In view of the fact that the NHS is one of the largest employers of women, there is a surprising dearth of discussion in staff periodicals about the problems of recruiting or retaining staff with young children, and about the extent to which child care provision should be made available to staff. This report aims to redress the balance by presenting information gathered from day nurseries for the children of staff in NHS hospitals.
We present six cases of cancer associated with Crohn's disease and stress the importance of the earlier age of onset than spontaneously arising small-bowel carcinoma, the long period of latency from the time of diagnosis of Crohn's disease to that of carcinoma, and the generally poor prognosis. We emphasize, furthermore, the frequent association with fistulas and the predisposition of bypassed or excluded segments of bowel to undergo malignant transformation. The occurrence of carcinoma in an excluded rectal stump has not previously been reported and stresses the necessity of resecting, rather than excluding, segments of bowel involved with Crohn's disease. These tumors are often not readily apparent by radiographic or endoscopic examination and in fact, may be discovered only after microscopic examination of resected or biopsied tissue.

Asymptomatic infection with either Entameba histolytica or Giardia lamblia was found in 61 per cent of the residents of a dormitory in an institution for the mentally retarded; two other dormitories had rates of 20 per cent and 22 per cent. Drug therapy was successfully undertaken in all three dormitories, and environmental improvements were introduced in the heavily infected dormitory. A one-year follow-up showed a reduction in parasitic disease in two dormitories but, in the most heavily infected dormitory, infection had returned to pretreatment levels.

Compression of the cauda equina from subluxation, stenosis, or spondylosis of the lumbosacral articulation was found in 20 dogs, 13 of which were German Shepherd Dogs or crossbred German Shepherd Dogs. Pain, posterior paresis, and urinary or fecal incontinence were the most frequent problems associated with the compression. Dorsal laminectomy was performed on 13 cases, with good to excellent results on 10 of these.

High rates of acetylene (C(2)H(2)) reduction (nitrogenase activity) were observed in woodroom effluent from a neutral sulfite semi-chemical mill under aerobic (up to 644 nmol of C(2)H(4) produced per ml per h) and under anaerobic (up to 135 nmol of C(2)H(4) produced per ml per h) conditions. Pasteurized effluent developed C(2)H(2) reduction activity when incubated under anaerobic but not under aerobic conditions. Activities were increased by addition of 0.5 to 3.0% glucose or xylose. Enrichment and enumeration studies showed that N(2)-fixing Azotobacter and Klebsiella were abundant, and N(2)-fixing Bacillus was present. Of 129 isolates of Klebsiella from pulp mills, lakes, rivers, and drainage and sewage systems, 32% possessed nitrogen-fixing ability.

OBJECTIVE: To test preterm neonates' physiologic and behavioral responses when placed skin-to-skin on their mother's chests, called kangaroo care (KC), for the first 6 hours after birth, instead of having the neonates go to an intensive care unit. DESIGN: Convenience sampling was used in this descriptive study to enroll neonates who were given continuous KC beginning soon after birth in the delivery room and continuing for 6 hours. Heart rate, respiratory rate, oxygen saturation, abdominal temperature, and behavioral state were recorded each minute. SETTING: Data were collected in the delivery room and in a private labor room in tropical Cali, Colombia. PARTICIPANTS: Six 34-36-week preterm neonates with 5-minute APGAR scores of 6 or more were enrolled. Two neonates had grunting respirations before KC was begun. RESULTS: Temperature rose rapidly to thermoneutral range. With few exceptions, heart rate, respiratory rate, and oxygen saturation remained within normal limits; grunting respirations in two neonates disappeared with warmed humidified oxygen and continuous KC. Sleep predominated, and neonates were discharged home by 48 hours being fully breastfed, suggesting that KC was an environment conducive to recovery from fatigue. CONCLUSIONS: These data suggest that KC beginning in the delivery room can be given safely and perhaps with benefit to 34-36-week gestation neonates who appear healthy at birth. Kangaroo care was conducive to recovery from birth-related fatigue.
One hundred patients supported with enteral tube feeding were evaluated for biochemical abnormalities. The following metabolic complications were observed: hyperglycemia (29%), hypoglycermia (2%) hypernatremia (10%), hyponatremia (31%), hyperkalemia (40%) hypokalemia (8%), hyperphosphatemia (14%) hypophosphatemia (30%) hypomagnesemia (3%), hypozincemia (11%), and hypocupremia (3%). Because of the extensive range of biochemical problems, it is recommended that subjects on tube feeding be evaluated periodically for metabolic abnormalities.
