Trophoblastic embolism in sudden maternal death.

Trophoblastic embolism (TE) was shown in three out of eight maternal autopsy cases. The trophoblasts of TE were easily identified by the immunocytochemical method with antisera to placental antigen (PA) and human placental lactogen (HPL). Two types of TE were differentiated in the morphology of trophoblasts: One was formed of whole trophoblastic cells (TE-Type I) and the other was composed of amorphous trophoblastic fragments (TE-Type II). The TE-Type II was confirmed in the central nervous system (trophoblastic emboli in brain) and appeared to be the sudden maternal death cause.

[99mTc-MAA perfusion studies in the treatment of preoperative intra-arterial infusion chemotherapy of malignant bone and soft tissue sarcoma].

Preoperative intra-arterial continuous chemotherapy combined with surgery in the treatment of malignant bone and soft tissue sarcomas has been used since 1968 in our clinic. This approach offers a theoretical advantage over systemic chemotherapy in that it delivers higher concentrations of chemotherapeutic agents to the tumor bed without increasing systemic toxicity. Tumor response depends on tumor sensitivity and the maximum amount of agent which is deliverable to the tumor tissues, so it is important to maintain a proper catheter placement to obtain good tumor response. 99mTc-Labeled macroaggregated albumin (99mTc-MAA) perfusion studies were carried out 19 patients who had histologically proven malignant bone and soft tissue sarcomas. The use of 99mTc-MAA perfusion studies in the treatment of intra-arterial chemotherapy offers an excellent way to evaluate catheter placement and tumor perfusion, in addition to providing a way to evaluate tumor A-Vshunts with lung scanning. The dynamic images of 99mTc-MAA obtained at each stage of the chemotherapy period also demonstrate the degree of tumor tissue response to intra-arterial chemotherapy. Therefore we conclude that 99mTc-MAA perfusion combined with preoperative intra-arterial infusion is a very useful method in cases involving limb salvage surgery.

Ultrasonic evaluation of pediatric superficial masses.

Mechanical sector scanning using a balloon and electronic scanning using transrectal probes were applied in the diagnosis of pediatric superficial masses. In 135 cases of pediatric superficial masses definitely diagnosed by surgical findings and histopathologic examinations, a comparative investigation was performed between the surgical and histopathologic findings and ultrasonographic findings. Among these 135 cases, 117 of the masses were benign, and 18 were malignant. By means of these scans, it was possible to diagnose correctly not only the size and shape of the mass but also the internal characteristics. However, it was difficult to determine if the mass was benign or malignant.

The effects of Escherichia coli endotoxin as a trigger for hepatic infection of rabbits with Fusobacterium necrophorum.

To evaluate the effects of endotoxin in hepatic infection with Fusobacterium necrophorum, rabbits were given several combinations of F. necrophorum and Escherichia coli endotoxin. Severe hepatic necrosis with multiplication of F. necrophorum was induced by inoculation of endotoxin via the bile duct and following inoculation of both endotoxin and F. necrophorum. Inoculation with F. necrophorum alone, preceded by inoculation of endotoxin via the bile duct, also induced hepatic necrosis, whereas rabbits which received a single inoculation of endotoxin or F. necrophorum had only slight necrotic lesions. The pathogenetic mechanism of experimental hepatic infection with F. necrophorum is discussed.

Clinical problems of invasive hydatidiform mole in patients aged 40 or more.

Seventy-six patients with invasive hydatidiform mole (HM) were reviewed as to clinical course, particularly treatment and outcome, in relation to their age. The results were as follows: (i) metastatic cases showed approximately a twofold increase in patients over 40 compared with younger patients, (ii) more courses of chemotherapy were required to achieve a cure in patients over 40 than in younger patients and (iii) 4 of 19 patients (21.1%) over 40 developed choriocarcinoma, whereas none of younger patients did.

Implications of circulating autoantibodies and peripheral blood lymphocyte subsets for the genesis of premature ovarian failure.

Several kinds of circulating autoantibodies and peripheral blood lymphocyte subsets were studied in 20 patients with secondary amenorrhea manifesting hormonal and clinical features of premature ovarian failure (POF). More than one kind of autoantibody was detected in 14 patients (70%). Seven patients (35%) had anti-thyroglobulin antibody, 6 (30%) had anti-parietal cell antibody, 8 (40%) had anti-nuclear antibody, and one patient with chronic thyroiditis had anti-TSH receptor antibody. Anti-adrenal cortex antibody and RA test were negative in all patients. Two patients had clinically evident autoimmune disease; one had myasthenia gravis and the other had chronic thyroiditis. Examination of peripheral blood lymphocyte subsets of 19 patients by flow cytometry revealed an increase in the percentage of OKT3+ cells and OKT4+ cells and a decrease in OKT8+ cells in POF patients compared with age-matched controls, but these differences were not significant. An increase in the OKT4/OKT8 ratio was, however, significant. It has been suggested that an autoimmune mechanism may participate in the genesis of POF, at least in patients with autoimmune diseases; however, the findings in this study support the hypothesis that some pure POF may also be caused by an autoimmune process resulting from a subclinical imbalance in the immunoregulatory system before manifestation of the autoimmune disease.

Proteolytic zymograms of Staphylococcus hyicus subsp. hyicus isolated from pigs, chickens and cows.

The extracellular proteases of Staphylococcus hyicus subsp. hyicus were assayed by a zymogram showing caseinolysis and gelatinolysis. Four bands were associated with caseinolysis or with gelatinolysis. The patterns shown by strains isolated from pigs, chickens and cows were compared; isolates from pigs differed from those isolated from chickens or cows but strains isolated from diseased and healthy pigs could not be differentiated.

Incidence of gestational trophoblastic disease by regional registration in Japan.

This presentation deals with the large-scale statistics of 19 951 patients of trophoblastic diseases (TDs). The information was obtained by regional registration of 16 prefectures in Japan since 1974, and based on a definite classification of TDs given by the Japanese Society of Obstetrics and Gynaecology. The population of these prefectures comprises approximately 40% of that in all Japan. The number of registrations accumulated in this series was estimated to be greater than 90% of all affected patients in all regions, indicating a high degree of reliability of these statistics. The overall incidence of hydatidiform mole (HM) including invasive mole (IM) in 16 regions between 1974 and 1983 was one HM per 306 live births or 482 pregnancies. The incidence of HM with or without IM per 100,000 women showed a tendency to decrease in parallel with the reduction in birth rate during this period, thus indicating no significant change in the incidence of HM per 1000 live births. In this series the number of cases 'undetermined' because of a lack of appropriate histology was 943 (or approximately 5% of all cases). These may potentially be categorized as either IM or choriocarcinoma. Also, criteria for the distinction between a partial and a complete mole could not be defined satisfactorily.

The rearrangement of cytoskeletal systems in epithelial cells accompanying the transition from a stationary to a motile state at the start of epithelial spreading.

The changes in the alignment of F-actin bundles in epithelial cells were studied, mainly using NBD-phallacidin, as the isolated corneal epithelium started to spread along the surface of Millipore filter. The epithelial cells at the margin were found to make the transition from a stationary to a motile state in two steps: first, from polygonal cells, in which F-actin was deposited along the cell border, to hemispherical cells, in which F-actin was distributed homogeneously along the inner cell surface; and second, from the hemispherical cells to spread cells in which F-actin bundles ran radially or in parallel to each other through the leading lamella from a cytocentre to the edges, as in fibroblastic cells. During the second step, an amorphous mass of F-actin was transiently accumulated at the distal end in the half-extended lamella and structured into bundles during development of the lamella. Since this proceeded only on substrata to which marginal cells could make contact firmly enough to enable the cells to extend, it was concluded that F-actin bundles were aligned within extending lamellae in relation to the tension exerted by the lamellae, and might be associated with other components to be microfilaments.
