Identification of alpha 1-adrenoceptor subtypes in the rabbit prostate.

1. The alpha 1-adrenoceptor subtypes of rabbit prostate were characterized in binding and functional experiments. 2. In saturation experiments, [3H]-prazosin bound to two distinct affinity sites in the rabbit prostate (pKD = 11.20 +/- 0.22 and 8.39 +/- 0.11, Bmax = 15.3 and 736 fmol mg protein-1). 3. In the displacement experiments, the binding was inhibited with shallow displacement curves by unlabelled prazosin, WB4101, and 5-methylurapidil, suggesting the presence of two distinct affinity sites for prazosin, WB4101, or 5-methylurapidil. On the other hand, HV723 displaced the [3H]-prazosin binding monophasically with a low affinity. From the results, the presence of two distinct alpha 1-adrenoceptor subtypes was suggested; presumably one is the classical alpha 1A (cloned alpha 1C) subtype with high affinity for prazosin, WB4101 and 5-methylurapidil but not for HV723 and the other corresponds to the alpha 1L subtype, which shows low affinity for the four antagonists. 4. In the functional experiments, prazosin, WB4101, HV723 and 5-methylurapidil competitively antagonized the contractile response to noradrenaline with low affinities close to those for the alpha 1L subtype determined in binding experiments. These results suggest that contractile response to noradrenaline in the rabbit prostate is predominantly mediated through the alpha 1L subtype.

Methicillin-resistant Staphylococcus aureus: colonization and development of infection in patients with haematological disorders.

A retrospective study of 53 patients with haematological disorders whose bacterial cultures were positive for methicillin-resistant Staphylococcus aureus (MRSA), was performed to analyse the risk factors for MRSA infection, and the prognostic factors. Sixteen patients showed colonization by MRSA but never developed infection(C), 16 showed colonization and subsequent infection(C-I), while 21 had MRSA infection at the time of first culture (I). Poor performance status, thrombocytopenia, increased serum urea nitrogen and decreased serum cholinesterase were more prominent in (I) than (C) + (C-I). The risk factors associated with the development of infection from colonization were age and serum cholinesterase. In addition, lower respiratory tract infection as a type of infection, non-remission status of the haematological malignancy and an inappropriate antibiotic therapy were associated with a poor prognosis for MRSA infection.

A variant alkaline phosphatase found in a case of gastric carcinoma with super bone scan.

A rare case of gastric carcinoma associated with increased serum variant alkaline phosphatase activities is presented. A 54 year old man had extremely high serum alkaline phosphatase activity (18,607 U/l) with normal calcium and phosphate concentrations. His bone scintigram showed abnormal findings, 'super bone scan'. He was diagnosed as having Borrmann type 4 gastric carcinoma with diffuse bone metastases by examinations of the upper gastrointestinal tract and iliac bone biopsy. The alkaline phosphatase isozyme of this patient was of the bone type as measured by cellulose acetate membrane electrophoresis and the placenta/bone type by agarose gel electrophoresis, respectively. Immunoelectrophoresis and the immunoprecipitation method using monoclonal antibodies against various alkaline phosphatase isozymes, however, showed that his serum alkaline phosphatase had the liver type antigenicity. Furthermore, it had a larger molecular size and different sugar chains compared with the common liver type alkaline phosphatase. These findings suggest that a unique variant alkaline phosphatase was produced by gastric cancer cells, which is possibly an explanation for the high serum alkaline phosphatase activities in this patient.

Analysis of bronchoalveolar lavage cells in chronic eosinophilic pneumonia before and during corticosteroid therapy.

Bronchoalveolar lavage was performed in patients with chronic eosinophilic pneumonia of unknown origin diagnosed histopathologically, and the alveolar fluid cells were investigated before and during corticosteroid therapy. The counts of eosinophils in bronchoalveolar lavage fluid decreased markedly during steroid therapy compared with before therapy, and the ratios of interleukin 2 receptor-positive T cells and HLA DR-positive T cells to all T lymphocytes in the lavage fluid also decreased. Both before and during steroid therapy, many macrophages in bronchoalveolar lavage fluid phagocytosed EG2-positive granules and stained immunohistochemically as CD71-positive 'responsive macrophages'. The decrease of activated T lymphocytes and the active phagocytosis of eosinophil granules by macrophages might play an important role in the mechanism of healing in chronic eosinophilic pneumonia.

Surface antigens on eosinophils in bronchial asthma and eosinophilic pneumonia.

Surface antigens on peripheral blood eosinophils from 23 patients with bronchial asthma, 6 with eosinophilic pneumonia and 8 controls were examined using a new direct method. Peripheral blood eosinophils in bronchial asthma and eosinophilic pneumonia showed higher complexity and/or granularity than those from controls. The percentage expression of HLA DR, CD4 and CD45RO on peripheral blood eosinophils from patients with bronchial asthma were increased compared with those from patients with eosinophilic pneumonia and from the controls. These results suggest that peripheral blood eosinophils in bronchial asthma may play a role in immunoregulation via the expression of human leukocyte antigens, such as HLA-DR, CD4 and CD45RO, that interact with lymphocytes, and may function as antigen-presenting cells. Furthermore this study suggests that there may be different phenotypes of eosinophils with differing surface antigens and intercellular reactions between eosinophils and lymphocytes.

Expression of aqualysin I (a thermophilic protease) in soluble form in Escherichia coli under a bacteriophage T7 promoter.

The thermophilic protease aqualysin I (AQI) gene (aquI), derived from Thermus aquaticus YT-1, was inserted under the control of the bacteriophage T7 promoter in an expression plasmid. The plasmid was introduced into two strains of E. coli JM109 (DE3), one carrying and one lacking an F' episome, which carries the lacIq gene. Upon cultivation the strain carrying an F' episome produced AQI as an insoluble fusion protein (74kDa) with the T7 gene 10 protein. This insoluble protein could not be processed into mature AQI by heat treatment and thus it had no proteolytic activity. On the other hand, when the strain lacking an F' episome was used as a host cell for aquI expression, non-induced, or leaky, expression occurred, and AQI was produced in a soluble form. This soluble protein could be processed into active AQI by heat treatment. Moreover, when a low concentration of IPTG (0.0125 mM) was added, the amount of active AQI was 2.7 times greater than that produced in a batch culture without induction.

Enhanced cardiac clearance of iodine-123-MIBG in chronic renal failure.

UNLABELLED: We studied myocardial accumulation and clearance of [123I]metaiodobenzylguanidine (MIBG) along with measurements of plasma norepinephrine concentrations. METHODS: Myocardial imaging with MIBG and plasma norepinephrine concentration measurements were performed in 21 patients with chronic renal failure on dialysis and 11 control subjects. Dynamic acquisitions, begun immediately after MIBG injection, and planar images 15 and 150 min after injection were used to measure early and late myocardial uptake and clearance rates of MIBG from the heart. RESULTS: Early and late MIBG uptake was not significantly different from that in controls but MIBG clearance was significantly more rapid in controls. Based on echocardiographic findings, the patients were divided into three subgroups: those without left ventricular hypertrophy and dysfunction (n = 10), with hypertrophy (n = 4) and with dysfunction (n = 7). Among controls and the three patient subgroups, MIBG clearance was significantly higher in all the subgroups and also was significantly higher in patients with dysfunction than in those without hypertrophy and dysfunction. Plasma norepinephrine levels were significantly higher in patients than in controls and correlated significantly with the clearance rate. CONCLUSION: MIBG clearance from the heart was rapid in patients with chronic renal failure on dialysis, particularly those with left ventricular dysfunction or hypertrophy, suggesting cardiac sympathetic overactivity in these patients.

[Interuncal distance measurements in normal controls and patients with dementia: MR imaging study].

To evaluate the utility of measuring interuncal distance (IUD) as a reflection of the limbic system, we compared the IUD of 60 dementia patients with that of 10 normal controls. We also measured the width of the intracranial compartment (W1 and W2) to correct for differences in individual brain size, and calculated the ratio of IUD/W1 and IUD/W2. IUD could not separate patients with dementia from normal controls, but there were significant differences in IUD/W1 and IUD/W2 between patients with dementia and normal controls. IUD, IUD/W1 and IUD/W2 did not correlate with Mini-Mental Examination score or ADAS score in patients with dementia. We conclude that IUD measurement is not helpful in distinguishing patients with mild stage dementia from normal aged people or as a scale for dementia. However, we suggest that IUD/W1 and IUD/W2 can discriminate between cases of mild dementia and normal aged people.

Serum autoantibody against interleukin-1 alpha is unrelated to the etiology or activity of liver disease but can be raised by interferon treatment.

OBJECTIVE: To clarify the clinical significance of serum levels of interleukin-1 alpha autoantibody in liver disease and their change during interferon therapy for chronic hepatitis. METHODS: By radioimmunoassay, we studied the incidence of serum interleukin-1 alpha autoantibody in 838 healthy controls and 180 patients with liver disease and monitored the change in antibody titer during the interferon therapy for chronic hepatitis. RESULTS: We detected the interleukin-1 alpha autoantibody in 12.6% (106/838) of healthy controls. In patients with liver disease, we found the antibody in 15.6% (5/32) in patients with acute hepatitis, 16.3% (13/80) in patients with chronic hepatitis, 18.8% (9/48) in patients with liver cirrhosis, and 15% (3/20) in patients with autoimmune liver disease. The incidence was not related to either etiology or inflammatory activity of liver disease. Two of three chronic hepatitis patients with initially high serum levels of the antibody (> 2000 ng/ml) showed transient increase in antibody titers during interferon therapy. CONCLUSION: The serum level of interleukin-1 alpha autoantibody was unrelated to the etiology or activity of liver disease. Interferon therapy can cause transient elevation of serum interleukin-1 alpha autoantibody levels.

[Inspection of carbon monoxide in imported tilapia].

Carbon monoxide in imported tilapia was determined with a gas chromatograph equipped with a molecular sieve 13x column (2.3 m), a methanizer and a FID for the inspection of imported food. The concentration of carbon monoxide in the sample, which was vacuum packed and suspected to be treated with carbon monoxide, was 16 x 10 micrograms/kg in fish meat, and 37 x 10(3) microliters/l in the bubble in the package. On the other hand, carbon monoxide in the reference, which was vacuum packed but was not treated with carbon monoxide, was 10 micrograms/kg in fish meat and 76 microliters/l in the bubble in the package. Carbon monoxide was less than 4 micrograms/kg in two vacuum packed fish meat of tilapia sold in a market in Tokyo. From these results, the suspected sample was concluded to be treated with carbon monoxide for color fixating of protoheme in fish meat.

6-(1H-imidazol-1-yl)-7-nitro-2,3(1H,4H)-quinoxalinedione hydrochloride (YM90K) and related compounds: structure-activity relationships for the AMPA-type non-NMDA receptor.

A novel series of quinoxalinediones possessing imidazolyl and related heteroaromatic substituents was synthesized and evaluated for their activity to inhibit [3H]AMPA binding from rat whole brain. From the structure-activity relationships, it was found that the 1H-imidazol-1-yl moiety could function as a bioisostere for the cyano and nitro groups, and that 6-(1H-imidazol-1-yl)-7-nitro-2,3(1H,4H)-quinoxalinedione (11) showed the most potent activity for the AMPA receptor. Compound 11 was evaluated for selectivity versus other excitatory amino acid receptors, and its action against AMPA at its receptor in the rat striatum was characterized. These data showed that compound 11 was a selective antagonist for the AMPA receptor with a Ki value of 0.084 microM, being approximately equipotent with 2,3-dihydro-6-nitro-7-sulfamoylbenzo(f)quinoxaline (3) (NBQX; Ki = 0.060 microM). Compound 11 was also found to give protection against sound-induced seizure on DBA/2 mice at the minimum effective dose of 3 mg/kg ip (3; 10 mg/kg ip).

Fermentation conditions for efficient production of thermophilic protease in Escherichia coli harboring a plasmid.

Escherichia coli TG1, transformed with an expression plasmid pAQN carrying the aqualysin I (AQI) gene derived from Thermus aquaticus YT-1 under the control of the tac promoter, was cultivated under various conditions in order to find fermentation conditions for the efficient production of the thermophilic protease, AQI. The amount of AQI produced was closely related to the growth phase at the time of isopropyl-beta-D-thiogalactopyranoside (IPTG) induction, and the highest production was obtained when it was added during the exponential growth phase. The addition of yeast extract had a greater effect on AQI production than did Polypeptone or casamino acids, and AQI productivity increased from 1.1 x 10(3) kU/g to 2.7 x 10(3) kU/g cells when 2 g/l yeast extract was supplied. Furthermore, the specific growth rate improved from 0.35 h-1 to 0.89 h-1 when 5 g/l yeast extract was supplied. The culture temperature also affected AQI gene expression. When the temperature was shifted from 37 degrees C to 34 degrees C at the time of IPTG induction, 19 kU/ml enzymatically active AQI was obtained, corresponding to a 28% increase over the amount produced in a batch culture without a shift. This is about a 44-fold higher yield than was obtained from the original strain, T. aquaticus YT-1.

Studies on the antigenicity and nucleotide sequence of the rabies virus Nishigahara strain, a current seed strain used for dog vaccine production in Japan.

The Nishigahara strain of rabies virus, a current seed strain used for animal vaccine production in Japan, is believed to derived from the original Pasteur strain obtained from Paris in or before 1915. In Japan, the virus was serially passaged through several kinds of animals and cell cultures. Reactions with anti-nucleocapsid protein monoclonal antibodies (MAb-N) indicated the Nishigahara strain had maintained the antigenic profile of the Pasteur virus. Reactions with monoclonal antibodies to the glycoprotein (MAb-G) revealed differences between the Nishigahara strain and the Pasteur strain; however, the Nishigahara strain maintained a closer resemblance to the Pasteur virus than to other Pasteur-related viruses or to rabies strains unrelated to the Pasteur strain. Comparative amino acid sequence analysis of cloned cDNA encoding the G gene confirmed the antigenic differences among these strains and the resemblance of the Nishigahara strain to the original Pasteur strain. Comparative nucleotide sequence analysis of the noncoding pseudogene region (Tordo et al., Proc Natl Acad Sci USA 83, 3914-3918, 1986) revealed different relationships. Unlike the Pasteur strain, which encodes a transcription-terminating signal at the end of the G gene (marking the beginning of the pseudogene), a long G-L intergenic sequence in the Nishigahara strain was connected to the 3' end of the cDNA, and the transcription-terminating signal was present only at the end of, but not before, the pseudogene. These results are not inconsistent with the documented origin of the Nishigahara strain, but the genome structure around the pseudogene region suggests divergence from the Pasteur strain and a closer resemblance to other strains of rabies virus.

Regulation of hepatocyte albumin and alpha 1-acid glycoprotein secretion by monokines, dexamethasone, and nitric oxide synthase pathway: significance of activated liver nonparenchymal cells.

To clarify the mechanism involved in regulating the secretion of albumin and alpha 1-acid glycoprotein by rat hepatocytes, we studied hepatocyte culture and cocultures of hepatocyte and liver nonparenchymal cells. The secretion of alpha 1-acid glycoprotein by hepatocytes was stimulated and that of albumin was inhibited by combinations of dexamethasone and monokines, especially by dexamethasone and interleukin-6. The secretion of these proteins was equally inhibited during stimulation by lipopolysaccharide in cocultures. The inhibitory effect of sinusoidal endothelial cells was smaller than that of Kupffer cells. This inhibition was partially abolished by blocking the nitric oxide synthase pathway in cocultured cells and was completely abolished by dexamethasone. In conclusion, the secretion of albumin and alpha 1-acid glycoprotein by hepatocytes was regulated by monokines, dexamethasone, and the inducible nitric oxide synthase pathway in hepatocytes and liver nonparenchymal cells in vitro.

Delayed gastric emptying in patients with liver cirrhosis.

Using a scintigraphic technique, we investigated gastric emptying of a semisolid meal in 20 patients with liver cirrhosis and in 10 control subjects and correlated gastric emptying with gastrointestinal symptoms and with autonomic nervous function as determined by R-R interval variation on the electrocardiogram. All subjects lacked endoscopic abnormalities that might explain their gastrointestinal symptoms. None had alcoholic liver disease, diabetes, or other diseases known to affect gastric emptying. The half-time for gastric emptying was significantly prolonged in the cirrhotic patients (51.3 +/- 16.6 minutes) as compared with control subjects (29.9 +/- 8.4) (P < 0.01). There was a significant correlation between the gastrointestinal symptom score and the half-time for gastric emptying (r = 0.46, P < 0.05) in the cirrhotic patients. However, the decreases in R-R interval variation and gastric emptying were not significantly correlated. These observations indicate that delayed gastric emptying is frequently present in patients with liver cirrhosis and may produce their gastrointestinal symptoms.

Diagnostic and prognostic significance of plasma endotoxin determination in febrile patients with haematological malignancies.

We evaluated the clinical utility of a new endotoxin-specific chromogenic limulus test in febrile patients with haematological malignancies. The specificity is assured by the removal of factor G, which is sensitive to (1-->3)-beta-D-glucan, from horseshoe crab amoebocyte lysate. The sensitivity and specificity of the test to systemic gram-negative bacterial infections were 69.7 and 96.3%, respectively. Meanwhile, gram-negative bacteria grew in only 39.7% of endotoxaemic samples. Thus, it seems appropriate to consider gram-negative bacteraemia and endotoxaemia as different entities. Endotoxaemia was significantly associated with septic shock and infectious death, especially in patients with neutropenia. The new test, the results of which are available within 3 h, should help physicians to recognise this ominous sign early and to initiate a prompt countermeasure to endotoxaemia.
