Effect of leprosy sera and foetal calf serum (FCS) on the T cell number of peripheral blood of leprosy patients.

Study was conducted in 24 cases of various types of leprosy and 10 healthy controls to find out the effect of various sera on the T cell count of peripheral blood lymphocytes by sheep erythrocyte rosetting method. The percentage of T lymphocytes in lepromatous and tuberculoid cases were significantly lower compared to that in normal healthy controls. All sera except FCS had a stimulatory effect on the number of T cells. The cells incubated for 24 hours in FCS did not show any stimulatory effect on the number of T cells, however, these FCS incubated cells showed a significant elevation in the number of T cells when further incubated in sera either from leprosy cases or from healthy subjects.

Port Moresby road traffic accident survey.

Road Traffic Accidents (RTA's) are an important cause of morbidity and mortality in Papua New Guinea today. A survey of casualties from RTA's attending Port Moresby General Hospital over a seven-month period from November 1982 to June 1983 showed that of 209 patients seen, one-third were in cars and one-quarter were in utilities. The most dangerous positions in vehicles were (1) in the open back of a utility or truck, and (2) in the front seat (driver or passenger) of a car or utility. Only 13% of injured front-seat occupants of cars or utilities were wearing a seat-belt; in one-third of vehicles no seat belts were fitted. A plea is made for the strengthening of legislation to ensure the safety of passengers travelling in the open back of trucks or utilities, and for the introduction of legislation for fitting and wearing of seat-belts by front-seat occupants.

A linear oscillator model for circadian rhythms: implications for phase response curves.

The underlying oscillator of circadian rhythms is assumed to be of a linear simple harmonic kind. Both pulse and step perturbations of positive and negative kinds are used to disturb the system theoretically. Phase response and phase-resetting curves are obtained and the results are compared with existing experimental data (namely Drosophila eclosion rhythm). It is demonstrated that the form and mirror-image symmetry of the response curves do appear similar to what is observed in real experiments. This is strongly indicative of the possibility that the underlying oscillator (or oscillators) is (are) not structurally stable.

Spontaneous hemoperitoneum due to rupture of hepatocellular carcinoma.

Spontaneous rupture with hemoperitoneum is one of the atypical modes of presentation of hepatocellular carcinoma (HCC), common in the orient (8-14% cases) but reported only once from India. Another case is being reported here. Hemostasis can be achieved by transcatheter hepatic artery ligation, intralesional alcohol injection, microwave application, curettage with suturing or gauze packing. Resectional surgery, preferably at a second stage, has the best long term outcome. Prognosis remains poor with few long term survivors.

Structural and functional aspects of the thoracic stomach.

The acid secretory and emptying function of the thoracic stomach was evaluated in 10 patients, following total oesophagectomy and gastric pull-up for oesophageal cancer. All patients had a Heinecke Mickulicz pyloroplasty. Endoscopic features and histological review of endoscopic biopsies were also studied in these patients. Acid secretion was higher in those with prolonged emptying. This was however, not statistically significant. Gastritis was more severe in those with prolonged emptying. Endoscopic diagnosis of gastritis was found unreliable while histopathology of endoscopic biopsy gave definite evidence of gastritis.

Short segment oesophago-cardiomyotomy for achalasia cardia.

Over a seven year period, 25 patients of achalasia cardia underwent a transabdominal short segment oesophago-cardiomyotomy. There was no operative mortality. Two patients had a mucosal tear, detected intraoperatively and promptly repaired. All patients were regularly followed up (range 1 to 7 years). Clinical results were excellent in 76%, good in 20% and fair in 4%. No patient developed reflux or required reoperation for residual dysphagia. We conclude that a transabdominal short segment oesophago-cardiomyotomy performed carefully is a safe and effective procedure in the treatment of achalasia.

Problem based module for the learning of biochemistry and molecular biology of hepatitis.

A problem based module for learning the concepts and practice of biochemistry and molecular biology in the understanding of hepatitis is suggested. The components of this module are (a) triggers--which represent case presentation, investigations and management strategies in the form of small statements organized in a logical sequence to provoke hypothetico-deductive reasoning and self directed identification of learning needs by the students, (b) guidelines for student tutor interaction--for conducting the learning exercise which is in the form of small group discussions facilitated by a tutor, (c) list of learning needs-essentially to be indentified in each group, (d) list of learning objectives--to ensure the necessary learning achievement, (e) few suggestions and guidelines for learning resources and (f) an overview of the problem. This module can be modified or altered for students at different phases of the medical curriculum. The suggested module can be best implemented through 3 tutorial sessions of 2 hours each, on alternate days, supported by 2-3 resource sessions (brief interactive/didactic lectures on selected difficult areas) in between. Alternate days can be organized for practical or skill sessions along with time for student's own learning and presentation in group during the tutorial session to be held on the following day.

Dumping syndrome in the intra-thoracic stomach.

Thirty seven patients who survived at least 6 months after oesophagectomy and cervical oesophagogastrostomy for benign and malignant diseases of the oesophagus were evaluated for dumping syndrome. Twenty two of these patients had a pyloroplasty and fifteen had no drainage procedure. All these patients were subjected to a dumping provocation test. Evidence of dumping was noted in four patients, all having a pyloroplasty. Clinically, these four patients had mild symptoms which improved with dietary adjustment. We conclude that dumping does occur following thoracic transposition of the stomach and seems to occur more in patients with pyloroplasty than those without it.

Splenic abscess--case report and review of literature.

Isolated splenic abscess is a rarity and remains a diagnostic dilemma. The presentation is nonspecific and diagnosis is often delayed. We present a case which had roentgenographic signs suggestive of splenic suppuration at admission. Of particular interest was the isolation of Salmonella paratyphi in this patient. Antibiotic therapy alone is insufficient and splenectomy remains the treatment of choice. The literature on splenic abscess is briefly reviewed.
