Superoxide production from nonenzymatically glycated protein.

Nonenzymatically glycated human serum albumin and glycated poly-lysine(Lys) in vitro brought about the reduction of nitroblue tetrazolium and ferricytochrome c at pH 9.06 and pH 7.8, respectively. This reduction was inhibited partially by superoxide dismutase (SOD). Glycated poly-Lys caused the oxidation of NADH in the presence of LDH at pH 7.0 which was completely inhibited by SOD. Glycated material was found to function both as a reductant and an oxidant. The reactivity of glycated material is discussed and a possible mechanism by which superoxide is produced is proposed. Results may give a clue to diabetic complications.

TaqI polymorphism in the LDL receptor gene and a TaqI 1.5-kb band associated with familial hypercholesterolemia.

The low density lipoprotein (LDL) receptor gene was analyzed in 67 unrelated healthy Japanese and 38 members of six consecutive families with familial hypercholesterolemia (FH) by Southern blot hybridization with TaqI, an LDL receptor cDNA fragment containing exons 1 to 8 being used as a probe. A new TaqI RFLP at the LDL receptor locus was detected with allele frequencies of 0.67 and 0.33. The data obtained with smaller cDNA subfragment probes revealed that the TaqI RFLP site is located within 1.1 kb of the 5' side of the EcoRI site of exon 5. The TaqI RFLP was in linkage disequilibrium with the PstI RFLP but showed no significant linkage disequilibrium with the RFLPs for AvaII, ApaLI/I15, PvuII, NcoI, and ApaLI/3'. Among the seven RFLPs at the LDL receptor locus, the TaqI RFLP was the only useful genetic marker in one of the six families with FH. Furthermore, the association of an additional TaqI 1.5-kb band with a mutant LDL receptor gene was observed in another family with FH in which the proband was homozygous for all of the seven RFLPs. The data obtained with various restriction enzymes and smaller cDNA subfragments probes suggested that a minor change in nucleotide sequences in the region including exons 5 to 8 is present in the mutant gene. These data suggest that the TaqI RFLP is a useful genetic marker at the LDL receptor locus and that TaqI serves for the analysis of some mutant LDL receptor genes, when used with small LDL receptor cDNA probes.

Prevention of uric acid stone formation by sodium bicarbonate in an ileostomy patient--a case report.

A 38 year old female underwent a proctocolectomy and ileostomy for ulcerative colitis in February, 1974. For 8 year post-operatively, she excreted innumerable renal stones, mainly composed of uric acid. Her urine was highly acidic and hyperuricosuric with a low concentration of sodium. Sodium bicarbonate 4 gm/day, t.i.d., was started in October 1985, after which her renal stone excretion completely ceased (up until March, 1987), except for one incidence of stone excretion when she discontinued therapy for a week. During the sodium bicarbonate therapy, her urinary pH and Na concentration were elevated. Furthermore, sodium bicarbonate significantly elevated the urinary pH and Na concentration of other ileostomy patients. Thus, sodium bicarbonate could be used for the possible prophylaxis of uric acid formation in selected ileostomy patients.

Characterization of a precursor T-cell line (THP-6) with rearranged T-cell receptor beta chain gene.

A previously established human leukemia cell line, designated THP-6, was further characterized with respect to cell surface antigen expression and immunoglobulin(Ig) and T-cell receptor(TCR) gene status. THP-6 cells were positive for CD7 and CD5 antigens and terminal deoxynucleotidyl transferase, but negative for CD2, CD1, CD4, CD8, CD10, cytoplasmic and surface CD3 and HLA-DR antigens, suggesting a precursor T-cell line. Analysis of Ig and TCR beta chain genes revealed that THP-6 had a rearranged TCR beta chain gene and a germline Ig gene. These results, in agreement with its phenotype, confirmed that THP-6 was of the T-cell lineage.

Gastric carcinoma metastatic to the breast diagnosed by mucosubstance histochemistry and electron microscopy.

Bilateral breast tumors with a histologically scirrhous pattern were conclusively diagnosed as gastric carcinoma metastatic to the breast using mucosubstance histochemistry and electron microscopy. The majority of the carcinoma cells gave histochemically positive reactions for galactose oxidase-Schiff, stable class III con A, and high-iron diamine-alcian blue (HID-AB). Also electron microscopically, numerous HID-positive mucus droplets (sulfomucin) were seen in the carcinoma cells. These findings invalidated the possibility that the carcinomas were primary breast cancer. The practical applications of distinctive patterns of mucus secretion are discussed.

Chronic granulomatous disease with neutrophil membrane cytochrome b deficiency: demonstration by immunochemical staining with monoclonal antibody.

Cytochrome b deficiency in the peripheral granulocytes of two male patients with chronic granulomatous disease was demonstrated by an immunocytochemical assay using a monoclonal antibody, 7D5, against human neutrophil cytochrome b. A mosaic of cytochrome b positive and negative neutrophils, indicating a carrier state in an X-linked trait, was found in the mother of patient 1 but not in the mother of patient 2.

[A case of intrahepatic bile duct cancer responding to 5-fluorouracil, adriamycin and mitomycin C chemotherapy].

A 64-year-old woman who was diagnosed as inoperable intrahepatic bile duct cancer with massive liver metastasis was treated with 5-fluorouracil (5-FU), adriamycin (ADM) and mitomycin-C (MMC). Obstructive jaundice was reduced and performance status was soon improved by this regimen. The reduction rate of the tumor was noted to be more than 50% on CT images, so this patient was considered to have achieved a partial response. In histological examination, well differentiated tubular adenocarcinoma cells were degenerated and replaced fibrous tissue. Thus, it was considered that the reduction of the tumor was caused by these phenomena. This case suggested the activity of 5-FU, ADM and MMC combination for bile duct cancer.

[Early detection and differential diagnosis of metastatic lung tumor].

In general, the prognosis of cases with metastatic pulmonary tumor is extremely poor. However, in cases where primary lesion is well controlled and no distant metastasis except the lung is clinically found, the pulmonary metastasis could be curable through the early detection of metastasis followed by radical surgery equally to the case with primary lung tumor. The candidate of this curable metastasis is the case with of nodular metastasis with a small number. I discuss the X-ray figure, differential diagnosis, diagnostic accuracy of the number of metastasis, and the interval of follow-up care after treatment of primary malignancy.

[Pathophysiology of bone disorder after total gastrectomy].

UNLABELLED: Sixty two total gastrectomized patients were studied in serum biochemistry, bone microdensitometry, Calcium infusion test and clinical symptoms related to bone disorder. Thirty total gastrectomized Wistar rats were studied in food-intake amount, serum biochemistry, bone histological changes and intestinal absorption of 45Ca. I. Clinical: 63% of the patients showed bone abnormality by microdensitometry, and 52% showed hypocalcemia and 47% showed high A1-P. Urinary excretion of calcium in bone disordered patients was below 28% by calcium infusion test, which suggested that the change of bone seemed to be osteomalacia. II. EXPERIMENT: The longer of time after gastrectomy, the thinner of cortex and lesser of trabeculation was observed in femoral bone of rats. Food-intake, serum calcium and vitamin D levels decreased, but on the other hand, the lipid content in feces increased, and disturbance of intestinal 45Ca absorption was marked in total gastrectomized rats. CONCLUSIONS: 1. After total gastrectomy, bone disorder was observed in 63% of patients and in all of rats. 2. The longer after operation, the worse of the bone change. 3. It resembled osteomalacia rather than osteoporosis. 4. Vitamin D and calcium malabsorption due to low food intake and fatty diarrhea may be the major etiologies.

[Distribution of recurrent lesions after radical resection of cancer of the thoracic esophagus].

In 150 patients who got cancer recurrence after curative resection for cancer of the thoracic esophagus, the sites where recurrent lesions were clinically detected for the first time were examined. The distribution of recurrent lesions in patients who did not undergo neck dissection at the operation (group A) differed from the distribution in those who underwent neck dissection (group B). Cervical and/or upper mediastinal recurrence occurred in 49% of cases in group A and in 11% of group B. On the contrary, middle or lower mediastinal recurrence was more often in group B. The distribution of recurrent lesions varied depending on the state of lymph node metastasis detected at surgery. Cervical and upper mediastinal recurrence was much more frequent than hematogenic recurrence in cases without lymph node involvement in group A, while hematogenic recurrence was more frequent in cases with both mediastinal and abdominal lymph node metastasis. In the upper mediastinum, recurrence along the recurrent laryngeal nerves was most frequent and it was supposed to have developed from residual lymphatic metastases. In the middle and lower mediastinum, recurrent lesions were located around the left main bronchus and descending aorta, and cancer infiltration of the neighboring organs was frequent. Recurrence at the abdominal paraaortic nodes was observed mainly in cases with perigastric lymph node involvement.

[Treatment of cancer of the esophago-gastric junction with special reference to dissection of the lymphatic system].

No means have yet been devised for detecting cancer of the esophago-gastric junction at an early stage, and no effective chemotherapy or irradiation regimen for this area is yet available. Recently, a very important lymphatic pathway and extension route for E. G-junction cancer has been discovered. A new operative procedure involving a thoraco-abdominal approach, wide dissection of the lymphatic system and safe reconstruction is discussed in this paper.

[Surgical treatment of lung cancer for both preserving lung function and maintaining radicality--bronchoplastic surgery].

Fifty cases with lung cancer given bronchoplastic surgery were discussed. Five-year survival rates of all of the cases, those with squamous cell carcinoma, pathological stage I + II cases and absolute curative cases were 36%, 42%, 46% and 59%, respectively, excluding operation death and hospital death. These results were virtually comparable with those of all of the cases with lung cancer which were ever resected in our hospital. The prognosis for cases with adenocarcinoma and of cases operated with bronchoplasty was poor because of direct bronchial invasion from inter-lobar lymph-node metastasis. Hence, this procedure could be better indicated for cases with squamous cell carcinoma, direct primary tumor invasion of the bronchus, and these in which absolute curative operation could be expected clinically. As is characteristic, suture failure occurred in 3 cases, and 2 of them which were followed by pulmonary arterial fistula died of massive bleeding. Preoperative BAI (Bronchial Arterial Infusion had been performed in all 3 cases. It is suggested that there may will be some relation between BAI and suture failure. From the viewpoint of technique, both blood supply preserving and tension relaxing procedures are the most important to avoid suture failure.
