Comparison of a polymerase chain reaction assay and a conventional microbiologic method for detection of methicillin-resistant Staphylococcus aureus.

The presence or absence of a methicillin resistance gene in 58 clinical isolates of Staphylococcus aureus was examined by the polymerase chain reaction (PCR) and Southern blot analyses. The results were analyzed in relation to those of the MIC assay of methicillin and oxacillin. PCR assay results were identical to those of Southern blot analysis of genomic DNA digested with HindIII (positive, 28 strains; negative, 30 strains). Among the 28 PCR-positive strains, 6 strains showed methicillin susceptibility by the conventional susceptibility test (MICs, less than or equal to 8 micrograms/ml). Culturing of the six strains with ceftizoxime led to an increase in the phenotypic level of resistance to methicillin and oxacillin, indicating that these strains should be classified as methicillin-resistant S. aureus (MRSA). The PCR assay was found to be a sensitive and reliable procedure for the rapid diagnosis of MRSA infection, even in cases in which the conventional MIC assay failed to detect MRSA.

Anticoagulant action of vanadate.

Sodium orthovanadate (vanadate) prolonged the clotting time of normal human plasma in a dose-dependent manner. The prolongation of clotting time by vanadate linearly decreased with an increase in the concentration of amiloride. Vanadate also was completely additive to prolongation by heparin. When factor Xa or thrombin was incubated with vanadate, the amidolytic activity of each decreased in a dose-dependent manner with vanadate. Amiloride protected the decrease of amidolytic activity of both factor Xa and thrombin by vanadate. The amidolytic activity of trypsin also was inhibited by vanadate, but that of alpha-chymotrypsin was not inhibited, suggesting that vanadate preferentially inhibits the amidolytic activity of trypsin and trypsin-like enzymes. These results show that vanadate prolongs the clotting time of plasma through mechanisms involving in part the inhibition of the activity of both factor Xa and thrombin.

Significance of anti-eye muscle antibody in patients with thyroid-associated ophthalmopathy by quantitative western blot.

To investigate the prevalence of antibody against rat eye muscle membrane antigen, as determined from SDS-polyacrylamide gel electrophoresis and western blotting, in sera from patients with thyroid-associated ophthalmopathy (TAO), we quantitatively analyzed the binding activity with a rat eye muscle membrane 64 kDa protein using chromato-scanner. Eye muscle antibody activity was expressed as ratio of density of the 64 kDa band to that at 66 kDa found with all normal sera and phosphate buffered saline. The mean (+/- SD) eye muscle antibody activity was 2.7 +/- 2.7 in TAO (P < 0.01 v.s. normal), 1.5 +/- 1.7 in Graves' disease without evident eye disease, 1.6 +/- 2.5 in Hashimoto's thyroiditis and 0.45 +/- 0.26 in normal subjects. A positive band at 64 kDa was found in 71% of patients with TAO, 36% of those of Graves' disease without evident eye disease and in 35% of patients with Hashimoto's thyroiditis without eye disease. The prevalence of this antibody activity tended to correlate to the severity of ophthalmopathy. Furthermore, the level of eye muscle antibody activity decreased in parallel with the improvement of eye signs in two patients. Sera reactive with rat eye muscle membrane 64 kDa protein reacted also with a human eye muscle membrane 64 kDa protein but not with human thyroid, liver, spleen or pancreas membrane preparations. In conclusion, antibody to rat eye muscle membrane 64 kDa protein is present in TAO and may be a useful clinical marker of ophthalmopathy.

Effects of arterial insufficiency on glucose uptake by fast and slow rat skeletal muscles.

1. Muscle wet weight and glucose uptake were measured in female Wistar rats 7 and 14 days after unilateral ligation and section of the artery. 2. Following the induction of arterial insufficiency, muscle wet weight decreased significantly in the extensor digitorum longus (EDL), but not in the soleus muscle (Sol). 3. Muscle glucose uptake per wet weight increased significantly in EDL after arterial insufficiency, but not in Sol. 4. It is suggested that arterial insufficiency influences the metabolic control of fast muscle via anoxia as a main etiologic factor.

[A case of flomoxef-induced pneumonitis].

We report a case of flomoxef-induced pneumonitis. A 22-year-old man was treated with flomoxef following liver biopsy. A few days later he developed a high fever and severe dyspnea, and his chest X-ray film revealed diffuse reticulo-nodular shadows in both lung fields. We suspected interstitial pneumonitis due to flomoxef, and pulse therapy with methylprednisolone was started. He showed rapid recovery of symptoms and marked regression of pulmonary infiltration in his chest X-ray. Lymphocyte stimulation test was positive to flomoxef, which was compatible with the diagnosis of drug-induced pneumonitis. To our knowledge, there has been no previous case of pulmonary hypersensitivity to flomoxef reported in Japan.

[The beta-lactamase activity in methicillin-resistant Staphylococcus aureus--a comparison of the beta-lactamase from a methicillin-resistant and from a methicillin-sensitive strain of Staphylococcus aureus].

Strains of Methicillin-resistant Staphylococcus aureus (MRSA) are more frequently beta-lactamase-negative than those of Methicillin-susceptible S. aureus (MSSA). In the strains of MRSA, the beta-lactamase activity tends to be inversely related to the MIC's of Methicillin. Such a marked tendency has been observed in recent years.

Endotoxin stimulates bronchial epithelial cells to release chemotactic factors for neutrophils. A potential mechanism for neutrophil recruitment, cytotoxicity, and inhibition of proliferation in bronchial inflammation.

To test the effect of endotoxin on bronchial epithelial cells (BEC), BEC were isolated from bovine lungs and cultured in the presence of bacterial endotoxin. The BEC culture supernatant fluids were harvested, and neutrophil chemotactic activity (NCA) was determined with a blindwell chamber technique; cytotoxicity determined by lactate dehydrogenase release and BEC proliferation determined by Coulter counting. Endotoxin caused a dose- and time-dependent release of NCA from BEC cultures compared with media alone (82.3 +/- 8.1 vs 12.0 +/- 3.1 cells/high power field, p less than 0.001). To further characterize this activity, reverse phase HPLC analysis of release eicosanoid metabolites after [3H]arachidonic acid incorporation was performed. Endotoxin stimulated the release of the neutrophil chemoattractants, leukotriene B4 and 12-hydroxyeicosatetraenoic acids. Endotoxin also resulted in a dose and time dependent release of lactate dehydrogenase (42.9 +/- 4.2 vs 20.2 +/- 2.2 U/liter, p less than 0.001) although higher doses were required to cause cytotoxicity than to stimulate chemotaxis. Finally, endotoxin resulted in a dose dependent inhibition of BEC proliferation (176 x 10(3) +/- 16 x 10(3) vs 1,080 x 10(3) +/- 38 x 10(3) cells/ml measured at day 14, p less than 0.001). These data suggest that bacterial release of endotoxin may contribute to the pathophysiologic changes observed in bronchial inflammation by stimulating BEC to release NCA, denuding airway epithelium by causing cytotoxicity of BEC, and inhibiting epithelial repair by inhibiting BEC proliferation.

Effects of triamcinolone acetonide on plasma amino acids and urinary urea output in rabbits.

1. The effect of administering triamcinolone acetonide (10 mg/kg/day), 6 consecutive s.c. injections given daily, on plasma free amino acids and urinary urea output was studied in rabbits. 2. The total free amino acids in plasma decreased significantly from day 2 while ammonia increased significantly only on day 2, glutamine, lysine and branched amino acids increased significantly from day 3 or 5. 3. The output of urinary urea increased significantly from day 3. 4. These findings suggest the inhibition of protein synthesis observed in steroid myopathy may result from a decrease in the amino acid pool in skeletal muscle.

Effect of thyroxine on basal and insulin-stimulated glucose uptake by fast and slow skeletal muscles of rats.

1. The sc injection of 1-thyroxine (2 mg/kg bw/day) for 8 days produced a significant decrease of body weight gain in young male Wistar rats. 2. In these hyperthyroid rats there was a significant decrease in the wet weight of the extensor digitorum longus (EDL) and soleus (Sol) muscles as compared with those of control rats. 3. The basal glucose uptake by the EDL and Sol muscles was unchanged in hyperthyroid rats using the wet weight of muscle as a reference. 4. In hyperthyroid rats, the insulin-stimulated uptake of glucose by both the EDL and Sol muscles was significantly decreased. This inhibition was stronger in Sol and there was no insulin stimulation of glucose uptake by Sol.

Regulation of glucose uptake in fast and slow skeletal muscles with fasting and refeeding.

1. The decrease in wet weight and noncollagen protein (NCP) was faster and greater in extensor digitorum longus (EDL) during fasting than in soleus (Sol) muscles in rats. 2. During refeeding, recovery was completed faster in Sol than in EDL. 3. Glucose uptake in skeletal muscle increased significantly during fasting on both a per wet weight and NCP basis. 4. This increase was faster and greater in EDL than Sol. 5. The initial increase in glucose uptake was greater during refeeding than fasting only in EDL.

Rate of urinary urea output correlates to degree of muscular atrophy induced by triamcinolone acetonide.

1. The effect of administering triamcinolone acetonide (10 mg kg-1 day-1) as seven consecutive single daily subcutaneous injections on urinary urea output and muscular atrophy was studied in rabbits. 2. The ratio of the wet weight of the lateral vastus of the quadriceps muscle to body weight (MI) varied greatly among the rabbits. 3. The ratio of urinary urea output after to before triamcinolone injection correlated significantly to MI. 4. These findings suggest that the ratio of urinary urea output can be used as a predictive index for steroid myopathy.
