Characteristics of blast crisis in chronic granulocytic leukemia.

The terminal phase of most patients with Ph1-positive chronic granulocytic leukemia (i.e., blast crisis) resembles acute leukemia. The clinical and hematologic features of blast crisis in 73 patients with chronic granulocytic leukemia have been reviewed. Two major morphological subgroups, lymphoblastic and myeloblastic, were identified. The lymphoblastic group in general had more profound thrombocytopenia and a greater number of blasts, while the myeloblastic group had more severa anemia. Extramedullary leukemia was documented in 27 patients. In 12 patients extramedullary leukemia preceded or occurred simultaneously with blast crisis in the bone marrow and peripheral blood. On the basis of this study we present hematologic criteria for the diagnosis of blast crisis and emphasize the importance of extramedullary leukemia in heralding the onset of blast crisis.

The structure of pre-messenger RNA and messenger RNA from erythroid cells.

Pre-mRNA fractions (greater than 45 S) were characterized by electron microscopy. High salt concentrations (0.2 M ammonium acetate, pH 8) yield linear molecules of different length (0.5--17 micrometer). In 10% of the molecules a compact-nonlinear contour (cn-contour) is detectable at one end. A significant enhancement of the number of cn-contour carrying molecules is observed after binding pre-mRNA to poly(U)-sepharose. The terminal cn-contour could be the depiction of a secondary and/or tertiary structure including the poly(A)-tail. 9 S globin mRNA appear in 80% with virtually the same cn-contour as detected in pre-mRNA molecules. After denaturing the mRNA in 80% formamide--4M urea in connection with heating to 90 degrees C from 10 min, a percentage of 77% of stretched, linear molecules results. This structural transformation is reversible when the denatured RNA is precipitated and redissolved in 0.2 M ammonium acetate. 73% of the stretched molecules are characterized by a mean length of 0.44 micrometer. This value is twice as high as commonly assumed for a globin mRNA chain.

[Studies on the vitality of human bone marrow during collection and deep-freeze preservation].

The cryoprotective effect of dimethylsulfoxide, glycerol and polyethylenglycol during freezing and thawing of human bone marrow was investigated by eosin staining test, an acridinorange fluochrome staining test and by RNA- and DNA-synthesis tests. In these tests the overall yield of vital nucleated cells, referred to the number in the absence of cryoprotectants and freezing and thawing, amounted to 50% with dimethylsulfoxide, 30% with glycerol, and 10% with polyethylenglycol. With dimethylsulfoxide and glycerol the loss of vital nucleated cells is almost entirely due to the addition of cryoprotectants. Polyethylenglycol freezing and thawing also leads to a great loss of vital nucleated cells. The results with dimethylsulfoxide show that the currently employed techniques of punction, preparation, freezing and thawing of bone marrow are suitable for clinical application.

Optic nerve metastasis from a mediastinal carcinoid tumour.

A 25-year-old man with a rapidly progressive metastatic carcinoid tumour arising in the mediastinum developed a metastasis to the optic nerve. This is the first instance in the literature of an ocular metastasis from this variant of carcinoid tumour. The ocular pathology, ultrastructure, and recent information on the derivation of this tumour, believed to originate in the thymus, are presented.

Prevalence of gram-negative rods in the normal pharyngeal flora.

We obtained throat cultures from 100 randomly selected people free from any chronic upper or lower respiratory disease who did not work in a hospital and who had not experienced any acute illness or received any antibacterial therapy in the 4 weeks preceding culture. Eighteen percent harbored either a species of Enterobacteriaceae or Pseudomonas aeruginosa in their pharynx. In all cases, colony counts were low, the majority being detected in broth media selective for Gram-negative rods. There were no clear-cut age or sex distributions of Gram-negative pharyngeal carriage. These data imply that, in at least some cases, isolation of Gram-negative rods from sputum of untreated patients may be a normal finding, and that in some patients with pulmonary infection, the pretreatment, upper respiratory tract flora may serve as the source of subsequent superinfection with Gram-negative rods.

Synchronization of rabbit bone-marrow cells in vivo.

The recovery of the rabbit bone-marrow from anaemia was investigated during an eight-day period of daily punctures of the tibiae after six days of phenylhydrazine treatment. A maximum of erythroid (range, 37.1 to 44.0%) and a minimum of leukoid cells (range, 8.4 to 13.4%) was observed on the fifth day of recovery. The rest, about 50% cells were reticulum cells. Signs of recovery were observed in peripheral blood as soon as on the first day after phenylhydrazine treatment. This led to the assumption that the tibiae became repopulated with active erythropoietic cells during anaemia, and that the reticulum cells might play a role as erythroid precursors in this process.
