Biliopancreatic bypass for obesity entails a 2/3 distal gastrectomy with Roux-en-Y reconstruction, small bowel being transected at its midpoint and the enteroenterostomy placed 50 cm proximal to the ileocecal valve. Plasma fasting and meal-stimulated gastrin concentrations were measured by radioimmunoassay in 13 nonobese healthy volunteers, 13 pre-surgery obese patients, 11 patients within two months, 12 patients between four and twelve months and 7 patients between fifteen and twenty months after operation. There were no significant differences in fasting and meal-stimulated peak plasma gastrin levels between obese group and the control group and between any of the postoperative groups and the preoperative group. The 15-20 month post-surgery group mean fasting value was lower than in the other groups, whilst the mean peak value was unchanged. Integrated gastrin response was significantly higher in this group than in the preoperative group.

The effects of a muscle twitch on the tremor of the extensor indicis muscle and of the tibial anterior muscle have been recorded in 14 patients with essential tremor and in 10 patients with Parkinson's disease. The muscle twitch evoked by the electrical stimulation of the motor nerve inhibits the tremor and synchronises it. The mean duration of the inhibition is 92.1 +/- 6.8 ms for essential tremor and 183.0 +/- 16.8 ms for Parkinsonian tremor. This inhibitory phase lasts longer when the muscle twitch is induced during the second half of the Parkinsonian tremor cycle. The effects of voluntary contraction and of unloading suggest that inhibition and resetting of the tremor can be attributed to the autogenic mechanism induced by Ib fibres discharges. The presence of a rhythmic inhibition in the cycle of Parkinsonian tremor accounts for the longer duration of the inhibitory phase. In practice, these techniques aid the diagnosis of tremor in these two conditions, for example they assist the identification of low frequency essential tremor and of postural tremor in Parkinson's disease.
Antimicrobial susceptibility determinations for three commonly used and seven seldom used antibacterial drugs against 482 strains of shigellae isolated in Ontario during 1977 and 1978 were carried out. Resistance to the first-line treatment drugs, ampicillin and tetracycline, occurred in 20.5 and 39.2% of strains, respectively. The emergence of trimethoprim-sulfamethoxazole resistance was noted for the first time in 3.2% of the strains in 1978. All strains retained their susceptibility to gentamicin, nalidixic acid, and polymyxin but were variably susceptible to chloramphenicol, cephalexin, and rifampin.

Data pertaining to 1,219 cultures of Yersinia enterocolitica isolated in Canada are summarized. Of the 977 cultures isolated from humans, Y. enterocolitica serotype 0:3, biotype 4, phage type IXb was the most predominant type in Ontario, Quebec and the four Eastern Provinces. In the Western Provinces the predominant strains were indol-positive, serotypes 0:8; 5,27 and 4,32. Most of the 242 cultures of nonhuman origin, except those isolated from swine and a few isolated from wild animals, were indol-positive, biochemically atypical and serologically nontypable or belonging to different serotypes which have seldom been found in human infections.
A study was done to determine the major antigenic factors of Bordetella pertussis strains isolated throughout Canada and whether these isolates have the same antigenic structure as the bacilli in the currently used vaccines. Testing for the major pertussis antigens, factors 1, 2 and 3, was conducted with 440 freshly isolated strains of B. pertussis received from seven canadian provinces between August 1976 and February 1978 and six batches of pertussis vaccine or immunizing agents containing pertussis vaccine. With the aid of specific antisera prepared in rabbits, the antigenic factors were detected by a slide agglutination technique. Almost all (98.9%) of the pertussis strains examined were serotype 1,3. All six batches of pertussis vaccine or immunizing agents containing pertussis vaccine proved to be rich in each of the three main pertussis agglutinogens.

Four enrichment procedures were used for examining 131 raw milk samples for the presence of Yersinia enterocolitica. Forty-two isolations were obtained from 19 pooled- (31.1 percent positive) and 10 individual-producer samples (14.3 percent positive). Enrichment by Butterfields phosphate buffer incubated at 4 degrees C for 14 days and then inoculation of modified Rappaport broth incubated at 23 degrees C for 5 days produced the greatest number of isolations. The majority of isolates were biotype 1, and many were atypical from clinical isolates in being rhamnose positive (47.6 percent), citrate positive (16.7 percent), and lactose positive (26.2 percent). Thirteen isolates were serotypable, belonging to seven different O serotypes, with O:5 occurring most frequently.
A total of 414 cultures of Yersinia enterocolitica isolated from human and nonhuman sources were examined for heat-stable enterotoxin (ST) production to determine whether enterotoxigenicity was related to the source of isolation, serotype, or biochemical characteristics of the organism. A total of 65% of all cultures were found to produce ST. Enterotoxin production was much more prevalent in strains isolated from humans (218/232) than in those isolated from animals (17/34), water (9/49), raw milk (14/44), and food (10/55). Strains belonging to the serotypes O:3; 8; 5,27; 6,30; 9, often isolated from human infections, were almost always enterotoxigenic (191/196), although ST production was also highly prevalent among a few other serotypes. The most significant difference was observed between the groups that differed in the ability to ferment rhamnose; only 13 of 130 rhamnose-positive isolates produced ST (10%) compared to 255 of 284 rhamnose-negative cultures (90%). These results suggest that ST production is ubiquitous in Y. enterocolitica, with the highest prevalence among strains associated with human infections.
A total of 989 Neisseria gonorrhoeae cultures were examined for acid production from dextrose in seven different carbohydrate media. Four of these media were found to be quite accurate for this purpose. Some pitfalls in the preparation and usage of cystine trypticase agar carbohydrate media are discussed. The term 'fermentation' of carbohydrates by Neisseria species is inaccurate and responsible for some N. gonorrhoeae false-dextrose-negative results.

Vibrio cholerae, biotype El Tor, was isolated in a hospital laboratory in Kingston, Ont. in 1974. Confirmation and complete identification by the Ontario regional and provincial public health laboratories was obtained within 3 days. Institution of well established infection-control and public health measures prevented spread of the infection within the hospital and the community.
