Clinicoepidemiological profile of kala azar patients in Delhi.

Search for livelihood makes many people move from kala azar endemic areas to metropolitans like Delhi from where indigenous cases have not been reported. During 1986-1989, 211 laboratory confirmed kala azar cases were detected in Delhi. Of these 202 (95.73 per cent) were from Bihar and remaining nine (4.27 per cent) belonged to Uttar Pradesh. Younger age groups were affected more and maximum cases (54.02 per cent) were in the age group of 5-15 years. Male:female ratio was 2:1. Fever (100 per cent) along with mass in abdomen (99.05 per cent) were the major presenting complaints. Splenomegaly was seen in all the patients whereas hepatomegaly was a feature in 184 (87.20 per cent) cases. Response to sodium antimony gluconate (SAG) was variable and number of injections administered reached upto 80 in some (23) cases. All 37 patients responded to pentamidine after two full courses of SAG failed to bring about any clinical and parasitological cure.

Bacteriological study of drinking water during epidemic of cholera in Delhi.

Sixty drinking water samples collected from various sources in different areas of Delhi during epidemic of cholera and gastroenteritis were analysed for bacteriological standards. Only 27 (45 per cent) samples were found to be satisfactory for human consumption. Remaining thirty-three (55 per cent) samples showed presence of coliform organisms with MPN value ranging from 10 to 1800+ per 100 ml. Among these positive samples, 31 (93.9 per cent) samples contained faecal coli. Twenty out of thirty (66.6 per cent) Hand pump, 9 out of 21 (42.8 per cent) Taps 2 out of 4 (50 per cent) Tube well and 2 out of 5 (40 per cent) of miscellaneous sources showed contamination with coliform organisms.

Evaluation of CIEP and ELISA in a rodent malaria model.

Circulating antigens could be detected by both ELISA and CIEP in the sera of mice infected with Plasmodium berghei. However, ELISA was positive for circulating antigen even though the Giemsa stained smears did not show significant parasites. Hence, ELISA can have a diagnostic value for detection of antigens as a marker of active infection.

Comparative evaluation of the protective effect of immune spleen cells and immune RNA against Plasmodium berghei.

Earlier studies from this laboratory indicated that passive transfer of viable or frozen-thawed cells from spleens and lymph nodes of immune mice resulted in a significant protective immunity against Plasmodium berghei in syngeneic recipients. To assess whether immune RNA played a role in conferring such protection, experiments were designed wherein immune RNA was isolated from immune monkeys, rats and mice and transferred to normal mice. The effect of transfer was assessed by challenging RNA-primed animals with Plasmodium berghei. Results indicated that immune RNA failed to confer resistance against P. berghei both in syngeneic and in heterologous systems.

Leprosy in young urban children.

Prevalence among leprosy patients of children in the age group of 0-14 years was 3.8%, with 9 male preponderance. The children were largely immigrants from the endemic neighboring states of Uttar Pradesh and Bihar. A borderline group formed the bulk of the patients. The disease had its onset between ages 5 and 9 in more than half the patients. There was a striking contrast between the age at onset and that of reporting. Usually, the lesions were multiple. Single lesions were, however, present on the exposed parts. Skin-slit-smears were positive in borderline borderline (BB), borderline lepromatous (BL), and lepromatous lepromatous (LL) only. A good clinicopathologic correlation was apparent in most patients.

Immune response studies in relation to protection induced by using MDP as an adjuvant in malaria.

Muramyl dipeptide (MDP) was an important compound conferring protection to mice against the lethal malaria parasite Plasmodium berghei. The mode of protection of this compound was studied using different humoral and cellular parameters. The observations indicate that MDP boosts both humoral antibody response as well as delayed type hypersensitivity reactions, but as far as phagocytosis by macrophages is concerned, malarial mice are already maximally stimulated and MDP makes a marginal difference in immune phagocytosis only.

AIDS in India: a report of three cases with review of literatures.

Sera from 3500 individuals including patients of non-Hodgkin's lymphoma, patients with sexually transmitted diseases, leprosy, jail inmates, healthy laboratory workers and patients on hemodialysis were screened for antibodies against human immunodeficiency virus (HIV) as a sero surveillance exercise of one of the centers set up by the India Council of Medical Research at the Postgraduate Institute of Medical Education & Research, Candigarh. Three individuals were found to be strongly positive by ELISA using the Wellcozyme kit, with no background noise in the population studies. These three patients were also evaluated by the Western blot technique and showed strong antibody reaction to all the major gene products. It was also possible to ascertain the approximate incubation period of infection in a case who rapidly developed full blown AIDS and died of wasting syndrome. Comprehensive data of 14 cases was also collected from all the centers in India and in every case one could trace the contact outside India. None of the 3 cases studied at our center had Kaposi's sarcoma. These observations could be important landmarks in the epidemiology of AIDS in India.
