Immunological abnormalities in a patient with lysinuric protein intolerance.

We report an 8-year-old girl with lysinuric protein intolerance and immunological abnormalities including impaired function of lymphocytes, the presence of LE cells, antinuclear antibodies, and hypergammaglobulinaemia. These abnormalities have not been reported before and may be due to an amino acid imbalance or protein malnutrition in cells or tissues. The coincidence of a pre-stage of systemic lupus erythematosus (SLE) and LPI is not excluded.

Anterior perineal anorectoplasty for intermediate and high imperforate anus.

Anterior perineal anorectoplasty is a new technique for the repair of high imperforate anus. This technique allows direct access and clear visualization of the puborectalis sling and rectal pouch; division of the fistula and mobilization of the rectum can be precisely performed under direct vision, thereby reducing injury to surrounding structures; the pull-through of the rectum is readily facilitated; it obviates the need to change the position of the patient during the procedure; and mobilization of the rectum required to pull the rectum through is minimal, hence the late complication of mucosal prolapse is avoided. Details of the operative procedure are described herein.

Segmental wall motion abnormalities in dilated cardiomyopathy: hemodynamic characteristics and comparison with thallium-201 myocardial scintigraphy.

This study assessed the hemodynamic characteristics of segmental wall motion abnormality of the left ventricle in patients with dilated cardiomyopathy (DCM) and its relation to the thallium-201 (TI-201) myocardial scintigraphy (MPI). Left ventriculograms and MPI in 23 patients were analyzed by the use of quantitative indexes of regional wall motion and TI-201 uptake based on a mean and a standard deviation of 13 normal subjects. Relative normokinesis in our definition was more frequently seen in the inferior wall than in the anterior wall (p less than 0.01). In contrast, severe asynergy was more often seen in the anterior wall than in the inferior wall (p less than 0.01). There were 11 patients who had relative normokinesis and asynergy together. By means of the index of wall motion, the DCM patients were divided into two groups, one with segmental wall motion abnormality (SWMA) and another with diffuse wall motion abnormality (DWMA). The DWMA group had higher left ventricular end-diastolic pressures (p less than 0.05) and the tendency of large left ventricular end-diastolic volumes than the SWMA group. There was a rough correlation (r = 0.58) between the quantitative indexes of TI-201 uptake and wall motion at the same region of the left ventricle. Thus, the nonuniformity of the left ventricular wall motion was recognized in the patients with DCM and more increased preload was shown in the patients with DWMA than in the group with SWMA. Further, the regional asynergy may be related to the localized fibrosis within the left ventricle in DCM, considering the result that the worse TI-201 uptake was roughly accompanied by the more severe asynergy.

Isolation and genetic analysis of operator-constitutive mutants of the H1 operon in Salmonella typhimurium.

In phase-2 cells of diphasic Salmonella strains, expression of H1 is repressor, coded for by the rh1 gene. A procedure for the isolation of operator-constitutive (H1-Oc) mutants of the H1 operon is described. Using three-factor crosses between an H1-Oc H1 strain and H1-O+ H1 strains, where motility recovery via H1-phase (or phase 1) flagellation was used as the selected marker and the H1-O character was the unselected marker, the relative position of the H1-Oc site to the H1 gene was determined. A diphasic H1-Oc strain produced, in phase 2, copolymer filaments composed of H1 and H2 flagellin.

Regions of Salmonella typhimurium flagellin essential for its polymerization and excretion.

Immunological methods were used to examine the flagellin production of Salmonella typhimurium strains that carried a mutation in one of the two possible genes for flagellin (H1 or H2) and also were incapable of expressing the other gene. Some mutants produced flagellin that was excreted into the culture medium; others accumulated flagellin intracellularly. These two phenotypes were detected in both H1 and H2 mutants. The mutation sites were mapped on the corresponding deletion map (consisting of 21 segments in the case of H1 and 31 segments in the case of H2). H1 and H2 mutations causing excretion of flagellin were clustered mainly in segment 12 and segment 6 from the proximal end, respectively, suggesting that the corresponding segments of the flagellins play a role in polymerization. Mutations causing accumulation in the cytoplasm were clustered in segments 19 to 21 of the H1 map and in segments 25 to 29 of the H2 map, suggesting that an essential region for flagellin transport exists toward the C terminus of flagellin.

Effect of recombinant interleukin 2 on hepatitis B e antigen positive chronic hepatitis.

Eleven patients with hepatitis B (HB) virus related chronic hepatitis were treated with recombinant interleukin 2 (rIL 2). Two hundred and fifty to 1000 units were given intravenously once daily for seven to 28 days. In five patients serum glutamic pyruvic transaminase activity rose transiently. Six patients showed a decrease in HBV DNA polymerase. One patient lost HBs, e antigens (Ags) and gained anti-HBs, e antibodies, while one lost HBs Ag and another HBe Ag. 2'-5' oligoadenylate synthetase activity in mononuclear cells in the peripheral blood did not change during treatment. The number of CD4 positive (helper/inducer) cells and natural killer cell activity increased after therapy (p less than 0.05, p less than 0.01). These results suggest that rIL 2 acts as an immunomodulatory agent enhancing host immune activity and may be beneficial in patients with chronic HB virus infection.

Self-rating depression scales correlated with regional cerebral blood flow in normal volunteers.

In order to investigate possible influences of satisfaction with life on regional cerebral blood flow (rCBF), we studied the relationship between the self-rating depression scale and rCBF in normal middle-aged volunteers. A significant inverse correlation between SDS scores and the rCBF was observed. This result was independent of the influences of risk factors for stroke and age. Our findings suggest that satisfaction in daily life is important in maintaining active levels of brain metabolism.

A case of glutaric aciduria type I with unique abnormalities in the cerebral CT findings.

A first Japanese case of glutaric aciduria type I (GA-I) was described. She was a 7-month-old girl presenting with poor head control, irritability and sleeplessness. The profile of urinary organic acids by gas chromatography mass spectrometry (GC/MS) suggesting GA-I were confirmed by no activity of glutaryl-CoA dehydrogenase in the fibroblasts. The cerebral computer tomography (CT) showed marked changes such as large fluid collections on bilateral frontotemporal regions and a slight enlargement of bilateral ventricles. The amounts of urinary glutarate excretion decreased after restriction of lysine and tryptophan in her diet and administration of carnitine improved the carnitine levels in blood and urine, while these were less effective for the neurological symptoms. On the other hand, oral administration of lioresal, an analogue of gamma-aminobutyrate (GABA), cleared her symptoms such as ill temper, irritability and sleeplessness dramatically, and the abnormalities of the CT examinations were not more deteriorative until 2 years of her age at least. The neurological manifestations of GA-I seemed to be affected by the unusual metabolism of GABA in the central nervous system.

Histological and immunohistochemical study on ontogeny of the endocrine cells in the quail gizzard.

The pre- and post-hatching development and differentiation of the endocrine cells in quail gizzard were examined histologically and immunohistochemically. A total of 158 heads (from 5th d of incubation to adult) were used in this study. The formation of gizzard tubular glands began from 11th d of incubation. 8 kinds of endocrine cells, argyrophil cells, and gastrin releasing polypeptide (GRP)-, 5-hydroxytryptamine (5-HT)-, somatostatin-, glucagon-, avian pancreatic polypeptide (APP)-, neurotensin-, and gastrin-immunoreactive cells were detected in the gizzard. These endocrine cells began to appear from 10th d of incubation. Argyrophil cells and GRP-immunoreactive cells in the gizzard were increased with age. Other kinds of immunoreactive cells were found rarely and irregularly, and some of them were found transitory in the embryonic stage.

[Primary carcinoma of the male urethra: a case report].

A case of primary carcinoma of the male urethra is reported. A 72-year-old male who complained of dysuria and a perineal mass, was admitted to our hospital in April, 1982. Irregularity and narrowing of bulbous urethra were detected on the urethrocystogram and the biopsied specimen from the perineal mass showed the histological findings of squamous cell carcinoma. Total penectomy, total cystectomy, construction of ileal conduit and pelvic lymphadenectomy were performed. The tumor, 10 X 6 X 5 cm in size, was located in the bulbous urethra and the histological diagnosis was well differentiated squamous cell carcinoma with no lymph node metastasis. In spite of postoperative administration of bleomycin, local recurrence appeared 3 months after operation and the patient died from disease progression in August, 1983. The age, histopathology, symptoms, past history, site of tumor, treatment and prognosis in 126 Japanese cases of primary male urethral tumor including the present case were reviewed. En bloc exenteration including resection of the inferior rami or pubic bone with chemotherapy and radiotherapy is recommended for advanced carcinoma of bulbomenbrous urethra.

[Squamous cell carcinoma of renal pelvis: a case report and review of the Japanese literature].

Squamous cell carcinoma of renal pelvis is relatively rare and its prognosis is very poor. A 72-year-old man was introduced to our institute because of macroscopic hematuria. He had no history of urolithiasis or urinary tract infection. Excretory urography showed a nonfunctioning right kidney. Cytologic examination of urine was positive for malignant cell from squamous cell carcinoma. Preoperative diagnosis was made as right renal pelvic tumor, but it appeared to be renal tumor on the roentgenogram. Right radical nephrectomy and transurethral ureterectomy was performed. Radiation therapy was done after operation. Pathological diagnosis was squamous cell carcinoma of renal pelvis extensively infiltrating to the renal parenchyma. The patient is alive with no recurrence or metastasis for eight months after operation. Statistical analysis was made on 136 cases of squamous cell carcinoma of renal pelvis reported in the Japanese literature including our case, and this disease is also briefly reviewed.
