Radiation absorbed doses at radiographic examination of third molars.

The radiation absorbed doses to critical organs, i.e. the thyroid and salivary glands and the gonadal region, were measured at radiographic examination of third molars. A tissue equivalent phantom was used together with ionization chamber detectors and TLDs. In the maxilla three, and in the mandible four different projections were used; also an extraoral lateral view. The greatest thyroid dose, 35 muGy, came from a mandibular disto-oblique projection with the circular tube collimator and Ultra-Speed film. the thyroid dose from an extraoral lateral view with high sensitivity screens was 3.7 muGy. The doses in different parts of the parotid gland from the disto-oblique mandibular projection with Ultra-Speed film ranged between 2.65 and 0.052 mGy. The corresponding doses in the submandibular gland were 1.74 mGy beneath the mandible and 0.458 mGy in the fovea. A rectangular tube collimator reduced the doses by approximately 50%. the Ekta-Speed film required approximately 40% lower exposure than the Ultra-Speed film. Without shielding the gonadal doses from a complete examination of four third molars were of the same order of magnitude as from a full survey with intraoral films, i.e. 3-7 muGy. A horizontal radiation shield reduced the thyroid doses by between 12 and 46% and the gonadal doses by between 50 and 95%. The reduction effect from the shield was relatively greater when using the larger aperture of the tube collimator. Combinations of leaded aprons and soft leaded collars reduced the thyroid doses by between 15 and 42% and the gonadal doses by two orders of magnitude.

[Necrotizing fasciitis--a frequently mistaken disease (author's transl)].

A case of Fasciitis necroticans on the leg is reported in a female patient, age 28. The disease began with symptoms of an erysipelas, but it was resistant to penicillin. Within a few days necrotic decay of circumscribed skin areas occurred and the adjacent skin was widely undermined by the necrotizing process. Beta-hemolytic streptococci and Staphylococcus aureus haemolyticus were isolated. However, the disease could not be managed by adequate antibiotic therapy. Finally, the process was brought under control by extended incisions and removal of the necrotic masses. Defects were covered by plastic surgery.

A survey of intraocular pressures with emphasis on patients under ten years of age.

This paper presents the results of a cross-sectional survey of intraocular pressures in patients of the University Optometric Center, the clinical facility of the State College of Optometry, SUNY. The study consisted of two parts: a pilot study conducted by one of the authors and a survey of patient records that were reviewed at random with particular emphasis on patients under 10. The results indicate that the intraocular pressure norms used to evaluate the general population are also appropriate for evaluation of patients under 10 years of age.

An embedded contact lens in the upper lid masquerading as a mass.

A patient who "lost" his right contact lens three years prior to our examination was found to have a mass in the right upper lid which contained the hard contact lens. A careful search of the superior fornix and the upper lid, with double eversion of the lids, is indicated in instances where a patient presents reporting loss of a lens he was wearing or inserting.

Functional significance of electrocardiographic changes after left ventricular aneurysmectomy.

Electrocardiographic (ECG) changes after left ventricular aneurysmectomy were analyzed in 20 patients, thirteen of whom had additional aorto coronary saphenous vein bypass surgery. ECG changes were correlated with postoperative clinical and hemodynamic results. Out of 14 patients (Group I) who showed hemodynamic and/or clinical improvement, eight had decrease of chronic ST segment elevation that was associated in five with loss of pathologic Q waves. In the remaining six patients (Group II) who showed no hemodynamic or clinical improvement as well as in six patients in Group I, chronic ST segment elevation persisted or increased and in some, new pathologic Q waves developed after surgery. The study suggests that loss of pathologic Q waves and/or decrease of chronic ST segment elevation in patients who undergo a left ventricular aneurysmectomy with aorto coronary saphenous vein bypass surgery may reflect postoperative clinical, hemodynamic and angiographic improvement. On the other hand, failure of these ECG changes to occur or, conversely, increased ST segment elevation and/or appearance of new Q waves may have no predictive value. The mechanisms for these ECG changes are discussed.

Morphometric investigations on endocrine glands. IV. The rat thyroid during liver regeneration after partial hepatectomy with and without application of carcinogenic substances.

Morphometric studies and counting of mitoses in thyroids during regeneration after partial hepatectomy had the following results: 1. During the first days after surgical operation laparotomy caused only a slight reduction of the thyroid function. 2. Up to the 3rd day and especially on the 10th day after partial hepatectomy there is a higher relative percentage of the epithelium in the thyroid, but the mitoses are diminished. 3. Prefeeding with methylnitrosourea and acetylaminofluorene before the surgical 2/3 removal of the liver was answered with subactive thyroids on the 3rd day.

Surgical treatment of solitary pulmonary metastasis.

Seventeen patients undergoing pulmonary excision for metastatic tumors of the lung from 1965 to 1975 are reviewed with an overall four-year survival of 72%. From this group, four patients are alive five years and more after operation. In two patients however, multiple metastases in the contralateral lung and bones have appeared. One of them underwent a second removal of metastatic tumor in the other lung. On the basis of this limited experience and the one reported in the literature, we believe that excision of pulmonary metastasis in selected cases improve the prognosis of those patients, prolong life, and in some cases may even result in cure of the basic disease.

[Age dependence of visual acuity after incomplicated cataractoperation (author's transl)].

The results of visual acuity in 1523 cataract-extractions were evaluated; comparing the visual acuity of the 7th to the 9th postoperative days, when the patients left the hospital. 73,1% of the patients had a supplemental disease of the eye or another general disease. 76,6% of the patients had a postoperative visual acuity between 5/4 and 5/15, clearly depending on the age of the patients. General diseases influence scarcely surgery, but the older the patients the more we find a reduced visual acuity that is reduced by generative senile macula disease. On an average they stayed in hospital for 9,4 days.
