HIV-1 RNA levels and the development of clinical disease. North American Lamivudine HIV Working Group.

OBJECTIVE: To assess the prognostic value of HIV RNA levels for predicting clinical disease independently of the CD4 lymphocyte count in patients on antiretroviral therapy. DESIGN: Cohort of HIV-infected patients from two trials of lamivudine therapy. PATIENTS: For 620 patients randomized in the North American NUCA3001 and NUCA3002 trials of lamivudine, HIV RNA levels were measured (median, seven measures per patient) and CD4 counts were assessed at a central laboratory (median, 13 counts per patient). Patients were in the 1993 Centers for Disease Control and Prevention (CDC) stages A (n = 439), B (n = 135) or C (n = 46) at baseline. OUTCOME MEASURES: For patients who were in CDC stage A at baseline we considered the ability of HIV RNA levels and CD4 counts to predict the development of CDC stage B or C disease. A Cox proportional hazards model was used. In a second analysis, patients who were AIDS-free at baseline were considered, and the endpoint was AIDS (CDC stage C). RESULTS: Patients' initial CD4 counts ranged (5-95% centiles) from 104 to 529 x 10(6)/l (median, 274 x 10(6)/l) and HIV RNA levels from 1900 to 339680 copies/ml (median, 44240 copies/ml). For the first analysis, with CDC stage B or C disease as endpoint, both the most recent HIV RNA level and CD4 count predicted the development of clinical disease [relative hazard (RH) for HIV RNA, 1.96 per 10-fold difference in HIV RNA; 95% confidence interval (CI), 1.41-2.73; P = 0.0001; and RH for CD4 count, 1.82 per twofold difference in CD4 count; 95% CI, 1.27-2.56; P = 0.0009]. When both HIV RNA and CD4 count were included in a multiple regression model, both markers provided information additional to that given by the other (RH for HIV RNA, 1.75; 95% CI, 1.23-2.50; P = 0.002; and RH for CD4 count, 1.40; 95% CI, 0.95-2.07; P = 0.09). In the second analysis, with AIDS as endpoint, both HIV RNA level (P = 0.02) and CD4 count (P = 0.004) were independently associated with clinical progression. These results were essentially unchanged after adjustment for treatment arm (zidovudine/lamivudine versus control arms). CONCLUSION: The HIV RNA level shows ability to predict the development of clinical disease and may thus be of importance in addition to the CD4 count in patient monitoring.

An immunodominant antigen of Brugia malayi is an asparaginyl-tRNA synthetase.

Lymphatic filariasis is caused by infection with the filarial nematodes Brugia malayi, Brugia timori, Wuchereria bancrofti and Onchocerca volvulus which collectively infect about 200 million persons throughout the world. Protein sequence homology analysis of a major nematode antigen suggested that it was a class II aminoacyl-tRNA synthetase. The overproduction, purification and verification that the major B. malayi antigen is an asparaginyl-tRNA synthetase is described.

'Corynebacterium ulcerans': a potential cause of diphtheria.

Symptoms similar to those of diphtheria may occasionally be caused by micro-organisms other than Corynebacterium diphtheriae, such as zoonotic corynebacteria. A case of pharyngitis caused by toxigenic 'C. ulcerans' in a 9 year old girl illustrates the importance of including diphtheroids in the differential diagnosis. The presenting symptoms may have been attenuated by previous immunisation against C. diphtheriae. 'C. ulcerans' should be considered in the differential diagnosis of pharyngitis, especially in patients with a history of contact with farm animals or the consumption of raw dairy products.

The special needs of children.

British literature related to the care of children in hospital states that they have 'special needs'. This concept is not defined; neither are different writers in total agreement about what these needs may be. This paper attempts to define needs, concluding that they are 'requirements that should be met'. Children are seen as dynamic beings because they are in a state of constant development, both physically and psychosocially. It is this state of development that makes their needs special to them. They have special requirements, which should be met in specific ways. Through the presentation of indicators of behaviours which would enable such needs to be met, it can be demonstrated that children should be cared for by people with the appropriate knowledge and understanding. When children are admitted to hospital there may be a number of different people involved in determining how a need should be met. An important part of this process should be nurses who have been specially educated for work with this age group.

Localization of immunoreactive epidermal growth factor receptor in neonatal and adult rat hippocampus.

The regional and developmental expression of epidermal growth factor (EGF) receptor in rat hippocampus was investigated utilizing immunocytochemical techniques at the light and electron microscopic levels. EGF receptor immunoreactivity in adult hippocampus was compared to that found at postnatal day 7 (P7). While the receptor was observed in P7 hippocampus, immunostaining was more prominent in the adult hippocampus, especially in the pyramidal CA2 field. Ultrastructural analysis of this region revealed that the receptor was localized to the cell bodies of both P7 and adult neurons rather than the axons or dendrites. The expression of EGF receptor in selected regions of the adult brain was verified by Western blotting. These results demonstrate the presence of EGF receptor in rat hippocampus as early as P7, localize the receptor to the pyramidal cell body, and establish the hippocampal formation, particularly CA2, as a major site of EGF receptor expression in rat brain.

Crystallization of the seryl-tRNA synthetase:tRNAS(ser) complex of Escherichia coli.

Crystals of the complex between seryl-tRNA synthetase and tRNA(2ser) from Escherichia coli have been obtained from ammonium sulphate solutions. The crystals are of the 1:2 enzyme:tRNA complex, belong to the space group C222(1), have cell dimensions of a = 128.9 A, b = 164.9 A, c = 127.3 A and diffract anisotropically from 3.5 to 4.5 A. An X-ray diffraction data set to 4 A has been collected. The combination of molecular replacement using the refined structure of the catalytic domain of the native enzyme, data from a heavy atom derivative and solvent flattening was used to produce a map at 4 A resolution. This shows that a tRNA molecule binds across the dimer, the anticodon stem and loop do not contact the protein and the helical arm of the enzyme contacts the T psi C loop and the long extra arm of the tRNA.

Methylene blue and rose bengal photoinactivation of RNA bacteriophages: comparative studies of 8-oxoguanine formation in isolated RNA.

Several reactive oxygen species, including singlet oxygen (1O2) and hydroxyl free radical (.OH), may potentially be involved in the photoinactivation of viruses by agents such as methylene blue (MB) and rose bengal (RB). Both 1O2 and .OH also mediate the formation of 8-oxoguanine (8-oxoGua) in DNA and RNA. Evidence that MB-or RB-induced bacteriophage (R17 or Q beta) inactivation and 8-oxoGua formation in RNA result from 1O2 rather than .OH was obtained utilizing complementary experimental approaches which show that: (i) the rate of phage photoinactivation by MB was unchanged by the presence of iron chelators or by different temperatures in the 13-37 degrees C range; (ii) MB- and RB-mediated rates of 8-oxoGua formation in isolated RNA have very little, if any, temperature dependence, in contrast to a significant temperature dependence of 8-oxoGua formation by a .OH generating system, the ultraviolet light irradiation of H2O2; and (iii) deuterium oxide (D2O) enhanced the RB-mediated rate of phage photoinactivation and 8-oxoGua formation in isolated RNA. The presence of superoxide dismutase in the RB photoinactivation reaction did not alter the rate of phage inactivation. The data suggest that 8-oxoGua serves as a marker that correlates qualitatively with 1O2-mediated lethal lesions in RNA bacteriophages.

Methylene blue plus light mediates 8-hydroxy 2'-deoxyguanosine formation in DNA preferentially over strand breakage.

Methylene blue (MB) plus light, in the presence of oxygen, mediates formation of 8-hydroxyguanine in DNA. The yield of 8-hydroxyguanine may be as much as from 2 to 4% of the guanines present. The results presented here show that treatment of supercoiled plasmid DNA with methylene blue plus light causes single-stranded nicks. However, single-stranded nicking occurs approximately 17-fold less frequently than does formation of 8-hydroxyguanine. The nicking rate is reduced in the presence of Mg ion but is not prevented by inhibitors of the iron-catalyzed Fenton reaction or by scavengers of hydroxyl free radicals. Extensive exposure of DNA to light in the presence of MB produces no detectable thiobarbital reactive material thus implicating that single strand nicking does not occur by hydroxyl free radical attack on deoxyribose. Formation of 8-hydroxyguanine is apparently not dependent upon intercalative binding of MB to DNA, since it is formed in polydeoxyguanylic acid.

The role of blood flow and/or muscle hypoxia in capillary growth in chronically stimulated fast muscles.

Capillary supply, the proportion of oxidative fibres and blood flow were studied in fast rat muscles (tibialis anterior, TA, and extensor digitorum, EDL) made ischaemic by ligation of the common iliac artery, in chronically stimulated muscles and in ischaemic chronically stimulated muscles. Stimulation was carried out for 6 h/day at 10 Hz (three periods of 2 h with 90-120-min intervals between stimulations) for 10-12 days using electrodes implanted in the vicinity of the lateral popliteal nerve. Blood flow (measured by radioactive microspheres) was 3.62 +/- 0.52 ml.100 g-1.min-1 at rest and 78.4 +/- 14.6 ml.100 g-1.min-1 (mean +/- SEM) during isometric contractions at 4 Hz. Ischaemic muscles had significantly lower blood flow at rest as well as during contractions (72 +/- 14% and 25 +/- 4% of the values in contralateral muscles respectively). Stimulated muscles had significantly higher flow than contralateral control muscles during contractions; stimulated ischaemic muscles had normal blood flow at rest, but the increase in flow during contractions was limited to a similar extent to that in ischaemic muscles alone. Of all anatomically present capillaries (staining for alkaline phosphatase in frozen sections) the capillary/fibre ratio increased by 36% in stimulated tibialis anterior, but was not significantly different from control muscles in stimulated ischaemic TA and was even lower than in control muscles in stimulated ischaemic EDL. The proportion of fast oxidative fibres (estimated on the basis of histochemical staining for myosin ATPase and succinate dehydrogenase) increased from 53.2 +/- 3.2% in normal EDL to 82.0 +/- 2.3% in chronically stimulated EDL and to 100% in chronically stimulated ischaemic muscles.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of torbafylline, pentoxifylline and buflomedil on vascularisation and fibre type of rat skeletal muscles subjected to limited blood supply.

1. Blood flow (measured by radio-labelled microspheres), fibre composition and capillary/fibre ratio were estimated in rat fast twitch skeletal muscles (tibialis anterior and extensor digitorum longus) five weeks after unilateral ligation of the common iliac artery in animals treated with either saline, torbafylline, pentoxifylline or buflomedil. 2. The resting blood flow was lower in muscles with limited blood supply than in their contralateral controls; this difference became statistically insignificant after treatment. Capillary/fibre ratio was similar in all muscles with either intact or limited blood supply and did not change after administration of any of the drugs. 3. The percentage of glycolytic fibres was not changed by ligation, but it decreased significantly in animals treated with torbafylline. This may improve performance in muscles with limited blood supply.

Correlated light and electron microscopic study of dopaminergic neurons and their synaptic junctions with DARPP-32-containing cells in three-dimensional reaggregate tissue culture.

An antibody to tyrosine hydroxylase has been used in a correlated light and electron microscopic study to characterize dopaminergic neurons and synaptic junctions in three-dimensional reaggregate cell culture. Dissociated fetal mesencephalic cells containing dopamine neurons were coaggregated with dissociated fetal striatal cells in rotatory culture for 21 days. Sections of the coaggregates were stained by the peroxidase anti-peroxidase technique to reveal tyrosine hydroxylase-immunoreactive structures. Clusters of immunoreactive perikarya as well as dendrites and axons were observed. Immunolabeled perikarya were round or oval and approximately 20 microns in diameter. Boutons immunoreactive for tyrosine hydroxylase formed symmetric synapses, primarily with unlabeled dendritic shafts. Symmetric membrane specializations were also observed between tyrosine hydroxylase-positive boutons and unlabeled dendritic spines as well as with the perikaryon of an unlabeled medium-size neuron possessing a slightly indented nucleus. To characterize the neurochemical nature of the neurons postsynaptic to tyrosine hydroxylase-positive boutons in the reaggregates, an antibody against DARPP-32 (a dopamine and adenosine 3':5'-monophosphate-regulated phosphoprotein) and an antibody against tyrosine hydroxylase were employed to visualize striatal dopaminoceptive neurons and dopaminergic structures, respectively, in the same section. Examination of reaggregate sections at the light microscopic level demonstrated that DARPP-32-immunoreactive cells were distributed into discrete clusters that were associated with patches of tyrosine hydroxylase-positive axonal varicosities. Ultrastructural analysis of tyrosine hydroxylase-positive boutons in such clusters revealed that dopaminergic axons synaptically contacted DARPP-32-immunoreactive neurons as well as unlabeled neuronal structures.

Effects of antibodies against odorant binding proteins on electrophysiological responses to odorants.

Monoclonal antibodies against two olfactory mucosal proteins, one with affinity for anisole-like and the other for benzaldehyde-like compounds, were applied to mouse olfactory epithelium. Responses to three odorants (anisole, benzaldehyde and amyl acetate) were measured. Of 26 antibodies, three (12%) inhibited responses only to the odorant with affinity for the antigen, nine (35%) inhibited responses to all three odorants, and 14 (54%) were without effect. None reduced responses by as much as 50%. The data support the hypothesis that there is a class of related proteins in olfactory neuronal cell membranes that function as receptor molecules and that other mechanisms also mediate odorant stimulation.

Comparative efficacies of imipenem-cilastatin and vancomycin in experimental aortic valve endocarditis due to methicillin resistant Staphylococcus aureus.

Activities of imipenem and vancomycin against methicillin resistant Staphylococcus aureus (MRSA) were compared in vitro and in a rabbit model of aortic valve endocarditis. Against 25 MRSA clinical isolates, imipenem was bacteriostatic (MIC90/MBC90, mg/l 8/32) in vitro while vancomycin was bactericidal (MIC90/MBC90, mg/l 2/4). Rabbit endocarditis was produced with a MRSA isolate against which both drugs were bactericidal. Imipenem-cilastatin had better efficacy than vancomycin by the following criteria, the number of survivors (9/13 vs 7/13), clearance of bacteraemia (9/9 vs 3/7; P = 0.019), sterility of cardiac vegetations (9/9 vs 1/7; P = 0.001) and sterility of distant organs (8/9 vs 2/7; P = 0.035). Thus, imipenem-cilastatin may be a potentially useful alternative agent to vancomycin in the therapy of MRSA endocarditis in the occasional situations when the drug demonstrates in-vitro bactericidal activity against the pathogen. Efficacy against MRSA strains with higher MBCs remains to be proved.
