Analysis of MHC class II antigens in Japanese IDDM by a novel HLA-typing method, hybridization protection assay.

We examined HLA Class II antigens in 116 Japanese IDDM patients [84 typical IDDM (T-IDD); 32 slowly progressive IDDM (S-IDD)] by the hybridization protection assay (HPA) which is a novel HLA typing method based on hybridization of acridinium-ester-labeled DNA probes to amplified DNA. We detected HLA-DRB1, -DQA1 and -DQB1 genes by this method which is capable of analyzing over 50 samples within 4 h with high sensitivity. Positive associations were found in DRB1*0405, DRB1*0802, DRB1*0901, DQA1*0301, DQB1*0303 and DQB1*0401, negative correlations in DRB1*0403, DR2, DR12, DRB1*0801 or 03, DQA1*0101 or 02, DQA1*0501, DQB1*0301 and DQB1*0602 alleles. The absence of aspartic acid (Asp) at position 57 of the DRB1 chain and the presence of arginine (Arg) at position 52 of the DQA1 chain correlated positively with both types of IDDM. There were no significant differences in HLA between T-IDD and S-IDD. These results suggest that the absence of Asp at position 57 of the DRB1 chain and the presence of Arg at position 52 of the DQA1 chain are significant Japanese IDDM patients and that DRB1*0802, in which the amino acid at position 57 is aspartic acid, may play a role in the pathogenesis of IDDM. Also, T-IDD and S-IDD have common bases in the HLA gene.

Differential dynamics of neurofilament-H protein and neurofilament-L protein in neurons.

Neurofilaments (NFs) are composed of triplet proteins, NF-H, NF-M, and NF-L. To understand the dynamics of NFs in vivo, we studied the dynamics of NF-H and compared them to those of NF-L, using the combination of microinjection technique and fluorescence recovery after photobleaching. In the case of NF-L protein, the bleached zone gradually restored its fluorescence intensity with a recovery half time of approximately 35 min. On the other hand, recovery of the bleached zone of NF-H was considerably faster, taking place in approximately 19 min. However, in both cases the bleached zone was stationary. Thus, it was suggested that NF-H is the dynamic component of the NF array and is interchangeable, but that it assembles with the other neurofilament triplet proteins in a more exchangeable way, implying that the location of NF-H is in the periphery of the core NF array mainly composed of NF-L subunits. Immunoelectron microscopy investigations of the incorporation sites of NF-H labeled with biotin compounds also revealed the lateral insertion of NF-H subunits into the preexisting NF array, taking after the pattern seen in the case of NF-L. In summary, our results demonstrate that the dynamics of the L and H subunit proteins in situ are quite different from each other, suggesting different and separated mechanisms or structural specialization underlying the behavior of the two proteins.

Variability in circadian activity patterns within the Culex pipiens complex (Diptera: Culicidae).

Circadian activity was compared among four strains of Culex pipiens molestus Forskal from Iran, Egypt, two localities (Ebira and Yanagawa) in Nagasaki, Japan, and one strain of Cx. p. pallens Coquillett from Japan. Under a 16:8 (L:D) h photoperiod, all the strains showed peaks of activity at lights on and off, with additional activity during both light and dark periods; Egyptian females and both sexes of Cx. p. pallens were inactive during the photophase. Under 0:24 (L:D) h, the length of the free-running activity period was significantly shorter in the "Iran" (21 h) than in the other three Cx. p. molestus strains (range, 21-23 h) and Cx. p. pallens (approximately 24 h). Under 24:0 (L:D) h, the activity rhythm was obscured. The length of the period was shorter in 24:0 (L:D) h than in 0:24 (L:D) h, violating Aschoff's circadian rule. Furthermore, the circadian peak did not always occur every day under 24:0 (L:D) h but rather exhibited "day-skipping."

A case of recurrent intramural uterine stromal tumor with epithelial differentiation effectively treated with oral low-dose administration of etoposide.

A 66-year-old woman was diagnosed as having myoma of the uterus and total hysterectomy with bilateral salpingo-oophorectomy was performed. The histopathological findings were puzzling, and the case was finally diagnosed as intramural uterine stromal tumor with epithelial differentiation. Three years after the operation, a firm tumor developed in the pelvic cavity. Laparotomy failed to remove the tumor and neither CAP (cyclophosphamide 500 mg, adriamycin 50 mg, CDDP 70 mg) therapy nor radiation therapy was effective. Oral administration of etoposide (25 mg/day), however, showed PR (50% decrease) as determined by CT scanning, and ultrasonography, and no metastatic lesions were found. This tumor was coincident with the endometrial stromal tumor with epithelial elements classified by Clement and Scully. The histological feature of the tumors and the efficacy of oral etoposide therapy are discussed in this study.

Autologous blood transfusion for the patient with placenta previa complicated by placenta increta: a case report.

The patient, who was 34 years of age, had previously had a transverse incision of the lower uterine segment cesarean section because of placenta previa. She was admitted to the hospital due to placenta previa again at 27 weeks of gestation in the current pregnancy. Ultrasound examination revealed placenta increta as well as placenta previa. In an attempt to avoid homologous blood transfusion at the time of profuse hemorrhage anticipated to occur during cesarean section, an autologous blood transfusion was planned. Fifteen hundred ml of autologous blood was collected by a leap-frog method during the 8 weeks prior to cesarean section. A cesarean hysterectomy was performed at 37 weeks of gestation because of placenta increta. Blood loss was estimated at 1,830 ml, and 1,500 ml of autologous blood was transfused. A leap-frog method of autologous blood collection for this pregnant woman with risk of massive hemorrhage was simple and beneficial, resulting in the preservation of more than 1,500 ml of autologous blood for transfusion.

Activities of phospholipase A2, cyclooxygenase, and PGI2 synthase of umbilical venous endothelial cells in preeclamptic women.

To elucidate the cause of low prostacyclin (PGI2) production in severe preeclampsia (PE), we studied the activities of phospholipase A2, cyclooxygenase, and PGI2 synthase in umbilical venous endothelial cells obtained from healthy pregnant women and from patients with mild or severe PE. Umbilical venous endothelial cells homogenized in a buffer solution were analysed by calculating the apparent Vmax (mean +/- SEM: p mol/min mg protein) and Km (mean +/- SEM: microM) values for phospholipase A2 activity by the release of arachidonic acid from phosphatidylcholine, for the activity of a complex of cyclooxygenase and PGI2 synthase by the conversion of arachidonic acid to PGI2, and the activity of PGI2 synthase by conversion of PGH2 to PGI2. The phospholipase A2 activity of normal-pregnancy cells (Vmax: 17.0 +/- 2.7 Km: 0.26 +/- 0.04) (n = 10) significantly exceeded that of cells from women with either mild PE (5.8 +/- 0.5, 0.12 +/- 0.02) (n = 4) or severe PE (6.3 +/- 2.0, 0.08 +/- 0.03) (n = 5). The apparent combined activity of cyclooxygenase and PGI2 synthase in mild PE (552 +/- 142, 0.29 +/- 0.07) (n = 8) significantly exceeded that of a normal pregnancy (176 +/- 42, 0.76 +/- 0.25) (n = 7), whereas that in severe PE (326 +/- 36, 3.26 +/- 0.78) (n = 3) was significantly lower than that of a normal pregnancy. PGI2 synthase activity in mild PE (305 +/- 50, 0.12 +/- 0.07) (n = 4) exceeded that of a normal pregnancy (220 +/- 45, 0.13 +/- 0.06) (n = 5), whereas that in severe PE (55 +/- 12, 0.16 +/- 0.04) (n = 3) was lower than that of a normal pregnancy. The phospholipase A2 activity in cells of normal pregnant women exceeded that of cells of women with mild or severe PE. The combined activity of cyclooxygenase and PGI2 synthase in a normal pregnancy was lower than in mild PE, but higher than in severe PE. Similar results were found for PGI2 synthase activity; in normal pregnancy the activity was less than in mild PE, but higher than in severe PE.(ABSTRACT TRUNCATED AT 250 WORDS)

Intestinal bacterial hydrolysis is indispensable to absorption of 18 beta-glycyrrhetic acid after oral administration of glycyrrhizin in rats.

Gnotobiote rats were prepared by infecting germ-free rats with Eubacterium sp. strain GLH, a human intestinal bacterium capable of hydrolysing glycyrrhizin to 18 beta-glycyrrhetic acid. Their faeces and caecal contents showed glycyrrhizin-hydrolysing activities (31.7 and 31.3 pmol min-1 (mg protein)-1, respectively) similar to those (81.0 and 39.9 pmol min-1 (mg protein)-1, respectively) of conventional rats, although there was no detectable activity in germ-free rats. When glycyrrhizin (100 mg kg-1) was orally administered to conventional, germ-free and gnotobiote rats, no glycyrrhizin could be detected in plasma 4 or 17 h after the administration, using EIA and HPLC assays. Plasma 18 beta-glycyrrhetic acid was not detected 4 or 17 h after the administration of glycyrrhizin to germ-free rats nor could this compound be detected in caecal contents or in the faeces. However, 18 beta-glycyrrhetic acid (0.6-2.6 nmol mL-1) was detected in plasma of the conventional and the gnotobiote rats 4 and 17 h after the administration, and the caecal contents after 4 h and the cumulative faeces up to 17 h of the conventional and the gnotobiote rats contained considerable amounts of 18 beta-glycyrrhetic acid. These findings indicate that orally administered glycyrrhizin is poorly absorbed from the gut, but is hydrolysed to 18 beta-glycyrrhetic acid by intestinal bacteria such as E. sp. strain GLH, and the resulting 18 beta-glycyrrhetic acid is absorbed.

[A case report: cryptococcal meningitis and fluconazole therapy].

An 18-year-old female was given prednisolone and azathioprine for treatment of systemic lups erythematosus. She was admitted to Fukuoka University Hospital because of headache and vomiting. Examinations revealed she was suffering from cryptococcal meningitis. Fluconazole (FLCZ) 400 mg a day was administrated for therapy. Her general condition improved and the serum level of cryptococcal antigen decreased one month after therapy. This therapy resulted in abatement of subjected symptoms and sterilization of the cerebral spinal fluid. This case suggested that therapy with FLCZ alone is useful for patients with cryptococcal meningitis, therefore the standard therapy with amphotericin B (AMPH) or the combination.

Antibacterial activity of epigallocatechin gallate against methicillin-resistant Staphylococcus aureus.

The antibacterial activity of epigallocatechin gallate (EGCg), a catechin, against 53 clinical isolates of methicillin-resistant Staphylococcus aureus (MRSA) was evaluated and expressed as minimal inhibitory concentration (MIC). The MIC50 and MIC90 of the strains were found be 64 and 126 micrograms/ml, respectively by the microdilution method. A time-kill study using an isolate showed that EGCg appeared to be bacteriostatic at 1-2 x MIC and bactericidal at 6 x MIC against MRSA. In addition, the activity of EGCg was stable to various physical conditions including boiling or freezing. These findings suggest that EGCg could be a useful agent for treating MRSA infection.

Carcinosarcoma of the lung. Report of a case and review of the literature.

We describe a case of carcinosarcoma of the lung in a 60-year-old female who was admitted with complaints of productive cough and an abnormal shadow in the chest X-ray. Sputum cytology showed both squamous cell carcinoma and adenocarcinoma cells. Malignant nonepithelial cells were identified in the needle biopsy specimen. Right middle and lower lobectomy was performed to reveal a carcinosarcoma by microscopic and immunohistochemical examination. This case was histologically unusual because it presented multiple carcinomatous elements (squamous cell carcinoma and adenocarcinoma). It was classified as peripheral type according to the clinical classification, showing an extremely poor prognosis. The histogenesis and clinical features of this rare tumor are discussed.

The protective effect of preconditioning on reperfusion-induced arrhythmia is lost by treatment with superoxide dismutase.

The purpose of this study was to investigate the relation between free radicals and the effect of preconditioning on reperfusion arrhythmias using the isolated working rat heart model. Rats were divided into 3 groups: control, preconditioned without superoxide dismutase (SOD) and preconditioned with SOD (n = 8 each group). The incidence of reperfusion-induced ventricular fibrillation (Vf) in the preconditioned without SOD group was reduced from 100% to 13% (p < 0.005 vs control). This protective effect of preconditioning was not observed in the SOD-treated group (incidence of Vf increased from 13% to 75%, p < 0.025). The protective effect of preconditioning on reperfusion arrhythmias is related to the presence of oxygen free radicals which occur during the reperfusion period of the preconditioning phase.

Two cases of scimitar variant.

The scimitar sign is characteristic of partial anomalous pulmonary venous drainage into the inferior vena cava (IVC). We encountered two variant cases of scimitar sign. In one case, the scimitar vein entered both the IVC and the left atrium (LA) without any intracardiac shunts. Surgical repair was made by simple ligation of the scimitar vein to correct the left to right shunt. Retrograde balloon occlusion angiography of the scimitar vein was diagnostic. In the other case, the scimitar vein showed a meandering course, and then drained into the LA without any connection with the IVC, and surgical intervention was not required.

[Determination of urinary metabolites of organic solvents among chemical industry workers].

With the revision made to the Regulation on the Prevention of Organic Solvent Poisoning in June 30, 1989, analysis of urine must be made for metabolites as a part of the health examination of workers exposed to xylene, N,N-dimethylformamide, styrene, 1,1,1-trichloroethane, tetrachloroethylene, trichloroethylene, toluene, and n-hexane. In accordance with this Regulation, health examinations were conducted at eight factories of five chemical manufactures in 1990. This report presents the results obtained for 1,073 workers as compiled by the Committee of Industrial Health of the Japan Urea and Ammonium Sulfate Industry Association. 1) The results of data analysis have shown that workers whose urine was collected when they had been exposed to an imperceptible degree to organic solvent or who underwent urine collection during an unknown period of time after exposure numbered 615 or 57.3% of all the workers. 2) Workers whose urine was collected within about 5 hours after exposure numbered 458 (42.7%). The metabolite concentrations in these workers were significantly lower than the BEI values. 3) It may be concluded from the results that workers were exposed to organic solvents to an extremely small extent during organic solvent handling operations at chemical industry factories which maintain good control over toxic materials.

Chronic subdural hematoma associated with middle fossa arachnoid cysts--three case reports.

Three patients with chronic subdural hematoma associated with middle fossa arachnoid cyst were treated by irrigating the hematoma through burr holes, because the symptoms were considered mainly due to increased intracranial pressure caused by the subdural hematoma. The symptoms disappeared immediately afterwards, so no surgery for the middle fossa arachnoid cyst was done. The patients were followed by magnetic resonance imaging. No subdural hematoma recurred during a postoperative period of 11 months to 2.5 years, and the arachnoid cyst reduced in size in two patients. We recommend irrigation of the subdural hematoma as the initial procedure of choice for such cases.

Absence of mutagenicity in peripheral lymphocytes of workers occupationally exposed to methyl methacrylate.

Chromosome aberration rates and sister chromatid exchange frequency were examined in the peripheral lymphocytes of 38 male workers who were engaged in organic glass production and exposed to methyl methacrylate (MMA) vapors at the concentrations of 0.9 ppm to 71.9 ppm. The results were compared with the findings in the concurrent nonexposed male subjects. Comparison of the exposed group with the nonexposed controls showed that there were no exposure-related changes in chromosome aberration rate. SCE frequency was higher in the exposed group than in the controls, but this was considered to be due to higher ages of the former group than that of the latter. In fact, selection of nonsmokers and further classification of the exposed nonsmokers into two groups of those with exposure below and above a median MMA concentration (ca. 4 ppm) failed to show any difference among the three nonsmoking groups in cytogenetic parameters, or any dose-dependency. The present results, although in a limited number of subjects, indicate that occupational methyl methacrylate exposure under the conditions studied is not associated with mutagenicity. This conclusion confirms the absence of mutagenicity of methyl methacrylate in humans, and is in general agreement with a majority of the results of studies on mutagenicity in vitro, animal carcinogenicity and occupational cancer epidemiology of methyl methacrylate.

[Clinical study of Mallory-Weiss syndrome in the aged patients over 75 year--mainly five cases induced by the endoscopic examination].

Cases of Mallory-Weiss syndrome aged over 75 year were analyzed with regard to their clinical features. In a 5-year period, there were 9 such cases, representing 19% of the total number of Mallory-Weiss syndrome cases. Four patients had a history of retching or vomiting, but in the other 5 patients, Mallory-Weiss tears occurred during endoscopic examination with slight vomiting reflex or even almost vomiting reflex. The latter cases had a spindle-shaped tear which was located on the lesser curvature at the cardia of the stomach. All patients were managed conservatively by observation without endoscopic hemostatic procedure, because the amount of bleeding was small. The 5 patients were thin and three of them had chronic atrophic gastritis. Mucosal weakness is an important etiologic factor in aged patients who develop Mallory-Weiss syndrome.

Hand-arm vibration exposure and the development of vibration syndrome.

To evaluate the circulatory disturbances, sensory disturbances and damage to muscles and joints by chain saw vibration exposure, the process of the deterioration of the symptoms with chain saw operating time was studied. Subjects were classified into eight groups according to TOT (Total Operating Time): Group 0, 46 controls; Group A, 39 operators (< 2,000 hours); Group B, 53 operators (2,000-4,000 hours); Group C, 45 operators (4,000-6,000 hours); Group D, 29 operators (6,000-8,000 hours); Group E, 31 operators (8,000-10,000 hours); Group F, 35 operators (10,000-15,000 hours); and Group G, 34 operators (> 15,000 hours). The subjective symptoms and clinical findings due to operating chain saws were divided into three main categories of peripheral circulatory disturbances, sensory disturbances and damage to muscles and joints. According to the criteria, the total score of each disturbance was calculated per individual in Group 0 and Groups A through G, respectively. The scores for the three (circulatory, sensory, muscles and joints) disturbances increased significantly with the increase of TOT. The scores for circulatory disturbances increased significantly in Group A and B, as compared with those in its previous Group, respectively. The scores for sensory disturbances increased significantly in Groups A, B, and F. The scores for damage to muscles and joints increased significantly in Group B.
