Probably, the most important evidence concerning the breakdown of the BBB is a large inflow of hematogenous fluid into the extravascular spaces. Thus all parenchymal cell elements represent freely floating cells in this fluid medium called the edema fluid. These essential morphological alterations, such as extremely expanded extracellular space and freely floating cells within the fluid, were also observed in the developing normal fetal brain. Many neuroblasts were vigorously migrating in the fetal brain, as were the macrophages and reactive astrocytes in the edema fluid. Obviously, hematogenous cells and reactive astrocytes in the edematous lesion take part in its repair. Many astrocytes, GFAP positive, in the 3 or 4 day-old lesions, revealed mitosis. And in vitro, when we immersed these astrocytes in the protein and glucose rich medium, they demonstrated a remarkably changed morphology and were moved into the M and G1 phases, thus gaining the ability of cell motility. This was also true in the edema fluid. Brain edema is definitely a serious "pathological" condition. But it is also conceivable from a different biological aspect that as a result of BBB "opening", free extracellular space essential for cell motility and a source of their energy is ingeniously provided for these cells, and thus the lesion can be effectively repaired. The biological significance of the edema fluid was emphasized and stereotaxic morphology and cinematography, supporting the above evidence, have also been presented.
A 69-year-old female was admitted to the hospital because of tarry stool and dyspnea on exertion. Multiple hemangiomas were noticed on the tongue, right arm and abdomen. Gastrointestinal examination revealed multiple polypoid lesions in the stomach and small intestine. In addition, coronary artery-left ventricular fistula formation and pulmonary arteriovenous malformation were shown by angiography. Compared with other reported cases of blue rubber bleb nevus syndrome, the present case was unique because of the coronary artery-left ventricular fistula, heart failure and the familiar incidence of heart failure.
Male Wistar rats were exposed to immobilization stress for various periods (1 to 5 hr) with or without an IP injection of probenecid at 400 mg/kg. The regional characteristics of stress-induced increases in noradrenaline (NA) release in the rat brain related to the time-course of stress were demonstrated by measuring levels of the major metabolite of NA, 3-methoxy-4-hydroxyphenylethyleneglycol sulfate (MHPG-SO4). Increases in MHPG-SO4 levels occurred mainly within the first hr of stress in the hypothalamus, amygdala and thalamus, while the peak elevations of the metabolite levels were delayed in the hippocampus, cerebral cortex, pons + medulla oblongata and basal ganglia. According to the accumulation of MHPG-SO4 during each 1-hr period of stress, regional characteristics of NA release were classified into the following four types based upon regions where the most marked increase in MHPG-SO4 levels occurs mainly: (1) within the first hr of stress (the hypothalamus, amygdala and thalamus), (2) during the first and second hr (the hippocampus and cerebral cortex), (3) during the third hr (the basal ganglia) and (4) to the same extent from the first to the fourth hr of stress (the pons + medulla oblongata). These results suggest that noradrenergic neurons in different brain regions respond differentially to stress and reflect their own characteristic patterns depending upon nature and time-course of the stressor.
We have shown that Escherichia coli extracts catalyze recombination between mouse immunoglobulin mu and alpha genes inserted separately in lambda phage vectors carrying different genetic markers. Most of the recombination sites in the inserts are located in the switch regions of the heavy chain genes, as previously found in the expressed genes of myeloma cells. The recombination took place at relatively high frequency (10(-4)). The recombinational system in E. coli or lambda phage seems to prefer short nucleotide sequences similar to those used in the class switch recombination.

In view of the reduction of side effects as well as the enhancement of anti-tumor activities of anticancer drugs, we have been interested in the combined use of Chinese medicines with them. In the present study, we attempted to examine the effects of Chinese prescription, juzentaihoto (JTX), combined with mitomycin C (MMC). JTX is consisted of Astragali radix, Chinnamomi cortex, Rehmanniae radix, Paeonia radix, Cnidii rhizoma, Atractylodis lanceae rhizoma, Angelicae radix, Ginseng radix, Hoelen and Glycyrrhizae radix. In BDF1-mice which were implanted with P-388 leukemic cells, JTX prolonged significantly the average survival days of MMC-treated group. In tumor-free BDF1-mice, JTX improved the leukopenia and the body weight loss which were caused by MMC. Additionally, JTX delayed the appearance of deaths by lethal dosis of MMC. These results indicate that JTX enhances the anti-tumor activity of MMC and lessens the adverse effects of it. JTX may be useful for patients undertaking MMC treatment.

We report a patient with synchronous triple cancers arising in the digestive tract. It was a combination of esophageal, gastric and colonic cancers; total excision was performed in one operation. Histological study confirmed that each cancer had developed independently. The esophageal cancer was of ulcerated type; histologically it was poorly differentiated squamous cell carcinoma. The gastric and colonic cancers were both of Borrmann II type and well differentiated adenocarcinoma.
Ten cases of congenital cystic dilatation of the common bile duct (including insufficiency form) were examined for mode of pancreatico-biliary anastomosis and cholangiographic findings. The results were as follows. Male to female ratio was 1:9 in this series. Regarding the mode of pancreatico-biliary anastomosis, type I (pancreatic duct flowing into common bile duct) tended to occur frequently in patients 30 years old or more, while type II (common bile duct flowing into pancreatic duct) in patients under 30 years old. With respect to clinical symptoms, no cases had triad (jaundice, abdominal pain and palpation of mass), but abdominal pain occurred in all the cases. Jaundice and palpation of mass were seen in 30% and 10% of the cases, respectively. Regarding the age at the onset and the duration of the disease, type I occurred in elderly cases with a longer duration of disease, while type II occurred in young cases with a shorter duration of disease. The degree of distention increased in proportion to the severity of the peripheral stenosis of the biliary duct. Complication with biliary tract carcinoma was seen in 2 type I cases. Among the type I cases, those with mild distention were considered to be insufficient form.

Among 113 consecutive patients with disseminated intravascular coagulation (DIC), twenty cases who developed this condition under surgical intervention are the subject of this study. At the onset of DIC, fourteen cases were suffered from severe infection. Coagulation study in these patients revealed decreased platelet counts and prolonged prothrombin time. There was marked increase in fibrinogen degradation products and positive ethanol gelation test was frequently observed. These findings were analogous to those found in the more chronic presentations of DIC in the patients with neoplastic diseases, however, plasma concentration of fibrinogen was normal. Antithrombin III level was extremely low in the patients with surgical DIC. The patients with DIC in surgery took acute or subacute courses with very high incidence of major organ failure. The patients with dysfunction of more than three organs were seen in eight of 20 cases. The lungs, kidneys, liver and gastrointestinal tracts were involved in the descending order of frequency. Fourteen patients died and six of these were autopsied. All had multiple fibrin thrombi in more than two organs. The numbers and sites of microthrombi in the major organs were well matched with the clinical manifestations. These results suggested that DIC might be one of the precipitating factors of multiple organ failure.
