[Hemodynamics in atrioventricular block in acute myocardial infarct and its changes under the influence of electric stimulation of the heart].

Hemodynamics was studied in the initial condition and against the background of electric stimulation of the heart in 18 patients with acute myocardial infarction complicated by complete atrioventricular block. It is shown that block promotes the development of circulatory insufficiency in patients with myocardial infarction. The marked character of hemodynamic disorders and the changes induced in it by electric stimulation are determined in many respects by the functional condition of the left ventricle. In most patients with acute myocardial infarction complicated by complete atrioventricular block, with the exception of those with a clinical picture of cardiogenic shock, the main values of hemodynamics demonstrated against the background of electric stimulation hardly differ from the corresponding values in patients with myocardial infarction and no disturbances of atrioventricular conduction. During electric stimulation of the heart in the contingent of patients examined, the external work of the heart and the "tension/time" index considerably increase which suggests a marked increase in the myocardium requirements in oxygen. The probability of unbalanced increase in the requirements of the myocardium in oxygen against the background of electric stimulation dictates the necessity for raising its parameters to optimum levels.

[P-controlled electrical heart stimulation in complete transverse block in acute myocardial infarct].

The article discusses the results of the comparative study of certain hemodynamic indices in patients with myocardial infarction complicated by complete atrioventricular block, during routine electric stimulation of the ventricles and during P-controlled electric stimulation of the ventricles. Changes in hemodynamics were more marked in the last case, but only the values of the minute cardiac volume and of central venous pressure were found to differ substantially in these two groups. The authors presume that such a hemodynamic situation may occur in myocardial infarction in which P-controlled electric stimulation will produce more essential changes in hemodynamics that those induced by routine electric stimulation. Synchronized electric stimulation may be more preferable in such cases.

[Renin and various factors of its regulation in acute myocardial infarct].

The activity of blood plasma renin and the content of aldosterone were studied in dynamics in 55 patients with acute myocardial infarction following a different course. The dependence of renin activity and potassium content is analysed. The authors did not find any dependence between the degree of growth of blood plasma renin activity and the course of the disease; exception to this were patients with disorders of rhythm and conductivity. It is presumed that changes in potassium metabolism play an important role in the changes in renin activity in myocardial infarction.

[Optimal frequency for electrical stimulation of the heart in transverse block in acute myocardial infarct].

Study of hemodynamics did not disclose any sole criterion for determining the optimum frequency of electric stimulation of the heart in the IV degree atrioventricular block in patients with acute myocardial infarction. Only a complex of indices serves the purpose, among which the first and foremost is the patient's general condition appraised by means of the routine physical methods and, whenever possible, by measuring the cardiac minute volume and the central venous pressure. The necessary frequency of electric stimulation in the prevention of arrhythmia should be chosen empirically.

The effect of light and temperature on the growth of two subtropical and two temperate grass species.

The effect of light intensity and temperature on the growth of young plants of two temperate (Lolium perenne, Festuca arundinacea) and two subtropical (Lolium rigidum, Bromus unioloides) grass species was studied in the northern part of the GDR under open-air conditions. It was found that the subtropical species reacted highly sensitive to changes in the two environmental factors and showed absolutely higher growth performance during the summer as compared with the temperate species. The multiple regression analysis showed positive correlations between the relative growth rate (RGR) and the net assimilation rate (NAR) on the one hand and temperature and light intensity on the other hand. For most regression functions the range of variation was found to be between 63.5 and 91.3 per cent. No clear proof could be found of the dependence of the leaf area ratio (LAR) on light and temperature. The results are discussed and compared with similar investigations.

Ultrasonic diagnosis of intrauterine fetal death.

When there is doubt concerning fetal viability, the confirmation or exclusion of fetal death assumes major importance in the management of pregnancy. Frequently the clinical diagnosis is uncertain and recourse must be had to ancillary investigation. Radiologic methods may confirm the clinical suspicion of intrauterine fetal death in many cases, but these are accompanied by the hazard of irradiation. Ultrasound can provide valuable information regarding fetal viability. Ultrasonic signs of fetal death are discussed in the light of the literature and our own experience, with emphasis on the value of the M-mode in detecting fetal heart pulsations.

The role of angiography in the management of intestinal carcinoid.

Carcinoid tumors of the intestine may present with bowel obstruction or with the carcinoid syndrome. Confirmation of the diagnosis by biochemical means is easy. Management is often surgical and a number of different procedures have been advocated, e.g. resection of the primary, hemicolectomy and lately, partial hepatectomy. The angiographic features of two cases are presented and discussed; the usefulness of the information so gleaned in the choice of management is stressed.

Ultrasonography of the thyroid and parathyroid glands.

High-frequency ultrasonography is ideally suited to imaging of the thyroid and parathyroid glands by virtue of their superficial location in the neck. Ultrasonography of the thyroid is most commonly used in the evaluation of the solitary nodule. Ultrasonography of the parathyroid glands is usually performed to evaluate and localise parathyroid abnormalities in patients with hypercalcemia. The increased diagnostic acumen offered by this modality allows a realistic working diagnosis and leads to a rational management strategy.

Decision making in thyroid and parathyroid surgery: the influence of imaging.

Surgical decision making is essentially based on experience, augmented by a management philosophy. In this manuscript we review concepts of thyroid and parathyroid surgical decision making in relation to diagnostic imaging input. These two endocrine glands have crucial anatomic associations, but very different pathologic conditions; it is the specific pathologic considerations that ultimately determine the decision-making process. For us, diagnostic imaging has enabled a more effective surgical decision-making process through thorough pre-operative planning. The strategy for parathyroid surgery is based upon the pathologic localization obtained by high resolution ultrasound. Thyroid surgical strategy is more heavily influenced by experience, and our policy is total thyroidectomy for all malignant and benign tumors with airway compression.
