[Magnetic resonance imaging of the adrenal gland].

Adrenal imaging was performed using magnetic resonance (MR) was in 100 patients who had no clinical or biochemical evidence of adrenal abnormality and in 19 patients with 24 adrenal lesions (adenoma in 5, hyperplasia in 2, metastasis in 5, (lung cancer in 1, hepatoma in 4) adrenal cancer in 1, pheochromocytoma in 3, neuroblastoma in 3). Normal adrenal glands showed intermediate intensity between muscle and liver, and were detected in over 90% of cases on T1-weighted images (T1-weighted SE, inversion recovery). Adenomas and hyperplasias had the same intensity as normal glands. Medullary masses showed extreme hyperintensity on T2-weighted images and could be differentiated from cortical masses. Neuroblastomas were detected as hyperintense tumors with intratumoral hemorrhage and necrosis on T2-weighted MR images. Metastatic adrenal tumors from lung cancer were hyperintense on T2-weighted images, while metastasis from hepatoma showed low intensity on the same pulse sequence. In diagnosing adrenal metastasis, we must compare and contrast the tumor intensity and structure with those of the primary lesions. MR is considered a useful modality in characterizing adrenal tissue.

Protective effect of CCK-8 and ceruletide on glutamate-induced neuronal cell death in rat neuron cultures: possible involvement of CCK-B receptors.

Cholecystokinin octapeptide (CCK-8), ceruletide (CLT) and gastrin-I, which were added simultaneously with glutamate to rat neuron cultures, significantly suppressed the neuronal cell death induced by glutamate which can be observed from the efflux of lactate dehydroxylase into the culture medium. However, gastrin-I (1-13) had no effect on the response to glutamate. The inhibitory effect of CLT on glutamate-induced neuronal cell death could be completely blocked by a selective antagonist for CCK-B receptors, (+)L-365,260. These findings clearly indicate that CCK-8, CLT and gastrin-I exhibit a protective effect against glutamate-induced neurotoxicity via the CCK-B receptor.

[Results of radiotherapy combined with BAI (bronchial artery infusion) for non-small cell lung cancer--analysis of 104 cases].

Results of 104 cases of non-small cell lung cancer (NSCLC) treated by radiotherapy combined with BAI (bronchial artery infusion) were analyzed retrospectively. In 104 cases (84 males and 20 females), 14 cases of stage I, 10 of stage II, 33 of stage IIIA, 29 of stage IIIB and 18 cases of stage IV were included. Overall cumulative survival rate was 11.4% and 50% survival time was 12 months. On PS (performance status), 50% survival time was 1 year and 3 months for both PS 0 and PS 1, 8 months for PS 2 and 2 year for PS 3 (n = 3), respectively. On stage, 50% survival time was 3 year and 4 months for stage I, 1 year and 2 months for stage II, 12 months for stage IIIA, 11 months for stage IIIB and 8 months for stage IV. There was no significant difference of survival rate between squamous cell carcinoma (n = 41) and adenocarcinoma (n = 43). On anticancer agents used for BAI, CDDP group (n = 68) showed better significant 2-year survival rate than that of MMC and/or ADR group (n = 36). Response rate (CR + PR) was 85% (75/88) by radiotherapy with BAI, and 10 cases of CR were obtained. Among 27 cases of 2-year survivors, 21 cases of responders (CR + PR) were included and 4 cases of them showed CR. On these results, radiotherapy combined with BAI seemed to contribute to improve long-term treatment results of NSCLC by higher response rate with the progress of anticancer agent used for BAI.

Evaluation of energy metabolism in brain using epimerization of 2-deoxy-2-fluoro-D-glucose by 19F NMR: the effect of anesthesia.

The effect of anesthesia on the epimerization of 2-deoxy-2-fluoro-D-glucose(-6-phosphate) to 2-deoxy-2-fluoro-D-mannose(-6-phosphate) in mice was demonstrated with 19F NMR spectroscopy of excised tissue specimens from heart, muscle, and brain. Spectra from each of six excised organs confirmed that pretreatment with an anesthetic suppressed the conversion ratio, FDM/(FDG + FDM), in brain at 3 h from 52 +/- 5 to 30 +/- 6% under the dose of 100 mg/kg. We propose potential applications for NMR monitoring of FDG-FDM conversion, including the assessment of regional brain metabolism and the evaluation of various pharmacologic agents.

[Study on recurrence of hypertensive intracerebral hemorrhage].

Recurrence of hypertensive intracerebral hemorrhage confirmed by computed tomography scan has rarely been reported. The authors have experienced nine cases of recurrent hemorrhage among 494 cases of hypertensive intracerebral hemorrhage. Recurrence rate was 1.8%. There were eight males and one female with an average age of 60.5 years. Six cases had their first and second attacks in ganglionic regions. Among them, three cases had the second attack in the ipsilateral side, and the other three cases had the second attack in the contralateral side. Two cases had the first attack in the thalamic and the second in the cerebellar regions. One case had the first attack in the pontine and the second in the putaminal regions. Intervals between the first and second attacks were within 6 months for ipsilateral ganglionic attacks and over 4 years for contralateral ganglionic attacks. In all cases systemic blood pressure was normalized or well controlled by antihypertensive agents after the first attack. The mechanism of rebleeding has not been clarified.

[An adult case of the left coronary artery anomalously originating from the pulmonary artery (BWG syndrome), demonstrating remarkable improvement in postoperative thallium-201 myocardial imaging].

Stress thallium-201 myocardial scintigraphy were performed in a 44-year-old female with BWG syndrome before and after aorto-coronary bypass surgery. Preoperative stress thallium-201 studies demonstrated anterior reversible perfusion defect and posterior persistent defect. But postoperative studies demonstrated no defects in both areas. These data suggest that preoperative anterior defect was due to transient myocardial ischemia, but the finding of posterior persistent defect wasn't due to infarction or scarring. Thus, stress thallium-201 myocardial imaging provided useful informations to clarify the myocardial perfusion patterns pre- and postoperatively.

[Associations between blood PCB level and symptoms of Yusho patients, twenty years after outbreak].

To investigate relationships between blood PCB (polychlorinated biphenyls) level and symptoms of Yusho patients, twenty years after outbreak, a comparative study was designed in accordance with the results of medical examination conducted in 1988. For this examination, standardized forms were introduced in 1986, and a whole system for data-processing has been completed through two-years' efforts. Study subjects were 259 Yusho patients (male: 136, female: 123) for whom PCB concentration in blood was reported, among a total of 285 patients examined (91%). And 31 examination items which correspond or relate to the diagnostic criteria for Yusho (1976) were selected from the examination form for analysis. Mean blood PCB concentration in the subjects was 4.78 ppb ranging 0.6-32.0 ppb. The subjects were stratified for four blood PCB levels, less than or equal to 2.6 ppb; less than or equal to 4.0 ppb; less than or equal to 6.0 ppb; greater than 6.0 ppb, by dichotomized categories (normal/abnormal) for the results of each examination. This two-by-four table was transformed to two-by-two table with a cut-off point where the largest chi 2-value was obtained. Then the odds ratio (OR) adjusted for gender and age was calculated for the examination item which showed a statistically significant decrease or increase in crude odds ratio, to assess the association between blood PCB level and symptoms of Yusho patients. [201Tl-reverse redistribution in a case with stunned myocardium].

A 58-year-old man was admitted to our hospital because of chest pain. Electrocardiogram showed giant negative T waves in leads I, II, III, aVl, aVf and V2 through V6. Echocardiogram showed anterior wall motion abnormality. Rest-redistribution thallium scan obtained 3 days after admission revealed reverse redistribution in antero-apical area. Coronary angiogram showed no significant stenotic lesions and left ventriculogram in chronic stage showed improvement of wall motion abnormality. Repeat thallium scan in chronic stage showed disappearance of perfusion defect in apical area. It was difficult to diagnose myocardial viability in our case with stunned myocardium because rest-redistribution thallium scan showed reverse redistribution. As the mechanism of this finding, early coronary recanalization in acute stage of the event was suspected.

[A case of a rare anomaly of the abdominal aorta--magnetic resonance image and angiographic finding].

A case of a rare anomaly of the abdominal aorta was reported. The abdominal aorta was divided into two abnormal vessels and one vessel was celiac artery, and the other was considered to be abdominal aorta. To review literature, this type of anomaly of abdominal aorta is rare. Anatomy of this case had been well demonstrated by MR imaging and angiography. Those findings and the embryological basis of this case were discussed.

[Differentiation of infiltrates between lung cancer and noncancerous lung disease by 123I-IMP lung imaging].

Lung imaging with N-isopropyl-p-123I-iodoamphetamine (123I-IMP) was performed to estimate the pulmonary lesion imaging findings in 3 patients with bronchogenic carcinoma (2: bronchioloalveolar carcinoma and 1: adenocarcinoma) and 18 with noncancerous lung diseases (10: bacterial pneumonia, 1: viral pneumonia, 1: aspiration pneumonia, 1: radiation pneumonitis, 4: pulmonary tuberculosis and 1: obstructive pneumonitis due to an endobronchial lipoma) at 30 min and 4 hr after i.v. injection of 111 MBq of 123I-IMP. These patients all exhibited infiltrates only in the chest radiograms. Decreased uptake of 123I-IMP was observed in the cancerous infiltrating lesions in 3 patients with bronchogenic carcinoma at 30 min and 4 hr, while the uptake of 123I-IMP was normal or increased at 30 min and intense at 4 hr in all 18 noncancerous infiltrating lesions. Therefore 123I-IMP lung imaging can be used to differentiate bronchogenic carcinoma from noncancerous lung disease in patients who exhibit infiltrates only in the chest radiograms.

[An experimental trial for quantitative estimation of lipiodol using CT value--application to hepatic arterial infusion therapy].

An experimental trial for quantitative estimation of Lipiodol (Lp.) using CT scan was performed. Simulating the average CT value of Lp. to known iodine concentration calculated from the standard curve of KI solution/CT value, quantity of Lp. can be given by following formula; (Lp-dose/slice) = KS d/480 (K: known iodine concentration, S: area square of Lp., d: slice thickness). Satisfactory data were given, and clinical application was expected.

The influence of age on N-isopropyl-p-[123I]iodoamphetamine accumulation in the human heart.

Variations in heart intensity in the 30 min and 4 hr chest images of the radiolabelled lipophilic amine, N-isopropyl-p-[123I]iodoamphetamine (123I-IMP) were observed in 130 patients with lung diseases, aged 23 to 85 yrs. The heart intensity had a significant positive linear correlation with age (r = 0.43 at 30 min, 0.66 at 4 hr). The ratio of 4 hr heart intensity to 30 min heart intensity also had a positive linear correlation (r = 0.59), suggesting slower clearance of the radioactivity from the heart in older than in younger patients during this interval. Other parameters including sex, EKG findings, liver function, blood pressure, the presence of diabetes mellitus and smoking history had no relationship to heart intensity. A significant difference between heart intensities in bronchogenic carcinoma and pneumonia patient groups might be probably due to the age difference between the two groups. Therefore heart intensity in the 4 hr 123I-IMP image may reflect certain metabolic and/or myocardial change(s) with aging.

Differentiation of bronchogenic carcinoma from secondary changes, obstructive pneumonitis and/or collapse by I-123 IMP lung imaging.

Serial lung images with N-isopropyl-p-[I-123]-iodoamphetamine (I-123 IMP) were obtained to assess the imaging findings and to clarify the lesion to uptake relationships in 74 lesions in 73 patients with various histological types of bronchogenic carcinoma. A decreased uptake area was observed in all 74 lesions in the initial one or two-min I-123 IMP image. The initial image was analogous to a Tc-99m MAA lung perfusion image in 70 patients in whom both lung imaging procedures were performed. The imaging findings changed following this initial phase. At 4 hr, the lesion was depicted as either areas of decreased uptake or increased uptake or a combination of the two. Comparison between the lesion findings in the 4-hr I-123 IMP images, radiograms and removed specimens revealed that areas of decreased uptake corresponded to the cancerous portions of the lung mass or pleural effusion and areas of increased uptake corresponded to inflammatory portions including obstructive pneumonitis and/or collapse. Thus, the 4-hr I-123 IMP lung images can be used to discriminate the cancerous portion from associated secondary changes, obstructive pneumonitis and/or collapse.

Cancer and other causes of death among Koreans in Fukuoka, Japan, 1976-1986.

Mortalities from cancer and other causes among Koreans living in Fukuoka, Japan, between 1976 and 1986 were examined as compared with those of Japanese in the prefecture. Korean males had a marked excess in all-cause mortality, while the excess among females was less prominent. In both sexes, mortalities from liver cancer, liver cirrhosis, accident and suicide were markedly increased in the Korean population. These findings are in agreement with those observed among Koreans in Osaka. Although 20-30% lower-than-Japanese mortality from stomach cancer has been reported for Koreans in Osaka, those in Fukuoka had a risk of this cancer comparable to that of Japanese. A life-style survey of Koreans in Japan might provide a better understanding of the disease patterns observed in this population.
