Squamous cell carcinoma associated with disturbance of the IgA system.

A case of cell carcinoma of the skin associated with disturbance of the IgA system is described. Immunological observations revealed a marked low level of serum IgA and disturbance of the secretory IgA synthesis in the rectal mucosa. After the surgical removal of the skin tumor, the serum level of IgA was increased to half the normal-range level. The relationship of the disturbance of the IgA system to the occurrence of squamous cell carcinoma is discussed.

"Two route chemotherapy" using cis-diamminedichloro-platinum(II) and its antidote, sodium thiosulfate, for peritoneally disseminated cancer in rats.

The effects of "two route chemotherapy" using cis-diamminedichloroplatinum-(II) (DDP) and its antidote sodium thiosulfate (STS), given ip and sc, respectively, were assessed in the case of peritoneally disseminated cancer in rats. Administration of STS sc to mice 1 min before DDP ip was found to be the most effective in protecting the animals against the lethal toxicity of DDP. The LD50 values of DDP ip with and without STS were about 55 mg/kg and 14 mg/kg, respectively. This "two route chemotherapy" using DDP and STS was significantly superior in therapeutic efficacy to single treatment with DDP.

[Thin-section CT scan of the pituitary gland (author's transl)].

Topographic anatomy of the pituitary fossa was studied by 2 mm thin-section CT scan (Somatom II). Nineteen with normal pituitary (control group) and 20 with suspected pituitary abnormality were selected. Plain and contrast CT were performed in all cases. Contrast CT was carried out immediately after the rapid infusion of 220 ml of 30% iodinated contrast medium. In all of control group but two, pituitary gland was detected as homogenous density and its density was as same as density of normal brain tissues, and was enhanced in degree of about 25 CT number. In 2 cases, small low density was visualized in the pituitary gland. Pituitary gland was differentiated from cavernous sinus was usually higher than the pituitary gland. In abnormal group, the microadenoma of the pituitary gland was diagnosed in 5 cases and 3 out of 5 cases was proved by surgery. All 3 microadenomas proved slightly dense by plain CT and enhanced higher than normal pituitary gland by contrast CT. Polytomograms showed no abnormality of the sella turcica in one of these 3 cases. Although 3 microadenomas were detected by the abnormal enhancement, we are not sure whether all of microadenoma can be detected by CT alone. Arachnoid herniation into the pituitary fossa was diagnosed in 7 of control group and 2 of abnormal group. Four out of these 9 cases were verified by using Metrizamide CT. By plain thin-section CT, the diagnosis of arachnoid herniation seems to be possible without Metrizamide CT.

[Giant cell tumor of the temporal bone--case report (author's transl)].

The majority of giant cell tumors occurs in the long bones, especially around the knee, and only 1.4-1.8% of cases are found in the skull. In this communication, a case of giant cell tumor of the temporal bone is reported because of a rarity of the lesion in the skull. The 37-year-old housewife was admitted to our clinic complaining of swelling in right temporal region which had gradually developed over a two-month period. On examination, there was a swelling about 5cm in diameter beneath right temporal muscle; its surface was smooth and its consistency was hard. Tenderness was found in its center. Routine x-ray of the skull showed osteolytic lesion in right temporal bone with relatively clear margins. Computed tomography demonstrated heterogeneously high-density mass in right temporal bone and right middle fossa. The tumor located mainly within the temporal bone at the middle fossa, and intracranial epidural space at the convexity. After bolus injection of contrast material, the tumor of temporal bone and the peripheral portion of the intracranial tumor were enhanced markedly. Right external carotid angiograms disclosed tumor stain in capillary and venous phases. Abnormal RI uptake was disclosed by bone scintigrams with 99mTC-methylene diphosphonic acid. Her preoperative diagnosis was calvarial tumor with intracranial extension. A right fronto-temporal osteoplastic craniotomy was performed. The tumor located within the temporal bone and in epidural space which invaded temporal muscle and underlying dura. The tumor was elastic soft and vascular with central necrosis. The tumor was extirpated totally including surrounding temporal muscle and dura. Microscopic examination of the operative specimen revealed giant cell tumor of the skull. Therefore, she was treated with 60CO irradiation (3,000 rads) to the temporal region. In the 13 months since the completion of treatment, there has been no sign of recurrence nor metastasis. Giant cell tumor of the skull were reviewed with special references to diagnosis and treatment. Computed tomographic findings of this tumor were also discussed briefly.

Prenatal diagnosis of Hurler's syndrome--biochemical studies on the affected fetus.

A prenatal diagnosis of Hurler's syndrome was made in a pregnancy at risk in a family with two affected children. The fetus was diagnosed as having Hurler's syndrome on the basis of a deficiency of alpha-L-iduronidase in the cultured amniotic cells. The glycosaminoglycans (GAG) content in the supernatant of the amniotic fluid was increased about 1.5 fold compared with that in the control, and increases of heparan sulfate and dermatan sulfate were observed on electrophoresis. The diagnosis could be confirmed by the deficiencies of alpha-L-iduronidase in the liver and brain from the affected fetus. GAG content in the liver from the affected fetus was increased approximately 10 fold as compared with that in the control fetal liver, and most of the GAG were degraded. The GAG content was observed to be increased two fold in the brain, and dermatan sulfate, which was not detected in normal fetal brain, was identified. beta-Galactosidase activities in the affected liver and brain were decreased to 30-50% of the control, and an altered hexosaminidase A was also observed in the liver.

Postcontrast computed tomography in subarachnoid hemorrhage from ruptured aneurysms.

Computed tomography (CT) scans of 49 patients with subarachnoid hemorrhage (SAH) secondary to ruptured aneurysms were reviewed. Subarachnoid blood was detected in 95% when CT was performed within 5 days after the bleeding. Prediction of the site of the ruptured aneurysm from distribution of the cisternal blood varied from 92% for the anterior cerebral artery aneurysms to 70% for the middle cerebral and 47% for the internal carotid artery aneurysms. Following plain CT scans, postcontrast studies were performed in 27 patients. Aneurysms were directly visualized in 8 cases and subarachnoid enhancement was noted in 21. In 3 cases, subarachnoid enhancement made possible a correct diagnosis of SAH in the absence of evidence of cisternal blood on precontrast CT scans.

Colchicine in the treatment of the cutaneous manifestations of Behçet's disease.

Five patients with Behçet's disease were satisfactorily treated with colchicine. Oral aphthosis, erythema nodosum-like lesions and genital erosions improved greatly within a month as did laboratory findings. We believe colchicine to be the first choice in the management of the cutaneous and ocular lesions of Behçet's disease.

Effects of pH on cytotoxicity of carboquone.

To assess the effects of pH on the cytotoxicity of carboquone (CQ), use was made of the mouse tail skin and HeLa cells in culture. CQ had the most potent cytotoxic effect at pH 6 rather than at pH7 or 8. Regarding the interaction between 14C-CQ and HeLa cells, both the intracellular accumulation of free 14C-CQ and the ratio of bound 14C-CQ to total 14C-CQ uptake were enhanced at pH6. Among the fractionated biomolecules of DNA, RNA and protein, DNA was the most active in binding CQ, under the same conditions of pH. The 14C-CQ binding to nucleic acids at pH 6 was more apparent than was the binding to protein. Thus, the enhancement of CQ cytotoxicity at low pHs is probably due to an increase in the intracellular accumulation of free CQ as well as to an enhanced reactivity of CQ with DNA, within the ranges of a lower pH.

Protection by sodium thiosulfate and thiourea against lethal toxicity of cis-diamminedichloroplatinum (II) in bacteria and mice.

The protective effect of sodium thiosulfate and thiourea on the lethal toxicity of the antitumor drug, cis-diamminedichloroplatinum (II) (cis-DDP), was investigated in bacteria and mice. Initially, the agents capable of antagonizing bactericidal activity of cis-DDP were screened using WP2 uvra, a strain of E. coli sensitive to this drug. Of the ten sulfur-containing compounds tested, thiourea and sodium thiosulfate exhibited potent protecting effects against cis-DDP cytotoxicity in bacteria. Propylthiouracil and methimazole showed intermediate levels of such protection, but the other 6 compounds had little or no protective effects. Thiourea and sodium thiosulfate were then subjected to the acute lethal toxicity test in mice to assess their protective activity in vivo. We found that cis-DDP i.v. lethality against mice can be blocked almost completely by excess amounts of thiourea or sodium thiosulfate. Thiourea protected against cis-DDP toxicity with a narrow range among the effective doses, while sodium thiosulfate was protective with a remarkably wide range of effective doses. The effectiveness of sodium thiosulfate was also indicated in experiments in which the LD50 dose of cis-DDP (16 mg/kg) i.p. increased over the level of greater than 200 mg/kg with concomitant administration of sodium thiosulfate i.p.

[Cranial computed tomography of the neurofibromatosis (author's transl)].

The computed tomography (CT) was performed in 10 cases of neurofibromatosis. The CT scan showed the abnormal findings in 8 cases out of 10. Skull lesions were noted in 3 cases and intracranial tumors were found in 5 among which multiple neoplasms were seen in 3. Although reported cases were not large enough in number, the incidence and variety of the tumors were similar to others reported before CT era.

Erythema multiforme: demonstration of immune complexes in the sera and skin lesions.

In twenty patients with erythema multiforme we investigated circulating immune complexes and their deposition in the skin lesions. CIq-binding activity was elevated in ten of twenty patients, and the platelet aggregation titre was high in three of twelve tested sera. Decreased levels of C3 were seen in two and of C4 in one out of eighteen patients. Direct immunofluorescence showed a deposition of C3, IgM or IgG in the blood vessel walls of the upper dermis in four of twelve patients. These findings may suggest that transient production of circulating immune complexes and their deposition play an important role in the pathogenesis of this disease.

Sequential computed tomography scans in acute cerebral infarction.

Sequential computed tomography (CT) scans were obtained in 30 patients with acute cerebral infarction. In some cases, infarction was seen on the scans as early as three to six hours after onset of symptoms. Although small areas of petechial hemorrhagic infarction are not easily detected by currently available equipment, positive contrast enhancement helps locate petechial hemorrhagic infarction, which is usually in the cortical gray matter of the cerebral mantle and the central gray matter of the deep central gray matter. Contrast-enhanced CT scans are a useful method for detecting small infarctions. However, when a CT scan is performed within five days of ictus, contrast enhancement is usually not required.
