Significance of renal hyaline arteriolosclerosis and tubulo-interstitial change in IgA glomerulonephropathy and focal glomerular sclerosis.

The relationship between glomerular sclerosis and hyaline arteriolosclerosis (HA) or tubulo-interstitial (T-I) change was investigated in 66 patients with IgA glomerulonephropathy (IgA-GN) and in 12 patients with idiopathic focal segmental glomerulosclerosis (FGS). More than a 10% incidence of global sclerosis was noted in 15 patients with IgA-GN and in 9 patients with FGS. There was no significant discrepancy of age between patients with IgA-GN and those with FGS (IgA-GN: 37.5 +/- 12.5; FGS: 35.7 +/- 10.3). In patients with IgA-GN, 7 out of 15 were HA-positive (46.7%), whereas in patients with FGS, 7 out of 9 were HA-positive (77.8%). However, a close correlation was observed between glomerular sclerosis and T-I change only in patients with IgA-GN (r = 0.82; p less than 0.001). The results suggested that in the case of IgA-GN, T-I change was related to the prognosis of glomerular sclerosis. In addition, it is assumed that a possible correlation exists between FGS and HA.

Stripe sign in pulmonary perfusion scintigraphy: central pattern of pulmonary emphysema. Work in progress.

Morphologic changes and ventilation abnormalities in the lung that resulted in the peripheral perfusion stripe (the stripe sign) on pulmonary perfusion scintigrams were studied in six patients. Chest radiographs, computed tomography (CT) scans, and positron emission tomography (PET) scans obtained using N-13 were compared. Although there was no apparent correlation between the presence of the stripe sign and the findings on the chest radiographs, PET scans depicted ventilation abnormalities in all of 11 lung areas with the stripe sign in six patients. In addition, ventilation was affected more severely in the central part of the lung than in the subpleural region in ten of the 11 lung areas. CT scans demonstrated emphysematous changes, with low attenuation values located mainly in the central part of the lung in all four patients examined. The results suggest an intimate relationship between the stripe sign and the central pattern of pulmonary emphysema.

[A phase II study of etoposide (VP-16) injection in primary lung cancer by cooperative study group].

A Phase II Study of VP-16 was performed in 58 patients with primary lung cancer. VP-16 was administered via infusion at a dosage of 60-100 mg/m2 daily for 5 days, and repeated every 3 to 4 weeks. Of the 58 patients who entered into the study, 38 were evaluable. Partial response was observed in 6 patients who had been diagnosed as having small cell carcinoma of the lung. Response rates were 15.8% for all evaluated patients and 33.3% for the patients with small cell carcinoma of the lung. As for dose-limiting toxicity, leukopenia (less than 3,000/mm3) was observed in 53.1% of cases. Other major adverse reactions were hematologic toxicities such as anemia and thrombocytopenia, gastrointestinal toxicities and alopecia, but these were all well tolerated.

Hepatocellular carcinoma presenting extrahepatic obstructive jaundice due to bile duct invasion--clinicopathological study of two cases.

Two rare cases of autopsy and surgery presenting extrahepatic biliary obstruction due to intrabile-duct growth of hepatocellular carcinoma were reported. Clinically obstructive jaundice was predominant in comparison with the other symptoms in both cases. In one autopsy case, hepatocellular carcinoma developed in the right lobe of the cirrhotic liver (posthepatitic). It involved the secondary branch of the right hepatic duct and grew into the common hepatic duct. In the other case of surgical operation, hepatocellular carcinoma, which developed in the posterior portion of the right lobe of the cirrhotic liver (posthepatitic), destroyed the posterior wall of the bifurcation of the bilateral hepatic duct and obstructed the common hepatic duct due to the intraductal cancer growth. From the site of the bile duct invasion or permeation by the tumor, two cases were classified into the peripheral (the former case) and proximal (the latter case) types, respectively. Furthermore, as far as obstructive jaundice is clinically concerned, the possibility should be kept in mind that hepatocellular carcinoma may proliferate into the large bile ducts, apart from that of cholangiocarcinoma or cholelithiasis.

[A double-blind comparative study on the efficacy of S6472, cefaclor, in the treatment of bacterial bronchitis].

Clinical efficacy and safety of S6472 (long active cefaclor (CCL)) and CCL were compared by a double-blind study using 248 patients with bacterial bronchitis. Patients were orally administered with either 375 mg of S6472 2 times or 250 mg of CCL 3 times daily for 7 days, and the following results were obtained. Clinical efficacy judged by a committee: The efficacy rates on acute bronchitis were 87.2% for S6472 and 82.6% for CCL, and the efficacy rates on chronic bronchitis were 70.3% for S 6472 and 64.7% for CCL. There was no significant difference between the 2 drugs in clinical efficacy. Clinical efficacy judged by doctors in charge: Results of the clinical efficacy judged by doctors in charge were similar to the efficacy judged by the committee. Bacteriological efficacy judged by the committee: Eradication rate for the S6472 group was 71.1% (32 of 45 patients) and that for the CCL group was 67.4% (29 of 43 patients). There was no significant difference between the 2 drugs in the bacteriological efficacy. Side effects and abnormal laboratory findings: Side effects were observed in 5 cases (4.2%) in the S6472 group and in 5 cases (4.0%) in the CCL group. These side effects, however, were not serious, and no remarkable abnormal laboratory values were observed. There was no significant difference between the 2 drugs in the incidence of side effects or abnormal laboratory findings. Clinical utility: When the clinical utility was expressed by the utility rate (very useful and useful), it was 79.5% in the S6472 group (117 patients) and 76.2% in the CCL group (122 patients) (judged by the committee). When the utility was judged by doctors in charge, it was 73.3% in S6472 group (120 patients) and 71.4% in CCL group (126 patients). From the above results, it has been concluded that S6472 taken twice daily is equivalent to CCL in effectiveness on bacterial bronchitis.

[An autopsy case of adult neuroblastoma].

The patient was a 31-year-old man with neuroblastoma of the right adrenal gland, which presented at first as metastatic spinal epidural tumor. Catecholamine and its products in the blood and urine and neuron-specific enolase in the blood were increased. Six months after right adrenectomy followed by chemotherapy, the patient died due to cachexia. Autopsy revealed widespread metastasis. By electron microscopy, the centers of the Homer-Wright rosettes were occupied by neuropil-like structures consisting of a number of cytoplasmic processes of tumor cells, but no cored vesicles were found either in the processes or perikaryons of the tumor cells.

Migration inhibition and stimulation factors produced from peripheral blood lymphocyte cultures of sensitised guinea pigs.

The supernatants of peripheral blood lymphocytes of tuberculin-sensitive guinea pigs incubated with PPD were investigated using macrophage and leukocyte migration inhibition tests. Inhibition as well as stimulation of cell migration were observed. The effect upon migration cultures seemed to be dependent upon the immunological state of the host; animals of the V-group (vaccinated once without challenge) showed inhibitory activity to both macrophages and leukocytes, while those in the VC-group (vaccinated and challenged) had stimulatory activity only to leukocytes. The addition of antithymocyte serum stopped all activity (macrophage and leukocyte inhibition and leukocyte stimulation), suggesting that thymus-dependent lymphocytes are necessary for such activity.
