A new synchronous video urodynamics.

A new synchronous video urodynamics to evaluate the function of the lower urinary tract is described. The urodynamic study together with the transrectal longitudinal ultrasonotomography of the bladder and urethra can be seen on the colour display monitor through the display controller. An image on the colour display monitor is also stored on the video cassette recorder. This new technique will be indispensable for evaluation of the lower urinary tract dysfunction.

A new modified catheter for voiding cystourethrography.

We made a new modified catheter for voiding cystourethrography to evaluate the function of the external urethral sphincter. Using this catheter, we can evaluate it precisely, excluding the influences of the posterior urethra. The findings of voiding cystourethrography observed by using this catheter were equal to the findings of the simultaneous recording of cystourethrogram and external urethral sphincter electromyogram, and permitted us to assess the function of each portion of the urethra separately, such as the sphincteric urethra (prostatic urethra) and external urethral sphincter (membranous urethra). In addition, this urodynamic procedure can be performed safely and easily.

Immunological characterization of gamma-glutamyltransferases in human serum.

The immunological properties of gamma-glutamyltransferases (gamma-GTs) from human serum, liver and tonsil were studied by using a monospecific antibody to human kidney gamma-GT for the purpose of elucidating their isozymic relationships. gamma-GTs partially purified from liver and tonsil were indistinguishable in this respect from kidney gamma-GT. gamma-GT in sera from patients with hepato-biliary diseases, on the other hand, was heterogeneous in molecular size as revealed by sucrose density gradient centrifugation and Sephadex G-150 gel filtration, and was inhibited and precipitated by the above antibody relatively poorly as compared with the kidney enzyme. When these sera were treated with bromelain, however, the molecular size of gamma-GT was reduced and the enzyme now reacted with the antibody as strongly as kidney gamma-GT. gamma-GT from bromelain-treated sera also exhibited a single immunoprecipitin line smoothly fusible with that from kidney gamma-GT; the enzyme-antibody complex still exhibited gamma-GT activity. The major form of gamma-GT partially purified from papain-treated sera, even though indistinguishable from kidney gamma-GT immunologically and in molecular size, exhibited a mobility on polyacrylamide gel electrophoresis which was higher than that of kidney gamma-GT but similar to that of liver gamma-GT. It is suggested that gamma-GT in human sera is heterogeneous in molecular size and electric charge but is composed of common peptide chains, probably identical to those of kidney gamma-GT.

Initiation and propagation of canine renal pelvic peristalsis.

A new in vitro method was employed which enabled detailed examination of the initiation and propagation of the peristaltic contraction of the renal pelvis and ureter. Both the kidney and its attached ureter were carefully removed and the renal parenchyma concealing the renal pelvis and calyces removed under the dissection microscope. At the pelvicalyceal border, ripple-like peristaltic contraction of a constant frequency were observed. At the same time, electromyograms with a slow rising phase were recorded at constant intervals from the pelvicalyceal border, leading us to deduce that these might represent pacemaker potentials. The discharge interval of electromyograms recorded at both the center of the pelvis and the pelviureteric junction were found to be a multiple of the 'pacemaker' interval. Electrical activity (peristalsis) arising from the pacemaker might be blocked at only the center of the pelvis but also at the pelviureteric junction.

Regional arterial infusion of an anticancer drug combined with direct hemoperfusion.

The capacity of activated charcoal to adsorb anticancer drugs, Mitomycin C, Adriamycin and Bleomycin was examined in vitro in a closed circuit through a perfuser containing uncoated activated charcoal. Samples were obtained from the inlet and outlet of the perfuser every 30 min. As in vivo experiments, regional arterial infusion of anticancer drugs combined with direct hemoperfusion was performed on dogs. In 20 male mongrel dogs a polyethylene catheter was indwelt in the internal iliac artery and anticancer drugs were infused through the catheter for 1 hr. By using the femoral vessels for the blood access, direct hemoperfusion was performed for 3 hr. The adsorptive capacity of activated charcoal was high in both in vitro and in vivo experiments. These results indicate that regional arterial infusion of anticancer drugs combined with direct hemoperfusion would be useful for preventing their toxicity.

Changes in lymphocytes as an early sign of relapse in adult acute leukemia.

This study was undertaken to find out some clue to detect early relapse of adult acute leukemia which had been in remission. Laboratory data such as LDH, erythrocyte sedimentation, or immunoglobulin level did not show any difference at the time of relapse from the remission period. Complete blood count with reticulocyte count was evaluated every two weeks retrospectively for 12 weeks before relapse. No significant change was observed at the time of relapse. However, both the percentage and absolute number of peripheral lymphocytes significantly increased, whereas percentages of both T and B cells markedly decreased at the time of relapse, suggesting some derangement of lymphocyte function. Results in bone marrow culture did not yield any remarkable difference. It was suggested from this study that it is the time of prerelapse of adult acute leukemia when lymphocytes show an increase in either percentage or absolute number in the patients who are followed under their maintenance therapy schedule.

Enzyme deviation patterns in primary rat hepatomas induced by sequential administration of two chemically different carcinogens.

Enzyme deviation patterns were examined in primary rat hepatomas induced by short-term sequential administration of two chemical carcinogens from among 2-fluorenylacetamide (FAA), diethylnitrosamine (DENA), and 3'-methyl-4-dimethylaminoazobenzene (3'-Me-DAB) or by FAA or 3'-Me-DAB followed by phenobarbital as a promoter. The purpose was to discern how the patterns are influenced by different administration schedules of carcinogens and which of the two carcinogens in the sequence affects the pattern more. Biochemical differentiation of hyperplastic hepatic nodules and hepatomas was determined by simultaneous assays of activities and isozyme composition of glucose-adenosine triphosphate phosphotransferase, pyruvate kinase, glucose-6-phosphatase, fructose-1,6-bisphosphatase, and gamma-glutamyltransferase with consideration of histological classification of nodules and tumors. Poorly differentiated hepatomas were predominantly induced by 3'-Me-DAB followed by FAA or DENA except for hepatomas induced by 3'-Me-DAB followed by phenobarbital, which were mainly well and moderately differentiated; well and moderately differentiated hepatomas were predominantly induced by FAA followed by 3'-Me-DAB or phenobarbital. The degree of enzyme deviation of the hepatomas induced by DENA as the first carcinogen was intermediate between those of hepatomas induced by FAA or 3'-Me-DAB, although the degree tended to increase with increased dose or term of DENA. These results indicate that deviations of some enzymes, such as pyruvate kinase and fructose-1,6-bisphosphatase, as well as histological differentiation of the primary hepatomas are more strongly influenced by the first carcinogen than by the second under our administration schedules and that the degree of enzyme deviation shown by hepatomas produced by a particular carcinogen treatment regimen principally related to the potential of that regimen to induce the more anaplastic tumors.

Initiation and propagation of stimulus from the renal pelvic pacemaker in pig kidney.

A new in vitro method was used to examine the initiation and the propagation of the peristaltic contraction in the renal pelvis and ureter of the pig. The method enabled the direct observation of peristalsis, both microscopically and electrophysiologically. A microscopic ripplelike contraction of a constant frequency was generated spontaneously at the border between the upper, middle, and lower major calyces and their minor calyces. At the same time, electromyograms were recorded in the same regions, with a constant discharge interval that suggested to us that these might be pacemaker potentials. The waves originating from the upper calyx, middle calyx, and lower calyx propagated at different times toward the ureter. Pressure changes of an almost constant interval were recorded in each calyx and corresponded in a 1:1 ratio to the action potentials at the same calyx. Pelvic pressure changes reflected the electrical activity of the renal pelvis, but did not reflect the pacemaker activity of the renal calyx.U

Renin release and renal pelvic pressure in the isolated perfused dog kidney.

The perfused dog kidney was used for a study of the relationship between renin secretion and renal pelvic pressure. The kidney was completely isolated and perfused with perfluorochemical emulsion. During the elevation of renal pelvic pressure, renin secretion rate increased from 0.93 +/- 0.43 to 5.54 +/- 1.57 ng/g min, perfusion flow rate decreased temporarily and recovered within a few minutes.

[Clinical effect of KW-1062 on chronic complicated urinary tract infection].

KW-1062 was administered intramuscularly at a daily dose of 360 mg for 5 days to 50 patients suffering from chronic complicated urinary tract infections. The following results were obtained. 1. Clinical response was excellent in 3 cases (6%), moderate in 23 (46%) and poor in 24 (48%); the overall effectiveness rate was 52%. 2. In bacteriological results, 58 (75.3%) out of 77 strains were eradicated and 19 strains (24.7%) persisted. P. aeruginosa was eradicated in 9 (52.9%) of 17 strains and Serratia in 10 (83.3%) of 12 strains. 3. The MIC levels of KW-1062 were slightly lower than those of gentamicin (GM) and almost the same as those of dibekacin (DKB). But against clinically isolated Serratia, KW-1062 was found to be more active than DKB. 4. As to side effects, this series showed no evidence of hepatic and renal dysfunction and hearing disturbance except a case which showed elevated GOT and GPT.
