Functional and geometrical interference and interdependency between the right and left ventricle in cor pulmonale: an experimental study on simultaneous measurement of biventricular geometry of acute right ventricular pressure overload.

To clarify the effects of right ventricular (RV) pressure overload on functional and geometrical interference and interdependency between the right and left ventricle, both ventricular internal diameters were measured by the microcrystal technique during lycopodium induced pulmonary embolization in the dog. By repeated embolization, RV systolic pressure was increased progressively until it reached a peak value of about 60-70 mmHg, then it began to fall. At the same time, the hemodynamics deteriorated progressively resulting in death. During the experiment, gradual leftward displacement of the interventricular septum (IVS) without any change in left ventricular (LV) free wall geometry was observed. In pulmonary embolic shock, which showed a fall in LV pressure to about 60 mmHg and cardiac output to about 40% of control, the leftward displacement of IVS became marked, and the cooperative movement of IVS to LV contraction disappeared. The IVS position during acute RV pressure overload was able to account for the transseptal pressure gradient. The importance of IVS position and motion in cardiac function during acute RV pressure overload was stressed. Furthermore, to establish the theoretical treatment in acute cardiopulmonary resuscitation, ligation of the descending aorta (AoL) or norepinephrine ("N") or isoproterenol ("I") administration were examined in a canine pulmonary embolic shock model. AoL or "N" improved the deteriorated hemodynamics with restoration of biventricular geometry. However, "I" did not restore the biventricular geometry despite the transiently improved hemodynamics, and the experimental animals were unable to survive. These results suggest the importance of the maintainance of systemic pressure for the restoration of failed RV function. Further integrated studies are required to understand biventricular interference and interdependency.

[Clinical study on the prognosis of complete denture wearers covering 10 years. 4. Masticatory efficiency].

The purpose of this study was to estimate masticatory efficiency of complete denture wearers. The subjects were 72 patients, treated before 5-10 years at students' clinic of Tsurumi University, School of Dental Medicine. The masticatory efficiencies with peanuts and Kamaboko were calculated. Results 1. The mean masticatory efficiency with peanuts for 63 patients was 35.8%, and with Kamaboko (steamed fishpast) for 64 patients was 94.5%, in comparison with natural dentition. 2. The mean masticatory efficiency with peanuts was 124.3%, and with Kamaboko was 99.6%, for complete denture wearers. 3. Comparing the measured value of masticatory efficiency with 5 to 10 years wearing period, there was significant difference in the masticatory efficiency between peanuts and Kamaboko. 4. There was significant difference in the masticatory efficiency with peanuts between the group needed new denture and the group not needed but there was not significant difference with Kamaboko.

[Clinical evaluation of the metal-based denture using superplastic titanium alloy for upper edentulous patients].

The purpose of this study was to evaluate clinically superplastic titanium alloy for maxillary complete denture. The subjects that were the 30 patients with upper edentulous ridges were selected from Tsurumi University Dental Hospital. 17 patients were maxillary and mandibular edentulous, the other patients were only maxillary edentulous. Each of metal base was made of 90 Ti-6 Al-4 V alloy by Sankin Dental Co.. The portion of a resin was used of PMMA resin with 4-META (4-methacryloxyethyl trimellitic anhydride). At the time of inserting the denture and one month after, the dentures were clinically observed about the retentive force and the adhesive condition on finish line. At the same time, the patients were examined oral mucose and inquired of the feeling relating to wear new denture. Furthermore each weight of new dentures was compared with each of old dentures. The results were as following: 1. The width and depth of the palate were measured previously on posterior palatal sealed area. The mean value of the depth was 8.8 +/- 1.8 mm and the mean value of the width was 44.9 +/- 2.7 mm. 2. The retentive force of new dentures was clinically adequate enough to achievement of function without two cases. 3. The adhesive condition on the finish line was adequate in all cases. 4. The mean weight value of the new denture was 20.1 +/- 3.9 g. 5. 2 of 30 patients were finded the slightly redness without pain on the middle palatal mucose. 6. The feelings of patients relating to wear were as following: 1) Lighter than old denture. 2) More extensive tongue space.(ABSTRACT TRUNCATED AT 250 WORDS)

Mechanism of anti-ulcer effect of calf blood extract.

The mechanism of anti-ulcer effect of Solcoseryl, an extract from calf blood, was studied. Solcoseryl (10 ml/kg, i. v.) increased basal and chemically reduced potential difference of the gastric mucosa in rats. Solcoseryl (1-10 ml/kg, i.v.) also increased gastric mucosal blood flow and gastroduodenal HCO3- secretion. These results indicate that Solcoseryl has a mucosal protective effect which may contribute to the mechanism of its anti-ulcer effect.

[Two cases of miliary tuberculosis with SIADH].

Two cases of miliary tuberculosis with syndrome of inappropriate secretion of antidiuretic hormone (SIADH) were reported. Case 1. A 70-year-old woman suffering from general fatigue and appetite loss developed neck stiffness and stupor three days after admission. The chest X-ray film showed a miliary pattern in both lungs. The lumber puncture showed high pressure and increased leucocytes in the cerebrospinal fluid. Serum natrium concentration was 113 mEq/L. Tubercle bacilli were seen in the broncho-alveolar lavage fluid by the Ziehl-Nielsen staining. An improvement in electrolytes balance was produced by 2.5% NaCl and antituberculous treatment, then her mental function recovered. Case 2. A 71-year-old man was admitted with gastric ulcer. When he developed dry cough thirty days after admission, the chest X-ray film showed a miliary pattern in both lungs. Acute respiratory failure advanced concomitantly. Tubercle bacilli were seen in the sputum (Gaffky 5) by the Ziehl-Nielsen staining. Antituberculous treatment was started. Although the miliary shadow improved gradually, hyponatremia was rather progressing. The following values for serum constituents were determined: sodium, 118 mEq/L; antidiuretic hormone, 10.3 pg/ml. Antituberculous treatment and supplement of NaCl (10 g/day) improved serum natrium level. He had no mental disturbance in his clinical course. In both cases, thyroid, renal and adrenal function were normal. Systemic edema and dehydration did not exist at the state of hyponatremia, and it was very clear that laboratory data were compatible with SIADH criteria. Miliary tuberculosis is one of the least commonly recognized causes of SIADH.

Tentative identification of arterial baroreceptors associated with arteriovenous anastomoses.

Nerve endings of the trigeminal nerve were examined in the lip, nostril, upper jaw and supraorbital skin of cat by the horseradish peroxidase (HRP) tracing method. Wheatgerm agglutinin-HRP (WGA-HRP) as injected into the spinal trigeminal nucleus was transported transganglionically to nerve endings. The present study deals with presumptive baroreceptive nerve endings found on arteriovenous anastomoses and on some other large arteries. By light microscopy HRP-labeled, complexly arborized nerve endings were found on the walls of arteries located deep in the subcutaneous tissue of the lip and nostril. These arteries were identified in serial sections to be an arterial segment of an arteriovenous anastomosis. By electron microscopy characteristic nerve endings were located in the adventitia and partly extended into the adjacent connective tissue of the arterial wall. These terminals had no connective tissue capsule. Axon terminals were somewhat enlarged (1-2 microns in diameter) and extended through bundles of collagen fibrils. The terminals contained an abundance of mitochondria and some vesicles. Axon terminals were typically covered by thin Schwann cell processes, but parts of the axolemma were sometimes devoid of such Schwann cell coverings, being invested only by basal laminae. Cell bodies of Schwann cells were located apart from axons. These light and electron microscopic features of the endings resembled those of other well-defined baroreceptors reported in the carotid sinus, aortic arch and endocardium, as well as of Ruffini terminals and Golgi tendon organs, suggesting that they would be baroreceptors of arteriovenous anastomosis. In addition HRP-labeled single nerve fibers with varicosities were found in the walls of some large arteries in the facial skin. By electron microscopy, such a HRP-positive nerve fiber contained some mitochondria and vesicles in varicosities and coursed with a bundle of HRP-negative fine fibers in the adventitia of arteries. These HRP-labeled single fibers were considered to be sensory derived from trigeminal nerves.

[Unexpected renal cell carcinomas found during the operation in renal calculus: report of two cases].

Case 1: A 58-year-old man was seen with dull pain over the left flank. The X-ray examination revealed a renal stone in the left kidney. On operation, a globular hard mass was found in the lower portion of the posterior aspect of the kidney from which a biopsy specimen was obtained. A frozen section study showed renal cell carcinoma, and nephrectomy was performed. The patient is well four years after the operation without recurrence or metastasis. Case 2: A 50-year-old man consulted with microhematuria pointed out in a physical examination. At the operation for renal stone, we found the mass in the lower portion of the kidney. A frozen section study of the mass revealed renal cell carcinoma. We performed nephrectomy and lymphadenectomy on the renal pedicle. The patient is well three years after the operation. We reviewed the cases of renal cell carcinoma associated with renal calculi reported in Japan and discussed how the nephrectomy should be performed according to the frozen section study.

[Obstructive sleep apnea syndrome with reversible interventricular septal displacement during sleep: a case report].

A case of the obstructive sleep apnea syndrome revealed reversible leftward displacement of the interventricular septum by echocardiography during sleep. A 46-year-old housewife with congenital micrognathia was admitted to our hospital complaining of severe dyspnea and general edema. On admission, she had severe hypoxemia (PaO2 = 35.2 mmHg), pulmonary hypertension (mean pulmonary artery pressure = 70 mmHg) and right heart failure. Her echocardiograms revealed enlargement of the right ventricle with a flattened left ventricle. A sleep study performed after partial resolution of her right heart failure disclosed that severe hypoxemia and pulmonary hypertension (mean pulmonary artery pressure = 70 mmHg) occurred after relatively long periods of apnea. With vigorous inspiratory efforts during sleep apnea, transient enlargement of the right ventricle and leftward displacement of the septum causing the flattened left ventricle were observed echocardiographically. A concomitant decrease in left ventricular inflow velocities was also observed by the pulsed Doppler method. However, these findings immediately returned to normal with the resumption of ventilation. We concluded that these repetitive apneic events due to obstruction of the airway during sleep might accelerate complete eventual pulmonary hypertension and right heart failure.
