[Diagnosis of gastrointestinal amyloidosis with special reference to the relationship with amyloid fibril protein].

Forty-five patients with proved gastrointestinal amyloidosis were examined to study the clinical and pathological features and to determine the correlation with amyloid fibril proteins. The examinations included physical examination, laboratory study, plain X-ray film of the abdomen, gastrointestinal radiography, gastrointestinal endoscopy, endoscopic biopsy, surgery, and autopsy. The results were as follows: 1) Amyloid fibril proteins consisted of amyloid light chain protein (AL) in 7 patients, amyloid A protein (AA) in 36, beta 2-microglobulin (AH) in one, and prealbumin (AF) in one. 2) At the time of examination, gastrointestinal symptoms were present in 43 (96%) of the 45 patients. The incidence of diarrhea and gastrointestinal bleeding was higher in patients with the AA type than in those with other types, whereas abdominal distension, nausea, and vomiting were more frequently evident in patients with the AL type. 3) The incidence of hypoalbuminemia, hypokalemia, and positive occult blood was higher in patients with the AA type, whereas abnormal electrocardiogram and cardiac failure were more frequently observed in patients with the AL type. The thyroid gland was enlarged in 7 patients with the AA type, and the macroglossia was evident in only 2 patients with the AL type. 4) The frequency of radiographic abnormalities in the gastrointestinal tract was as follows: 79% of the patients in the jejunum and ileum, 61% in the duodenum, 53% in the stomach, 32% in the colorectum, and 14% in the esophagus. Fine granular shadows were most frequently seen in patients with the AA type, whereas multiple polypoid protrusions and thickening of the folds were characteristic in patients with the AL type. 5) Endoscopic abnormalities were found as follows: 82% in the duodenum, 81% in the jejunum, 73% in the stomach, 42% in the colorectum, and 20% in the esophagus. Endoscopy revealed fine granular appearance in 31 (86%) of the 36 patients with the AA type, whereas multiple polypoid protrusions and thickening of the folds were evident in 5 (71%) of the 7 patients with the AL type. 6) On the histological examination of the biopsy specimens, amyloid deposition was found in the duodenum in 98% of the 45 patients studied, in the jejunum in 96% of the 26 patients, in the antrum in 93% of the 41 patients, and in the rectum in 86% of the 43 patients. 7) Histological findings of the biopsy and autopsy specimens revealed that the the degree of amyloid deposition was the highest in the duodenum and jejunum of the gastrointestinal tract.(ABSTRACT TRUNCATED AT 400 WORDS)

Endoscopic removal of inflammatory fibroid polyps of the stomach.

Three cases of gastric inflammatory fibroid polyp removed by endoscopic polypectomy are reported. All lesions were located in the gastric antrum or prepyloric region, and their depth was restricted to the submucosa. The correct diagnosis was obtained, and the digestive symptoms disappeared after polypectomy. In contrast to observations in previous reports, we believe these lesions are suitable for removal by endoscopic means, provided they are small in size and strictly submucosal in location.

[Studies on IgG subclass antibodies in adult asthma. 1. Serum antigen specific IgG subclass antibodies in asthmatics with late asthmatic response].

It is clear that immediate asthmatic response is mediated by IgE-dependent mechanisms. However, late asthmatic response is induced by inhalation of antigens without antigen specific IgE antibodies in some asthmatics, especially in intractable asthma induced by Candida antigen. To elucidate the relationship between those bronchial responses and antibodies, antigen specific IgG subclass antibodies in sera from asthmatics were measured and compared with IgE antibody. The results were as follows. 1. Avidin-biotin ELISA (enzyme-linked immunosorbent assay) method was established for the measurement of specific IgG and IgG subclass antibodies to mite or Candida antigen. 2. Serum levels of antigen specific IgG and IgG1 antibodies in asthmatics with LAR provoked by mite or Candida antigen were significantly higher than those in asthmatics without LAR (p less than 0.01). 3. Serum levels of specific IgE antibody to these antigens in asthmatics with LAR provoked by mite or Candida antigen were slightly lower than those in asthmatics without LAR, though the difference is not significant. These results suggest that high serum levels of specific IgG and IgG1 antibodies to these antigens play a role in inducing LAR in asthmatics with LAR.

The formation of oxalate from glycolate in rat and human liver.

In this study, we attempted to elucidate the metabolic pathway and enzymes actually involved in oxalate formation from glycolate in rat and human liver. In rat liver, the formation of oxalate from glycolate appeared to take place predominantly via glyoxylate. The oxalate formation from glycolate observed with crude enzyme preparations was almost entirely accounted for by the sequential actions of glycolate oxidase and xanthine oxidase (XOD) or lactate dehydrogenase (LDH). Under the conditions used, no significant activity was attributable to glycolate dehydrogenase, an enzyme reported to catalyze the direct oxidation of glycolate to oxalate. Among the three enzymes known to catalyze the oxidation of glyoxylate to oxalate, glycolate oxidase and XOD showed much lower activities (a higher Km and lower Vmax) toward glyoxylate than those with the respective primary substrates. As to LDH, none of the LDH subunit-deficient patients examined showed profoundly lowered urinary oxalate excretion. Based on the results obtained, the presumed efficacies in vivo of individual enzymes, as catalysts of glyoxylate oxidation, and the in vivo conditions assumed to allow their catalysis of oxalate production are discussed.

Peptide mapping of the four subunits of the mouse DNA polymerase alpha-primase complex.

We report a simple, two-step method (phosphocellulose and immunoaffinity column chromatographies) for purification of the mouse DNA polymerase alpha-primase complex. The advantages of this method over other procedures are its simplicity and rapidity, with little loss by proteolysis. Sedimentation analysis in a glycerol density gradient of the immunoaffinity-purified fraction revealed that four polypeptides with molecular weights of 180,000, 68,000, 54,000 and 46,000 in the enzyme fraction form a physical complex. Peptide mapping by reversed phase-high performance liquid chromatography demonstrated unequivocally that these four polypeptides constituting the complex are different entities.

Barium meal study for amyloidosis of the small intestine: measurements on radiograph.

In order to determine barium meal radiographic findings characteristic of amyloidosis, we measured the jejunal diameter, valvular width, and intervalvular distance in 25 patients with small bowel amyloidosis and in 30 control individuals, and compared the two groups with each other. As a result, jejunal diameter demonstrated no difference between amyloidosis cases and controls, while there was a significant difference in valvular width or intervalvular distance between the two groups. Our results indicate that objective estimates of the thickening of the valvulae and the shortening of the intervalvular distance by means of measurements of the given roentgenograms greatly contribute to the clinical diagnosis of this disease.

Infarction of the conus medullaris.

We encountered a patient with acute cauda equina syndrome. The clinical course, laboratory data and imaging study, including magnetic resonance imaging (MRI) strongly suggested that this episode was caused by a vascular disorder of the conus medullaris. We emphasized the diagnostic usefulness of MRI in the vascular disease of the conus medullaris as well as the clinical importance of this disease in the differential diagnosis of acute cauda equina syndrome.

Isolation of multiple cytomegalovirus strains from a patient with adult T cell leukemia.

A 66-year-old male with adult T cell leukemia had an ulcer on the left medial thigh. The biopsy of the skin lesion revealed enlarged endothelial cells with acidophilic intranuclear inclusion bodies, suggesting cytomegalovirus (CMV) infection. At autopsy, CMV was isolated from a nodular skin lesion of the scrotum. The urine constantly tested positive for CMV. Restriction endonuclease cleavage analysis of DNA of the isolates from the skin and urine indicated that this patient was infected with two different strains of CMV.

[Evaluation of mica crystal glass ceramic crown marginal fitness].

The purpose of this study is to investigate the influence of three marginal shape of abutment teeth (Flat, Moderate and Steep) on the fitness of castable glass ceramics (DICOR) crown clinically. The following results were obtained: 1. The change of width of three marginal shapes showed the same tendency on each firing procedure. 2. The vertical change of three marginal shapes showed the most constant on Flat and decreased on Steep. 3. On the marginal edge, the Labiopalatal gaps were larger than Mesiodistal gaps. 4. On Labiopalatal section, the gaps of Steep were larger than those of Flat and Moderate especially as cast. 5. The gaps Flat were almost same on Labial, Palatal, Mesial and Distal parts.

[Treatment of four cases of B-cell lymphoma].

Four cases of B cell lymphoma, seen at the Department of Dermatology, Miyazaki Medical College, for the previous approximately 10 years, were reviewed. Case 1 was characterized by a localized tumours lesion on the head; the histologic picture was of the follicular, medium sized cell type. Complete remission for 8 years has been obtained after electron beam therapy and surgical removal of the tumor. Cases 2 and 3 had a histologic picture of diffuse, large cell type and were assumed to have a bad prognosis with standard chemotherapy. In addition to conventional chemotherapy, they were placed on noncross-resistant alternating combination chemotherapy with medium dosages of MTX, VP-16, VDS, PCZ MXT, PEP and ACR, so as not to develop cross resistance. Case 2 has been in complete remission for 11 months and case 3 for 7 months. Case 4 was a nodal lymphoma with skin lesions. This patient was treated mainly by local radiation therapy, but later died with leukemic changes and renal failure. Cutaneous B cell lymphoma should be treated vigorously and carefully, since it has a poorer prognosis than that of lymphoma of lymph node origin.

Granular cell tumor of the esophagus: endoscopic ultrasonographic demonstration and endoscopic removal.

A 35-yr-old Japanese man with a granular cell tumor of the esophagus that was removed by endoscopic polypectomy is presented. Radiography and endoscopy showed a 20 x 12 mm sessile protrusion in the distal esophagus. Endoscopic ultrasonography demonstrated the hypoechoic mass in the submucosa without continuity to the muscularis propria. The lesion was successfully treated by endoscopic polypectomy without complications. The cross-sections of the resected specimen were quite in agreement with the ultrasonographic findings. Endoscopic ultrasonography is valuable to assess the exact location and extent of the tumor, and to determine the indication for endoscopic polypectomy.

[CT of abdominal blunt trauma].

We studied CT findings and Interventional Radiology including therapeutic procedures in 43 cases with abdominal blunt trauma, retrospectively. All of parenchymal organ's injuries, and injuries of duodenum & retroperitoneum were correctly diagnosed by CT. In 14 cases with only hemoperitoneum or no positive findings on CT, 4 cases were jejunal perforations, and remaining 10 cases were conservatively treated and relieved. We have to take care that the similar CT findings include the opposite results to need surgery and not.

[Characteristic of cell components in bronchoalveolar lavage fluid (BALF) in patients with bronchial asthma classified by clinical symptoms].

The characteristics of cell components in BALF were examined in three groups of 104 patients with bronchial asthma, classified by clinical symptoms. 1. The frequency of neutrophils in BALF was significantly higher in patients with hypersecretion type (Ib) and in cases with bronchiolar obstruction type (II) than in cases with simple bronchoconstriction type (Ia). 2. A significantly higher proportions of neutrophils in BALF was observed in atopic asthmatics with type Ib than in non-atopic asthmatics with type Ib. 3. More frequency with moderate or high grade of eosinophilia in BALF were observed in cases with type Ib, whether they were atopic or non-atopic type. 4. More frequency with moderate or high grade of neutrophilia in BALF were observed in atopic cases with Ib and in both atopic and non-atopic cases with II. 5. Cases with moderate and high grade of both eosinophilia and neutrophilia in BALF were more frequent in atopic cases with Ib. The results suggest followings--1) both eosinophils and neutrophils participate in hypersecretion of type Ib in atopic cases, and only eosinophils in non-atopic cases. 2) neutrophils participate in bronchiolar obstruction of type II, whether they are atopic or non-atopic cases.

[Temporal changes in internal echoes in hepatic hemangiomas].

We have discovered a fact that there is a temporal changes of internal echo levels in hepatic hemangiomas. Nine hemangiomas in 6 cases diagnosed by CT were studied ultrasonographically by the real-time scanner during the observation period from 4 to 30 minutes. Low echo portions occupying within the tumor (low echo level ratio) showed temporal changes in 8 of 9 hemangiomas during this period. It is important to take temporal factors into consideration for the ultrasonographic diagnosis of hepatic masses. We coined the term "wax and wane" sign to the temporal changes of internal echo levels in the hepatic hemangioma.

Ultrasonographic findings in gastric cancer: in vitro and in vivo studies.

Ultrasonography was performed in 45 cases of gastric cancer. Specimens from all 45 cases of gastric cancer were subjects to ultrasonographic study by the water immersion method for comparison with histology. In 32 of these 45 cases in vivo ultrasonographic evaluation was performed prospectively. The overall accuracy rates for the diagnosis of the depth of cancerous invasion were almost 80% in both in vitro and in vivo studies. In vivo ultrasonographic findings agreed well with those from the specimen studies. Ultrasonography was considered to be useful in the diagnosis of gastric malignancies.

The value of double-contrast barium enema in amebic colitis.

We report here our findings on the double-contrast barium enema given to 8 patients with amebic colitis and a comparison is made with endoscopic features. A definite diagnosis of amebic colitis was made by fecal examination or by endoscopic biopsy. Radiologic findings included fine marginal serration, aphthoid ulcers, minute barium flecks, marginal defects, loss of haustration, and deformities of the bowel. Aphthoid ulcers and marginal defects are both characteristic of amebic colitis. The double-contrast barium enema is most useful for differentiating amebiasis from other forms of colitis.
