Arterial chemoembolization of hepatocellular carcinoma with mitomycin C microcapsules.

Chemoembolization by selective intra-arterial infusion of mitomycin C in microcapsule form exerts potential therapeutic effects through infarction and prolonged activity. Of 20 patients with inoperable hepatocellular carcinoma (HCC) treated with one or more courses via a hepatic arterial catheter, a change in tumor size could be measured in 16. Measurable tumor regression amounting to greater than 50% was shown in 6 patients (38%), 25-50% in 3 (19%), and less than or equal to 25% in 6 (38%), while tumor increased in 1. Serum alpha-fetoprotein, which was greater than 300 ng/ml before treatment in 13 patients, clearly fell after treatment in 10 (77%). The survival rate at 3, 6, and 12 months after treatment was 75%, 65%, and 24%, respectively, for all patients, and 92%, 85%, and 38% in 11 patients without a tumor cast in the portal vein or its major branches. Systemic toxicity was mild, and all patients tolerated treatment well. These results indicate that chemoembolization with mitomycin C microcapsules is a useful palliative measure for treatment of inoperable HCC.

T-cell subsets analyzed by monoclonal antibodies in patients with primary immunodeficiency diseases.

The proportion of T-cell subsets was normal in all patients with X-linked agammaglobulinemia except for an 8-month patient who showed a decrease in OKT8+ T-cell population. In two patients with hypogammaglobulinemia with IgM production, T-cell subsets were normal in distribution and the patients' B cells produced only IgM in culture with autologous T cells. A patient with common variable immunodeficiency showed no imbalance in distribution of T-cell subsets. Imbalance of regulatory T-cell subsets was found in a patient with DiGeorge syndrome and severe combined immunodeficiency.

An Ia-like antigen positive null cell leukemia cell line (THP-5) lacking in the stimulating capacity in autologous and allogeneic mixed lymphocyte reaction.

A null cell leukemia cell line, THP-5, was established from the peripheral blood of a patient with acute null cell leukemia. THP-5 cells were characterized by the presence of Ia-like antigen and the absence of other cell surface markers examined. Despite the presence of Ia like antigen, THP-5 cells had no stimulating capacity in autologous or allogeneic mixed lymphocyte reaction.

A clinical trial of live attenuated varicella vaccine (Biken) in children with malignant diseases.

The live attenuated varicella vaccine (Biken) derived from the Oka strain was used for immunization of childhood cancer patients in remission but on chemotherapy. Thirty nine patients were immunized without any severe adverse effects but 6 recipients had a small number of vesicles. Seroresponses were observed in 90% of recipients examined. During the follow-up period 6 leukemia recipients were suffered from natural varicella in family or community contacts. No vaccinee developed herpes-zoster during this study period.

[Anti-tumor activity of peripheral lymphocytes of tumor-bearing rats by in vitro culture].

This basic study was performed to determine whether the anti-tumor effect of lymphocytes obtained from the peripheral blood of a tumor bearing host can be increased when cultured in vitro with Mitomycin C (MMC)-treated AH109A tumor cells, using interleukin-2 (IL-2). Donryu rats were used as tumor bearing hosts. The following results were obtained. Weak anti-tumor activity of lymphocytes was noted 7 days after lymphocytes were cultured in the medium to which only IL-2 was added. Anti-tumor activity was augmented by adding antigen (MMC treated tumor cells) to the above (1). Anti-tumor activity was further augmented by adding to (2) above antigen presenting cells such as intraperitoneal exudate macrophages or peripheral hole leukocytes. Anti-tumor activity of lymphocytes was detected when IL-2 and MMC treated antigen were added to the peripheral hole leukocytes containing the lymphocytes.

Spontaneous partial regression of primary melanoma with death due to metastases.

A 42-year-old man had a metastatic melanoma in the brain, but no obvious primary melanoma could be detected. A halo nevus-like depigmented lesion was found on the left side of the patient's back. The histopathologic features of the depigmented lesion were compatible with those of a spontaneously regressed primary lesion of malignant melanoma. Enhanced immune responses to autologous and homologous melanoma cells were not detected when investigated in the late disease stage. Subsequently, the patient died of disseminated melanoma.

[Ultrastructural study of human breast cancer--relation to the basement membrane with invasive breast cancer].

The Japan Mammary Cancer Society's General Rules for Pathological Record prescribe differential diagnosis of the noninvasive or invasive status of human breast cancer upon discovery of basement membrane. We discovered basement membranes not only in normal mammary glands and non invasive breast cancers, but in metastatic breast cancers to the lungs and regional axillary lymph nodes. We concluded that, for such diagnosis, it is necessary to observe the cellular arrangement and condition of the surrounding stromal tissues with light microscopy. Nuclear chromatin patterns should also be examined; they showed predominantly euchromatin in invasive breast cancer and heterochromatin in non-invasive breast cancer with electron microscopy.

Resection of cancer of the thoracic esophagus without thoracotomy.

We developed a new surgical technique to manually resect the thoracic esophagus without thoracotomy and employed it in 31 patients with cancer of the esophagus who were considered poor risks for resection by traditional thoracotomy. Postoperative complications consisted of left hemothorax in seven patients, hoarseness in five, sternal osteomyelitis in three, a perforated trachea in one, and cardiorenal failure in three patients. No severe pulmonary complications occurred, and there were no hospital deaths. The long-term results of this surgical treatment have not been assessed.

Cell surface glycolipids of transformed NIH 3T3 cells transfected with DNAs of human bladder and lung carcinomas.

Neutral glycolipids and gangliosides of NIH 3T3 cells oncogenically transformed by transfection of DNAs from human lung carcinoma (Lx-1) and human bladder carcinoma (Ej) have been investigated. The chemical quantity and the degree of cell surface exposure of gangliotriaosylceramide (Gg3) were greatly enhanced in NIH 3T3 cells transformed by transfection of DNAs of either Lx-1 or Ej carcinoma cells. An identical but more conspicuous change in cell surface exposure of Gg3 was observed in BALB/c 3T3 cells transformed by murine sarcoma virus Kirsten strain, but the same glycolipid was absent in the original Lx-1 or Ej human carcinomas. The mechanism that defines the chemical quantity and the organization of glycolipids is controlled by multiple factors. These include not only the quantity but also the organization of glycosyl transferases and hydrolases in membranes. This also involves membrane dynamics regulated through a cytoskeletal-membrane conjunction which may determine the degree of glycolipid exposure at the cell surface. The similarity of the chemical and organizational change of a single glycolipid, Gg3, between 3T3 transformants by Kirsten murine sarcoma virus and those by transfection of human cancer DNAs may indicate a common biochemical basis triggered by activation of the oncogene.

Effects of intra- and extrahepatic portal systemic shunts on insulin metabolism.

To study the effects of intra- and extrahepatic portal-systemic shunts on insulin degradation, 11 patients with liver cirrhosis and 7 noncirrhotic patients with liver disease were studied with percutaneous transhepatic catheterization. Insulin levels in portal and peripheral blood were measured simultaneously for 1-2 hr after intravenous administration of glucose. The degrees of intra- and extrahepatic portal-systemic shunting were measured with this technique using 131I-macroaggregated albumin and 99mTc-macroaggregated albumin. The amount of insulin secreted and insulin degraded were assessed from the areas under blood concentration curves for portal and peripheral blood. Insulin degradation was significantly reduced in cirrhotics compared to noncirrhotics with liver disease, although there was no difference in the amount of insulin secreted between these two groups. It was also correlated significantly with the degree of intrahepatic shunting but not with the degree of extrahepatic shunting. These results suggest that intrahepatic shunting plays an important role in the reduction of insulin degradation in cirrhosis.

Short chain fatty acid in intestinal obstruction.

Short chain fatty acid (SCFA) and enteric bacteria in enteric fluid of 26 patients with non-strangulated intestinal obstruction were investigated. Sixteen to 905 mg/L of SCFA was detected in all the samples and acetic, propionic and lactic acid were prominent in the fraction. Concentration of SCFA reached a peak around the time of operation and then gradually decreased, in most cases. It also gradually decreased with clinical improvement in conservatively treated cases. Cultures of these enteric samples revealed 10(4-11)/ml of SCFA producing bacteria. SCFA may be one factor related to inhibition of water and electrolytes absorption and lead to distention of the bowel.

Extraction of retained gallstones using a fiber-choledochoscope through a PTC-drainage fistula.

After unsuccessful attempts to completely remove gallstones, endoscopic papillotomy was carried out to remove the stones after closure of the T-tube tract. We treated two patients with successful extraction of retained gallstones using a fiber-choledochoscope through a percutaneous transhepatic cholangio-drainage (PTCD) fistula. This technique can be applied in cases when endoscopic papillotomy cannot be performed or for patients who are poor risks for repeat surgery.
