Emergency treatment of rupture of the proximal anastomosis of an infected thoracoabdominal aortic graft with bleeding into a postpneumonectomy empyema cavity.

The thoracic part of a huge thoracoabdominal aortic aneurysm was resected. A prosthetic graft was implanted between the descending aorta and the iliac arteries. The celiac, superior mesenteric, and the renal arteries were connected to the aortic prosthesis with two bifurcation grafts. A life-threatening hemorrhage into an empyema cavity occurred postoperatively. An emergency two-stage procedure was employed involving: (1) exclusion of the infected intrathoracic segment and exclusion of an infected sutured aortic stump; (2) subsequent removal of necrotic and infected tissue, including the excluded segment of the aorta and the graft in a second operation. The patient recovered and is now well and works full-time. Angiography 3 years after the operation shows that all anastomoses are patent. A scheme of management of these rare cases is proposed.

Management of postinfarction ventricular septal rupture.

Thirty-five patients were operated on for ventricular septal rupture after acute myocardial infarction (AMI). The overall operative mortality rate was 46%. Hemodynamic and clinical stability was achieved in 14 patients preoperatively. They were operated on after 4 weeks or more. Late development of cardiac failure was the main cause of an operative mortality rate of 21%. From a surgical point of view there are no reasons to delay the operation of these patients for more than 4-8 weeks. The operative mortality rate among 21 patients in cardiogenic shock was 62%. The present study shows, however, that it was possible to save lives by emergency operation of patients who otherwise would have died of cardiogenic shock. The time-interval from diagnosis to surgical treatment, and the preoperative condition of the patients appeared to be important prognostic factors. The location of the myocardial infarction did not significantly influence the prognosis in this series. The 19 long-term survivors returned to normal life, although 8 of them had a small residual shunt. More than half of the patients, who were examined, had multivessel disease. The operative mortality rate for those patients without concomitant coronary artery bypass grafting (CABG) was 80%. Technical failures and some new surgical principles are presented. The indications for IAPB and coronary angiography are discussed as well as the optimum time for surgical repair. The clinical course varies greatly and calls for a differentiated approach to treatment. A plan for surgical management of these patients is suggested.

Iron, zinc and folate status during pregnancy and two months after delivery.

Iron, zinc and folate statuses of 45 women were determined during pregnancy around 12, 20, 28, 32 and 36 weeks, and again 2 months after delivery. Analyses of plasma ferritin, Hb, MCV, folate and zinc in plasma and whole blood were performed. Iron supplementation was recommended from mid-pregnancy but 13 of the participants did not use the iron supplements. This group had significantly decreased levels of plasma ferritin and MCV at the end of pregnancy, but none developed anemia. Two months post partum the plasma ferritin of the unsupplemented group had normalized and was in the same range as in the supplemented group. The concentrations of zinc in plasma and whole blood and the calculated levels of red cells were low even at the first examination around 12 weeks of gestation, compared with non-pregnant women. Throughout the course of pregnancy the plasma zinc levels continued to decrease, while the whole blood and red cell levels showed a significant rise. At term of gestation almost half the women had subnormal plasma folate levels (L. casei), which persisted during the post partum follow-up. The corresponding value for red cell folate was 10% below normal values at term and 30% subnormal 2 months after parturition. These findings stress the importance of extending the observation period to include also the lactating period, in order to judge the need for folate supplementation.

Structural studies on the O-glycosidically linked carbohydrate chains of glucoamylase G1 from Aspergillus niger.

Glucoamylase G1 from Aspergillus niger contains an unusual type of carbohydrate-protein linkage, involving mannose O-glycosidically linked to serine and threonine. The majority of the neutral oligosaccharides of glucoamylase G1 are located in a region of about 70 amino acid residues which carries about 35 oligosaccharide units [(1983) Carlsberg Res. Commun. 48, 517-527]. Structural analysis was performed on the O-linked carbohydrates of a tryptic fragment from glucoamylase G1 comprising the segment characterized by a high degree of glycosylation. The carbohydrate structures released by trifluoroacetolysis were elucidated using sugar analysis, methylation analysis, mass spectrometry, chromium trioxide oxidation, digestion with alpha-mannosidase and 1H-NMR spectroscopy. The following structures could be identified. (formula; see text)

Degradation of chondroitin 4-sulphate by tri-fluoroacetolysis: isolation of oligosaccharides from the carbohydrate-protein linkage region.

Oligosaccharides from the linkage-region tetrasaccharide, beta-D-GlcpA-(1----3)-beta-D-Galp-(1----3)-beta-D-Galp-(1----4)-D- Xylp, of chondroitin 4-sulphate were isolated after trifluoroacetolysis. The oligosaccharides were purified by ion-exchange chromatography and paper chromatography and subjected to sugar and methylation analysis and g.l.c.-m.s. The recovery of linkage-region oligosaccharides was approximately 45% after trifluoroacetolysis, calculated according to the D-xylose present in the chondroitin 4-sulphate preparation. The following structures were identified: beta-D-Galp-(1----4)-D-Xylp, beta-D-Galp-(1----3)-D-Galp, beta-D-Galp-(1----3)-beta-D-Galp-(1----4)-D-Xylp, beta-D-GlcpA-(1----3)-beta-D-Galp-(1----3)-beta-D-Galp-(1----4)-D- Xylp.

Synthesis of the disaccharide 6-O-beta-D-galactopyranosyl-2-acetamido-2-deoxy-D-galactose using immobilized beta-galactosidase.

The disaccharide 6-O-beta-D-galactopyranosyl-2-acetamido-2-deoxy-D-galactose has been synthesized by transfer of the beta-D-galactopyranosyl residue from lactose to 2-acetamido-2-deoxy-D-galactose utilizing the transferase activity of beta-galactosidase from E. coli. To make the enzyme reusable, it was applied in an immobilized form covalently bound to Sepharose CL-4B. The yield of the disaccharide was about 20%, calculated on the amount of acetamido-deoxy-D-galactose added. The disaccharide could also be obtained by reversal of the hydrolytic activity of the enzyme, using D-galactose and 2-acetamido-2-deoxy-D-galactose as substrate. The yield in this reaction, however, was only 2-3% under the conditions applied.

Haemodynamic effects of a single large dose of insulin in open heart surgery.

The haemodynamic effects of a single dose of 0.35, 1.5 or 7.5 IU fast acting insulin X kg bw-1 were studied in 24 patients undergoing aortocoronary bypass grafting. No inotropic drugs were used. Blood glucose was measured using a continuous blood glucose monitoring system, and was kept at the preinsulin level by administration of a 40% glucose solution. Injection of 0.35 or 1.5 IU X kg bw-1 of insulin did not have significant haemodynamic effects. Injection of 7.5 IU X kg bw-1 of insulin resulted in significant changes in cardiac index (+20.8%) and in total peripheral resistance (-13.9%) within 2 min. After 10 min a reduction of 16.8% was found in the diastolic pulmonary artery pressure. These haemodynamic effects occurred before glucose had been infused. The arterial pressure and the heart rate were unaffected. It is concluded that the injection of 7.5 IU X kg bw-1 of insulin results in an increase in cardiac index in patients who have undergone open heart surgery. The effects are not primarily related to stimulation of glycolysis.

Oligosaccharide structures mediating agglutination of sheep erythrocytes by Staphylococcus saprophyticus.

Agglutination of sheep erythrocytes by Staphylococcus saprophyticus was used as a model system for adherence studies. Glycolipids were isolated from sheep erythrocyte membranes, and oligosaccharides were prepared by trifluoroacetolysis. The oligosaccharides were characterized by sugar analyses, methylation analyses, gas-liquid chromatography-mass spectrometry, and nuclear magnetic resonance spectroscopy. We showed that oligosaccharides containing terminal beta-D-galactose-p-(1-4)-beta-D-2-acetamido-2-deoxyglucose-p-(1- were good inhibitors of the hemagglutination of sheep erythrocytes by S. saprophyticus.

Specific cleavage of the glycosidic bond between the carbohydrate and ceramide portions in glycosphingolipids using trifluoroacetolysis.

A number of glycosphingolipids have been subjected to trifluoroacetolysis. Heating the glycosphingolipid at 100 degrees C for 48 h in mixtures of trifluoroacetic acid (TFA) and trifluoroacetic anhydride (TFAA) varying from 1:1 to 1:100 (v/v) resulted in complete cleavage of the glycosidic bond between the carbohydrate and the unsaturated base of the ceramide. The fatty acid(s) linked to the amino group of sphingosine were also released by transamidation. The carbohydrate portion was stable under these conditions due to stabilization of the glycosidic bonds by the electronegative O-trifluoroacetyl groups. The 2-acetamido-2-deoxy-hexose units in the glycosphingolipids were converted into their N-trifluoroacetyl analogues. The N-trifluoroacetyl functions could be removed and the 2-acetamido-2-deoxy-hexoses could be reconstituted by N-acetylation.

Haemagglutination by vaginal anaerobic curved rods and its inhibition by oligosaccharides.

Except for occasional (6/45) strains, both the short (21/24) and long (16/21) variants of anaerobic curved rods isolated from the vagina of women with bacterial vaginosis were capable of agglutinating human, guinea pig and sheep erythrocytes. The agglutination was not affected by heat treatment, i.e. up to 80 degrees C for 30 minutes, of the bacteria prior to use. Nor was the agglutination influenced by 50 mM EDTA or 50 mM D-mannose. To elucidate receptors on the erythrocyte membrane responsible for the agglutination, various free oligosaccharides and glycoproteins were tested for haemagglutination-inhibiting capacity, using representative strains of the short variant. Glycoproteins containing terminal lactosamine structures inhibited the agglutination.

Zinc, copper, magnesium, and calcium in blood and plasma after phlebotomy.

Zinc levels in plasma and whole blood were investigated in 2 groups of blood donors. The first group consisted of 19 men and 11 women who donated blood for the first time and were followed during 3 phlebotomies. The second group comprised 44 men with 9 or more previous blood donations, their blood being analysed on a single occasion. Plasma concentrations of copper, magnesium, calcium and ferritin were also analysed. The zinc levels in whole blood and plasma in the second group were significantly lower as compared to the first group (P less than 0.001). Furthermore, the second group had significantly higher levels of copper and copper:zinc quotient in plasma as compared to those of the new blood donors (P less than 0.001). Our results indicate that besides the earlier known effect of blood donation on iron stores, there are also signs of zinc depletion when judged from the zinc levels in plasma and in erythrocytes. No significant changes in calcium and magnesium concentrations were seen during phlebotomy.

Experimental induction of irreversible precancerous changes in the palatal epithelium of the rat.

The development of irreversible precancerous changes in the palatal mucosa of the rat was studied macroscopically and histologically after painting the palate 3 times a week with 0.5% 4-nitroquinoline-N-oxide for 1-22 weeks. Control animals were painted with the solvent propylene glycol. After the application periods, the spontaneous development of premalignant epithelial changes and carcinomas was recorded by repeated biopsy procedures. Short application periods (1-6 weeks) were consistently followed by prolonged expectancy periods (51-80 weeks) before the development of carcinomas. Comparably long application periods (more than 12 weeks), however, induced carcinomas after relatively short expectancy periods (8-21 weeks). The total experimental periods (application plus expectancy periods) for animals painted for more than 12 weeks were frequently less than 1/2 of that for animals painted for 1-6 weeks. All experimentally induced tumours represented highly differentiated squamous cell carcinomas with excessive keratin formation. They were consistently preceded by different stages of epithelial dysplasia. Advanced carcinomatous changes were frequently not visible macroscopically. Irreversible precancerous transformation had already occurred in epithelium at early hyperplastic, non-dysplastic stages.
