Cast-saw burns: comparison of technique versus material versus saws.

Inadvertent cast-saw burns are attributable to inadequately padded casts, improper technique, or uncooperative patients. Cast-saw blade temperatures recorded while splitting plaster and fiberglass casts are elevated to a degree that would increase the risk of a second- or third-degree burn. Using standardized plaster and fiberglass cast models, this study attempted to determine whether the elevated temperature was a product of the saw, blade design, blade material, technique of cast removal, or casting materials. The results demonstrated that cast-saw blades manufactured with stainless steel are poorly suited for a device that uses friction to separate a cast. Modifications to the technique and varying the cast-saw models did not reduce the blade temperature consistently to <120-130 degrees F during normal operations.

Interventional ERCP: techniques and nursing care.

As a diagnostic and interventional procedure endoscopic retrograde choledochopancreatography (ERCP) is growing in popularity. Interventional endoscopists can diagnose and treat problems of the hepatobiliary system and technology is expanding the scope of the technique. Increasingly nursing staff on surgical wards are required to provide care for patients undergoing ERCP. A sound understanding of the procedure and potential complications allows accurate nursing assessment and individualised planning to ensure the highest standard of care.

Inhibition of growth of subcutaneous xenografts and metastasis of human breast carcinoma by swainsonine: modulation of tumor cell HLA class I antigens and host immune effector mechanisms.

Swainsonine, an indolizidine alkaloid, can decrease the organ colonization potential of metastatic murine tumor cells by augmentation of host immune effector mechanisms. In this report the above findings were extended by the demonstration that systemic administration of swainsonine strongly suppressed the growth of human breast carcinoma subcutaneous xenografts and experimentally induced lung metastases. This inhibition was not due to a direct effect of swainsonine on cell growth. However swainsonine treatment of tumor cells resulted in enhanced expression of HLA Class I antigens, and HLA class I mRNA. Swainsonine was a potent immunodulator as evidenced by the increased (a) cytotoxicity of splenocytes and macrophages, and, (b) proliferative potential of splenocytes and bone marrow cells. These data suggest that swainsonine-induced inhibition of tumor growth and metastases may be mediated via activation of host effector cells and/or alteration of tumor cell antigenicity.

Cost-benefit analysis of an automated medication system.

Automated medication distribution systems have helped solve issues of efficiency and effectiveness. Cost-benefit analysis conducted by nursing and pharmacy departments can help to objectify the decision to adopt this technology.
