[A comparative morphologic study of Carabelli cusp between Chinese and Japanese students.].

The study of teeth,dentition and occlusion is helpful to guide oral medical clinic and the study of anthropology.Our department of Osaka Dental University and had many items of research work about teeth,dentition and occlusion between Chinese and Japanese students.This article reports the study of Carabelli cusp.The results revealed:(1)the rate of Carabelli cusp is higher in Japanese than in Chinese and is higher in male than in female.(2)the rate of Carabelli cusp of the first maxillary molar of both side is very high.(3)The rate of Carabelli cusp is related to the height and the type of cusp numbers.These all illustrated the occurence of Carabeiil cusp were influenced by race, individual and the development of teeth.

[Low-dose consecutive intra-arterial infusion of CDDP to treat advanced cervical carcinoma: usefulness of a vascular access device with alteration of intrapelvic blood flow].

Percutaneous transarterial implantation of a vascular access device (VAD) was used to treat advanced cervical carcinoma in 12 patients (IIb 3, IIIb 5, IVa 4). After altering intrapelvic flow by embolization under pelvic angiography, a 5 F catheter was placed in the internal iliac artery on the side of tumor dominance. RI angiography with 99mTc-MAA via the VAD showed 99% higher accumulation in the anterior division than in the posterior division of the pelvis. Consecutive infusion of cisplatin, 12.5 mg/body, via the VAD showed less toxicity than systemic chemotherapy. Overall response rate was 75% (CR 4, PR 5, NC 3). Curative surgical treatment was successful in nine of these 12 patients. This technique was evaluated to be effective for treatment of advanced cervical carcinoma.

[MR imaging of infected spondylodiskitis: interval signal change in vertebral body (including enhanced image of vertebral body)].

A retrospective evaluation of MRI was carried out in 22 patients with spondylodiskitis. Low signal intensity on T1-weighted images and high or low signal intensity on T2-weighted images were observed. Sixteen patients underwent follow-up examinations after the initial examination. Various signal changes in the vertebral body were recognized. The longer the period of follow-up, the less abnormal the signal intensity became on both T1- and T2-weighted images. Several cases showed no change in signal intensity. In all of 17 patients re-examined after injection of Gd-DTPA, infected vertebral bodies were enhanced. We classified the enhancing patterns of infected vertebral bodies in three types: A) homogeneous, B) homogeneous with strong enhancement adjacent to the end plate, C) inhomogeneous. Pyogenic spondylodiskitis tended to show patterns A or B, and tuberculous spondylodiskitis pattern C. The enhancement effect disappeared after normalization or remission of laboratory findings. In two cases, the enhancement effect remained even after laboratory findings became normal. Contrast MRI was useful for understanding infectious state.

[Determinations of microproteinuria in early diabetic nephropathy].

Microalbuminuria, transferrin and IgG were determined in the urine of diabetic patients with normal renal function. Microalbuminuria was assayed by the immunoturbidity method, transferrin by the latex agglutination method and IgG by the ELISA method using biotin labelled IgG.HRP labelled protein G. The correlation between microalbuminuria and urinary transferrin concentrations was high (r = 0.902). However, a low titer of transferrinuria was observed, (25.5%) among the patients with microalbuminuria; a wider distribution of transferrinuria was noted than microalbuminuria. Determination of transferrinuria did not appear to be superior to the determination of microalbuminuria in early diabetic nephropathy. The urinary IgG levels assayed by ELISA using biotin labelled IgG.HRP labelled protein G method were distributed from zero to 5 mg/g Cr in the patients with normoalbuminuria. The reference intervals of the urinary IgG concentration should be settled below 5 mg/g Cr. In the patients with microalbuminuria, the concentration of urinary IgG showed a wide variation and no correlation between concentrations of microalbuminuria and urinary IgG was noted (r = 0.02). Determination of urinary IgG is not useful as an early diagnostic procedure for diabetic nephropathy.

[Clinical study of bronchial asthma in adult, intractable asthmatics after introduction of guideline therapy].

Introduction of Guideline for asthma treatment proposed by the committee of Japanese allergology have a tremendous impact on patients with bronchial asthma. Intractable asthmatics who have had to take some oral steroid to overcome disease severity, may have also some merit by this treatment, so that some of them might be no longer considered as intractable asthmatics. To clarify this, multicenter study was conducted. In this study, a case who have had more than 5 mg of prednisolone and/or 800 mu g of beclomethasone dipropionate throughout the year, was diagnosed as intractable asthmatics. In 845 case, 14.7%, 123 cases were diagnosed as intractable. These cases were significantly to be non-atopic and adult onset. Also, they have a significant tendency to be deteriorated by infection and careless drug administrations. Using the multiquantification method to examine the most powerful factor on intractable asthmatics, type of asthmatics was the most important and the past history of severe attack was the second. When intractable asthmatics diagnosed mainly by their BDP usage (BDP-intractable) were compared with intractable diagnosed by oral PSL (PSL-intractables), BDP-intractables were significantly atopic compared to PSL-intractables.

Molecular cloning of cDNA encoding human DNA helicase Q1 which has homology to Escherichia coli Rec Q helicase and localization of the gene at chromosome 12p12.

A complementary DNA encoding DNA-dependent ATPase Q1 possessing DNA helicase activity, which is the major DNA-dependent ATPase in human cell extracts, was cloned from a cDNA library of human KB cells. The predicted amino acid sequence has seven consecutive motifs conserved in the RNA and DNA helicase super family and DNA helicase Q1 belongs to DEXH helicase family. A homology search indicated that helicase Q1 had 47% homology in the seven conserved regions with Escherichia coli RecQ protein. Three RNA bands of 4.0, 3.3, and 2.2 kilobases were detected in HeLa cells by Northern blotting. Analysis of the genomic DNA indicated the presence of a homologous gene in mouse cells. The DNA helicase Q1 gene was localized on the short arm of human chromosome 12 at 12p12.

A clinicopathologic study of small flat colorectal carcinoma.

BACKGROUND: Recent studies have noted that small flat carcinomas do exist in the human colon, but their clinicopathologic features have not been fully delineated. METHODS: A clinicopathologic study of 62 flat colorectal carcinomas (FCC) and 80 polypoid colorectal carcinomas (PCC) was performed. FCC were defined for the study as colorectal adenocarcinomas confined to the mucosa or submucosa, macroscopically characterized as slightly elevated, often nearly flat, and sometimes with central shallow depressions. RESULTS: In intramucosal and submucosal invasive carcinomas, the mean size of FCCs was significantly smaller than that of PCCs, (P < 0.001). When compared with PCCs, FCCs were found more often in the proximal colon, less frequently well differentiated, and had fewer adenomatous remnants. In submucosal invasive cases, FCCs showed more frequent deep invasion (67 versus 32%, P < 0.05) and lymphovascular permeation (41% versus 16%, P < 0.05) compared with PCCs. CONCLUSIONS: These results suggest that FCCs are characterized by frequent deep invasion with lymphovascular permeation even when small, and that they may be more prevalent away from the rectosigmoid area.

[Ability of subtraction and dynamic MR imaging to detect breast tumors: comparison with ultrasonography and mammography].

We evaluated the ability of subtraction and dynamic MR imaging to accurately detect breast tumors. Sixty-five breast carcinomas and 24 fibroadenomas were examined by an SE pulse sequence using a 0.2 Tesla unit. Subtraction MR images were obtained every minute during dynamic study with Gd-DTPA. Almost all breast tumors were seen as very bright masses, and the margin of the mass was clearly demonstrated on subtraction MR images. Breast carcinomas and fibroadenomas showed characteristic time-intensity curves on dynamic study, Time-intensity curves of the early peak type and plateau type were seen in 97% of breast carcinomas, while the gradually increasing type was seen in 92% of fibroadenomas. The detectability of breast carcinoma was 98% by MRI, 98% by ultrasonography, and 87% by mammography. That of fibroadenoma was 95% by MRI, 91% by ultrasonography and 60% by mammography. Sensitivity and specificity for breast carcinoma were 98% and 92% for MRI and 97% and 71% for ultrasonography. For fibroadenoma, they were 96% and 98% for MRI and 89% and 92% for ultrasonography.

[Ultrasonography in primary malignant lymphoma of the thyroid].

We analyzed retrospectively the sonographic appearance of eight cases of primary malignant lymphoma of the thyroid. Sonography showed hypoechoic masses replacing thyroid parenchyma with posterior acoustic enhancement. A hypoechoic and/or nearly anechoic confluent nodular appearance characterized malignant lymphoma of the thyroid.

Early detection of nonpolypoid cancers in the rectal remnant in patients with familial adenomatous polyposis/Gardner's syndrome.

BACKGROUND: Invasive cancer occurs in the rectal remnant of patients with familial adenomatous polyposis (FAP). METHODS: A long term surveillance proctoscopy program was performed on 25 patients with an established diagnosis of FAP or Gardner's syndrome. RESULTS: The surveillance revealed small nonpolypoid cancer of the rectal remnant in two patients. One cancer, which measured 5 mm, was restricted to the mucosa, whereas the other, measuring 10 mm at its greatest dimension had invaded the submucosa. On proctoscopy, both the lesions were characterized by a reddish depression, surrounded by marginal elevations. Both of these cancers were composed of well differentiated adenocarcinoma without any adenomatous component. CONCLUSIONS: The authors' experience suggests that nonpolypoid cancers do exist in FAP/Gardner's syndrome and that careful follow-up seems necessary in patients with a diagnosis, especially in consideration of the possible development of nonpolypoid lesions.

[Relationship between oral food intake and nausea caused by intravenous injection of iodinated contrast material].

From December, 1990 to July, 1992 we investigated the relationship between the time after oral food intake and nausea and vomiting caused by the intravenous injection of iodinated contrast media in 2,414 patients who underwent contrast enhanced CT. The contrast media used were as follows: amidotrizoic acid (high-osmolality contrast medium, HOCM, iodine 292 mg/ml) in 1173 patients, iopamidol (low-osmolality contrast medium, LOCM, iodine 300m g/ml) in 641 patients, and iohexol (LOCM, iodine 300 mg/ml) in 600 patients. All the patients had an intravenous injection of 100ml in volume. The overall incidence of nausea and vomiting was 3.8% (96/2412 patients). The incidence was 6. 7% (79/1173 patients) in the HOCM group and 1.4% (17/1241 patients) in the LOCM group. The incidence of nausea and vomiting increased with the interval between the oral intake of food and the intravenous injection of contrast medium. It is concluded that fasting before contrast-enhanced CT enhances the adverse effect of nausea and vomiting.

A multivariate analysis of assessment of myometrial invasion of endometrial carcinoma by magnetic resonance imaging.

Thirty-nine cases of endometrial carcinoma were analyzed by magnetic resonance imaging (0.15 or 1.5 tesla (T) MRI) prior to surgery to evaluate the depth of myometrial invasion of endometrial carcinoma. Several diagnostic parameters of MRI were set: maximum area of high-intensity area (HIA) in sagittal (HIASI) and transverse (HIATI) images of the uterus, minimum width of remaining myometrium (MWRM), and ratio of minimum to maximum width of normal myometrial thickness (RMMT). In analysis of correlation between myometrial invasion and MRI parameters, the correlation coefficient values of HIASI, HIATI, and MWRM were higher than those of other parameters at 0.71, 0.69, and -0.74, respectively. When cases were classified into three groups according to tumor invasion (tumor limited to endometrium, invasion to less than 50% of the myometrium, invasion to greater than 50% of the myometrium), MWRM was the most useful as a single parameter. Since HIASI and MWRM were independent parameters of myometrial invasion, they were selected for multivariate analysis. An equation to calculate the predicted depth of myometrial invasion was derived using both HIASI and MWRM by the multiple linear regression model. The equation could estimate the degree of myometrial invasion from the parameters of 1.5 T MRI. The values obtained from the equation are more useful for prediction of myometrial invasion of endometrial carcinoma than single parameters in both 0.15- and 1.5-T MRI.

Changes of antigen expression on human hepatoma cell lines caused by sodium butyrate, a differentiation inducer.

We have previously reported the effect of a differentiation inducer, sodium butyrate (SB), on human hepatocellular carcinoma (HCC) cell lines, demonstrating that it was a potent inducer of differentiation. In the present study, we investigated the alteration in expression of an antigen defined by a murine monoclonal antibody, H2, as well as alterations in the expression of other antigens, on the HCC cell lines HCC-T, HCC-M, and PLC/PRF/5, since it is known that specific antigenic changes occur during the differentiation of leukemic cells. The expression of the antigen defined by H2 increased immunocytochemically on HCC-T, HCC-M, and PLC/PRF/5 during treatment with SB. A flowcytometric study showed that almost all the HCC-T and HCC-M cells treated with SB highly expressed this antigen after 5 days' treatment. The antigen expression detected by H2 decreased after the removal of SB from the medium. On the other hand, antigen expression detected by another monoclonal antibody, 5C11, was not changed by this treatment. The expression of intracellular adhesion molecule (ICAM)-1 in HCC-T increased slightly, but that of beta 2-microglobulin and HLA-DR did not change. These results demonstrated that some antigen expression was changed by SB treatment and that the antigen defined by H2 seemed to be highly expressed on human HCC cells in the differentiated state.

Isolation and characterization of cDNAs encoding imidazoleglycerolphosphate dehydratase from Arabidopsis thaliana.

cDNA clones encoding imidazoleglycerolphosphate dehydratase (IGPD; EC 4.2.1.19) from Arabidopsis thaliana were isolated by complementation of a bacterial auxotroph. The predicted primary translation product shared significant identity with the corresponding sequences from bacteria and fungi. As in yeast, the plant enzyme is monofunctional, lacking the histidinol phosphatase activity present in the Escherichia coli protein. IGPD mRNA was present in major organs at all developmental stages assayed. The Arabidopsis genome appears to contain two genes encoding this enzyme, based on DNA gel blot and polymerase chain reaction analysis.

Depressed adenoma of the colorectum: analysis of proliferative activity using immunohistochemical staining for proliferating cell nuclear antigen (PCNA).

Recently, depressed adenomas (DA) of the colorectum have been frequently detected in Japan. The natural history and carcinogenesis of these DA remain unclear. The histological features and cell kinetics of 13 DA were analyzed, using proliferating cell nuclear antigen (PCNA), and compared with 24 non-depressed adenomas (NDA), 12 non-neoplastic mucosa and 12 adenocarcinomas. The DA had a characteristic architecture when compared with the NDA, as demonstrated by a crowded collection of adenomatous tubules with small diameters. The adenomatous tubules tended to occupy the full thickness of the lamina propria at the center with superficial growth to the periphery. The PCNA-positive cells were mostly restricted to the lower half of the crypts in the non-neoplastic mucosa, and were distributed predominantly across the upper half area of the crypts in both the DA and NDA, whereas they were randomly distributed across any area in the adenocarcinomas. The mean labeling index of PCNA in the DA (43.2%) was significantly higher than that in the non-neoplastic mucosa (26.5%) and NDA (29.3%), but lower than that in the intramucosal component of the adenocarcinomas (70.0%). The DA should be seriously considered as a precancerous lesion because of its unusual histologic architecture and high proliferative activity.
