Evaluation of flow electromyography in patients with benign prostatic hyperplasia.

Combined uroflowmetry and external sphincter electromyography studies were done both preoperatively and postoperatively in patients with benign prostatic hyperplasia. In 56 patients preoperative electromyography during voiding showed silent or markedly suppressed activity in 41 patients (73.2%), while the remaining 15 patients (27.8%) revealed active electromyography. In the postoperative studies, 12 patients revealed active electromyography even after the release of obstruction. The urine flow rate following operation was not drastically improved in the active electromyography group compared to the silent electromyography group. We suppose that long-term urethral obstruction with prostatic adenoma could influence the function of relaxation of the external sphincter muscle in some patients with benign prostatic hyperplasia.

Clinical studies on the effectiveness of prazosin HCl (Minipress tablets) in the treatment of dysuria accompanying benign prostatic hyperplasia.

Prazosin HCl was administered in dosages of 1.5-4.5 mg/day for 4 weeks to 40 patients suffering from dysuria accompanying prostatic hyperplasia. Significant reductions were observed in the mean frequency of night-time and 24-hour urination. Additionally, significant improvements were recorded for the time for start of urination, time from start to finish of urination and related variables. The improvements correlated with dosage levels. Objective variables measured by urine flow rates also exhibited significant improvement. A statistically significant decrease in systolic without a decline in diastolic blood pressure was observed. Slight dizziness was observed in 1 patient and elevated laboratory test values in another, but neither warranted discontinuation of the medication; however, 1 patient did have two doses of medication held. Overall, the drug was judged beneficial to some degree in 95% of the patients.

Recurrent herpetic keratitis: failure to detect herpes simplex virus infection using the Syva MicroTrak HSV1/HSV2 direct specimen identification/typing test.

A 35-year-old man had developed recurrent herpetic keratitis characterized by dendritic keratitis at intervals of a year. We were able to culture cytopathic agents repeatedly from his lesions by inoculating Vero cells. The cultures yielded definitive evidence of a virus that caused a cytopathic effect within 3 days. However, these virus strains could not be identified as herpes simplex virus (HSV) in immunofluorescence assays using the Syva MicroTrak HSV1/HSV2 direct specimen identification/typing test. Rather they were identified as strains of HSV type 1 (HSV-1) on the basis of plaque morphology, neutralization tests, electron-microscopic examination and DNA restriction endonuclease analysis. Our results allow us to assume the existence of HSV-1 strains isolated clinically that are negative to analysis using the Syva Micro-Trak HSV1/HSV2 direct specimen identification/typing test.

[99mTc-MDP abnormal uptake in the femur shaft in hemi-lateral hip joint disorders].

During the past 4 years, three-phase bone scintigraphy using 99mTc-MDP has been studied in 68 patients suffering from hemi-lateral hip-joint disorders. We were impressed on abnormal uptake of the shaft of femur on the involved side. 99mTc-MDP uptake in the shaft of femur is compared with between involved leg and normal leg of 68 cases with hemi-lateral hip-joint disorders (76 examinations). We excluded cases of osteomyelitis, cases of malignant tumor, and post-operative cases. In early images, the 99mTc-MDP uptake is not always increased in involved side. But in delayed images, there are no patients whose normal side's 99mTc-MDP uptake is increased. The 99mTc-MDP uptake in early image is not related only with blood flow of the thigh but with the amount of soft tissue of the thigh. And the 99mTc-MDP uptake in delayed image is related with disuse osteoporosis.

The effect of selenium ions on the catabolism of prostaglandin E2 in rabbit kidney cortex.

The effect of selenium ions on the catabolism of prostaglandin E2 by 15-hydroxy prostaglandin dehydrogenase and prostaglandin delta 13 reductase in rabbit kidney cortex was examined. Selenium ions had no effect on the 15-hydroxy prostaglandin dehydrogenase activity. On the other hand, selenium ions inhibited the prostaglandin delta 13 reductase activity at concentrations ranging from 0.05 to 0.5 mM. The effect was concentration-dependent. The inhibition by selenium ions was non-competitive with regard to NADH and 15-keto PGE2, respectively. These results suggest that selenium ions have the potential to increase 15-keto PG levels in the kidney by inhibiting the prostaglandin delta 13 reductase activity and that such action of selenium ions may participate in renal damage of selenium poisoning.

Abnormal chromosome 9 in a neonate program. Report of three cases.

We describe three cases with abnormal chromosome 9. Patient 1 shows translocation in a homologous chromosome, with a karyotype of 46,XX,t(9;9)(9pter----cen----9pter; 9qter----cen::9q13----9qter), 1qh+. This case has a variety of anomalies, including brain anomalies. Patient 2 shows a partial trisomy 9p with a karyotype of 47,XY,+del(9)(pter----q11:). The patient has the typical clinical features of 9p trisomy syndrome. Patient 3 is unique because of partial 9p tetrasomy mosaicism without phenotypic abnormalities; the karyotype is mos 46,XY/47,XY,+dic(9)(pter----cen----q21::q21----cen----pter).

[Evaluation of the left ventricular myocardial infarction by Tc-99m cardiac pool SPECT and TI-201 myocardial SPECT--diagnoses in the inferior wall of the left ventricle and septum in the vicinity of the base of the heart].

Thallium-201 myocardial single photon emission computed tomography (myocardial SPECT) has been believed likely to miss lesions or false lesions in the inferior wall of the left ventricle and septum in the vicinity of the basis of the heart. In 35 patients having undergone both coronary arterio-angiography and contrast left ventriculography, technetium-99m cardiac pool SPECT synchronized with electrocardiography (cardiac pool SPECT) and myocardial SPECT were performed simultaneously. Both cinematographic and phase techniques were used for the cardiac pool SPECT. Visual and Bull's eye techniques were used for the myocardial SPECT. The rate of correct diagnoses made by myocardial SPECT was compared by site with that by cardiac pool SPECT. With the inferior wall of the left ventricle of diagnoses made by cinematographic and phase techniques of cardiac pool SPECT, 97.0% each was correct. The rates of accurate diagnoses made by visual and Bull's eye techniques of myocardial SPECT were 97.0 and 93.9%, respectively. The rate at which at least one of the SPECT methods was correct was 97.1%. With the septum, the corresponding rates of accurate diagnoses were 97.1, 94.3, 90.0, and 90.9%, with rate of 94.3% at least one of these two methods made correct diagnoses. In conclusion, for diagnosing infarcts of the septal wall of the basis, the cardiac pool SPECT is more accurate than myocardial SPECT.

[A case of intramedullary spinal cord metastasis from adenocarcinoma of corpus uteri].

The authors present a case of a woman with intramedullary spinal cord metastasis from adenocarcinoma of the corpus uteri. Such a case was not able to be found in the literature. A 54 year-old woman complained of right upper extremity weakness and headache. CT scan showed abnormality, and removal of a brain tumor was performed. After that, total hysterectomy, retroperitoneal lymphadenectomy and chemotherapy was carried out. Three months later, she suddenly experienced bilateral weakness in her lower extremities. After a few days she was unable to walk. Gd-DTPA enhanced MRI showed an abnormal mass at the upper part of the conus medullaris. Total removal of the intramedullary tumor was performed and adenocarcinoma was diagnosed histopathologically.

[Evaluation of endobronchial foreign bodies by 99mTc-MAA pulmonary perfusion scintigraphy].

We evaluated the lung perfusion scintigraphic findings of endobronchial foreign bodies in four children. We could detect the bronchial stenosis due to foreign bodies by lung perfusion scintigraphy more easily than plain chest radiograph and predict the location at lobar bronchi level according to the hypoperfusion area. We guessed that the scintigraphic abnormal finding after plain chest radiographic normalization reflect bronchial inflammatory change after foreign body removal and so it is very important to perform the follow up scintigraphic examination.

Complete amino acid sequence of an immunomodulatory protein, ling zhi-8 (LZ-8). An immunomodulator from a fungus, Ganoderma lucidium, having similarity to immunoglobulin variable regions.

The complete amino acid sequence of a novel immunomodulatory protein, ling zhi-8 (LZ-8), isolated from a fungus, Ganoderma lucidium (Kino, K., Yamashita, A., Yamaoka, K., Watanabe, J., Tanaka, S., Ko, K., Shimizu, K., and Tsunoo, H. (1989) J. Biol. Chem. 264, 472-478), was determined by protein sequencing. The polypeptide consists of 110 amino acid residues with an acetylated amino end and has a molecular mass of 12,420 Da including an amino-end blocking group. There is no attachment site for an Asn-linked oligosaccharide chain, consistent with the very low carbohydrate content of LZ-8. These results indicate that the native form of LZ-8 with a molecular mass of 24 kDa is a homodimer of the LZ-8 polypeptide whose sequence is described here. Furthermore, the LZ-8 chain shows considerable similarity to the variable region of immunoglobulin heavy chain both in its sequence and in its predicted secondary structure. The interesting possibility that LZ-8 is related to an ancestral protein of the immunoglobulin superfamily is also discussed.

Calcium induces membrane translocation of 12-lipoxygenase in rat platelets.

Translocation of soluble 12-lipoxygenase to membranes was examined in rat platelets. Preincubation of platelet homogenates with 0.1-10 microM Ca2+ resulted in an increase in 12-lipoxygenase activity of the particulate fraction with a concomitant decrease in that of the soluble fraction. Kinetic parameters of 12-lipoxygenase of the soluble and membrane fractions were not changes in the presence of 10 microM Ca2+. Ca2+-induced association of 12-lipoxygenase to the particulate fraction was dependent on the amounts of platelet-soluble and membrane fractions but not on the incubation temperature. 12-Lipoxygenase activity associated with the particulate fraction was completely dissociated by reducing the concentration of Ca2+ to 10 nM. Ca2+-induced association of the enzyme also occurred in the boiled- and trypsin-treated membranes but was significantly reduced in the phospholipase A2-treated membranes. Soluble 12-lipoxygenase also associated to liposomes in a Ca2+-dependent manner. Pretreatment of platelets with thrombin (0.5-5 units/ml) significantly caused a translocation of soluble 12-lipoxygenase to particulate fraction; in the time course study, the translocation was observed at the thrombin pretreatment of 1, 5, and 10 min. These results suggest that stimulation of platelets is followed by the translocation of soluble 12-lipoxygenase to membranes, which is mediated by physiological concentration of Ca2+.

[Evaluation of CRT images of digitized film angiographs].

X-ray sheet film images of the test chart, the vascular phantom and angiography were digitized at sampling pitch of 0.2 mm and 0.15 mm using film digitizer TFR-01 (Toshiba) and transferred to a device for image storage and display system with 1635-line display monitor (TDF-500AS, Toshiba). Comparison of image qualities between film- and CRT-images was performed in fundamental and clinical studies. Resolution of the test chart image of conventional radiography was worse on CRT than on the original film, although it was improved when film image was digitized at resolution of 0.15 mm/pixel in comparison with that at resolution of 0.2 mm/pixel. Moiré stripes which occurred due to interference were found on CRT images taken using a grid technique. On CRT images of X-ray sheet film using direct magnification technique moiré stripes were not produced because of non grid technique, and the resolution approached that of the original film. In the study using vascular phantom, the optimal image on CRT could be obtained by various image processing procedures, and image quality on CRT with resolution of 0.15 mm approached that of original film. In case of direct magnification CRT images were superior to film images. Subtraction image of the vascular phantom at resolution of 0.2 mm/pixel was obtained on CRT and compared with film subtraction image. On conventional subtraction CRT image moiré stripes impaired the image quality in comparison with the film subtraction. However, magnification subtraction image of the vascular phantom on CRT was superior to the film subtraction. The results obtained in the test chart studies and phantom studies were also confirmed in clinical studies using various kind of angiograms. In addition, ROC study using clinical angiograms showed no significant statistical differences between the original film and CRT image even with 0.2 mm matrix size. Angiographic image on CRT at resolution of 0.15 mm/pixel or less is available for clinical use in place of conventional film image.

Immunohistochemical studies on the somatostatin- and growth hormone-releasing factor (GRF)-neurons in the hypothalamus of the novel dwarf rat; the spontaneous dwarf rat (dr).

Immunoreactivities of somatostatin and growth hormone-releasing factor (GRF) in the hypothalamus of spontaneous dwarf rats (SDRs, gene symbol; dr), which show isolated growth hormone (GH) deficiency, and normal rats were studied with an avidin-biotin complex (ABC) immunohistochemical method. Somatostatin and GRF immunoreactivities were observed in the median eminence and each afferent nucleus with no difference between these animals. The observation suggests that the etiology of the GH deficiency lies in GH cells themselves and is not hypothalamic in origin. This observation indicates that the SDR is a new model animal for type I dwarfism.

[RGSS-IDJ and its application to cranial computed tomography].

RGSS-IDJ is developed as the Japanese version of Report Generation Support System for Imaging Diagnosis (RGSS-ID), which is a developmental computer system that applies artificial intelligence (AI) methods to a reporting system. Now RGSS-IDJ supports the report generation of cranial computed tomography. A representation scheme called Generalized Finding Representation (GFR) is proposed, to bridge the gap between natural language expressions in the radiographic report and AI methods. GRF for RGSS-IDJ is the same as for RGSS-ID. The basic style for entering the findings on the radiograph is the dialogue system with the routine of query and answering it by selecting items with a mouse. This system encodes the input findings into the network expressions, which are represented as the list form in the LISP computer language. And, it reserves them into the knowledge data base. The content of the report will be able to be utilized for various analyses within AI paradigm. The final radiographic report is made in the natural Japanese language.

Identification of an enhancer-type sequence that is responsive to Z and R trans-activators of Epstein-Barr virus.

We studied the regulatory region of the open reading frame BHRF1 of Epstein-Barr virus (EBV) DNA that was responsive to two trans-activators (Z and R) encoded by the BZLF1 and BRLF1, respectively. The 200-bp sequence, nucleotide numbers 53,617 to 53,817 on the EBV map, was sufficient for conferring responsiveness to Z and R. This 200-bp sequence also enhanced expression of the chloramphenicol acetyltransferase (CAT) gene from the simian virus 40 promoter in response to both Z and R, even when inserted downstream of the cat gene. The results indicate that the Z and R response sequence upstream of the BHRF1 has the properties of an enhancer.

Computer-assisted conformation radiotherapy with a variable thickness multi-leaf filter.

A new computer-assisted conformation radiotherapy with a variable thickness multi-leaf filter system was developed. Each wedgeshaped compensating filter (aluminum, tan theta = 0.1) travels against the movement of the ipsilateral leaf and according to defined parameters. By using our filter, a homogenous dose distribution in the target area was obtained and high-dose levels outside the target area decreased significantly.

Nucleotide sequences of gag and env genes of a Japanese isolate of HIV-1 and their expression in bacteria.

A DNA clone of HIV-1, JH3, was isolated from a Japanese patient with hemophilia and the gag and env genes were sequenced. The nucleotide and deduced amino acid sequences were similar to those reported and showed high divergence in the env gene, particularly in the extracellular domain of the env. The genetic variation of JH3 isolated from a Mongolian was within the range of those of isolates from whites and blacks. The gag and env polypeptides were efficiently expressed in E. coli as fusion proteins with beta-galactosidase, and the products were shown to be useful as diagnostic reagents.

Seroepidemiology of herpes simplex virus in Yogyakarta, Indonesia.

Sera from 487 individuals in Yogyakarta, Indonesia, were tested for antibodies to herpes simplex virus (HSV) by a passive hemagglutination method. Age-specific incidence rates for antibodies to HSV were calculated. For sera from persons other than prostitutes, in the age group from 10 to 19, the positive rate was 48% but in the age group higher than 20, it was more than 87%. Fifty of 59 pregnant women (85%) were positive. The positive rate and the distribution of antibody levels in prostitutes were higher than in the general population.
