[Study of growth fraction on fine needle aspirated prostatic tissue smear using monoclonal antibody Ki-67].

Immunohistochemical staining using monoclonal antibody Ki-67 was performed in 30 patients with benign prostatic hypertrophy (BPH), one with prostatic tuberculosis (TB), 22 with prostatic adenocarcinoma, one with prostatic transitional cell carcinoma and one with prostatic invasion from a bladder cancer. Specimens were aspirated from the prostate transrectally and a cytological smear were made. This antibody is specific for a proliferation-associated nuclear antigen. Alkaline phosphatase anti-alkaline phosphatase stained immunopositive nuclei red making positive or negative specimens easy to recognize. In BPH and TB smears, no immunopositive cell was reactive with Ki-67. In prostatic malignancy were found many immunopositive cells ranging from 2.5 to 10.2% (mean 5.9%) in prostatic adenocarcinoma (n = 22), and from 11.9 to 24.3% (mean 18.1%) in prostatic transitional cell carcinoma. Transitional cell carcinoma may have a much greater growth fraction than adenocarcinoma in prostatic tissue. Poorly differentiated adenocarcinoma showed a higher growth fraction (from 4.2 to 10.2%, mean 6.9%) than well differentiated (from 3.1 to 8.9%, mean 5.8%) and moderately differentiated adenocarcinoma (from 2.5 to 10.1%, mean 5.6%), but this difference was not significant. There was no correlation with age, clinical stage, bone metastasis or Böcking's cytological grade. In conclusion, immunohistochemical staining using Ki-67 on aspirated prostatic smear is visualizes the growth fraction of prostatic disease well and is useful to diagnose prostate cancer.

Use of bovine lung heparin to obviate anaphylactic shock caused by porcine gut heparin.

Intracardiac repair in a 7-year-old girl with tetralogy of Fallot was hampered because of an anaphylactic reaction with cardiopulmonary collapse caused by porcine gut heparin. This patient had an extremely rare case of hypersensitivity to porcine gut heparin but not to bovine lung heparin. Uneventful radical operation could be performed with the use of bovine lung heparin for systemic heparinization during cardiopulmonary bypass.

[Granulomatous prostatitis after intravesical BCG immunotherapy of superficial bladder cancer: a case report].

We report a case of granulomatous prostatitis after intravesical BCG immunotherapy for superficial bladder cancer. A 58-year-old man presented with gross hematuria. Cystoscopic examination revealed multiple tumors at the posterior wall of the bladder. The patient underwent transurethral resection of the tumor. Intravesical BCG immunotherapy was postoperatively followed and it eradicated the disease. Digital examination revealed that the prostate became stony-hard and larger 10 weeks after the initial BCG immunotherapy. A needle aspiration cytology and biopsy of the prostate revealed the granulomatous prostatitis due to BCG immunotherapy.

[A case report of the pectoralis major muscle flap in the successful management of severe mediastinal infection].

A severe, post-mediansternotomy, mediastinitis was treated with pectoral muscle flap method. About 10 days after CABG this patient had purulent discharge, high fever, and unstable sternum. Under a diagnosis of mediastinitis, wound irrigation and systemic administration with antibiotics began, but these managements were ineffective. Therefore, we closed the chest primarily by the pectoral muscle flap method. Successful primary closure of the chest was accomplished.

Effects of nonsteroidal antiinflammatory drugs on collagen biosynthesis of cultured chondrocytes.

In conclusion, the data obtained from the present study show that TA has less effect than indomethacin or ASA on cultured chondrocytes in gene expression and synthesis of collagen. These findings cannot be directly interpolated to clinical application, but attention to pharmacologic activities of NSAIDs on chondrocytes may have future applications. Two major NSAIDs, TA, indomethacin, and ASA, were examined for their effects on the biosynthesis and gene expression of articular cartilage collagen. The biosynthesis of type II collagen was suppressed to 70% to 80% of the control by indomethacin and ASA. TA did not suppress collagen synthesis. To know the gene expression of type II collagen, dot blot hybridization was performed using type II collagen cDNA. The effects of NSAIDs on the amount of type II collagen mRNA were consistent with those on collagen biosynthesis. Thus, TA had the least effect on cultured chondrocytes.

[Clinical study of ectopic eruption of permanent incisors and first molars].

UNLABELLED: Using the X-ray films, plastic models and pathological photograph of 4824 patients seen at the Department of Pedodontics of Iwate Medical University, the patterns of ectopic eruptions of permanent incisors and first molars (restricted to these teeth in this study) were classified into 4 groups (I-IV) to investigate the conditions of its incidence, size of the teeth and dental arch, and conditions at the stage of the mixed and permanent dentitions. RESULTS: 1. The total frequency of ectopic eruptions was 5.6%. No difference based on sex was found. 2. By classification, ectopic eruptions of the upper and lower permanent central incisors (Group I) amounted to more than half of the total frequency. The incidence of ectopic eruptions of the upper and lower permanent first molars (Group III) was very low and amounted to only 4.6% of the total frequency. 3. In Groups I and III, the incidence was higher in the maxilla than in the mandible. 4. Based on sex, the incidence was higher in boys in Group III and in girls in group II. 5. Ectopia of the permanent central incisors was frequently bilateral and those of the lower permanent lateral incisors and upper permanent first molars were frequently unilateral. 6. The mesiodistal width of the ectopically erupted permanent teeth tended to be somewhat enlarged, but showed no significant differences from Ono's mean value. 7. In ectopic eruptions at the area of the incisors, the C-C distance was narrowed with an increase in the ectopically erupted permanent teeth. 8. In patients in whom the permanent dentition was formed without occlusal guidance, the basal arch length and dental arch length were large and basal arch width and dental arch width were small. 9. In cases which were followed from the deciduous dentitions to permanent dentitions, the anterior tooth area at the stage of the mixed dentition was frequently crowded. Occlusal guidance was carried out in more than half of these cases.

[A case of superficial squamous cell carcinoma of the esophagus showing multicentric development].

A 43-year-old male was admitted to the hospital with a complaint of mild dysphagia. A barium esophagogram showed an annular narrowing in the lower esophagus. Further, an endoscopic examination revealed erosive mucosa of the esophagus about 35 cm anal to the incisor. Lugol staining also revealed a geographical unstained lesion. The histologic diagnosis of a biopsy specimen indicated a squamous cell carcinoma and a total resection of the esophagus was performed. The operated material macroscopically showed a tumor of a superficially mixed type (IIa + IIc + IIb). The resected specimen histologically consisted of 18 squamous carcinomas that were separated with the normal squamous epithelium from each other. The major part of these lesions were limited to the epithelium, and only a few specimens microscopically showed an invasion of cancer nests in the propria mucosae. Great permeation of cancer cells into lymph vessels and the metastasis of one regional lymph node also were noted. Taken together, these findings indicated that the squamous cell carcinomas of this case seemed to have multicentrically developed from the normal esophageal mucosa.

[A case of leiomyosarcoma of the kidney].

A case of renal leiomyosarcoma seen in a 65-year-old man is reported. On November 15, 1985, he complained of right flank pain and high fever. On right renal angiography and computed tomography showed a mass on lower pole of the right kidney. On December 10, 1985, the patient underwent right radical nephrectomy. The histological diagnosis was leiomyosarcoma probably originating from the right renal capsule. Forty-six cases of renal leiomyosarcoma, including this case, in the Japanese literature have been reviewed and are discussed.

[Aortocoronary bypass in patient with chronic renal failure].

Aorto-coronary bypass surgery was performed successfully for an anuric patient who had been on hemodialysis three times a week for ten years because of chronic renal failure. Hemodialysis was performed up to twenty-four hours prior to the operation. Cardiopulmonary bypass was done using a bubble oxygenator primed with 2000 ml of homologous blood and 500 ml of osmotic diuretic and electrolytes. Peritoneal dialysis started immediately after the surgery followed by routine chronic hemodialysis from the 5th postoperative day. This patient recovered uneventfully and is now relieved from severe chest pain.

[Construction of cDNA clones encoding for cartilage proteoglycan core protein].

In order to investigate cartilage proteoglycan mRNAs and genes, a cDNA expression library was constructed from chick embryonic sternal mRNAs, and cDNA clones encoding for chondroitin sulfate proteoglycan core protein were screened from the library. Total RNA extracted from chick embryonic sterna was translated in a cell-free system using rabbit reticulocyte lysate. Immunoprecipitation of products of cell-free translation using antiserum against cartilage proteoglycan yielded a component of about 340 kd polypeptide. This polypeptide is most likely the translational product of proteoglycan core protein mRNA. A chicken embryo sternal cartilage cDNA library, created in the plasmid expression vector pUC 9, was screened for sequences coding for immunologically detectable core protein, and two cDNA clones were isolated. Northern blot hybridization of sternal cartilage RNA revealed a single mRNA of about 8.1 kb for proteoglycan core protein.

Cartilage proteoglycan core protein gene expression during limb cartilage differentiation.

Changes in the steady-state cytoplasmic levels of mRNA for the core protein of the major sulfated proteoglycan of cartilage were examined during the course of limb chondrogenesis in vitro using cloned cDNA probes. Cytoplasmic core protein mRNA begins to accumulate at the onset of overt chondrogenesis in micromass culture coincident with the crucial condensation phase of the process, in which prechondrogenic mesenchymal cells become closely juxtaposed prior to depositing a cartilage matrix. The initiation of core protein mRNA accumulation coincides with a dramatic increase in the accumulation of mRNA for type II collagen, the other major constituent of hyaline cartilage matrix. Following condensation, there is a concomitant progressive increase in cytoplasmic core protein and type II collagen mRNA accumulation which parallels the progressive accumulation of cartilage matrix by the cells. The relative rate of accumulation of cytoplasmic type II collagen mRNA is greater than twice that of core protein mRNA during chondrogenesis in micromass culture. Cyclic AMP, an agent implicated in the regulation of chondrogenesis elicits a concomitant two- to fourfold increase in both cartilage core protein and type II collagen mRNA levels by limb mesenchymal cells. Core protein gene expression is more sensitive to cAMP than type II collagen gene expression. These results suggest that the cartilage proteoglycan core protein and type II collagen genes are coordinately regulated during the course of limb cartilage differentiation, although there are quantitative differences in the extent of expression of the two genes.

Cloning and sequence analysis of a partial cDNA for chicken cartilage proteoglycan core protein.

A chicken embryo sternal cartilage cDNA library, created in the plasmid expression vector pUC9, was screened for sequences coding for immunologically detectable core protein of the large, major proteoglycan of cartilage. A 1229-base-pair cDNA clone was isolated that contained only one extended open reading frame, which had sequences coding for a polypeptide of 379 amino acid residues. These deduced sequences corresponded to those anticipated from current models of proteoglycan structure; a deduced sequence encompassing 21 amino acids was almost identical to a known sequence of bovine nasal cartilage proteoglycan. Significant homology was found between the deduced amino acid sequence of the proteoglycan and two regions of a chicken hepatic lectin. Immunoprecipitation of the products of cell-free translation yielded a component of about 340 kDa, and transfer blot hybridization of sternal cartilage RNA showed a single mRNA of about 8.1 kilobases. Hybridizable mRNA sequences were readily detectable by dot-blot analyses of the cytoplasm of cartilaginous tissues of the chicken embryo, whereas similar analyses of prechondrogenic limb mesenchymal cells did not demonstrate such hybridizable mRNA signals.

[Clinical application of a new fibrin adhesive (Tisseel) in urologic surgery].

Our experience with the use of a new biological tissue adhesive (Tisseel) consisting of highly concentrated human fibrinogen, thrombin and factor XIII in urologic surgery is reported. In 21 operations (18 patients), Tisseel was used for the purpose of tissue adhesion and reduction of sutures of the renal parenchymal wound in nephrolithotomy, water tight sealing in suture part of expanded pyelolithtomy and vasovasostomy, and local hemostasis of oozing in prostatectomized fossa. Tisseel was not responsible for the two unsuccessful trials; and, undesirable complications due to this adhesive were few. Our results revealed that this new fibrin adhesive will be useful for urologic surgery.

[A case of aortoenteric fistula with iliac embolization].

A sixty-eight year-old woman complained of acute hematemesis and numbness of the right lower extremity. Physiocal examination on amission demonstrated revealed a pulsating mass in the midabdomen, absence of pulsation of the right femoral artery and cold pale skin of the right leg. The diagnosis of abdominal aneurysm with aortoenteric fistula and embolization embolectomy to the right iliac artery was mode by computed tomography although the fistula was not clearly visualized. The abdominal aneurysm was replaced with a Y-graft after embolectomy and the fistula to the duodenum was completely closed. Early diagnosis and emergency surgery are necessary to save the patient with this rare disease.
