Birth-related fatigue in 34-36-week preterm neonates: rapid recovery with very early kangaroo (skin-to-skin) care.

OBJECTIVE: To test preterm neonates' physiologic and behavioral responses when placed skin-to-skin on their mother's chests, called kangaroo care (KC), for the first 6 hours after birth, instead of having the neonates go to an intensive care unit. DESIGN: Convenience sampling was used in this descriptive study to enroll neonates who were given continuous KC beginning soon after birth in the delivery room and continuing for 6 hours. Heart rate, respiratory rate, oxygen saturation, abdominal temperature, and behavioral state were recorded each minute. SETTING: Data were collected in the delivery room and in a private labor room in tropical Cali, Colombia. PARTICIPANTS: Six 34-36-week preterm neonates with 5-minute APGAR scores of 6 or more were enrolled. Two neonates had grunting respirations before KC was begun. RESULTS: Temperature rose rapidly to thermoneutral range. With few exceptions, heart rate, respiratory rate, and oxygen saturation remained within normal limits; grunting respirations in two neonates disappeared with warmed humidified oxygen and continuous KC. Sleep predominated, and neonates were discharged home by 48 hours being fully breastfed, suggesting that KC was an environment conducive to recovery from fatigue. CONCLUSIONS: These data suggest that KC beginning in the delivery room can be given safely and perhaps with benefit to 34-36-week gestation neonates who appear healthy at birth. Kangaroo care was conducive to recovery from birth-related fatigue.

Metabolic abnormalities in patients supported with enteral tube feeding.

One hundred patients supported with enteral tube feeding were evaluated for biochemical abnormalities. The following metabolic complications were observed: hyperglycemia (29%), hypoglycermia (2%) hypernatremia (10%), hyponatremia (31%), hyperkalemia (40%) hypokalemia (8%), hyperphosphatemia (14%) hypophosphatemia (30%) hypomagnesemia (3%), hypozincemia (11%), and hypocupremia (3%). Because of the extensive range of biochemical problems, it is recommended that subjects on tube feeding be evaluated periodically for metabolic abnormalities.

Wheat germ agglutinin binding to cells derived from in vivo grown solid tumors.

Binding of Wheat Germ Agglutinin (WGA) to cell suspensions derived from in vivo grown solid rat tumors of independent origin and of different metastatic potential has been examined, and results compared with previous work in other systems. The assay used to assess binding was based on the reaction of iodine 125-labelled lectin to tumor cells in suspension and values expressed as per cent of values of a cell line used as internal standard. Inhibition studies showed that WGA binding was principally to sialic acid and/or N-acetyl glucosamine residues. Binding parameters Bmax (maximal binding) and Kd (affinity of binding), derived from Woolf plots and expressed as per cent of internal standard cell line, were found for rat tumors Mc7, Mc40A, Sp24 and Walker 256. It was observed that cell suspensions derived from the metastatic and most tumorigenic tumor, Walker 256, exhibited a different pattern of lectin binding to the other non-metastatic and less tumorigenic tumors; in particular, Kd values were lower. These results are in accord with work using cell lines selected in vitro and are further evidence for a correlation between the density and/or configuration of cell surface sialic acid residues and metastatic potential.

Follow-up of comprehensive geriatric assessment in a family medicine residency clinic.

BACKGROUND: The objective of this study was to evaluate the effect of outpatient geriatric consultation by referring academic physicians and to verify the results of a previous study. METHODS: We conducted a retrospective review of charts of 37 patients referred for geriatric consultation during a 7-month period of a university- and community-hospital-affiliated family practice residency clinic in urban northeast Washington, DC. The consultation involved team assessment, which led to formal recommendations to the attending physician. Main outcome measures included total number and category of recommendations made, as well as a total number and category of recommendations adhered to by referring physicians. RESULTS: There were 29 women and 8 men with an average age of 79.1 years; 5 were white and 32 were African-American. For the 23 patients for whom follow-up could be determined, the mean number of total diagnoses per patient was 11.4 (SD 3.5). The mean total number of recommendations made per patient was 18.1 (SD 5.9). The mean total number of recommendations acted upon per patient by referring physicians was 9.5 (SD 4.4). The recommendations fell into the following categories: rehabilitative 64 percent, radiologic 57.1 percent, laboratory 56.9 percent, total medication 55.6 percent, medical 50 percent, health maintenance 47.1 percent, social service 46.2 percent, sensory 33 percent, other 28.6 percent, educational 20 percent, and nutritional 14.3 percent. CONCLUSIONS: Multidisciplinary geriatric assessment in an academic outpatient setting provides a comprehensive assessment for faculty and resident physicians in training. Recommendations will be adhered to only 50 to 70 percent of the time, possibly because of the demographic and socioeconomic mix and overall health of the patient population, health care priorities of the referring physician, and costs and availability of various interventions. Physicians in training should be exposed through continuing medical education to various aspects of geriatric assessment.

Paediatric basic life support--an update.

This paper highlights the need for health-care personnel to receive specific training in paediatric resuscitation. The aetiology of cardiopulmonary collapse in children, and the rationale behind the management of basic life support in this situation is outlined.
