[Curative limb saving operation (curative wide resection)].

The recent results of surgical treatment of soft tissue sarcomas have dramatically improved. The result of the group study of soft tissue sarcomas, which was supported by the Ministry of Health and Welfare (1962-1976) showed that the local recurrence rate was 50%, and the metastatic rate was 60%. In contrast with this result, the recent results of our curative wide resection showed the local recurrence rate of 10%, and the metastatic rate of 30%. These procedures are based on the clinico-pathological findings which we studied. Curative wide resection is a method of radical wide resection, similar to radical local resection (reported by Enneking) and radical local excision (reported by Stenar). Our procedures are the least aggressive of the three radical wide resections, but our results are the same as the other wide resection methods. To obtain good results with these procedures, accurate preoperative diagnosis and skillful surgery are necessary.

[Studies of the cellular immune response in patients with urinary bladder carcinoma. X. Evaluation of serum IAP and ASP levels].

Serum levels of immunosuppressive acidic protein (IAP) and acid soluble glycoprotein (ASP) were measured in 48 patients with urinary bladder carcinoma and 51 control subjects. Both IAP and ASP levels were significantly higher in the patients than in the control subjects, and the increase of these two serum substances seemed to be closely related to grading and staging of urinary bladder carcinoma. In many patients, serum IAP and ASP levels decreased after resection of tumors. There was an inverse correlation between IAP or ASP level and phytohemagglutinin (PHA)-induced lymphocyte blastogenesis (stimulation index, SI). This suggested an immunosuppressive effect of these serum factors. After chemotherapy of urinary bladder carcinoma, both IAP and ASP levels would decrease. It is suggested that serum IAP and ASP levels could be good indicators for evaluating the effect of chemotherapy.

[Origin of components P 11 and P 13 in short latency somato-sensory evoked potentials (SSEP): correlative study of SSEP and intraoperative evoked potentials].

In 15 patients with cervical or posterior fossa lesion, SSEPs were recorded between the skull and the non-cephalic reference electrodes during the surgical operation and compared with the evoked potentials directly recorded at the same time from the surface of the cervical spinal cord and the brain stem. The directly recorded evoked potential consisted of three main components appeared within about 25 ms., they were a small negative spike wave, a large positive spike wave and a subsequent slow potential. The positive spike wave of the evoked potentials recorded from the surface of the dorsal column was not only coincided in latency with component P 11 of SSEP, but also showed the greatest amplitude at the lower cervical level. Moreover, the positive spike wave gradually delayed in latency and reduced in amplitude from lower to upper cervical segments. The amplitude of the positive spike wave was greater at the surface of the dorsal column ipsilateral to the stimulated median nerve than that of contra-lateral recording. No polarity change was observed between the anterior and posterior surface of the spinal cord. Similarly, the positive spike wave of the evoked potentials, recorded from the surface of the brain stem, showed fairly same latency with P 13 and a maximal amplitude at the surface of the cuneate tuberculum ipsilateral to the median nerve stimulated, and those positive spike wave traveled to contra-lateral ventral surface of the pons, presumably from ipsilateral cuneate nucleus to the contra-lateral medial lemniscus.(ABSTRACT TRUNCATED AT 250 WORDS)

[Radiation therapy for cancer of the uterine cervix in the aged].

Since a majority of old aged patients have the troublesome complications and their physical or mental emaciation is clearly appeared, it is necessary for them to adopt a new treatment method which was considered about their such conditions. The crude survival rate of old aged, over 71, patients suffered from carcinoma of the uterine cervix were 63.6% (7/11) for stage 1, 60% (36/60) for stage 2, 50% (53/106) for stage 3 and 28.6% (8/28) for stage 4. About 20% of patients in each stages were suffered from the complications. We considered the treatment for the old aged patients such as follows: the radiation dose must be reduced 10% for 71 to 75 year old and 20% for 76 to 80 year old. In the case of over 81 year old, an intracavitary irradiation is only applied for the palliative aim at the out patient clinic.

[Antiepileptic effects of globulin-N as an intact human immunoglobulin and its tissue-distribution in kindled cats].

The antiepileptic effects of Globulin-N as an intact human immunoglobulin were examined using the amygdaloid kindled animals, which was prepared by the method of Goddard et al (1969) and Wada et al (1974). In the non-treated kindled cats, generalized convulsion (GC) and after-discharge (AD) in amygdala, hippocampus and neocortex was caused synchronously by electrical stimulation with generalized seizure-triggering threshold (GST) at constant intervals of 24 hours. By contrast, in kindled cats treated intravenously with 200 mg/kg Globulin-N, the GC and AD due to the GST-stimulation disappeared entirely for the period 4 to 8 days after medication in 8 out of 10 cases. Further, kindled cats treated with anticonvulsant, phenobarbital or phenytoin inhibited GC and AD to GST-stimulation until 24-48 hours after medication. On the other hand, the plasma half-life of Globulin-N was determined as about 10 days in the kindled cats, and Globulin-N was distributed immunohistochemically not only in the visceral organ-tissues but also in the tissues of the brain and spinal cord. The administered Globulin-N or 14C-human immunoglobulin pass through the blood-brain barrier easily in kindled cats with GST induced-epileptic seizure, and was apparently taken up by the nerve cells and glial cells in the cerebral cortex and other extensive deep structure involved in the central nervous system. These results seem to support the clinical usefulness of Globulin-N in patients with epilepsy (Ariizumi et al, 1982) and also epileptic psychosis.

[A clinico-physiological study on the generators of short latency somatosensory evoked potential].

The purpose of this paper is to locate the generators of each wave component in the short latency somatosensory evoked potential (SSEP), by means of cumulative analysis of the SSEP obtained from various localized lesions in the upper cervical cord through brain stem and cerebral subcortical structures. Since there are considerable inconsistency of naming each component in the literature, SSEP to median nerve stimulation of 10 normal subjects were examined by two different recordings, i.e. recording from an electrode at the parietal scalp with a reference electrode on Erb's point (Par.-Erb), and the other at the frontal scalp with a reference electrode on Cv 7 (Fro.-Cv 7), and the SSEP was carefully studied. In normal subjects, the SSEP by Par.-Erb lead yielded 5 negative components (N 7, N 11, N 16, N 18 & N 26) and 4 positive components (P 9, P 13, P 22 & P 42), while by Fro.-Cv 7 5 negative components (N 7, N 10, N 12, N 16 & N 28) and 5 positive components (P 9, P 11, P 13, P 20 & P 44). Thirty three patients were subjected to analyse the influence of localized lesions to each component of the SSEP and the recording was evaluated in regard to (a) identification of each component, (b) latency of each component and inter-peak latency difference exceeding 2 SD, and (c) over 50% asymmetry and laterality of the amplitude. Cervical spondylotic myelopathy, high cervical cord tumor, tonsillar herniation, pontine infarct and hemorrhage, circumscribed thalamic lesion, and vascular lesion of centrum semiovale were carefully examined with CT scan and the findings were compared with neurological findings periodically. SSEP was taken repeatedly, especially before and after operative intervention, and alteration of the component was referred to the clinical progress of the lesion. In conclusion, results obtained from our present observation indicated that P 9 was the extramedullary projection, P 11 was intramedullary origin of the lower cervical cord, P 13 was medulla oblongata origin and P 13-N 16 was projection from medulla oblongata to thalamus. N 16-N 18 and N 26 were considered projection from thalamus to hand area of the parietal lobe with some association area and N 28 had the generator widely based on frontal projection system. These findings appeared to be quite useful for topographic diagnosis and functional evaluation of the lesions in central nervous system.

[Evaluation of blink reflexes in cerebellopontine angle tumors].

Blink reflexes were analysed in 28 cases with cerebello-pontine angle tumors (22 cases of acoustic neurinoma and 6 cases of meningioma). In patients with acoustic neurinoma, delay or absence of R1 was found in all but two cases (91%) and abnormality of R2 was in 11 (50%). Estimating the mean latency difference of R1 on both sides it was found useful for establishing early diagnosis of small acoustic neurinomas, which had no apparent clinical signs of facial or trigeminal nerve involvement. In patient with obsolete hemorrhage (5 cases) or large cyst (6 cases) within the acoustic neurinomas, which were proven at operation, abnormality of blink reflexes was more prominent than in patient with solid tumors (11 cases). In patients with meningioma, although all were huge tumors, abnormality of R1 was less conspicuous in comparison with acoustic neurinoma. Bilateral delay of R2 was found in 3 of 6 patients. In 13 patients who underwent spinal accessory-facial anastomosis for facial palsy after removal of the tumors, R1-like response were recorded in all but one on the affected side and R2-like response was recorded in only one patient. Observing alteration of blink reflexes in patients with cerebello-pontine angle tumor was useful for establishing early diagnosis and evaluating function of trigeminal nerve, facial nerve and brainstem.

[A case of Buschke-Loewenstein tumor].

A 67-year-old man with a tumor resembling carcinoma of the penis was seen at our Department. The tumor appeared clinically to be malignant and partial amputation of the penis was therefore performed. But biopsy specimens from parts of the mass were benign and histological examination revealed Buschke-Loewenstein tumor.

[Intracranial giant osteochondroma with unusual clinical features].

A case of giant osteochondroma arising from the paramedian skull basis was reported because of unsual clinical features in cranial nerve involvement and pyramidal tract signs, and especially because of sudden death due to intracranial hemorrhage. The patient was a 24-year-old male, who had been followed for over 3 years and 9 months, and received two operations because of bilateral extension of the tumor. During follow-up he had experienced frequent euphoric spells untill mental deterioration became evident. Then he went suddenly into deep coma and intracranial hemorrhage was found with CT scan. Hemorrhage was probably attributed to the marked stretching of cerebral arteries around tumor, although autopsy could not confirm the source of hemorrhage. Neurological characteristics of a giant osteochondroma arising from the skull basis were described in comparison with over skull basis tumor such as chordoma, and special reference was made for CT scan diagnosis.

Reticular neuron activities associated with locomotion in thalamic cats.

Unit discharges of the reticular neurons and EMGs of extensors and flexors were recorded in the 4 limbs of thalamic cats during stepping on a motor-driven treadmill to study the functional role of the spino-bulbo-spinal (SBS) reflex in locomotion. (1) Reticular neurons showed alterations of increased or decreased discharge rate depending on the phase of the step cycle; they were divided into input (ascending), output (reticulospinal, R-S) and internuncial neurons underlying the SBS reflex. (2) R-S neurons showed increased spikes from the end o the stance to the beginning of the swing phase; in this period, flexor muscle EMGS were obtained. The spikes decreased or disappeared when extensor EMGs were observed. To analyze the relationship between activity in R-S neurons and flexor muscles, R-S neuron spikes were used for triggering and post-spike EMGs were averaged. Averaged EMGs of the correlated flexor revealed a 12 and 6 ms latency for hind- and forelimbs, respectively. Most R-S neurons elicited EMGs of flexor in muscles of one ipsilateral leg, some elicited flexor EMGs of both the ipsilateral fore-and hindlimbs. (3) Input neurons of the reticular formation from spinal ascending tracts fired spikes upon touching of the skin over the correlated limb. During stepping, they showed two types of response patterns: a burst discharge appeared for the leg which was swinging and touching the belt, the other pattern was of continuous discharges at the end of stance and at the beginning of swing phases. (4) Internuncial neurons which relay impulses between the input and output neurons also showed alternate firings depending on the phase of the step cycle. Spikes could be elicited by touching the skin over the 4 limbs. (5) The relationship among the activities of the 3 groups of reticular neurons was analyzed by the post- or pre-spike averaging technique and appropriate latency responses were obtained. (6) The results indicate that flexor EMGs were excited by cutaneous afferents via the SBS reflex in cats stepping on the treadmill. The role of the SBS reflex in locomotion is discussed.

Effects of diltiazem hydrochloride on serum lipids: comparison with beta-blockers.

The effects of diltiazem hydrochloride, a vasodilator often used to treat hypertension, and beta-blockers (pindolol and propranolol hydrochloride) on lipid metabolism and carbohydrate metabolism were assessed by measuring serum levels of total cholesterol, high-density-lipoproteins (HDL)-cholesterol, triglycerides, and glucose in patients taking these drugs for three months. No significant changes were observed in any of the values in patients taking either diltiazem hydrochloride (180 mg/day) or beta-blockers (15 mg of pindolol or 60 mg of propranolol daily). The decrease in HDL-cholesterol levels and increase in triglyceride levels reported in 1978 to be associated with use of propranolol, and with the concomitant use of hydrochlorothiazide and propranolol, was not found.
