: a case report.

A 59-year-old woman was admitted to the hospital because of recurrence of thymoma with appearance of myasthenia gravis 18 years after excision of an invasive thymoma. Recurrence was observed in the anterior mediastinum, right pleura, pericardium and left supraclavicular lymph node. Hashimoto's thyroiditis, vitiligo vulgaris and leukopenia were also seen. After creation of a pericardial window for pericardial effusion, she received vincristine, then carbazilquinone intravenously as single chemotherapeutic agents based on the chemosensitivity by clonogenic assay of the supraclavicular tumor, but response was not achieved. At one and a half years after radiotherapy to the mediastinum, she is now doing well without symptoms and is receiving a fairly low dose of anticholinesterase medication.

[Hydrocephalus due to membranous obstruction of the fourth ventricle aperture].

Three cases of hydrocephalus due to membranous obstruction of the fourth ventricle aperture are reported. Patient's age was 4, 34 and 68 years old and they complained of slowly progressive cerebellar ataxia, gait disturbance and other hydrocephalic symptoms. CT scan revealed symmetrical dilatation of lateral ventricles, and, especially, the fourth ventricle aperture was confirmed by ventriculography or metrizamide CT cisternography. Suboccipital craniectomy was performed in all cases and cerebellum was of normal shape and semilucent membrane was found obliterating the foramen of Magendie. Histological finding of the membrane was chiefly composed of fibroblast, but inflammatory evidence suggesting any particular etiological process. The cause of membranous obstruction was undetermined and obsolete infectious or inflammatory change was suspected.

Mechanism for adaptive modification during cold acclimation of phospholipid acyl chain composition in Tetrahymena. I. Principal involvement of deacylation-reacylation.

The purpose of the present studies with in vivo isotope labeling was to explore the mechanism by which the fatty acyl composition of membrane phospholipids was altered quickly after a downward temperature-shift in Tetrahymena pyriformis. 1. When 39 degrees C-grown Tetrahymena cells were shifted to 15 degrees C, unsaturated fatty acids, especially gamma-linolenic acid, were increased, with compensating decrease of palmitic acid in phospholipids. 2. However, in the [32P]Pi-prelabeled cells, the specific radioactivities of major membrane phospholipids, phosphatidylethanolamine, phosphatidylcholine and 2-aminoethylphosphonolipid, were not changed within 10 h after shift-down. Furthermore, in the [14C]palmitic acid-prelabeled cells, the specific radioactivities toward phospholipids also did not change after temperature-shift. 3. After the shift, the rate of incorporation of [14C]acetate into fatty acids was reduced to less than one-tenth and the principal fatty acid newly synthesized was linoleic acid, and palmitic and gamma-linolenic acids were also formed to almost same degree. 4. In contrast, when cells prelabeled with both [32P]Pi and [14C]palmitic acid were shifted to 15 degrees C and then linoleic and gamma-linolenic acids, which are known to increase during cold acclimation, were added to the medium, the 14C/32P ratios of major phospholipids were progressively decreased until 5 h after the shift. These results suggest the mechanism for the alteration of phospholipid fatty acyl composition at lowered growth temperature by which preexisting fatty acids of membrane phospholipids would be deacylated and then, after modification to adequate unsaturated fatty acids by desaturation and/or elongation, be reacylated again into lysophospholipids.

Mechanism for adaptive modification during cold acclimation of phospholipid acyl chain composition in Tetrahymena. II. Activities of 2-acyl-sn-glycerol-3-phosphorylcholine and 2-acyl-sn-glycerol-3- phosphorylethanolamine acyltransferases involving the reacylation.

The deacylation-reacylation process is very important for the alteration of phospholipid fatty acyl composition on lowering of growth temperature in Tetrahymena pyriformis (Kameyama, Y., Yoshioka, S. and Nozawa, Y., (1984) Biochim. Biophys. Acta 793, 28-33). Microsomes isolated from Tetrahymena cells have reacylation activities not only for 1-acyl-sn-glycerol-3-phosphorylcholine (1-acyl-GPC) and 1-acyl-sn-glycerol-3-phosphorylethanolamine (1-acyl-GPE) but also for 2-acyl-GPC and 2-acyl-GPE. Unsaturated fatty acyl-CoAs were in general much better substrates than saturated fatty acyl-CoAs for acylations of 1-acyl-GPC and 1-acyl-GPE. The acylation rates for 1-acyl-GPE were almost the same in palmitoleoyl-CoA, oleoyl-CoA, linoleoyl-CoA and gamma-linoleoyl-CoA. However, the acylation activity for 1-acyl-GPC was more than 2-fold higher with palmitoleoyl-CoA than with any other unsaturated fatty acyl-CoAs. In contrast, both 2-acyl-GPC and 2-acyl-GPE acyltransferases did not show a distinct preference for various acyl-CoAs, although palmitoyl-CoA was incorporated into both 2-acylphospholipids at higher rates than into 1-acylphospholipids. These specificities for various acyl-CoAs of 1-acyl- and 2-acyl-GPC and 1-acyl- and 2-acyl-GPE acyltransferases were not changed in the microsomes isolated from cells grown isothermally at 39 degrees C and 15 degrees C and cells shifted from 39 degrees C to 15 degrees C. However, the acylating ratio of linoleoyl-CoA to palmitoyl-CoA, which were chosen as typical unsaturated and saturated fatty acyl-CoAs, in the microsomes from cells grown at 15 degrees C was 1.5-3.0-times higher than in the microsomes from 39 degrees C-grown cells in four acyltransferase activities. These results suggest that the changes of acyl-CoA specificities in reacylation enzyme activities during temperature down-shift would make little contribution to the increase in unsaturated fatty acids in phospholipids, although reacylating enzymes from isothermally grown cells at lower temperature would partially participate in those increases.

Effects of selective vagotomy with or without drainage on glucose tolerance test.

Effects of selective proximal vagotomy (SPV) with pyloroplasty, SPV without a drainage, selective gastric vagotomy with antrectomy and distal gastrectomy with gastro-duodenostomy on the oral glucose tolerance test (OGTT) were studied in 25 patients with duodenal ulcer. Hyperglycemia occurred at 30 and 60 min after glucose loading and hypoglycemia occurred at 120 min in all three types of selective vagotomy. The pattern of the OGTT, however, was not remarkably changed after gastrectomy. These findings suggested that an altered pattern in the OGTT might relate to vagotomy, but not to gastrectomy or pyloroplasty. Hormonal factors, such as immunoreactive insulin (IRI), pancreatic glucagon (PG) and gut glucagon-like immunoreactivity (GLI) were measured to elucidate the mechanism of the altered patterns of OGTT after selective vagotomy. The enhanced response of IRI was found in selective vagotomy groups, in comparison with the preoperative study or gastrectomy group. The exaggerated response of IRI might be responsible for hypoglycemia. GLI and PG had little or no effect on blood glucose level on the OGTT.

Paradoxical tubular obstruction after release of ureteral obstruction in rat kidney.

The healing process of hydronephrosis was studied using rat kidney. Following release of 2 weeks' unilateral hydronephrosis, renal papilla and cortex were examined by scanning electron microscopy. Shortly after the release of obstruction, massive loss of brush border and ballooning of epithelial cells were noted in proximal tubules. Distal tubules and collecting tubules also showed severe ballooning of epithelial cells. Actually most of them were completely or nearly obliterated in spite of the release of ureteral obstruction. Two weeks later, on the other hand, the many tubular components became patent with normal epithelial lining. Some of them, however, deteriorated further in spite of the release of obstruction. Thus there was a marked tubular heterogenicity in recovery. The sudden decrease of the pelvic pressure and chronic ischemic state during ureteral obstruction may cause the tubular changes which were observed in this study.
