[Infiltration of bile duct carcinoma along the wall].

The characteristics of the infiltration of bile duct carcinoma along the wall were studied from 25 cases with resection. The histologic mode of the spread of carcinoma was the mucosal infiltration in 1 case, mucosal and transmural infiltration in 2 cases, transmural infiltration in 22 cases. The average distance from the gross margin of the tumor to the margin of spread of growth in the bile duct as determined microscopically was 19.1 mm in the direction of the liver (the longest was 85 mm), and 5.8 mm toward the duodenum (the longest was 20 mm). The distance of the polypoid or nodular type of the gross appearance of the tumor is shorter than that of the infiltrating type. The polypoid type showed inverted U-type in obstructive or stenotic feature of bile duct radiologically and the infiltrating type showed U-type, V-type or constricted type. The distance of the tubular adenocarcinoma in histological cellular type tended to be longer than that of papillary or undifferentiated type. The severer the invasion of the carcinoma to the lymphatic vessels was, the longer the distance was.

[The clinical and pathologic correlations in acute cholangitis].

The liver histology of acute cholangitis was studied in order to determine whether microscopic changes correspond to clinical status. Patients with acute cholangitis were divided into mild cholangitis (1st group, n = 18) and severe cholangitis (2nd, 3rd and 4th groups) accompanied by hypotension. The 2nd group consisted of 6 surviving patients who underwent liver biopsy during hypotension. The 3rd group consisted of 3 non-surviving patients with biopsy during hypotension. The 4th group consisted of 7 patients with biopsy during autopsy. The frequency of endotoxemia, gram negative bacteremia, disseminated intravascular coagulopathy and hepatic failure were significantly higher in severe cholangitis than in mild cholangitis. The incidence of neutrophilic infiltration in the sinusoid and microabscess in lobule were significantly higher in severe cholangitis than in mild one. The incidence of portal thrombosis and massive necrosis of hepatic cells were also significantly higher in severe cholangitis, especially in the 3rd and 4th groups. It was suggested that the neutrophilic infiltration in the sinusoid and microabscess in lobules which proved the existence of endotoxemia or bacteremia were characteristic findings in severe cholangitis.

Cardio-respiratory effects of paraquat with and without emetics on Wistar rats.

We investigated the effects of paraquat products both containing and devoid of emetics on the cardio-respiratory functions of Wister rats. Anesthetized rats were administered intravenously 0.46 ml/kg Gramoxone, a commercial product containing 24% paraquat and an emetic. Paraquat equivalence equalled 110 mg/kg body weight. ECG and respiratory curves were recorded with a polygraph. Fifteen seconds after injection, A-V block and bradycardia occurred and the heart stopped beating soon after. Though the heart was severely affected, the respiratory curve differed very little from pre-injection values. Administration of pure, 24% paraquat solution (110 mg/kg body weight) inhibited respiration and almost no change was recorded by the ECG. Administration of Paragreen A, a commercial product containing 24% paraquat with no emetic, produced slight change in the ECG but respiration soon became strained and stopped. In accordance with this data, we propose that the emetic contained in Gramoxone is primarily responsible for the cardiovascular failure.

[Management of Crohn's disease].

Our therapeutic management to Crohn's disease was analyzed retrospectively. Among 17 cases with Crohn's disease, 14 (82%) were operated. Operative indications were due to obstruction (7), perforation (2), fistula (2), stenosis (2) and intractability (1). Postoperative recurrence was developed in 8 out of 14 (57%) and 4 were reoperated. Most of recurrent lesions were around the previous anastomosis. Operation per se does not cure the Crohn's disease but operation should be indicated when complicated obstruction, fistula and perforation. Nutritional treatment has been also performed 8 times. Crohn's disease activity index (CDAI) was decreased in all but one. In individual item of CDAI, frequency of bowel movement, abdominal pain and general status were improved but items such as anal lesion, fistula and fever were not improved. CDAI values were turned to remission value in all of cases with complicated stenotic lesions, but it remained in active stage in all cases with complicated fistula, obstruction and abscess. Nutritional treatment seemed less effective in cases complicated fistula, obstruction and abscess.

New fluorescence of nonenzymatically glucosylated human serum albumin.

Glucosylated human serum albumin (G-HSA) obtained under incubation with glucose at 37 degrees C for 8 days showed a new fluorescence with a maximum at 430 nm, resulting in quenching of the fluorescence of only one tryptophan residue on HSA. The quantum yield of new fluorescence is 0.024 at 25 degrees C. The analysis of the excitation spectra allowed us to conclude the absence of energy transfer. In G-HSA, non-disulfide cross-linking hexamer was confirmed by SDS-PAGE.

Nonenzymatic glucosylation of human serum albumin and its influence on binding capacity of sulfonylureas.

To estimate the functional change occurring in human serum albumin by nonenzymatic glucosylation, glucosylated human serum albumin was prepared by in vitro incubation with glucose. The rate of glucosylation proceeded as a first-order reaction. The binding of sulfonylureas to serum albumin was determined by equilibrium gel filtration. Through this method, it was possible to estimate the binding capacity of a low water solubility drug with a high affinity to protein. The amounts of the sulfonylureas bound to glucosylated HSA decreased by 44% with tolazamide and acetohexamide, 50% with glibenclamide, and 52% with tolbutamide, compared to human serum albumin (HSA). This suggests that a high concentration of glucosylated HSA in diabetic patients may possibly cause an increase in free drug concentration exceeding normal levels. This study shows that the decrease in the binding capacity of sulfonylureas with protein is due to the modification of albumin molecules by the covalent binding of glucose.

Characterization of microenvironments of 2-(4'-hydroxyphenylazo)benzoic acid bound to bovine serum albumin by studying the solvent effects.

The binding and thermodynamic parameters of the two binding forms of 2-(4'-hydroxyphenylazo)benzoic acid to bovine serum albumin were estimated. The number of the binding sites of the azo form was 3.2 and that of the hydrazone form was 0.7. The binding of the hydrazone form was affected only by enthalpy changes, in contrast to that of the azo form which was affected by both enthalpy and entropy changes. To estimate the complex microenvironments of 2-(4'-hydroxyphenylazo)benzoic acid molecules on bovine serum albumin, the solvent effects were studied. The 2-carboxyl group of 2-(4'-hydroxyphenylazo)benzoic acid participates in the azo-hydrazone tautomerism . The 2-carboxylate ion forms an ion pair with triethylamine in ethylene dichloride, chloroform, and benzene, resulting in the appearance of the hydrazone form. The hydrazone formation in the system of 2-(4'-hydroxyphenylazo)benzoic acid-triethylamine (1:1) in chloroform was affected only by enthalpy changes, in the same manner as in the system of 2-(4'-hydroxyphenylazo)benzoic acid-bovine serum albumin. We speculate the presence of the two kinds of ion pairs on the basis of the changes of the azo-hydrazone tautomerism in chloroform, and that the azo form takes the contact ion pair and the hydrazone form takes the solvent-separated ion pair. A new possible model for the interaction of the azo and hydrazone forms and bovine serum albumin is proposed.

Adenocarcinoma of the sublingual gland: case report and 50 year review of the literature.

Primary adenocarcinoma arising in the sublingual gland is extremely rara, accounting for six cases reported in extensive survey of the literature for the period of 1930 to 1981. A case of adenocarcinoma of the sublingual gland is documented, demonstrating a highly malignant nature of the disease. This neoplasm possesses particular therapeutic problems owing to the relative lack of symptoms and the delay of initial medical observation after the onset of symptoms.

Pathogenesis and clinical features of acute cholangitis accompanied by shock.

We investigated 91 cases of acute cholangitis, including 42 of severe cholangitis and 49 of mild cholangitis. The incidence of endotoxemia was 78.6 percent in 42 and 32.6 percent in the 49 patients. In the 42 with severe cholangitis, remarkable leukocytosis, thrombocytopenia, decrease of serum CH50, C3, plasma fibronectin and phagocytic index were characteristic. Disseminated intravascular coagulation (DIC) was observed in 76.2 percent. Since there was a positive correlation between platelet counts and levels of CH50 and C3, the decrease of platelet count, and the occurrence of DIC in patients with endotoxemia were thought to be closely related to the consumption of complements. There was no difference in mortality rate between nonsurgical treatment (57.8 percent) and the emergency bile drainage treatment (56.5 percent). The results of therapy depended on the degree of complicating DIC. We conclude that acute cholangitis was aggravated by endotoxemia and that severe cholangitis was accompanied by DIC induced by a decline in phylaxis.

Bile acid metabolism in patients with non-strangulated intestinal obstruction.

The intraluminal changes of bile acids were studied in 42 patients with intestinal obstruction, treated from 1978-1982. Twenty-two patients received surgical treatment and twenty were treated conservatively. Bile acid analysis of the intestinal contents was performed by thin-layer and gas-liquid chromatography. A marked reduction of bile acid concentration was observed before treatment. A decreased percentage of deoxycholic acid, a lowered G : T ratio, and presence of deconjugated bile acids were also present. After relief and/or removal of the obstruction, the concentration of bile acids, the percentage of deoxycholic acid, and the G : T ratio increased. Bile acid deconjugation decreased immediately after surgical treatment but was not affected by conservative treatment. We conclude that bile acid metabolism is altered quantitatively and qualitatively in patients with an intestinal obstruction.
