[Extent of lymphadenectomy in stage I-IIIA non-small cell lung cancer: a randomized clinical trial].

OBJECTIVE: To study the role of radical systematic mediastinal lymphadenectomy for non-small cell lung cancer (NSCLC). METHODS: All 504 operable eligible cases with NSCLC were randomly divided to a radical lymphadenectomy (RL) group and a conventional lymph node dissection group (control) treated between Aug. 1989 and Dec. 1995. For patients postoperatively eligible, thirteen parameters (operation type, pathological type and grade, tumor size, total number of dissected lymph nodes, number of metastatic lymph nodes, metastasis ratio of lymph nodes, postoperative TNM staging, adjuvant therapy, recurrence or metastasis, morbidity, survival and life quality) were evaluated. The end point of follow-up was Dec. 31, 1998. Lost follow-up rate was 1.9%. The results were analyzed with soft were SPSS7.5. The cumulative survival was calculated by the Kaplan-Meier method and compared by the log rank test. The prognostic factors were analyzed by the Cox model. RESULTS: There were 320 cases, 160 cases in each group, who entered the study. The mean numbers of dissected lymph nodes was 9.49 in the RL group and 3.63 in the control group. For stage I NSCLC patients, the 1,3,5,9-year survival rate was 91.8%, 86.9%, 81.4%, 74.2% respectively in the RL group and 88.7%, 72.5%, 58.5%, 52.1% respectively in the control group (P < 0.014). However, no statistically significant difference in survival rates between RL and control groups of patients with stage II and IIIA NSCLC. The postoperative TNM staging, metastasis ratio of lymph nodes, extent of lymphadenectomy were the factors influencing long term survival upon multivariable analysis. CONCLUSION: Classical lobectomy or pneumonectomy with radical systematic mediastinal lymphadenectomy is the surgical treatment of choice for NSCLC.

[Skip metastasis to the mediastinal lymph nodes in non-small cell lung cancer].

OBJECTIVE: To ascertain if any difference exists in clinical characteristics between resected non-small cell lung cancer (NSCLC) with either skip or ordinary mediastinal lymph node metastases. METHODS: Among 176 patients with stage IIIAN2 disease between 1982-1994 treated in our hospital, 53 had no metastasis at the hilar lymph nodes [skip(+) group] while 123 had [skip(-) group]. To investigate the extent of nodal involvement, the mediastinal lymph nodes were divided into three regions. RESULTS: In the skip(+) group, mediastinal node metastasis was found in only one region in 49 of the 53 patients(92.4%), whereas 45 of the 123 patients(36.6%) from the skip (-) group revealed medinastinal metastasis at two or three regions The overall survival rate at 5 years after operation was 29.3% in the skip(+) group and 12.2% in the skip (-) group (P = 0.038). Furthermore, regarding patients with mediactint node metantion in one region, the skip(+) group had a better prognosis than the skip (-) group (32.1% vs 15.3%, P = 0.042). CONCLUSION: These results suggest that patients with skip mediastinal lymph node metastases represent a unique subgroup of N2 disease.

[Telomerase activity in cervical intraepithelial neoplasia].

OBJECTIVE: To investigate the relationship between telomerase expression and the carcinogenesis of cervix, and the possibility of using telomerase as a marker to evaluate the natural history of cervical intraepithelial neoplasia (CIN). METHODS: Use the telomere repeat amplification protocol (TRAP) assay to detect cervical lesion in cervical scraping samples and cervical biopsy tissues. RESULTS: Sixty two point five percent, 60.0 percent, 82.4 percent and 61.9 percent cervical scraping samples exhibited telomerase activity in CINI, CINII, CINIII and cervical cancer, respectively, compared 50.0 percent, 45.0 percent, 96.4 percent and 95.2 percent in cervical biopsy tissue. In contrast, 20.0 percent and 26.7 percent in cervical scraping samples and cervical biopsy tissues of normal cervix. With the development of cervical lesion, the expression of tolemerase activation was increasing (P < 0.05). There was no significant difference between CINI and CINII (P > 0.05). There were significant differences between CINIII and CINI, CINII (P < 0.05) Telomerase activity was coincidence between the cervical scraping samples and cervical biopsy tissues. CONCLUSIONS: These results seem to suggest that the Telomerase expression may associate with the carcinogenesis of cervix. The telomerase activity assay of cervical scraping sample could be used as a marker to monitor and predicate the transit of CIN in clinical practice.

[A summary description of development in immunosensor].

In this review, an introduction to the concept and fundamental principles of immunosensor is given, and immunosensor is classified into many types according to the difference of transducers, each type being compared with others and explicated in derivation and developing process respectively. Two problems about the fabrication of immunosensor, which are immobilization and reproductivity, are addressed fleetly. In addition, the application and developing tendency of immunosensor are described briefly.

[Clinical analysis of 68 patients with ruptured sinus of Valsalva aneurysm].

OBJECTIVE: To evaluate the clinical characteristics, diagnosis, treatment and prognosis of ruptured aneurysm of the sinus of Valsalva (ASV). METHODS: Sixty-eight cases of ruptured ASV treated in our hospital from 1986 to 1998 were retrospectively analyzed. RESULTS: The ratio of male to female was 1.8:1 in 68 patients (44 males, 24 females). The mean age was (29.5 +/- 10.7) years (8-60 years). The proportion of ruptured aneurysms originating from the right coronary and noncoronary sinus was 79.4% and 20.6% respectively. Sixty-seven point six percent of ASV ruptured into right ventricle and 32.3% into right atrium. The incidence of ventricular septal defect and aortic valve insufficiency was 48.5% and 20.6% respectively. The former only existed in those patients whose ASV originating from the right coronary sinus and rupturing into right ventricles. There was a significant correlation between sudden onset of symptoms and fistulating of ASV into right atrium and a significant difference as compared with those fistulating into right ventricle (P < 0.001). Diagnostic sensitivity and specificity of echocardiography in ruptured ASV was 97.1% and 100% respectively, but the rate of missing subarterial ventricular septal defect was 24.2%. Sixty-seven patients received operation, one died of pulmonary embolism six weeks after operation. Thirty-six patients were followed up, the mean follow-up period was 6.5 years (range 1 to 16 years). Two patients were reoperated for leak of ventricular septal defect and aortic regurgitation. The cardiac function of sixteen patients were found to be of NYHA class I, seventeen of class II and three of class III. CONCLUSION: Echocardiography in combination with aortography is an accurate diagnostic method for ruptured ASV and the prognosis in majority of the patients operated in time is good.

[The application of laser therapy plus endobronchial stenting in treatment of complete airway obstruction].

OBJECTIVE: To assess the efficacy of laser therapy plus endobronchial stenting in the treatment of complete airway obstruction. METHODS: 17 cases with complete airway obstruction were treated with Nd: YAG laser and endobronchial stenting from April 1998 to November 2000. The patients' dyspnea, airway gauge, pulmonary function and complications were observed. RESULTS: The airways were all restructured following a single or multiple laser treatment; the airway gauge enlarged and the average diameter increased (5.82 +/- 1.01) mm. Immediate relief of dyspnea was achieved in sixteen patients and dyspnea index lessened one grade in 4 patients, 2 grades in 11 and 3 grades in one. Obvious improvement of forced expiratory volume in one second (FEV(1)) was noted in all patients;the improvement averaged (0.49 +/- 0.18) L. Endobronchial stents were placed for 14 patients after laser treatment. After stent placement, the airway was further opened and the average airway gauge increased (3.50 +/- 0.94) mm further in diameter. Patients' dyspnea and FEV(1) were further improved; dyspnea index lowered one grade in eight patients and 2 grades in two patients. FEV(1) improved further (0.41 +/- 0.20) L in average. Complications after laser therapy included hypoxemia in 15 patients, hemorrhage in 2 and postoperative dyspnea in 3. Follow-up duration ranged from 2-20 months. Complications after stenting included recurrent pneumonia in 4 patients, airway obstruction due to granuloma or tumour growth in 5 and stent migration in 1. CONCLUSION: Laser therapy and endobronchial stent placement were effective in the treatment of patients with totally occluded airway. Nd: YAG laser can be first chosen to restructure the occluded airway, then endobronchial stent must be used to further enlarge the airway and maintain the therapeutic effect.

[Adrenomedullin and hypoxia-induced pulmonary hypertension].

OBJECTIVE: To study adrenomedullin (AM) in rat and human lung during hypoxia-induced pulmonary hypertension. METHODS: Normobaric hypoxic pulmonary hypertension animal model was adopted, pulmonary artery pressure and RVdp/dt were measured. The changes of AM contents in blood and bronchoalveolar lavage fluid were studied. The plasma levels of AM in patients with pulmonary hypertension were measured. RESULTS: During hypoxia, pulmonary artery pressure, RVdp/dt, AM contents in blood and bronchoalveolar lavage fluid in rats increased. Plasma levels of AM were significantly higher in patients with pulmonary hypertension than in the control subjects. CONCLUSIONS: AM may play an important role in developing hypoxic pulmonary hypertension in rats and human. The increase of AM level is in proportion to the extent of pulmonary hypertension.

[Association of HLA-DRB1 genes with pulmonary tuberculosis].

OBJECTIVE: To assess the association of HLA-DRB1 genes with pulmonary tuberculosis (PTB) and the correlation between HLA-DRB1 alleles and clinical manifestations of patients with PTB. METHOD: Alleles at the HLA-DRB1 typing were analyzed by polymerase chain reaction with sequence-specific primers (PCR-SSP) in 74 patients and 90 healthy controls. RESULTS: Compared with controls, a significant increased frequency of DRB1 * 15 was found (34.3% vs 17.0%, Pc < 0.05, RR = 2.91). The frequency of DRB1 * 12 was also increased, but the difference was not statistically significant(15.4% vs 7.5%, Pc > 0.05). The frequency of DRB1 * 11 was significantly decreased (1.4% vs 9.9%, Pc < 0.05, RR = 0.12). More retreated and drug-resistant cases were found in DRB1 * 15 positive patients than in DRB1 * 15 negative patients. CONCLUSIONS: These results show that HLA-DRB1 * 15 allele is susceptible gene in PTB, DRB1 * 11 may be a protective allele, HLA-DRB1 * 15 allele may be related to patients with refractory PTB.

[A surgery oriented serial treatments of 191 patients with large primary liver cancer].

OBJECTIVE: To discuss the methods and effects of a series of surgery oriented therapies in the treatment of large primary liver cancers. METHODS: From January 1993 to June 1999, 191 patients with large liver cancer were treated chiefly by operation. The size of the primary cancer varied from 5.2 to 19.7 cm (median 9.4 cm). Various kinds of liver resections were performed in 121 patients and, as a supplement, deep cryosurgery was carried for out the rest 70 patients. Importable drug delivery system (IDDS) was instituted intraoperatively. Transcatheter arterial chemo-embolization (TACE, THP 30-60 mg, E-ADM 20-40 mg, CDDP 40-80 mg, MMC 10-20 mg, iodine oil 5-30 ml), percutaneous ethanol injection (PEI), bioimmunotherapy and the traditional Chinese medicine were used pre- and post-operatively. CT angiography (CTA) and CT during arterial portography (CTAP) were used to find satellite nodules. Early recurrence was diagnosed by AFPmRNA monitor in peripheral blood. Child's classification plus branch chain amino acid/aromatic amino acid (BCAA/AAA) were adopted in evaluating the pre-operative liver functions. RESULTS: Remarkable results were observed after this surgery oriented serial treatment. The 1-, 3- and 5-year survival rates in the resection group were 75.8%, 45.6% and 30.4%, respectively. The 1- and 3-year survival rates in the cryosurgery group were 63.2% and 37.0%. The operative mortality was 1.6%. Recurrence rates were 69.2% in AFPmRNA positive group and 33.3% in AFPmRNA negative group. Comparing this two groups, significant difference was observed (P < 0.05). BCAA/AAA were lower than 1.5 in two patients died after resection. CONCLUSION: A serial treatment with surgery as the chief modality gives satisfactory results in large primary liver cancers. This regimen should be adhered to as a main strategy in dealing with large liver cancers. AFPmRNA in the peripheral blood, signifying a recurrence, may become a hope of a new clinical parameter. BCAA/AAA plus Child's classification is able to evaluate more accurately the hepatic functional reserve before surgery.

[Changes of crop yields and soil fertility under long-term application of fertilizer and recycled nutrients in manure on a black soil].

The results from a field experiment over thirteen years on a black soil in Northeast China indicated that nitrogen fertilizer increased crop yields by 724 kg.hm-2 per year on average, with an increase rate of 27.5% and about 9.4 kg increased yield from 1 kg applied N. In the early stage, the crop yields almost gave no response to phosphorus fertilizer, owing to the high level of soil available phosphorus at the beginning of the experiment. The average increase of crop yields by phosphorus in thirteen years was only 241 kg.hm-2 per year, with an increase rate of 7%, about 12.7 kg increased yield from 1 kg applied P. Annual application of recycled pig manure made by 80% of harvested grain fed to pigs and the stolks as bedding materials could increase crop yields by 268, 258 and 255 kg.hm-2 per year with an increase rate of 9.8%, 7.6% and 7.0%, based on no fertilizer, with N and with P and N, respectively. There was a trend that the effect of recycled manure on crop yield was gradually increased during the period of experiment, implying the existence of accumulative residual effect from the manure.

[Determination of triacontanol and octacosanol in fenglasu and fenglasu capsules by capillary gas chromatography].

OBJECTIVE: The method was established for the determination of triacontanol and octacosanol in fenglasu and fenglasu capsules. METHODS: The solvent was chloroform. The internal standard was hexatriacontanol. Using SIMPLCITY-1 capillary column, flame ionization detector was specified, injector temperature was 300 degrees C, and the column temperature rise by programming. [Initial temperature was 240 degrees C, maintaining 1 min and then rising the temperature (5 degrees C.min-1) to 300 degrees C for 15 min]. The flow rate of Nitrogen was 27 ml.min-1. RESULTS: The calibration curves were linear in the range of 0.4-1.6 mg/ml for triacontanol(r = 0.999), 0.2-0.6 mg/ml for octacosanol(r = 0.9996) respectively. The average recovery (n = 9) was 99.2% for triacontanol (RSD = 1.15%), 99.91% for octacosanol (RSD = 1.40%). CONCLUSION: The method is rapid, accurate and reliable for the quality control of fenglasu and fenglasu capsules.

[Treatment of 2-naphtholwastewater using air oxidation and ion exchange].

The properties of 2-naphtholwastewater were studied in details in this paper, the wastewater was characterized by the strong acidity, high buffer capability and concentrated salt. Air oxidation and anion exchange were employed for the treatment of the wastewater. Under formal pH, room temperature and 1 BV/h rate, the result showed that the removal of COD reached to 97% and the recovery of sodium naphthalensulfonate to 98%. Besides, wastewater treated can be reused for washing the sodium naphthalensulfonate.

[Non-filamentous activated sludge bulking caused by the deficiency of nitrogen].

The effect of nitrogen deficiency on activated sludge bulking was studied specially in some experiments carried on a sequencing batch reactor fed with beer wastewater in this paper. The experimental results showed that the sludge settled properly at a influent BOD/N value of 100/4. When the value of BOD/N was 100/3 and 100/2 respectively, non-filamentous activated sludge bulking caused by an excessive growth of viscous Zoogloea with high moisture content occurred. When the value of influent BOD/N was 100/0.94, more serious non-filamentous bulking occurred. Furthermore, the effect of nitrogen deficiency on the nitrogen sources and phosphorus sources utilization rate and the COD removal rate was studied in the experiments.

[Characteristics of nitrogen and phosphorus pollution in the middle and small creeks, suburban Shanghai].

1. The middle and small creeks in suburban Shanghai were under heavy eutropic condition, with high loadings of nitrogen, phosphorus and organic pollutants. KN, TP and CODCr of the surface water of them were several times greater than the critical values of Types V, the worst level of water. 2. Spatial variations of N and P pollution were observed, as the creeks in different places accepted different types and amounts of pollutants. Affected by domestic sewage, the creeks nearby residential areas were often heavily polluted by NH4+ and P. The creeks in agricultural fields were more easily polluted by NO3- than by NH4+ and phosphorus. The creeks nearby livestock farms often witnessed sudden soaring of NH4+ and P. 3. N and P loadings of the creeks were also affected by the seasonal evolvement of ecological environment. With the increase of temperature and biomass, NH4+ and NO3- in the surface water were largely consumed and decreased as a result; while organic N and particle P increased as the enhancement of suspending biological particles. NO2- in the surface water increased with temperature as nitrifying bacteria became active. Total P and water soluble P also showed the trend of enhancement with the intensification of biological activities, however, they were also controlled by the condition of pH and DO in the water. 4. At the height of the summer, the surface and bottom water of the creeks differed in N and P loadings. NH4+, soluble P and total reactive P in the bottom water were higher than those in the surface; while NO3- and NO2- in the surface were higher than those in the bottom. Such water-quality stratification inevitably resulted from the enhancement of phytoplankton in the surface water and intensified release of NH4+ and P from the sediments. In the autumn, as-the temperature decrease, such phenomenon declined gradually.

[Study on the quality of life among the elderly in the rural and pastoral districts in Baotou city].

OBJECTIVE: In order to explore the situation of QOL of the elderly in the rural and pastoral districts in Baotou city, to compare the differences of QOL between the two districts, and to find out the major factors influencing the QOL of the elderly. METHODS: SF-36, recommended by WHO was adopted to study the QOL of 1 421 old people in the rural and pastoral districts in Baotou city. RESULTS: (1) The average weighted score based on the SF-36 was 64.16 +/- 18.76 in the rural area district and 73.94 +/- 17.48 in the pastoral district (F = 137.8, P < 0.01). The level of QOL of the elderly in the pastoral district was significantly higher than that in the rural district. (2) Results from the multiple liner regression analysis showed that the main factors influencing the QOL of the elderly were chronic disease, age, hospitalization, education and gender in the rural district, but chronic disease, age, hospitalization, income and marital status in the pastoral district. CONCLUSION: Population with older age, low education, no spouse and low income should be viewed as the priority of attention and their special difficulties fully considered when setting up the policy of health care and social security.

[Preparation of genetically engineered retinal pigment epithelial cells capable of secreting nerve growth factor].

OBJECTIVE: To develop genetically engineered retinal pigment epithelial (RPE) cells which are able to express bioactive nerve growth factor (NGF) steadily and efficiently and to combine gene therapy with retinal transplantation techniques to treat retinal degeneration. METHODS: Replication-deficient retrovirus-plasmid recombinant pLXSN-NGF was constructed via subclone and clone, and then packaged into pT67 cells to obtain virion containing a secretable form of NGF gene. We evaluated titer of the retroviral recombinant with NIH 3T3 cells and then transduced NGF gene into cultured RPE cells by the retroviral recombinant. We also analyzed the expression of NGF gene in transduced and untransduced RPE cells via reverse transcription (RT)-PCR and Western blot. The bioactivity of expressed NGF were evaluated by stimulating PC 12 cells with conditioned medium from transduced and untransduced RPE cells. RESULTS: The titer of replication-deficient retroviral recombinant reached 1.2 x 10(7) cfu/ml. NGF protein and mRNA were detected in the conditioned medium from transduced RPE cells, but not from the normal RPE cells. Marked neurite process outgrowth from PC 12 cells was observed after being stimulated with the conditioned medium from transduced RPE cells. CONCLUSIONS: RPE cells are able to be transduced by replication-deficient retrovirus with high efficiency, and secret bioactive NGF at a high level, which might be an ideal candidate for engineered cells.
