A case of erythroleukemia coexistent with pulmonary emphysema is reported. A 67-year-old male was admitted to our hospital in May 1981, with a few year history of cough, sputum and fatigue. He had already been diagnosed as having pulmonary emphysema and moderate anemia. On physical examination, except for pallor, no other findings were remarkable. The initial hematological examination showed hemoglobin, 9.6 g/dl, red cell count, 251 x 10(4)/microliters, platelet count, 7.3 x 10(4)/microliters, white cell count, 2600/microliters with neither myeloblasts nor erythroblasts. A sternal marrow aspiration revealed 21% myeloblasts and 40% erythroblasts including 7.5% megaloblastoids. Periodic Acid Schiff staining was strongly positive for a part of erythroblasts. A chest X-P finding was typical for pulmonary emphysema. Pulmonary function was moderately damaged. He was started on chemotherapy with AAAP (ACNU 50 mg/d i.v. drip over 4 hr x 4d, adriamycin 20 mg/d i.v. push x 4d, Methotrexate 20 mg i.v. push x 4d). The first course of AAAP brought him a complete remission with both disappearance of myeloblasts and erythroid precursors with megaloblastoid nuclei in the marrow and the normalization of white cell count and platelet count in the blood. He was discharged in September 1981 after completion of a consolidation chemotherapy with AAAP. Since then, he received two courses of AAAP as an intensification chemotherapy and has been in complete remission for more than 13 months. His pulmonary function has not been affected and no myocardial damage has been seen throughout AAAP therapy. Thus, AAAP therapy seems to be an excellent chemotherapy even for an aged patient with erythroleukemia.
Based on our experience on more than one hundred cases, bone remodelling in rickets and osteomalacia was investigated. Histomorphometry using tetracycline labelling on nineteen ribs and fibula biopsy specimen revealed a marked increase in number of osteon with osteoid seam, a slight decrease in number of resorption cavity, and extremely depressed tetracycline uptake, of 9.7% on average. The attempt to measure bone formation rate by double tetracycline labelling has failed. Analysing the dynamic behaviour of serum alkaline phosphatase, the hypothesis that the serum alkaline phosphatase was regulated by the three factors, activity of osteoblast, number of osteoblast and elution coefficient from the cell, was presented. Both accretion rate and urinary hydroxyproline excretion showed variable values, from low to high. Although bone remodelling in rickets and osteomalacia was always low at the cellular level, it could be low, normal or high at tissue or organ levels.

A 23-year-old man with primary hyperparathyroidism which was typical except for reduced alkaline phosphatase activity is reported. Histological examination of surgical specimens revealed chief cell hyperplasia of the parathyroid glands. Systemic abnormalities of alkaline phosphatase were demonstrated, i.e., marked reduction of all isoenzymes and undetectable osseous enzyme in the serum, abnormal distribution of the enzyme in hepatocytes and diminished enzyme activities in leukocytes. In addition, diminished bone remodeling activity was revealed in a biopsy specimen of the rib. The association of hypophosphatasia is highly unlikely, because of normal urinary excretion of phosphoethanolamine, lack of osteomalacia, and no indication of an hereditary factor. The causal relationship between low remodeling activity and abnormalities in alkaline phosphatase was suggested.

A gas chromatographic method has been developed that permits the accurate and specific determination of the antianginal agent ANP-4364 in plasma. ANP-4364 is extracted with n-hexane containing ethyl chloroformate and, after a clean-up procedure, derivatized to the trichloroethyl carbamate, which is assayed on a gas chromatograph equipped with an electron-capture detector. Accurate determinations are possible over a concentration range from 1 to 50 ng/ml of ANP-4364 in plasma with a relative standard deviation of 7.5%. The minimum detectable concentration is 0.5 ng/ml. Plasma levels of ANP-4364 in dogs receiving oral (10 mg/kg) or intravenous (0.1 mg/kg) dosing have been determined.
Hadjdu-Cheney syndrome is a rare type of syndrome characterized by acro-osteolysis, dolichocephaly with multiple Wormian bones, absence of frontal sinuses and joint laxity. A case of this syndrome is presented. A histological study of the osteolytic lesion revealed destruction characterized by microfractures with a poor reparative process. It is postulated that an abnormality of osteoblast or osteoid function is the pathogenesis of this syndrome.
The relation of the infected to nephritis, rheumatic and heart diseases has been recognized but that to dermatoses has not been fully established nor applied clinically by dermatogists or otolaryngologists in the treatment of such patients. In order to assess to attitude of dermatologists as regards the dermatoses due to focal tonsillar infection, a questionnaire was sent to the departments of dermatology of 72 universities in Japan: 44 (61.1%) have replied. There were 40 dermatoses listed as possibly associated with a focal infection. About 60% of the universities endeavoured to detect the focus although the focus was not always removed when identified.
Muscles from two cases of osteomalacia were studied histochemically and electron-microscopically. Histopathological finding were common in these two cases. There are (1) myopathic changes such as scattered muscle fiber atrophy, necrosis, and internal uuclei, (2) derangement of intermyofibrillar network. and (3) type II fiber atrophy. Electron-microscopical finding corresponded well with light microscopical findings. These are distinct pathological features and deserves to be called osteomalacic myopathy. As the pathogenetic mechanism of this myopathy, phosphate depletion in the muscle cells resulting in disturbed glycolysis, and decreased vitamin D effects on muscle cells resulting in diminished calcium uptake by sarcoplasmic reticulum, are considered to be the most important two factors.
