Morphological features of Spitz naevus as observed by digital videomicroscopy.

A characteristic epiluminescence pattern of pigmented epithelioid and/or spindle cell naevus, or Spitz naevus, has been described previously. The aim of this study was (i) to evaluate the characteristic morphological features both of pigmented and non-pigmented epithelioid and/or spindle cell naevi observed employing a videomicroscope, (ii) to identify their histopathological correlates and (iii) to assess the improvement in diagnostic accuracy for epithelioid and/or spindle cell naevi obtained by means of this new instrumental device. Clinical, videomicroscopic and histopathological diagnoses were performed on 26 epithelioid and/or spindle cell naevi. Moreover, the videomicroscopic pattern of each lesion was described using appropriate morphological parameters. Based on their morphological aspect detected by digital videomicroscopy, epithelioid and/or spindle cell naevi can be subdivided into three main groups: (i) darkly pigmented lesions, (ii) red or light brown ESC naevi, and (iii) lesions with dark or brown areas on a light-brown background. Whereas most epithelioid and/or spindle cell naevi of the spindle cell type belonged to the morphological group I and group 3, most epithelioid cell lesions appeared as red or light-brown coloured naevi. Finally, instrumental observation by means of a videomicroscope enabled an improvement in diagnostic accuracy with respect to the naked eye observation, with an increase in sensitivity from 15% to 58%.

[Contact sensitization to thimerosal in healthy subjects].

The aim of our study was to evaluate the prevalence of thimerosal allergy in subjects free of dermatitis. 256 recruits, selected by the Military Health Authorities as healthy subjects, without dermatitis or ocular defects, underwent allergological examination. The percentage of sensitization observed (6.25%) was compared to that found among 2150 patients, consecutively admitted to the Allergological Service of our Department, with suspected allergic dermatitis. Here we found 76 positivities (3.53%). Contact sensitization to ammoniated-mercury is lower among thimerosal-negative subjects (1.88%) than among both the healthy group of thimerosal-positive subjects (12.5%) and thimerosal-positive patients (11.84%). In the group of 76 positive patients it was not possible to relate the sensitization to thimerosal to either a particular clinical dermatological picture or to a specific exposure.

Evaluation of the efficacy of oral cromolyn sodium or an oligoantigenic diet in children with atopic dermatitis: a multicenter study of 1085 patients.

One thousand eighty-five children with atopic dermatitis were enrolled in a multicenter study to evaluate the efficacy of 4 weeks of oral sodium cromoglycate or 4 weeks of a restricted diet. One thousand-eleven children (93%) concluded the study. At the end of the trial there was a significant improvement in skin lesions in the two groups: 61% of the patients in the sodium cromoglycate group and 69% in the restricted diet showed a significant improvement in atopic dermatitis. We concluded that, at least in our experimental design, both sodium cromoglycate and a restricted diet are equally effective in atopic dermatitis.
