Studies on the enhanced effect of acupuncture analgesia and acupuncture anesthesia by D-phenylalanine (2nd report)--schedule of administration and clinical effects in low back pain and tooth extraction.

D-phenylalanine (DPA) is known to block the activity of carboxypeptidase, an enzyme which degrades enkephalins, endogenous morphine-like substances. Therefore, it is considered that DPA administered as an inhibiting drug of this degrading enzyme might prolong analgesia induced by acupuncture. 1) Thirty patients suffering from chronic low back pain were treated with acupuncture 30 minutes after the oral administration of 4.0 grams of DPA. The results were: excellent in 7 cases, good in 11, fair in 6 and poor in 6. Cases graded excellent and good were then compared with a placebo group. The effect was increased 26% in the DPA-acupuncture group, which shows no statistically significant difference (P less than 0.1). 2) In 56 patients, tooth extraction was performed under acupuncture anesthesia: 18 had received 4.0 gram of DPA (P.O.) 30 minutes earlier. The results were excellent in 8, good in 6, fair in 3, and poor in 1. The excellent and good cases were compared with 38 placebo group cases. The effect in the DPA-acupuncture anesthesia group was significantly increased by 35% (P less than 0.01). 3) In order to determine the optimum time for the administration of DPA, two schedules of administration were compared. [1] DPA was given on the previous day in three 0.5 gram doses (26 cases). [2] A single 4 gram dose was administered 30 minutes before treatment (30 cases). The results from the "excellent", "good" and "fair" cases showed a 16% increase in effectiveness when DPA was administered the day before, not a statistically significant difference (P less than 0.1), but a clear tendency to increase was observed. The above findings show that DPA has an enhancing effect on acupuncture analgesia and anesthesia in clinical practice.

[123I-N-isopropyl-p-iodoamphetamine scintigraphy in patients with malignant melanoma].

N-Isopropyl-p-[123I]iodoamphetamine (123I-IMP) has been widely used in patients with cerebral vascular damage and epilepsy. The potential of 123I-IMP accumulation in the melanin producing cells has been reported since early stage of its development. Some authors reported the accumulation of 123I-IMP in the malignant melanoma of human and animals. We evaluated the 123I-IMP scintigraphy in 9 patients with malignant melanoma. Patients were classified into two groups: A, 4 patients with 8 lesions; B, 5 post-operative patients without lesions. In group A, the 123I-IMP uptake was seen in 4 of 8 lesions. The smallest true positive lesion was located at skin and its size was 15 mm in diameter. Two of the visualized 4 lesions were amelanotic malignant melanomas. This fact suggests that uptake of 123I-IMP in malignant melanoma may be related to the processes of melanin synthesis. In group B, two abnormal deposits had been seen in the right thigh of a female patient. However no abnormality was seen in the following 67Ga scintigraphy, TCT, MRI, and repeated 123I-IMP scintigraphy. Therefore the abnormal deposits were considered to be the false positive lesions due to urinary contamination.

[A clinical study of intraarterial infusion therapy with CDDP for primary osteosarcoma].

Eleven patients with primary osteosarcoma were treated by intra-arterial infusion of cisplatin. Cisplatin (50-100 mg/m2) was infused slowly into tumor feeding artery or proximal-to-the-lesion artery. An antidote, sodium thiosulfate, was also administered intravenously in 10 cases and angiotensin II was simultaneously used in 2 cases. Symptoms of heat sensation and local pain were decreased or disappeared in almost all cases and histopathologic changes were observed in 7 cases. No viable tumor cells were seen in 3 cases.

Incorporation of blood lactate and glucose into tissues in rats after short-term strenuous exercise.

The purpose of this study was to investigate the main site of removal of blood lactate and glucose and which is the more important substrate for muscle glycogen resynthesis in rats after short-term strenuous exercise, to exhaustion. Male Wistar rats ran to exhaustion at a speed of 70-100m.min-1. Immediately after the exercise the rats received an injection of [U-14C]lactate (LA, 0.025 microCi.g-1, n = 5) or [U-14C]glucose (GL, 0.015 microCi.g-1, n = 5) into the aorta through an indwelling catheter. The rats were sacrificed after 40 min of recovery. During 40 min of recovery, 20.4% +/- 2.0% (mean +/- SE) of 14C injected was recovered as 14CO2 in LA, while 4.1% +/- 0.4% of 14C was recovered as 14CO2 in GL. In LA, the content of 14C incorporated per tissue weight in the vastus lateralis was significantly greater than that in the kidney, heart, and blood, while in GL that in the vastus lateralis was significantly greater than in any other tissues measured. The incorporation of 14C-glucose into muscle glycogen (vastus lateralis) was about five times greater than that of 14C-lactate. Data from this study indicate that lactate and glucose are incorporated from the blood into the skeletal muscle which was active during exercise and that blood glucose is a more preferred substrate for muscle glycogen resynthesis in rats after strenuous exercise to exhaustion.

A case of parathyroid carcinoma visualized by 67Ga citrate and 201Tl chloride scintigraphy.

Both 67Ga citrate and 201Tl chloride accumulated in a parathyroid tumor of a 62 year-old woman with primary hyperparathyroidism. Histological studies disclosed that the tumor was a parathyroid carcinoma. The use of both tumor scanning agents, 67Ga citrate and 201Tl chloride to visualize parathyroid carcinoma in a patient with primary hyperparathyroidism has not been reported as far as we could determine.

[Statistical analysis of subjects with type A, BC and C pattern of blood PCB on GC].

Various combinations of measured data on PCBs, PCQs, and PCDFs in adipose tissue or blood were tested on its discriminating power between normal persons and persons poisoned by yusho oil. The most discriminative single parameter was concentration of PCDF in adipose tissue. Furthermore, discriminative power increased by using additional parameters.

[Introduction of the information processing system of medical examinations of yusho patients and some results obtained by the 1986 survey].

Information processing system for newly established, unified medical examinations of yusho patients which started in September 1985 was presented. Some comparative analysis was also shown on the "important findings" in yusho patients residing in Fukuoka or Nagasaki prefecture. In the 1986 survey, Fukuoka male patients showed higher percentages of pigmentation in toe nail, gingiva and palpebral conjunctiva than Nagasaki (p less than 0.01), whereas Nagasaki male patients showed higher percentages of pigmentation in buccal and lip mucosa. Average concentration of PCB in blood was higher in Nagasaki male patients (6.79 ppb) than in Fukuoka patients (4.39 ppb), the difference being significant at 1% level.

[Hemodynamic effects of dobutamine, dopamine and methoxamine during thoracic epidural anesthesia in dogs with coronary stenosis].

Dobutamine (DOB), dopamine (DOP) and methoxamine (MEX) were compared as to their effectiveness in controlling circulatory depression caused by thoracic epidural anesthesia in dogs with instituted coronary stenosis. DOB appeared to best serve the purpose of reestablishing systemic circulation, notably with an elevation of blood pressure. DOP achieved the best restoration of the absolute value of regional myocardial blood flow (RMBF), but DOB and DOP both caused RMBF in stenosed and non-stenosed areas to increase to virtually the same extent, and furthermore, worsened the intramyocardial distribution of RMBF in stenosed area. With MEX, an alpha-stimulant, there was noted a significant increase in blood flow through a stenosed area and, moreover, the increment tended to be greater than in a non-stenosed area, a fact suggesting involvement of such a mechanism as reverse coronary steal.

[Clinical evaluation of I-131 metaiodobenzylguanidine (MIBG) imaging in suspected neuroblastoma].

Twenty neuroblastoma and 4 nonneuroblastoma patients were studied by 131I-MIBG imaging. The primary tumor was detected in 89% of patients (8/9) before therapy. Bone marrow metastasis was also visualized in 4 of the 8 patients with primary positive scan. True negative results were obtained in 4 nonneuroblastoma patients. After therapy, of 10 tumor-bearing patients, eight showed positive scans and 9 of 12 lesions (75%) were visualized. The accuracies of presence or absence of neuroblastoma were compared between 131I-MIBG imaging and several tumor markers. The accuracies before and after therapy were as follows: 131I-MIBG imaging; 92% (12/13), 88% (15/17), serum NSE; 80% (4/5), 93% (13/14), serum LDH; 92% (11/12), 76% (13/17), urinary VMA; 54% (7/13), 56% (9/16), and urinary HVA; 77% (10/13), 56% (9/16). It appears that 131I-MIBG imaging is useful for both locating and excluding neuroblastoma. In addition, 131I-MIBG imaging appears to be the most efficient diagnostic and follow up study for neuroblastoma when it is combined with measurements of serum NSE.

The effects of ovarian hormones on glucose and fatty acid oxidation during exercise in female ovariectomized rats.

The effects of ovarian hormones on glucose and fatty acid oxidation during exercise were investigated in adult female ovariectomized rats. Rats subdivided into 3 groups received intraperitoneal injections of hormones or sesame oil for 8 days. Estrogen (E) treated rats received 17-beta estradiol in daily doses of 2 micrograms. Estrogen and progesterone treated rats (EP) received 17-beta estradiol in daily doses of 2 micrograms and 2 mg, respectively. Control rats (S) received sesame oil alone. After an overnight fast, rats ran at the speed of 25 m.min-1 for 60 min. [U-14C]glucose or [1-14C]palmitate was injected into rats at 5 min of exercise and before 10 min of exercise, respectively. Expired 14CO2 was collected using bottomless chamber on a treadmill belt. No significant differences were found in mean blood glucose, lactate and plasma free fatty acid concentrations after the exercise. Until the end of the exercise 34.7 +/- 2.6 (E, n = 5), 40.8 +/- 2.9 (EP, n = 5) and 43.7 +/- 3.5% (S, n = 6) (mean +/- SE) of 14C which was injected as 14C-glucose was recovered as 14CO2. During 60 min of the exercise 27.5 +/- 1.0 (E, n = 7), 19.8 +/- 2.7 (EP, n = 6) and 25.0 +/- 1.9% (S, n = 6) of 14C which was injected as 14C-palmitate was recovered as 14CO2. A significant difference was found in this rate between E and EP (P less than 0.05). It was concluded that estrogen treatment stimulated fatty acid oxidation compared with the estrogen plus progesterone treatment and tended to inhibit glucose oxidation during prolonged exercise.

Oxidation of lactate in rats after short-term strenuous exercise.

Oxidation of lactate and glucose was investigated in rats after short-term strenuous running to exhaustion at a speed of 80-110 m.min-1, lasting about 100 sec. Immediately after the exercise, 4 microCi of [U-14C]lactate (LA and AR) or 9.4 microCi of [U-14C]glucose (GL) was injected into the aorta through an indwelling catheter. In AR, the rats ran at a speed of 25 m.min-1 for 20 min after injection of [U-14C]lactate. Expired gas was collected by a bottomless metabolism chamber while the rats were on the treadmill for 120 min. Blood lactate concentration tended to decrease faster in AR than in LA. Peak evolution of 14CO2 expiration occurred at 12.5 min recovery in LA, 7.5 min of recovery in AR, and 35 min of recovery in GL. Cumulative percent recovery of 14C as 14CO2 was 48.5% +/- 2.8% in LA, 74.0% +/- 2.9% in AR, and 18.6% +/- 1.6% (mean +/- SE) in GL. Significant differences were found in these rates between groups (P less than 0.01). It was suggested that a great deal of lactate was oxidized directly, not after conversion to glucose in rats after short-term strenuous exercise to exhaustion and mild exercise following strenuous exercise (active recovery) enhanced lactate oxidation.

Primary role of hyperkalemia in the acidosis of hyporeninemic hypoaldosteronism.

A 65-year-old woman with mild renal insufficiency had persistent hyperkalemia and hyperchloremic acidosis. Her plasma aldosterone level was relatively low for her hyperkalemia, and her urine pH was low. Fludrocortisone acetate administration corrected both hyperkalemia and acidosis by increasing urinary excretion of potassium and net acid, implicating deficient mineralocorticoid activity in the distal renal tubule in this patient. During this medication urinary ammonium excretion increased, but urine pH remained low, so that urinary titratable acid excretion did not decrease. On the other hand, correction of hyperkalemia by administration of a potassium-calcium exchange resin alone also resolved the acidosis by increasing urinary ammonium excretion. This increment exceeded the decrement of urinary titratable acid excretion, which was caused by raised urine pH secondary to increased urinary ammonium excretion, and resulted in increase of net acid excretion. Thus, in this patient, hyperkalemia appears to be a decisive causative factor in the acidosis, with deficient mineralocorticoid effect only contributing in part to the reduction of net acid excretion and the acidosis.

Amphetamine-induced myoglobinuric acute renal failure.

A 36-year-old man was admitted because of sopor and dark urine after intravenous amphetamine injection. He subsequently developed myoglobinuria and acute renal failure. Serum myoglobin value was remarkably elevated to 83,000 ng/ml, and urine myoglobin was 400,000 ng/ml. Renal biopsy revealed tubular degeneration and tubular obstruction with myoglobin casts by immunofluorescence examination. Amphetamine-induced rhabdomyolysis was suspected to cause myoglobinuric acute renal failure.
