Serum levels of macrophage colony stimulating factor (M-CSF) in liver disease.

We investigated the serum level of macrophage colony stimulating factor in acute and chronic liver disease. Levels of macrophage colony stimulating factor (mean +/- SD, ng/ml) were significantly higher in acute hepatitis (5.67 +/- 1.01, p < 0.01) and chronic active hepatitis (3.34 +/- 1.19, p < 0.01) than in healthy volunteers (1.90 +/- 0.25), asymptomatic hepatitis B virus carriers (1.98 +/- 0.40), and chronic persistent hepatitis (2.34 +/- 0.43). Levels of macrophage colony stimulating factor showed a highly significant correlation with the serum alanine aminotransferase levels in acute hepatitis (p < 0.01, rs = 0.903) and in chronic active hepatis (p < 0.01, rs = 0.672). Levels of macrophage colony stimulating factor in patients with cirrhosis (cirrhosis; 3.11 +/- 0.93 and hepatocellular carcinoma; 3.30 +/- 0.74) were significantly higher than in patients with chronic persistent hepatitis although the alanine aminotransferase levels were not significantly different. In cirrhosis, levels of macrophage colony stimulating factor correlated positively with the serum alanine aminotransferase levels (p < 0.05), total bilirubin levels (p < 0.05), and indocyanine green clearance (p < 0.05). An immunohistochemical study showed an increased number of macrophage colony stimulating factor positive mononuclear cells in portal areas in acute hepatitis. Our findings suggest that; (a) the serum levels of macrophage colony stimulating factor represent ongoing hepatocellular necrosis in acute and chronic liver disease, (b) the source of the increase in the serum macrophage colony stimulating factor levels in hepatic inflammation may be, in part, its production by infiltrating mononuclear cells in the liver, and (c) cirrhosis also causes elevated serum levels of macrophage colony stimulating factor.

Childhood bullous pemphigoid: immunohistochemical, immunoelectron microscopic, and western blot analysis.

The most reliable diagnostic criteria of childhood bullous pemphigoid (BP) have been the linear deposition of IgG and C3 at the basement membrane zone (BMZ) and the frequent presence of circulating IgG BMZ antibodies. To characterize further the immunologic features of childhood BP, we performed direct and indirect immunoelectron microscopy (IEM) and Western immunoblotting on specimens from a 3-month-old boy with BP who had immunofluorescence evidence of linear BMZ IgG and C3 deposition and circulating IgG BMZ antibody. By direct IEM, autoantibody deposits were seen in the upper portion of the lamina lucida and on the undersurface of basal keratinocytes. Indirect IEM showed that the binding sites of BP antibody were closely related to the intracellular attachment plaques of hemidesmosomes. By Western blotting, the patient's serum recognized 180 kd epidermal protein.

Effects of Chinese herbal medicines on DNA-synthesizing enzyme activities in mammary tumors of mice.

Sho-saiko-to (SST) and Juzen-taiho-to (JTT), Japanese modified Chinese herbal prescriptions, suppressed the activities of thymidylate synthetase and thymidine kinase involved in de novo and salvage pathways for pyrimidine nucleotide synthesis, respectively, in mammary tumors of SHN mice with the reduction of serum prolactin level. These results indicate that SST and JTT may have the antitumor effects on mammary tumors.

Suppression by kampo medicine, sho-saiko-to, on papillomas induced by 7,12-dimethylbenz(a)anthracene in mice.

Sho-saiko-to (SST) is a Japanese modified, traditional Chinese herbal medicine (Kampo medicine) consisting of seven medical plants. We examined the effects of SST on formation and growth of squamous cell papillomas induced by 7,12-dimethylbenz(a)anthracene application in mouse skin. Chronic oral administration of SST reduced the incidence and growth of papillomas with the reduction of activities of succinate-dehydrogenase and thymidylate synthetase, which were evaluated as the cell viability and DNA synthesis via the de novo pathway, respectively.

Effects of recombinant human erythropoietin on DNA synthesis in rat hematopoietic organs.

In an attempt to define the effects of recombinant human erythropoietin (rEPO) on DNA synthesis in hematopoietic organs, we investigated DNA-synthesizing enzyme activities, i.e., thymidylate synthetase and thymidine kinase activities, and bromodeoxyuridine-immunohistochemistry during the recovery phase of hematopoietic cells in bone marrow and spleen after the hypoplastic period induced by cyclophosphamide (Cy) treatment in rats. Treatment with rEPO increased enzyme activities and cell number of erythroid series in bone marrow cells; it also increased organ weight and S-phase cells in the spleen, followed by an augmentation of the number of erythrocytes and a rise in the hemoglobin and hematocrit levels in peripheral blood with or without Cy treatment.

Clinical characteristics of halitosis: differences in two patient groups with primary and secondary complaints of halitosis.

Halitosis, defined as an unpleasant oral odor, has become a health concern among the general public. The objective of this study was to evaluate the diversity of clinical characteristics of halitosis of the patients who visited dental clinics. Sixty-eight patients with primary complaints of halitosis and 19 patients with primary complaints of periodontal diseases but secondary complaints of halitosis were studied by organoleptic examination. The patients with primary complaints were diagnosed as having halitosis in fewer cases than the patients with secondary complaints-25% and 53%, respectively. Patient complaints for halitosis were further categorized, by questionnaire, into three types: Type 1, self-conscious; Type 2, conscious by the indication of others; and Type 3, conscious by presumptions from the attitude of others. Although 80% of the patients of both groups were of Type 1, only 24.1% of the Type 1 patients with primary complaint, in comparison with 50% of the Type 1 patients with secondary complaint, were actually found to have halitosis. The results suggest that the majority of patients with primary complaints of halitosis at the dental clinic did not actually have halitosis, but suffered from an imaginary halitosis due to presumptions based upon others' attitudes. After treatment, these patients were more likely to be dissatisfied than patients who had visited the clinic with halitosis as their secondary complaint.

Changes with age of mammary glands in male and female soft-furred rat, Millardia meltada, in relation to prolactin and testosterone.

The soft-furred rat, millardia (Millardia meltada), is characterized by the development of androgen-dependent mammary tumours only in males. The age-related changes of the activities of thymidylate synthetase (TS) and thymidine kinase (TK), which contribute to DNA synthesis through de novo and salvage pathways, respectively, and structure in the mammary glands were studied in both males and females of this species between 5-28 months of age. While TK activity had no relation to age, TS activity decreased with age in males. In the females, TK activity increased with age, but not TS activity. These enzyme activities were generally higher in females than in males. The mammary glands of both sexes consisted of fine ducts with small end-buds and the glands of males contained mostly black pigments at any age examined. In either males or females, serum levels of prolactin and testosterone related little with age, DNA synthesizing enzyme activities or structure of the mammary glands. Furthermore, elevation by pituitary grafting of circulating prolactin affected neither DNA synthesizing enzyme activities nor structure of mammary glands in both sexes. The histological structures of adrenal, testis, ovary, ventral prostate and uterus of millardia were essentially similar to those of mice or rats.

[Negativity bias in evaluation of own personality traits in depressed persons].

There have been two views on how depressed people perceive themselves. One argues that they attend excessively to negative aspects of their selves. The other views that the depressed attend evenly to positive and negative sides of themselves, and the nondepressed persons attend more to their positive aspects. This study examined these views of cognitive bias, through a comparison of self-referent judgments of trait adjectives by mildly depressed and nondepressed groups of people. The Beck Depression Inventory was used to measure the severity of depression, and mildly depressed and nondepressed students, 342 in total, made self-referent judgments with a list of trait adjectives. The list consisted of pairs of semantically similar adjectives with contrasting desirability. With negative words, the mildly depressed tended to judge themselves more negatively than the nondepressed control. With positive words, no significant differences were found between the judgments by the two groups. It was concluded that negativity bias in self judgments was characteristic of the depressed persons.

Structural elucidation of aibellin, a new peptide antibiotic with efficiency enhancing activity on rumen fermentation.

A new peptide antibiotic, aibellin, that had the efficiency enhancing activity on rumen fermentation, was isolated from the culture broth of the fungus, Verticimonosporium ellipticum D1528, and its primary structure was elucidated from spectrometric analysis and chemical degradation. Aibellin is a 20-residue peptaibol, and it has a unique structural feature in the novel C-terminal amino alcohol. Moreover, aibellin is the first peptaibol that possesses two acidic amino acids in the C-terminal region and a Phe residue in the middle of the sequence.

[Side effects of interferon on endocrine and respiratory system in 545 cases of chronic hepatitis C].

We investigated the side effects of interferon (IFN) on the endocrine and respiratory system in 545 cases of chronic hepatitis C. Eleven of 494 (2.2%) patients with chronic hepatitis C who were treated with natural or recombinant interferon (IFN) developed thyroid disease while on treatment. Eight patients developed hyperthyroidism and 3 patients developed hypothyroidism. All 11 patients required definitive therapy, who became euthyroid after the therapy. Two patients received nIFN alpha and one patient received rIFN alpha 2b developed diabetes mellitus. Two patients received rIFN alpha 2a and rIFN alpha 2b, respectively, developed interstitial pneumonia 12 weeks and 24 weeks later, respectively. One patient showed positive reaction for RA test and LE factor and positive LE cell, and complained of fever, arthralgia and dry cough. These phenomenon disappeared after the cessation of IFN therapy.

[A case of lobar atrophy with stereotypic unusual laughing].

We report a case of a Pick's disease (frontal predominant type) with unusual laughing. The patient was a 54-year-old, right handed, female. The patient began to show low activity and inappropriate laughing at about 51 years. She did not speak at all and showed weird laughing with a voice "hi hi hi hi hi hi" frequently at first presentation. When asked some questions, she sometimes answered with a nod. Almost all of her laughing did not match with the situation and seemed to lack a relationship between affective change and observed expressions. The laughing could be elicited by non specific and varied stimuli. She often interrupt laughing voluntarily unlike pathological laughing. Neurological examination revealed visual rooting reflex and forced grasping reflex but there was no signs of pseudobulbar palsy often observed in patients with pathological laughing. Her electromyogram of facial expression during the laughing showed that electric discharge of M. corrugator supercilii was over 4 times as much as that in normal laughing. And it showed also that the duration of her laughing was several seconds as normal laughing. CT and MRI scans demonstrated atrophy in the fronto-temporo-parietal lobe bilaterally (L > R), most prominent in the frontal lobe. SPECT scan showed a significant diminution of blood flow in the atrophic region. An EEG was normal. We consider that the laughing of the present case is not due to an emotional disturbance but rather a disorder of affective expression as pathological laughing.

Benign recurrent intrahepatic cholestasis: a long-term follow-up.

We report on a case with a 50-year history of benign recurrent intrahepatic cholestasis, including 33 years of follow-up at our hospital. The attacks of jaundice began at the age of 1 year and have been repeated more than 14 times to date. During the follow-up period at our hospital, the patient has suffered 6 attacks of cholestasis. During the episodes of cholestasis, a dissociation between the serum level of bilirubin and that of bile acids was observed. The analysis of serum bile acids during cholestasis showed that cholic acid and chenodeoxycholic acid were the main bile acids, and no appreciable amounts of the secondary bile acids were detected. Between episodes the liver function tests, including serum bile acids, were normal. Despite multiple attacks of cholestasis over many years, no permanent liver damage has occurred. This case reaffirms the essentially benign nature of this disease.

[A case of acute promyelocytic leukemia (APL) with myeloblastoma in the oral cavity developing after receiving all-trans retinoic acid (ATRA)].

A 44-year-old woman was diagnosed as having acute promyelocytic leukemia (APL) in April 1988. On her first admission, chromosomal translocation (15; 17), +8, and +12 was detected. When she was readmitted to our hospital with the second relapse in May 1990, t(3; 13) and +8 was detected, instead of t(15;17). Complete remission was re-achieved with VP-16, MIT, and BHAC, but the third relapse occurred in September 1990. After obtaining informed consent, she was given etretinate 40 mg per day orally for 17 days, without any effect on leukemia. She was then given all-trans retinoic acid (ATRA) 60 mg per day orally for 29 days. Although a mild granulocytic recovery was observed, no sufficient hematological recovery was obtained (minor response). Besides common side effects of ATRA, such as dry skin and hypertriglycedemia, she had a myeloblastoma in the oral cavity, but it is unknown whether the symptom was a complication of ATRA therapy or not.

[Symptomatic Rathke's cleft cyst: a clinicopathologic study of 9 cases].

Nine cases of symptomatic Rathke's cleft cyst are reported. Their most frequent signs and symptoms included headache, chiasmal syndrome and hypopituitarism, while one of the cases developed a sudden onset of headache and vomiting following diabetes insipidus. Endocrinological findings showed a decreased ACTH, gonadotropin and growth hormone more frequently while there were 2 cases of hyperprolactinemia and 1 case of diabetes insipidus. In a neuroradiological examination, a plain skull X-ray showed 5 cases of ballooning of the sella turcica, and a CT scan demonstrated a low to high density of the cyst and 2 cases of marginal enhancement of the cyst. MRI mostly demonstrated a well delineated mass at the sella extending mostly into the suprasellar region and a low to high intensity of the cyst in the T1-weighted image. Two cases were marginally enhanced after gadolinium DTPA administration. The pathological examination, done on 6 cases, showed either single or multiple layers of the epithelium which were mostly ciliated. The epithelium was positive in PAS and Alcian blue in all cases and a histochemical examination showed 3 cases to be positive in EMA and 2 cases positive in CEA. A resection of the cyst wall and an opening of the cyst is thus recommended in symptomatic cases. Therefore, the transsphenoidal approach should be the choice of treatment in an intra- and suprasellar extension of the cysts with sellar enlargement.(ABSTRACT TRUNCATED AT 250 WORDS)

[Immunohistochemical studies on mycobacteria in tuberculous necrotizing granulomas].

For the purpose to know the pathogenesis of caseous necrosis in tuberculous lesion, authors compared the presence and the number of mycobacteria in the pulmonary tubercles of various histological types. Forty pulmonary tubercles obtained from 16 patients (9 males, 7 females; mean age 51) were examined. By Ziehl-Neelsen (ZN) method, acid-fast bacilli were detected in 40% of 5 proliferative, in 12.5% of 24 productive and none of 11 sclerotic granulomas. By immunohistological staining with anti-BCG antiserum, positive staining was observed in 60%, 87.5% and 91% of each type of granuloma, respectively. Types of necrosis were classified by silver-staining as necrosis after exudative reactions (EN), necrosis after productive reactions (PN) and filled materials after softening and liquification of necrotic lesions (LN). EN was observed in 24 tubercles, and mycobacteria were stained in 8% of EN by ZN method and in 75% by immunohistological method. PN was observed in 21 tubercles, and mycobacteria were detected in none and 5% of such type of necrosis by respective method. LN was observed in 28 tubercles, and mycobacteria were detected in 11% and 71% of these lesions by respective method. So, positive ratio by immunochemical method was significantly higher in EN and LN than in PN. Further, mycobacteria were stained with anti-BCG antiserum as granules and the number of such granules were much more numerous in EN and LN than in PN (p < 0.01). From these results, it was suggested that necrosis might occur by different mechanisms in exudative reactions and in productive reactions.
