Comparative analysis of tensile bond strength of two new fissure sealants using invasive and non-invasive techniques.

In this era of preventive dentistry, a galore of dental materials used for prevention of dental diseases is available. Since the last few decades efforts are being directed towards prevention of dental caries which is one of the major dental diseases tormenting mankind. The rationale of using a fissure sealant is that when it is applied on the caries prone fissures, it penetrates these pits and fissures and seals them from the oral environment. One of the main problems usually encountered is retention of the sealant material. This study was undertaken with the aim to evaluate the tensile bond strength of two new fissure sealants using invasive and non-invasive techniques. The results substantiated the use of invasive techniques over non-invasive techniques and resin sealants over glass ionomer sealants.

Dental age assessment: the applicability of Demirjian's method in south Indian children.

The paper concerns the testing of Demirjian's method of age assessment in South Indian children. Since previous studies have shown the inapplicability of Demirjian's method on other populations, an attempt to compile a maturity standard for South Indian children was made. An additional independent indicator of age was employed, namely the skeletal age. The material was 184 South Indian children aged 5 to 15 years and an additional 34 children as the test sample. It was found that Demirjian's method gave an overestimation of 3.04 and 2.82 years in males and females, respectively. The skeletal age was found to differ from the dental and chronologic age. It may be concluded that the accuracy of age estimation based on Demirjian's method is not applicable for the South Indian children. For the population to be tested, it is imperative that individual assessment parameters need to be put forward because of wide ethnic differences.

AgNORs in benign, borderline and transitional cell neoplasms of the urinary bladder.

Argyrophilic nucleolar organizer regions (AgNORs) were studied in 106 tissue samples from the urinary bladder (6 normal transitional epithelium, 5 cystitis, 12 hyperplastic, 14 dysplastic lesions, 12 carcinoma in situ, 4 transitional cell carcinoma grade 0, 12 grade I, 15 grade II and 12 grade III) to evaluate their role in differentiating benign, borderline and malignant lesions. The NOR counts presented a rising scale from normal (2.21), inflammatory (3.93 for both cystitis and hyperplasia), dysplastic (4.16), carcinoma in situ (5.08) to malignant lesions (5.28 grade I, 6.59 grade II and 8.37 grade III). It was concluded that AgNORs do not have a diagnostic role in these lesions, but that they can act as a reliable adjunct to existing parameters in the early detection of tumour recurrence and grading of malignant neoplasms.

Thoracic endometriosis is rare. Its associated clinical syndromes should be considered in menstruating women with pneumothorax or hemoptysis. The availability of chest CT scanning and danazol provide new technique in diagnosis and therapy.

Bite mark analysis in child abuse.

The science of bite mark analysis is relatively new and potentially valuable. This science has been greatly used in identifying the suspects involved in crimes related to sex, child abuse, and altercations of various types. Child abuse is presenting a serious social problem with global dimensions, which is increasing at an alarming rate in all socioeconomic strata and all ethnic or racial communities. The aim of the present study was to assess these bite marks and analyze the effectiveness of a simple technique used in the department of pedodontics and preventive dentistry, College of Dental Surgery, Manipal.

Compound composite odontoma in primary dentition--a case report.

Compound composite odontoma, is a tumor formed by the overgrowth of transitory or complete dental tissue, made up of one or more rudimentary teeth. Although odontomes are considered to be quite common of odontogenic tumors, these rarely occur solely in primary dentition. This paper is a report of case of a compound composite odontoma, five year old child who presented to the Department of Pedodontics and preventive Dentistry, College of Dental Surgery, Manipal.

A short-term plaque study using Lactitol as a non-sugar sweetener.

A short-term plaque study was undertaken in a group of 12 children of 8-12 years age to evaluate the effect of lactitol, 4-O (beta-galactosyl)-D-glucitol, on dental plaque pH, weight and microchemical composition. Sucrose which was taken as control and lactitol, were used in both solution form and sweets. Plaque pH recording was done at rest and post consumption periods at 5, 10, 20 and 30 minute intervals. Weight and microchemical analysis were done with 3-day-old plaque after consumption of sweets. Throughout the study period lactitol did not reach to a pH where the demineralization of enamel starts. A significant reduction in plaque formation, carbohydrate content, increase in calcium, phosphorus and protein in lactitol showed the potentiality of this bulk sweetener as non-cariogenic.

Cuspal fracture resistance and microleakage of glass ionomer cements in primary molars.

The purpose of this study was to comparatively evaluate the fracture strength and microleakage among two new glass ionomer cements. One hundred and ten primary first and second molars were divided into two main groups, one group comprising of fifty teeth for microleakage evaluation and second group of sixty for fracture strength. Both the groups were subdivided equally into four based on the materials used, consisting of the same number of teeth respectively, in each. Two controls were used for cuspal fracture strength and an additional trial group was added for microleakage evaluation. Ideal, non retentive class II were prepared and restored with amalgam, Fuji IX, Fuji II LC and Vitremer. The statistical analysis revealed highly significant differences among all the subgroups except between subgroup 3 vs. 4 and 1 for cuspal fracture strength. Similarly microleakage values were significantly different statistically among sub groups 4 and 5 vs 1 and 2. The present study revealed that Fuji IX was the best in terms of cuspal fracture strength and Fuji IX with an additional application of light cure resin sealant gave the lowest degree of microleakage.

The effect of chewing gums on acidogenicity of plaque after a sucrose challenge.

The aim of this investigation was to study the effect of chewing sugared, sugar free or gum with therapeutic agents like urea or fluoride, on pH of dental plaque after a sucrose challenge using a microtouch method. The 20 subjects aged 8 to 10 years refrained from toothbrushing for 2 days. Resting plaque pH was recorded, followed by sucrose 10% (w/v) rinse for 3 min. After 5 min chewing gum was given and pH recorded at time points of 5, 10, 20, 30 min. Highest plaque pH was found with Endekay (urea) at all time points followed by fluoride chewing gum; Trident, and Wrigley's (p < 0.0001). Sucrose 10% w/v (control) showed the maximum drop in plaque pH when compared to the rest of the groups. The comparative evaluation has shown Endekay having maximum pH against the acidogenic challenge. Therefore the present study indicates that chewing gum containing therapeutic agents like urea may be recommended in high risk caries children.

Observer variation in AgNOR counts in neoplastic breast lesions.

OBJECTIVE: To determine interobserver and intraobserver variability of AgNOR quantitation in neoplastic lesions of the breast. STUDY DESIGN: Forty-five cases, 20 benign and 25 malignant lesions, were included in the study. Counts were performed on one slide from each case within a pre-marked area of about 1 cm2 in a standardized manner by two observers blind to the histopathologic diagnosis and independent of each other and repeated after two weeks. Interobserver and intraobserver agreement was assessed using the Bland-Altman method. RESULTS: Our results showed small mean interobserver and intraobserver differences but wide limits of agreement. CONCLUSION: Observer variation in AgNOR counts is too high for the method to be of any diagnostic or prognostic relevance.

Evaluation of fluoride uptake by enamel in children from fluoride dentifrices.

Fluoride dentifrices are well accepted for their caries preventive effects. Although it is generally accepted that the presence of fluoride in the enamel protects teeth against carious attack, at the same time, there is some concern that ingestion of fluoride from toothpastes may substantially contribute to the total intake of this element. The present study was undertaken to clinically evaluate the fluoride uptake by human enamel after the use of lower concentrations of fluoride dentifrices and compare their potency to higher concentration in increasing the uptake of fluoride by enamel. Enamel biopsy was conducted on first permanent molars of 100 school children, aged 6-8 years before brushing and at intervals of one hour, 3 weeks, 6 weeks and 12 weeks following brushing with different concentrations of fluoride dentifrices. The fluoride content of the enamel biopsy samples were estimated using the Orion microprocessor ion analyser and by taking into consideration the enamel biopsy mass and depth of the etch. The results of the present study suggest that low fluoride concentration dentifrices provide similar fluoride uptake by enamel when compared with that of higher concentrations.

Prosthetic rehabilitation of a child with induced anodontia.

A case of prosthetic rehabilitation of a five and half year old child with induced anodontia is presented. Complete dentures were provided and patient was followed for a period of 20 months. As the permanent teeth erupt into the oral cavity, regular modifications have to be made on the denture base to facilitate their eruption.

Physiological response to dental anxiety in children.

The study was conducted among 50 children in the age group of 8-9 yrs. to evaluate the anxiety producing dental procedures in the dental operatory viz. waiting at the reception area, examination in the dental chairs, scaling, polishing, cavity cutting, injection, extraction and post extraction period. The pulse rates were very high during local anaesthesia, extraction, waiting at reception and examination. A significant correlation was observed between the pulse rate and blood pressure.

A comparison of manual vacuum aspiration (MVA) and sharp curettage in the management of incomplete abortion.

OBJECTIVES: A prospective longitudinal study was carried out in two Harare hospitals to determine whether manual vacuum aspiration (MVA) was as safe and as effective as sharp curettage for treatment of incomplete abortion. METHODS: Demographic and clinical data were collected over a 3-month period on 589 women treated with sharp curettage for incomplete abortion < or = 12 weeks gestation. One year later, after faculty and staff at the two hospitals were trained to use MVA, data were similarly collected on 834 women treated with MVA for incomplete abortion. RESULTS: Based on procedure-related complications at the time of treatment, MVA was found to be as safe as sharp curettage in treating incomplete abortion < or = 12 week gestation. MVA was more effective than sharp curettage in achieving complete uterine evacuation (0% incomplete evacuations vs. 0.7%, P < 0.05). CONCLUSIONS: Given the safety and effectiveness of the MVA procedure and the potential for reducing health care costs and improving patient management, this technology should be considered by health care systems in developing countries for improving treatment of abortion complications.
