Mechanotransduction and intracellular signaling mechanisms of stretch-induced remodeling in endothelial cells.

We investigated the signaling mechanism of stretch-induced cell remodeling in human umbilical vein endothelial cells (HUVECs). Freshly dissociated HUVECs were cultured on an elastic silicon membrane and subjected to uniaxial cyclic stretch (20% in length, 1 Hz). The cells started to change their morphology as early as 15 min after stretch onset, and most cells eventually aligned perpendicularly to the stretch axis within 1 h. This remodeling was dependent on the increase in intracellular calcium concentration ([Ca2+]i) via a Ca(2+)-permeable stretch-activated (SA) channel. During the process of remodeling, extensive rearrangement of stress fibers and focal adhesions was observed, which may be close to the final step in the intracellular signaling cascade. This event was [Ca2+]i-dependent, suggesting the existence of a Ca(2+)-dependent intermediate cascade that links [Ca2+]i to the rearrangement of cytoskeletons and focal adhesions. We found that some proteins, including pp125FAK (focal adhesion kinase) and paxillin, were tyrosine phosphorylated during cyclic stretch in a Ca(2+)-dependent manner. Inhibition of this tyrosine phosphorylation prohibited the stretch-dependent rearrangement of cytoskeletons and focal adhesions as well as the remodeling. Finally the tyrosine kinase src, which could phosphorylate pp125FAK, was found to be activated in a [Ca2+]i-dependent way during stretch. All of the above molecular events were consistently Ca(2+)-dependent, which led us to propose the signaling cascade: SA channel activation-->[Ca2+]i increase-->src activation-->protein tyrosine phosphorylation-->rearrangement of cytoskeletons and focal adhesions-->cell remodeling.

Sevoflurane inhibits human platelet aggregation and thromboxane A2 formation, possibly by suppression of cyclooxygenase activity.

BACKGROUND: Halothane increases bleeding time and suppresses platelet aggregation in vivo and in vitro. A previous study by the authors suggests that halothane inhibits platelet aggregation by reducing thromboxane (TX) A2 receptor-binding affinity. However, no studies of the effects of sevoflurane on platelet aggregation have been published. METHODS: The effects of sevoflurane, halothane, and isoflurane were examined at doses of 0.13-1.4 mM. Human platelet aggregation was induced by adenosine diphosphate, epinephrine, arachidonic acid, prostaglandin G2, and a TXA2 agonist ([+]-9, 11-epithia-11, 12-methano-TXA2, STA2) and measured by aggregometry. Platelet TXB2 levels were measured by radioimmunoassay, and the ligand-binding characteristics of the TXA2 receptors were examined by Scatchard analysis using a [3H]-labeled TXA2 receptor antagonist (5Z-7-(3-endo-([ring-4-[3H] phenyl) sulphonylamino-[2.2.1.] bicyclohept-2-exo-yl) heptenoic acid, [3H]S145). RESULTS: Isoflurane (0.28-0.84 mM) did not significantly affect platelet aggregation induced by adenosine diphosphate and epinephrine. Sevoflurane (0.13-0.91 mM) and halothane (0.49-1.25 mM) inhibited secondary platelet aggregation induced by adenosine diphosphate (1-10 microM) and epinephrine (1-10 microM) without altering primary aggregation. Sevoflurane (0.13 mM) also inhibited arachidonic acid-induced aggregation, but not that induced by prostaglandin G2 or STA2, although halothane (0.49 mM) inhibited the latter. Sevoflurane (3 mM) did not affect the binding of [3H]S145 to platelets, whereas halothane (3.3 mM) suppressed it strongly. Sevoflurane (0.26 mM) and halothane (0.98 mM) strongly suppressed TXB2 formation by arachidonic acid-stimulated platelets. CONCLUSIONS: The findings that sevoflurane suppressed the effects of arachidonic acid, but not those of prostaglandin G2 and STA2, suggest strongly that sevoflurane inhibited TXA2 formation by suppressing cyclooxygenase activity. Halothane appeared to suppress both TXA2 formation and binding to its receptors. Sevoflurane has strong antiaggregatory effects at subanesthetic concentrations (greater than 0.13 mM; i.e., approximately 0.5 vol/%), whereas halothane has similar effects at somewhat greater anesthetic concentrations (0.49 mM; i.e., approximately 0.54 vol/%). Isoflurane at clinical concentration (0.84 mM; i.e., approximately 1.82 vol/%) does not affect platelet aggregation significantly.

[Clinical statistics on operations during a ten-year period (1985-1994) at department of urology, Handa Hospital].

A clinical statistic survey was carried out on the operations performed at the Department of Urology, Handa Hospital between 1985 and 1994. The total number of operations was 2,532 and the number of operations other than extracorporeal shock-wave lithotripsy (ESWL) was 1,938. The mode of operation for urolithiasis has changed dramatically from open surgery to the ESWL and endourology.

[A case of visceral ischemia associated with acute type IIIb aortic dissection].

The case was a 61-year-old man who was transported to our hospital with complaining of severe chest and back pain of sudden onset and diagnosed as acute type IIIb aortic dissection. Inspite of intensive medical treatments, severe abdominal pain with abdominal distension and progressive metabolic acidosis appeared at around 11 hours after admission. Aortogram revealed obstruction of both superior and inferior mesenteric arteries suggesting visceral ischemia associated with acute type IIIb aortic dissection. Emergent operation consisting of bypass grafting for the superior and the inferior mesenteric arteries with saphenous vein, partial resection of small intestine, left hemicolectomy and construction of artificial anus was immediately carried out. The patient had no particular trouble after the operation and returned to the previous job.

Diverticulum of the superior vena cava.

A 14-year-old girl underwent operation with the diagnosis of diverticulum of the superior vena cava. Microscopic findings revealed a diverticulum with venous architecture. This represents a rare case of a giant diverticulum of the superior vena cava.

[A familial case of prostate cancer in three brothers].

We treated three brothers for prostate cancer. The first brother developed the disease of poorly differentiated adenocarcinoma at the age of 76, and was treated with endocrine therapy but died of recurrent cancer at age 80. The second brother was diagnosed poorly differentiated adenocarcinoma at age 75, and he is alive at age 80 without recurrence after endocrine therapy. The third brother developed moderately differentiated adenocarcinoma at age 58, and was treated with endocrine therapy but died of recurrent cancer at age 73. We have seen few familial cases of prostate cancer. The first such case is reported here in Japan. Eleven similar cases of familial prostate cancer involving three or more brothers have been reported in the Western literature.

Thoracic aortic aneurysmectomy with a sutureless intraluminal ringed graft.

Forty-two patients underwent replacement of the thoracic aorta with a sutureless intraluminal graft. Early and long-term results were evaluated. The operative mortality rate was 7.1%. There were two postoperative complications related to a ringed graft. One complication, the formation of a pseudoaneurysm, was caused by insufficient fixation of the graft; the other, a cerebral infarction, was related to the location of the proximal anastomosis with respect to the origin of the left subclavian artery. The 3- and 5-year actuarial survival rates were 86 and 69.5%, respectively. These were similar to survival rates of age-matched controls in the general Japanese population. Replacement of the thoracic aorta using a sutureless intraluminal graft can be performed with acceptable operative mortality, with good long-term results.

[Insertion of prosthetic tricuspid valve two years after the tricuspid valvulectomy].

A case of a 14-year-old boy who underwent a prosthetic tricuspid valve insertion two years after the tricuspid valvulectomy due to intractable right-sided active endocarditis is presented. At the initial operation, the tricuspid valve was thoroughly resected because of marked destruction and attachment of vegetations, and a concomitant ventricular septal defect was directly closed. When a prosthetic valve was placed in the tricuspid portion, a semicircular Dacron patch was attached to the right side of the interventricular septum in a fashion of up-chord and down-arc to cover the bundle of His. The straight edge of the patch was located near the AV node and was not sutured to the tricuspid valve annulus. A 31 mm of CarboMedics prosthetic valve was sutured to the tricuspid valve annulus and to the free edge of the patch at the position near the AV node to prevent AV block. Postoperative course was uneventful and ECG showed regular sinus rhythm.

Tricuspid valve myxoma in a child with coronary artery occlusion and aneurysms.

The case of a 13-year-old boy with a tricuspid valve myxoma and coronary artery occlusion and aneurysms suspected to be caused by Kawasaki's disease is presented. Simultaneous extirpation of the myxoma and coronary artery bypass grafting to the left anterior descending artery using the left internal mammary artery was performed. His postoperative course was satisfactory. Coronary artery bypass grafting in a child is also discussed.

Influence of LAK cells on expression of HLA-DR antigen on laryngeal carcinoma cell line in new culture systems.

We demonstrated the enhancement of HLA-DR antigen expression on cultured laryngeal carcinoma cells (Hep 2) by in vitro cultivation with LAK cells using flow cytometric and immunohistological analysis. For in vitro cultivation of tumor cells with LAK cells, we used newly developed experimental systems (the Transwell double-dish system and experimental three-dimensional tumors). In flow cytometric analysis, expression of HLA-DR antigen on tumor cells was compared before and after co-cultivation with LAK cells. When tumor cells were cultured separately with LAK cells in a Transwell Petri dish and the expression of HLA-DR antigen on tumor cells was analyzed by flow cytometry, the expression of HLA-DR antigen on tumor cells was increased in a dose-dependent manner related to the number of LAK cells used. Furthermore, when anti-interferon-gamma monoclonal antibody was added to the experimental system, enhancement of HLA-DR antigen expression was blocked. These findings were consistent with immunohistological studies, in which experimental three-dimensional Hep 2 cell tumors were established in double-layered agar with/without being co-cultivated with LAK cells. The expression of HLA-DR antigen in this system was significantly increased when compared to such expression before cultivation with LAK cells. These findings suggested that the culture systems employed in this study could be a possible model for examining solid tumor in vivo biological responses. This enhanced expression of HLA-DR antigen may also represent one of the multi-factorial responses seen with adoptive LAK cell immunotherapy for solid tumors.

Reconstruction of the posterior cruciate ligament with LAD-augmented semitendinosus and gracilis tendons: a preliminary report.

We present our technique for reconstruction of the posterior cruciate ligament (PCL) using the semitendinosus and gracilis tendons with the Kennedy ligament augmentation device (LAD). The safe and excellent exposure of the posterior aspect of the knee allowed us to identify the most isometric position in the intercondylar notch of the femur. In addition to this advantage, firm fixation of the LAD-augmented tendons with staples prevented the tibia from sagging posteriorly during early protected motion of the knee. Evaluation of 12 patients followed for more than 2 years showed 9 (75%) good results. In this small series no correlation was found between clinical results and the number of major structures injured, indicating that postoperative care is as important as isometric placement of the PCL in obtaining satisfactory results.

Inhibition of head and neck tumor cell colony growth by lymphokine activated killer cells.

The antiproliferative effect of lymphokine activated killer (LAK) cells on head and neck tumor cells has not previously been elucidated. We studied the inhibitory effects of recombinant interleukin-2 activated lymphocytes on tumor colony formation in semisolid agar, using head and neck tumor cells prepared from established tumor cell lines (K562, HT29, HLaC78) and xenografted head and neck squamous cell carcinomas on nude mice (XKN, XLL, XFL, XKF). LAK cells demonstrated a significant inhibitory effect on colony formation. The effects of LAK cells on cultured tumor cell lines were evaluated in a dose dependent manner using effector: target ratios. In addition, the colony formation of tumor cells derived from xenografted nude mouse was inhibited by LAK cells. These results suggest that LAK cells generate an antiproliferative effect on head and neck tumor cells.

Chemosensitivity testing of human mouth carcinoma cell line.

The chemosensitivity of KB cells derived from oral epidermal carcinoma to various antitumor agents was analyzed using the MTT[3-(4,5-dimethyl-2-thiazoyl)-2,5-diphenyl-2H tetrazolium bromide] assay. Optical density (OD) for MTT assay was measured with dual wavelengths. The chemosensitivity of the drugs was evaluated by the 50% OD (OD50) of each drug concentration in the control group. Five platinum (Pt) drugs and 3 anthracycline (AC) drugs were used in this study. The chemosensitivity differed among the 5 Pt drugs. No significant difference was observed among the 3 AC drugs. A linear increase in OD corresponding to an increase in number of cells was observed. When 0.1 M sodium succinate (S.S.) was added to 0.4% MTT, the sensitivity increased five-fold compared to the control group without S.S. The MTT assay is a precise, rapid, easy and inexpensive experimental system useful for evaluation of antitumor drug sensitivity on tumor cell lines.

[Adenocarcinoma of colon 19 years after ureterosigmoidostomy: a case report].

A 65-year-old man underwent total cystectomy in 1972 for a bladder tumor. Ureterosigmoidostomy was performed in this case. A physical examination in 1983 revealed occult blood in the feces, and colon fiberscopic examination disclosed a polyp in this patient, who then underwent polypectomy. Histology confirmed hyperplasia. In 1985, colon fiberscopic examination revealed a polyp in the sigmoid, resulting in another polypectomy. Adenoma was established by histology. In April 1991, fresh blood was found in the feces, and a Borrmann II tumor was confirmed by colon fibrerscopic examination. Sigmoidectomy, colostomy, and right ureterostomy were performed. Histology confirmed moderately differentiated adenocarcinoma.

[Antitumor effect of LAK cells on CDDP resistant head and neck carcinoma cell line].

Antitumor effects of lymphokine-activated killer (LAK) cells on cisplatin resistant oral carcinoma cell line were evaluated in vitro by 51Cr release assay and in vivo by nude mouse model. Oral carcinoma cell line (KB) and its subline KBrc resistant to cisplatin were used as target cells. We observed the antitumor effect of LAK cells on both KB and KBrc cells in terms of cytotoxicity of 51Cr release assay, and tumor growth inhibition of nude mouse assay. However, when we compared the antitumor effect between KB and KBrc cells, KBrc cells demonstrated a significant decrease in susceptibility to killing by LAK cells in the in vitro and in vivo model in comparison with the antitumor effect of LAK cells on KB cells. These results, which were not similar to the results of previous studies reported, suggest that alternations of characteristics of tumor cells such as acquired drug resistance result in the decrease in susceptibility of tumor cells against LAK cells.

S-antigen localization in developing rds mouse retina.

The morphology of the photoreceptor cell and localization of the cytoplasmic soluble protein, S-antigen, in the retina of the developing retinal degeneration slow (rds) mutant mouse (2-505 postnatal days) were studied by improved immunocytochemical and freeze-substitution methods. Anti-S-antigen antibody labeling was observed first in the postnatal 10-day retina under light microscope. Labeling signals increased progressively to a maximum level in 20 days, and then decreased gradually to an undetectable level by 505 postnatal days. By electron microscopic immunocytochemical methods, S-antigen was detected first in the photoreceptor cells at 3 postnatal days, and increased with development. It was located in the entire cytoplasm of the photoreceptor cell including the rudimentary outer segment, but degenerated and disappeared by 505 postnatal days. S-antigen was also present in the membranous vesicles budding off from the photoreceptor membrane to the subretinal space. The rds photoreceptor cell seems to lose other soluble proteins together with these vesicles. From these results and other published data, we speculate that the degeneration of the photoreceptor cells may be the secondary effects of the loss of a large amount of soluble and membrane proteins following the malfunction of membrane. Recent reports show the rds mouse must have a gene defect in the membrane component such as peripherin or the 39 kDa protein.

[Antitumor effect of carboplatin on in vitro three-dimensional tumor].

We studied the antitumor effect of Carboplatin on the three-dimensional tumor in vitro. First, we observed the three-dimensional growth of cultured tumor cell lines (Hep 2, KB and HT 29 cells), which were developed in the combined culture system of fibrin matrix and agar culture. The tumor cells developed a 3 x 3 mm tumor in diameter in the in vitro 10 day-culture system. This size was large enough to allow the histologic study. When we applied Carboplatin to the three-dimensional tumors, histologic change of the three-dimensional tumor developed from Hep 2 and KB cells were observed in a dose dependent manner. However, no remarkable histologic change was observed in the three-dimensional tumor developed from HT 29 cells. These results suggest the experimental system of three-dimensional tumor can predict histologically the antitumor effect of Carboplatin on various tumors at the solid tumor level.

[Serious complication of retrograde perfusion during repair of dissecting aneurysm: compression of the true lumen].

Between 1982 and 1990, 61 patients underwent the repair of dissecting aneurysm. Retrograde perfusion from the unilateral femoral artery were performed for 56 patients and the selective false lumen perfusion occurred in 3 patients. As for those 3 patients, compression of the true lumen and insufficient cerebral perfusion were suspected. Selective cerebral perfusion and antegrade systemic perfusion from the ascending aortic graft were performed. Although 2 patients took uneventful postoperative courses, 1 patient died of massive brain damage. It is very important to accomplish the means to maintain adequate cerebral circulation as soon as possible toward this fetal complication.
