Nocturnal enuresis and upper airway obstruction.

PURPOSE: The purpose of this paper is to investigate the relationship between nocturnal enuresis (NE) and upper airway obstruction (UAO) in pediatric population. MATERIAL AND METHODS: This study presents the results of our experience with 321 children who underwent adenotonsillar surgery. RESULTS: Among 321 children who were operated on, 111 (35%) had NE. Seventy-four of the 111 children who had NE have been evaluated postoperatively. After a follow-up period of 3 months 47 (63%) patients were free of their complaints, while three (4%) of them reported decrease in the frequency of NE and 24 had no change in their complaints. CONCLUSIONS: We do suggest that UAO is probably a more common etiological factor in NE than previously recognised.

Primary papillary adenocarcinoma confined to the middle ear and mastoid.

Primary adenocarcinoma is a rare tumor of the middle ear and temporal bone; its most frequent symptoms are hearing loss, otalgia, and facial paralysis. Otoscopic examination of a 27-year-old man revealed purulent discharge in the ear canal, diffuse edema, and hypertrophy of the right tympanic membrane. He presented with a grade III (House-Brachman) facial paralysis and right conductive hearing loss with a history of aural discharge for 6 months, otalgia, and facial weakness for 2 days. Computed tomography of the temporal bone showed an opacity filling the tympanic cavity, antrum, and aditus. Tympanotomy revealed diffuse edema of the middle ear mucosa, and granulation tissue was encountered during mastoidectomy filling the antrum and periantral cells and eroding the fallopian canal at the level of the oval window. After the histopathological examination revealed papillary adenocarcinoma, a subtotal temporal bone resection, facial nerve segmenter resection, and end-to-end anastomosis of the facial with the hypoglossal nerves were performed. The importance of histopathological examination in all cases of chronic otitis media with granulation tissue is stressed.

The relation between recurrent acute subglottic laryngitis and asthma in children.

OBJECTIVE: Acute subglottic laryngitis is caused by some viruses and may recur several times in children. Risk factors underlying the recurrences of this disease are obscure. The present study was undertaken to investigate the relation between acute subglottic laryngitis, asthma and allergy. METHODS: Twenty five children with recurrent acute subglottic laryngitis were evaluated for risk factors for recurrent respiratory tract infections and allergy. Eight of the children had high IgE levels and six of them were found to be allergic by skin prick tests. RESULTS: During a 1 year follow-up period four children showed asthma symptoms and in three of these patients recurrences of croup attacks disappeared after asthma treatment with nebulized budesonide. CONCLUSION: We concluded that recurrent acute subglottic laryngitis may be an early sign of subsequent asthma development.

Intraoperative management of the thyroid gland in laryngeal cancer surgery.

OBJECTIVE: To determine if it is necessary to perform a hemithyroidectomy routinely with all total laryngectomies or if it should be reserved for selected cases. DESIGN: A retrospective analysis of 215 cases who had been operated on due to laryngeal cancer in our clinic between 1985 and 1999. SETTING: In only 182 cases, hemithyroidectomy and isthmectomy were performed together with laryngeal surgery. Of these, 98% were male. Their ages ranged between 42 and 70 years. The tumour was located in the supraglottic region in 93 (51%) and in the glottic region in 24 (13%) cases. In 65 cases (36%), the tumour was transglottic. Twenty cases of transglottic tumours (31%) and 3 cases of glottic tumours (12.5%) were found to have subglottic extension. METHODS: Total laryngectomy with unilateral or bilateral neck dissection and hemithyroidectomy on the tumour side plus isthmectomy were performed on all patients. On the pathologic specimens, subglottic extension was measured anteriorly and posteriorly from the free edges of the vocal cords. The specimens were stained with hematoxylin and eosin and examined under a light microscope. MAIN OUTCOME MEASURES: With glottic and transglottic carcinomas, the need for thyroidectomy may be based on the intraoperative assessment of the thyroid gland. In subglottic carcinomas, a hemithyroidectomy should routinely be performed. There may be no need to perform thyroidectomy in all total laryngectomy cases. RESULTS: The thyroid gland was invaded by squamous cell carcinoma in only 2 cases (1%). Both of these cases were transglottic tumours staged as T3 and T4 and had a subglottic extension more than 1 cm. CONCLUSIONS: We recommend routine hemithyroidectomy and isthmectomy during total laryngectomy only in cases with subglottic extensions more than 1 cm or thyroid cartilage invasion with tumour. In the other cases, assessment of extralaryngeal invasion and thyroid gland invasion by the tumour will determine whether thyroidectomy should be performed.

Myoepithelioma of the parotid gland: a report of two cases.

We describe the clinical and pathologic features of two benign myoepitheliomas of the parotid gland. Through 1985, only 42 other cases had been reported in the literature--39 benign and three malignant. Fewer than 100 cases had been reported through 1993. Since then, two other reports have been published. But are these tumors really rare, or are they simply not well recognized? It is our opinion that they are not as rare as is generally believed because the number of case reports is increasing as pathologists have become more aware of their existence.

Choanal polyp of sphenoidal origin.

Sphenochoanal polyp is a rare entity which originates in the sphenoid sinus cavity and extends into the choana via the ostium. It presents in a similar manner to the more common antrochoanal polyp. Radiological examination is necessary to differentiate between these two types. We present a case of sphenochoanal polyp and review the clinical, radiological and pathological features. The role of endoscopic sinus surgery is emphasised.

Reliability of frozen section in determining tumor thickness intraoperatively in laryngeal cancer.

OBJECTIVES: The relationship between locoregional lymph metastasis and tumor thickness in head and neck cancer has been well documented in recent years. Determining tumor thickness by frozen section may help the surgeon decide intraoperatively whether to perform elective neck dissection, whereas paraffin section results could be obtained at a later time for this decision. The aim of this study was to evaluate the accuracy of tumor thickness measurements obtained by macroscopic measurement and by frozen section intraoperatively in laryngeal cancer. STUDY DESIGN: Prospectively we compared the tumor thickness results obtained by gross visual examination, by frozen section, and by paraffin section in 20 total, near-total, and horizontal supraglottic laryngectomy specimens. METHODS: The sections were stained with hematoxylin and eosin and tumor thickness was measured under a light microscope with an ocular micrometer. RESULTS: A strong correlation was found between frozen section and paraffin section tumor thickness measurements (Pearson correlation coefficient = 0.993, P <.001). Paired t test showed a 4.59 mm mean difference between macroscopic and paraffin section measurements, and a 0.76 mm mean difference between frozen and paraffin section measurements. CONCLUSION: Assessment of tumor thickness in laryngeal cancer intraoperatively by frozen section is a reliable method.

First experience with transnasal and transseptal endoscopic and microscopic repair of anterior skull base CSF fistulae.

Currently, endoscopic endonasal surgery is a valuable and safe procedure routinely performed for treatment of paranasal sinus disorders. Since the endoscopic technique has become popular in this area, interest has been increased in its possible use for some other indications such as transsphenoidal pituitary and anterior skull base surgery. In this paper, we present our experience in using the nasal endoscope for repair of anterior skull base cerebrospinal fluid (CSF) fistulae. Between 1994-1999, we observed 44 patients with CSF rhinorrhea. Out of 44 patients, 34 had improved with the conservative treatment in two weeks, in 10 patients endoscopic surgery was performed and in one patient endoscopic surgery failed and the defect was repaired with transsphenoidal microscopic surgery. The remaining 9 patients were doing well and no problem was encountered concerning the surgery.

Pneumatization in otosclerosis.

In this study, degrees of mastoid pneumatization in otosclerotic and normal patients were compared on CT scan and plain X-rays. This study was prospective, conducted on inpatients and outpatients at a large community hospital. Patients were consecutively evaluated with no sex or age predilection. Each patient had a CT scan and a Schuller graph. Temporal bone volumetric and planimetric measurements were done respectively on CT scans and Schuller graphs. Statistically no significant variations were observed between groups. No correlation could be established between the degree of pneumatization and otosclerosis. Neither imaging technique is superior to the other when they are compared.

Genes for D-arabinitol and ribitol catabolism from Klebsiella pneumoniae.

The enzymes for catabolism of the pentitols D-arabinitol (Dal) and ribitol (Rbt) and the corresponding genes from Klebsiella pneumoniae (dal and rbt) and Escherichia coli (atl and rtl) have been used intensively in experimental evolutionary studies. Four dal and four rbt genes from the chromosome of K. pneumoniae 1033-5P14 were cloned and sequenced. These genes are clustered in two adjacent but divergently transcribed operons and separated by two convergently transcribed repressor genes, dalR and rbtR. Each operon encodes an NAD-dependent pentose dehydrogenase (dalD and rbtD), and ATP-dependent pentulose kinase (dalK and rbtK) and a pentose-specific ion symporter (dalT and rbtT). Although the biochemical reactions which they catalyse are highly similar, the enzymes showed interesting deviations. Thus, DalR (313 aa) and RbtR (270 aa) belong to different repressor families, and DalD (455 aa) and RbtD (248 aa), which are active as a monomer or as tetramers, respectively, belong to different dehydrogenase families. Of the two kinases (19.3% identity), DalK (487 aa) belongs to the subfamily of short D-xylulokinases and RbtK (D-ribulokinase; 535 aa) to the subfamily of long kinases. The repressor, dehydrogenase and kinase genes did not show extensive similarity beyond local motifs. This contrasts with the ion symporters (86.6% identity) and their genes (82.7% identity). Due to their unusually high similarity, parts of dalT and rbtT have previously been claimed erroneously to correspond to 'inverted repeats' and possible remnants of a 'metabolic transposon' comprising the dal and rbt genes. Other characteristic structures, e.g. a secondary att lambda site and chi-like sites, as well as the conservation of this gene group in E. coli C are also discussed.

Postoperative hypertension after radical neck dissection.

Postoperative hypertension after radical neck dissection was detected in 20.2% of 109 neck dissections in our department between 1989 and 1993. It was probably caused by carotid sinus denervation and appeared after the vasodilation generated by anesthesia had subsided. If postoperative hypertension was encountered after the first operation, the risk of such hypertension after surgery on the contralateral side significantly increased.

Substrate recognition domains as revealed by active hybrids between the D-arabinitol and ribitol transporters from Klebsiella pneumoniae.

Two new genes, dalT and rbtT, have been cloned from the dal operon for D-arabinitol and the rbt operon for ribitol uptake and degradation, respectively, in Klebsiella pneumoniae 1033-5P14, derivative KAY2026. Each gene codes for a specific transporter which, based on sequence data, belongs to a large family of carbohydrate transporters which constitutes 12 transmembrane helices. DalT and RbtT show an unusually high similarity (86.2% identical residues for totals of 425 and 427 amino acids, respectively). This allowed the construction of DalT'-Rbt"T and RbtT'-Dal'T crossover hybrids by using a natural restriction site overlapping Met202. This site is located within the large cytoplasmic loop which connects the putative helices 6 and 7 and in particular the amino- and the carboxy-terminal halves of the transporters. Both hybrids have close to normal transport activities but essentially the substrate specificities and kinetic properties of the amino-terminal half. This result localizes essential substrate binding and recognition sites to the amino-terminal halves of the proteins in this important class of carbohydrate transporters.

Rhinolithiasis.

Rhinoliths are mineralized masses located in the nasal cavity. In this report, 12 patients with rhinolithiasis who were operated at the 2nd ENT Clinic of Ankara Numune Hospital are presented. The most frequently seen symptom is nasal obstruction, which has been seen in 9 patients. The disease most frequently seen in association with rhinolithiasis is chronic sinusitis. All masses have been extracted intranasally.

Isolated sphenoid sinusitis.

Isolated sphenoid sinusitis is a relatively rare clinical entity and can cause severe complications. Diagnostic nasal endoscopy using Hopkins telescopes and coronal and axial paranasal-sinus CT made the diagnosis of the sphenoid sinus disease easier. Eight out of 221 patients with paranasal sinus infection refractory to medical treatment--and treated surgically at the 2nd ENT Clinic of Ankara Numune Hospital between 1990-1995--had isolated sphenoid sinus infection. The most common symptom was headache felt in the retro-orbital region. Surgical procedure was intranasal endoscopic approach to the sphenoid sinus. The symptoms of the patients with isolated sphenoid sinusitis were completely resolved after surgery. As the literature is reviewed, it is concluded that endoscopic approach to the sphenoid sinus disease is the most appropriate method of surgery in order to reduce intra-operative morbidity and mortality.

Diagnosis and treatment of lingual thyroid: a review.

Lingual thyroid is a rare developmental anomaly. It occurs because of the defective descent of thyroid tissue through the thyroglossal duct to its normal pretracheal position. In this study, two patients who presented with a mass in the oropharynx, finally diagnosed as lingual thyroid are presented, and the literature is reviewed. The masses were 3 x 2.5 x 1.5 cm and 3.5 x 3 x 3 cm in size. The diagnosis was based on the clinical features, fine needle aspiration biopsy, laboratory tests and radiographic imaging studies. The first case was treated medically with thyroxine. No treatment was given for the second case because of the patient's refusal. Both cases have not required additional therapy so far.

Endoscopic anatomic distances on the lateral nasal wall.

OBJECTIVE: Functional endoscopic sinus surgery has become popularized during the last decade all over the world. Knowledge about the intricate anatomy of the nasal cavity, the ethmoid labyrinth, and related structures is of vital importance for the endoscopic sinus surgeon to avoid probable complications. This study was undertaken to provide a set of reference distances to the endoscopic surgeons. METHOD: In this surgical anatomic study, the distances of special anatomic landmarks from the anterior nasal spine were measured during the endoscopic dissection. The procedure was performed in 50 cadavers and measurements were done by the use of straight surgical guide under endoscopic vision. CONCLUSIONS: Optimum attention should be concentrated on vital structures such as optic nerve and internal carotid artery at posterior rhinobasis after approximately 75 mm. Surgical distances of critical anatomic landmarks on the lateral nasal wall from anterior nasal spine should be kept in mind during surgical interventions.
