Cerebral blood flow in both hemispheres was studied by the 133Xe inhalation method in 49 patients with cerebral infarction in the unilateral hemisphere. They were classified into three groups by computed tomographic findings as follows; relatively large low density lesion including the cerebral cortex and subcortex (cortical: C group), relatively large low density lesion including the subcortical white matter and basal ganglia (large subcortical: L group), and small low density lesion including the subcortical white matter (small subcortical: S group), respectively. Mean cerebral blood flow (mCBF) in the affected hemispheres was markedly low in C group, moderately low in L group, and slightly low in S group, in all of the examinations. Several cases in C and L groups revealed remarkable changes of mCBF less than one month after the onset. MCBF in both hemispheres was lower in C group than in L and S groups less than one week after the onset. Seven to twelve weeks after the onset, mCBF in the affected hemisphere was lower in C and L groups than in S group, and than in the unaffected hemisphere of C and L groups. There was no difference between mCBF in the affected hemisphere and that in the unaffected hemisphere in most of S group. Sequential changes of mCBF in both hemispheres were divided into seven types in 27 cases, who were examined first less than one week and repeatedly then. Regional cerebral blood flow was measured by the 133Xe inhalation method in 20 young patients with moyamoya disease and five young healthy volunteers. Most patients showed low values of mean hemispheric blood flow in both hemispheres. Regional cerebral blood flow was at a low value in the upper frontal region and at an almost average value in the posterotemporal and occipital regions, which was different from the "hyperfrontal" pattern in healthy volunteers. Regional cerebral blood flow was reduced evenly by hyperventilation. By 5% CO2 inhalation, regional cerebral blood flow was increased in the temporooccipital regions and was nearly unchanged or decreased in the frontal region.
Two cases of gestational choriocarcinoma have been examined for the expression of HLA and trophoblast antigens using the indirect immunoperoxidase technique on frozen tissue sections. Approximately 40-70% of tumour cells express MHC Class I antigen as detected by a panel of antibodies to monomorphic, or framework, MHC Class I antigenic determinants. Evidence in one case suggests that most of these cells may not express the paternal, polymorphic HLA antigenic determinants but that a small subpopulation do carry the fully antigenically active molecule. These latter may give rise to patient anti-paternal HLA antibodies. Class II (HLA DR or DC) antigens are expressed by none, or very few, tumour cells.
Between 1963 and 1980 we have had experience of 54 patients with gestational choriocarcinoma and 29 are alive, a survival rate of 53.7%. The disease had metastasized to the central nervous system in 15 of these 54 patients, an occurrence rate of cerebral metastases of 27.8%. 13 of these 15 patients have died and 2 are now in a state of remission. Autopsy was performed in 8 cases. The following points are emphasized: cases with the pulmonary disease risk intracranial spread; the serum/cerebrospinal fluid ratio of human chorionic gonadotropin concentrations and computerized tomography are recommended as useful investigations in the diagnosis of CNS disease; decompression surgery in patients with increased intracranial pressure is indispensable; radiotherapy is temporarily effective for brain deposits; powerful systemic chemotherapy using multiple anticancer drugs is suggested to be helpful both in preventing and in treating CNS disease.
The antitumour activity of ethylene-vinyl alcohol (EVAl) copolymer matrices containing 5-fluorouracil (5-FU) was evaluated against Ehrlich ascites carcinoma in mice. A prolongation of the life-span of tumour-bearing mice following intraperitoneal implantation of therapeutic matrices was noted. These results indicated that EVAl matrices containing 5-FU may be effective in cancer chemotherapy. Matrices composed of EVAl copolymer could be useful vehicles for implanted, inserted, or surface-applied delivery systems for anticancer agents.

The present report is Werner's-syndrome associated with a neurogenic bladder. The patient visited our clinic with the chief complaint of retention of urine and urinary tract infections. The diagnosis of Werner's syndrome was made because the patient had such clinical features as bird-like or "masked" face, short stature, graying of the hair in youth, cataracts in youth, skin changes like scleroderma principally on limbs, early menopause. After trans-urethral-resection of bladder neck and administration of distigmine bromide, her bladder function was improved, she could urinate by herself and she was free from urinary tract infections.

A clinicopathological analysis of 11 patients with Paget's disease of the vulva was reported. A close association of this disease with cancer was strongly suggested in the histological examination. Nine of 11 patients had wide primary lesion; 4 patients had coexisting undifferentiated carcinoma (one patient with in situ sweat gland carcinoma) and one patient had squamous cell carcinoma in other lesion of the vulva. Five patients had downward growth of the tumor from the epidermis proper. There were, thus, only two patients who had Paget's cells confined within the epidermis. Three patients died of generalized metastasis of carcinoma and two patients had vulvar recurrence. The results of staining for PAS, PAS diastase resistant reaction, alcian blue, mucicarmin and carcinoembryonic antigen (CEA) were all positive in specimens with similar stainings to coexisting undifferentiated carcinoma of the vulva and metastasized lesions. CEA staining was positive in cytoplasm of Paget's cells and basal cells widely in the epidermis. It could be considered that the Paget's cells of the vulva originated multifocally from primitive stratum germinativum, which gave rise to the surface epithelium as well as all of the dermal appendages.
