Lymphangiography in staging of carcinoma of the prostate: the potential value of percutaneous lymph node biopsy.

Bilateral pedal lymphangiography was performed in the staging of 208 patients with prostatic carcinoma. Forty-seven (23%) were found to have lymph node involvement, and 40 of these 47 had Stage C disease. Nine per cent of the Stage C1 patients (moderately advanced disease) and 37% of Stage C2 (massive carcinoma) had lymph node metastases. The distribution of the abnormal nodes was in continuity with the lymph node drainage from the prostate. The external iliac nodes were the most frequent site of metastases. Histological confirmation was obtained in 20 of the 40 positive Stage C2 cases. More recently, percutaneous aspiration biopsy of the opacified nodes was used to document the presence of metastic disease, thereby extending the value of lymphangiography.

"Mini" Gianturco stainless steel coils for transcatheter vascular occlusion.

Small stainless steel coils have been developed for transcatheter vascular occlusion procedures. The coils can be passed through a 5-French polyethylene catheter, extending the clinical applications of coil occlusion procedures for small and/or tortuous vessels. Two cases describing the use of these coils are reported.

Transcatheter arterial occlusion in the management of rectosigmoidal bleeding.

Transcatheter arterial occlusion was performed in five patients with recurrent rectosigmoidal bleeding. Four had malignant neoplasms, and the fifth, an arteriovenous malformation. The superior hemorrhoidal artery was embolized in two patients, the inferior mesenteric artery in two, and the left internal iliac artery in one patient. In four patients, immediate and complete control of rectal bleeding was achieved, and gradual control was accomplished in the fifth patient. Left lower quadrant pain and fever developed in three patients. The possibility of rectosigmoidal bleeding from the internal iliac artery is stressed.

Arterial occlusion in the management of pain from metastatic renal carcinoma.

Arterial embolization was performed in nine patients with metastases from renal carcinoma who had severe pain resistant to conventional therapy. Patients with metastases in the ilium (four), the lumbosacral spine (one), and the base of the skull (one) experienced pain relief lasting from one to six months. The other three patients, who had metastases in the proximal femur, underwent preoperative embolization to facilitate tumor curettage and internal hip fixation. No significant complications were seen with this therapeutic approach.

Computed tomography diagnosis of mesenteric masses.

The computed tomographic findings of 30 mesenteric masses are presented. To delineate the mesentery accurately, oral contrast material must fill the entire small bowel. Of the masses, 23 were secondary to non-Hodgkin's lymphoma. These masses were irregular in shape and homogeneous in their tissue attentuation. Two of the 23 had a normal lymphangiogram in the upper paraaortic area. Both lymphangiography and CT were necessary to define the extent of disease in such patients. The six mesenteric metastases were irregular in shape. Four of these had areas of decreased attentuation eccentrically located within the mass which may be secondary to necrosis. One mesenteric cyst had a circular shape with smooth, sharp borders, and a centrally located area of decreased attenuation.

Comparison of scintigraphy, sonography, and computed tomography in the evaluation of hepatic neoplasms.

A comparison of scintigraphy, ultrasonography, and computed tomography (CT) in 94 proven patients with clinically suspected liver disease is reported. CT proved to be the most accurate in detecting masses and assessing the complete extent of intrahepatic disease. The most reliable combination was CT and scintigraphy. The specific advantages and disadvantages of each method are discussed. The diagnostic scheme followed in the imaging evaluation of an hepatic mass in our clinical practice is discussed.

Radiographic features of olfactory neuroblastoma.

Olfactory neuroblastomas are malignant, slowly growing neurogenic tumors originating from the olfactory mucosa of the nasal cavity. Fourteen patients with this tumor were evaluated and showed variable extension into adjacent structures such as the ethmoid and sphenoid sinuses, orbit, and anterior cranial cavity.

Quantitative measurement of body motion using Schottky barrier silicon photodiode.

A new motion detection system has been developed using a laser beam and a Schottky barrier silicon photodiode. The system broadens the scope of gravitational center stabilography by facilitating the quantitative assessment of tremors of body appendages such as the hands and even the head. The system also eliminates cumbersome platforms and wire attachments previously used in gravitational center stabilography. The subject is asked to aim the beam at the photodiode. A quantitative off-center variation parameter, ARDS (Average Radial Distance Squared multiplied by time), was utilized in units of cm2 sec. A preliminary evaluation of the system shows that it is suitable for rapidly screening large numbers of subjects for localised neuromuscular control.

Intrarenal arteriovenous fistulas: transcatheter steel coil occlusion.

Intrarenal arteriovenous fistulas are either congenital (cirsoid), acquired or idiopathic. Transcatheter occlusion of these fistulas was accomplished using the Gianturco stainless steel coil. The anatomical configuration of the feeding and draining vessels dictated the technical approach. There was considerable conservation of renal tissue as the result of this non-surgical therapeutic alternative.

Lymphangiography in patients with malignancy in a non-descended testicle.

Lymphangiography was performed on 23 patients with malignancy in non-descended testicles, 14 of whom had seminomatous and 9 non-seminomatous tumors. Lymph node metastases were shown by lymphangiography in 8 patients: 3 in lumbar nodes, 1 in iliac nodes alone and 4 in lumbar and iliac lymph nodes. Microscopic metastases were shown in lumbar nodes at retroperitoneal lymph node dissection in 2 patients when the lymphangiograms were negative. Iliac lymph node metastases are rare in testicular tumors but may be seen in tumors of non-descended testicles, alone or in combination with lumbar metastatic disease. This information is extremely important for the radiologist as well as the clinician in the management of patients.

Computed tomography in the evaluation of musculoskeletal neoplasms.

In a study of 62 musculoskeletal neoplasms, computed tomography contributed little to establishing tissue diagnosis. Its primary value lay in the assessment of intraosseous and/or extraosseous extent of disease. CT also established the presence or absence of disease in a small number of cases with clinically questionable findings. In most cases, conventional radiographic methods were superior in diagnosing musculoskeletal neoplasms.

Radiographic aspects of massive bone osteoarticular allograft transplantation.

Experience with 56 cases of osteoarticular allograft transplantation of the extremities is presented. Cadaveric allograft including a joint surface and a variable amount of shaft is used in the treatment of aggressive benign and low-grade malignant tumors involving a long bone near a joint. The preoperative radiographic evaluation is outlined. Thirty-four patients had an uncomplicated course. Twenty-two patients had complications including aseptic fracture, infection, and tumor recurrence. The radiographic spectrum of both complicated and uncomplicated cases is illustrated.

Prostaglandin E1 (PGE1) in angiography of tumors of the extremities.

Prostaglandin E1 (PGE1) is a potent vasodilator. The value of this agent as an adjuvant in angiography of tumors of the extremities was evaluated in 10 patients. Except in one case involving a hemangioma, the diagnostic information concerning tumor vascularity and delineation of the extent of disease decreased after the use of PGE1. The visualization of small normal muscular arteries was improved. Therefore, the effect of PGE1 may be to preferentially increase the blood flow in normal tissues relative to the tumor. PGE1 seems to be of little value in enhancing the angiographic demonstration of neoplasms of the extremity.

Percutaneous needle biopsy of bone in the cancer patient.

Needle biopsy of skeletal lesions is easily accomplished by the percutaneous technique. Although this approach has been utilized for many years, the frequency has definitely increased of the 91 cases performed at M. D. Anderson Hospital and Tumor Institute from September 1976 to September 1977, adequate material was obtained in 92.3% of the patients, and the diagnosis was established accurately in 83.5% of the cases. No complications were encountered. The technique, indications, and contraindications are discussed and illustrated with representative cases. The contribution of this technique to the management of patients with malignant disease is significant. The benefits include rapid and accurate establishment of a tissue diagnosis, avoidance of a surgical procedure with its associated risks and cost, and simplicity and safety of the procedure facilitating its repetition if needed.

Angiography of abdominal leiomyosarcoma.

The spectrum of angiographic findings of abdominal leiomyosarcoma in 33 cases is presented, and the value of angiography in the diagnosis and management of such patients is discussed. Angiographic features of abdominal leiomyosarcomas are different depending upon the site of origin. Small bowel lesions are hypervascular, and those in the stomach and colon moderately vascular. Well circumscribed masses with enlarged feeding arteries and draining veins form the predominant presentation of small bowel leiomyosarcoma. In the retroperitoneum these neoplasms are usually hypovascular to moderately vascular, and displacement of major vessels, particularly the inferior vena cava, is the most common angiographic finding. Bladder leiomyosarcoma can be either moderately vascular or hypervascular. Vascularity of metastases is usually similar to that of the primary lesion.

Computed tomography in ocular neoplastic disease.

The computed tomographic findings of four patients with neoplastic disease of the global rim are discussed. All four patients demonstrated eccentric thickening of the scleraluveal rim with extension into the globe. Possible ways of differentiating these lesions from other orbital conditions are discussed. Window widths below 100 EMI units are best to visualize neoplasms of the global rim. For evaluation of the orbits, multiple window widths with varying window levels should be used.
