[Evaluation of the efficacy of photodynamic therapy on experimental hepatocellular carcinoma--using local injection of photosensitizer].

The effect on photodynamic therapy (PDT) of using pheophorbide-a as a photosensitizer and Nd: YAG laser (Q-switch) was evaluated. Two hundred micrograms (0.1 ml) of pheophorbide-a was injected into tumors that had been subcutaneously implanted into the backs of nude mice (BALB/c-nu). Interstitial Nd: YAG laser irradiation (Q-switch; mean power 0.5 W, duration 10 min.) was performed 72 hours after pheophorbide-a injection. Forty-eight hours after laser irradiation, the areas of tumor necrosis were measured; these were larger in the group with the injection of pheophorbide-a than in those without. The areas of tumor necrosis after Nd: YAG laser irradiation at a higher mean power (Q-switch; mean power 2 W, control temperature 43-43.5 degrees C, duration 10 min.) were also measured. Again, the areas were larger in the group injected with pheophorbide-a. These results show that the injection of pheophorbide-a and interstitial irradiation using Nd: YAG laser induce a photodynamic reaction, and that this combination is useful in the treatment of deep-seated tumors.

Design and synthesis of basic peptides having amphipathic beta-structure and their interaction with phospholipid membranes.

Basic amphipathic beta-structural peptides, Ac-(Ser-Val-Lys-Val)n-NHCH3 (1n, n = 1-3) and Ac-(Lys-Val)n-NHCH3 (2n, n = 2-4), were synthesized and their interaction with DPPC and DPPC-DPPG (3:1) bilayers was studied by CD, dye-leakage and fluorescence experiments. The CD data indicated that oligopeptides consisting of more than eight residues with alternating hydrophobic (Val) and hydrophilic amino acids (Ser and Lys) were able to form an amphipathic beta-structure in acidic phospholipid bilayers, but not or weakly in aqueous solution and in neutral phospholipid bilayers. The dye-leakage experiment showed that the basic amphipathic beta-structural peptides interact with acidic phospholipid bilayers to perturb them, but less effectively compared with basic amphipathic alpha-helical peptides. Fluorescent spectroscopic data suggest that hydrophobic side of the amphipathic peptides may immerse into membrane without deep penetration. Based on these results, we postulate that the formation of the basic amphipathic beta-structure on acidic lipid bilayers may be due to the combined effect of electrostatic and hydrophobic interactions between basic peptides and acidic lipid bilayers.

Monoclonal antibodies to Pgp-1/CD44 block lympho-hemopoiesis in long-term bone marrow cultures.

A new panel of mAbs was prepared to a stromal cell line known to support lymphocytes in Whitlock-Witte type long-term bone marrow cultures. These antibodies were then screened with a cell adhesion assay and four were selected that inhibited the binding of B lineage cells to stromal cell monolayers. Immunofluorescent and biochemical analyses revealed that these new antibodies detected epitopes of the previously described Pgp-1/CD44 antigen complex. Addition of Pgp-1/CD44 antibodies to Dexter-type long-term bone marrow cultures completely prevented emergence of myeloid cells and they also blocked lymphocyte growth in Whitlock-Witte type cultures. mAbs MEL-14, LFA-1, and CD45R did not inhibit under the same conditions and there was no apparent relationship to Ig isotype. Adherent layers in treated cultures were not unusual in terms of morphology and the antibodies did not affect factor-dependent replication of lymphoid or myeloid progenitor cells. Therefore, the mechanism of inhibition may not involve direct toxicity to precursors or microenvironmental elements. Previous studies in humans and mice have implicated Pgp-1/CD44-related glycoproteins in the migration of peripheral lymphoid cells, as well as interactions of cells with the extracellular matrix. These findings suggest that they may also be critical for formation of lymphoid and myeloid cells within bone marrow.

Cerebral circulation and oxygen metabolism associated with subclinical periventricular hyperintensity as shown by magnetic resonance imaging.

A combined magnetic resonance imaging and positron emission tomography study was performed on 21 patients with cerebrovascular risk factors but without neurological abnormalities. Our purpose was to investigate the hypothesis that periventricular hyperintensity (PVH) reflects ischemia. Periventricular hyperintensity was evaluated with a method we devised, and cerebral circulation and oxygen metabolism were evaluated with the oxygen-15 steady-state technique. We concluded that the brain with severe periventricular hyperintensity had abnormal circulation, although oxygen metabolism was not measurably affected. The role of a compensation mechanism under conditions of decreased oxygen supply was considered.

Involvement of I-A-restricted B-B cell interaction in the polyclonal B cell differentiation induced by lipopolysaccharide.

The present study has examined a functional role of Ia molecules expressed on murine B cells in polyclonal B cell differentiation induced by lipopolysaccharide (LPS). Reverse, IgM PFC responses of unprimed B cells induced by LPS in the apparent absence of T cells and adherent accessory cells were markedly inhibited in a haplotype-specific manner by Fab monomer fragment of anti-class II (Ia) but not anti-class I MHC monoclonal antibody (mAb). However, the degree of inhibition of LPS responses of H-2-heterozygous F1 B cells expressing both parental I-A products by either one of anti-I-A mAb was at best half that of the parental B cells. Interestingly, when (B10 x B10.-BR)F1 (H-2b/k) B cells were fractionated into adherent and nonadherent populations by their ability to bind to parental B10 B cell monolayers, LPS responses of F1 B cells adherent to and nonadherent to the B10 B cell monolayers were selectively inhibited by anti-I-Ab and anti-I-Ak mAb, respectively. These results suggest that LPS-responsive F1 B cells comprise at least two separate populations with restriction specificity for only one of the parental I-A products expressed on B cells. In addition, it was demonstrated that the I-A-restriction specificity of LPS-responsive B cells is "plastic" and determined by H-2-genotype of bone marrow cells present during B cell ontogeny but not by that of radiation-resistant host elements. Namely, the LPS responses of B10-derived B cells from (B10 + B10.BR) (H-2b x H - 2k)F1 radiation bone marrow chimeras but not from B10 (H-2b x H-2k)F1 chimeras became sensitive to the inhibition of anti-I-Ak mAb in the presence of mitomycin C-treated I-Ak-positive B cells, supporting a notion of receptor-Ia molecules interactions rather than like-like interactions. Thus, the present results provide evidence indicating that B-B cell interaction via recognition of self-I-A products is a crucial event in the polyclonal B cell differentiation induced by LPS.

The cerebral circulation in cases of carotid cavernous fistula. Findings of single photon emission computed tomography.

Ten patients with carotid cavernous fistula (CCF) were reviewed. Single photon emission computed tomography (SPECT) for regional cerebral blood flow (rCBF) study was performed in six patients with fast flow shunt. Five of these six patients with fast flow shunt angiographically demonstrated cortical venous drainage, and two of these patients suffered intracerebral hemorrhage. The rCBF decreased in the region demonstrating the cortical venous drainage in five patients, and in one patient the angiography demonstrated arterial steal and the rCBF decreased in a part of the ipsilateral cerebral hemisphere. SPECT proved to be useful in assessing the cerebral circulation in cases of CCF.

Amyotrophic lateral sclerosis: unusually low content of collagen in skin.

The skin tissues from patients with amyotrophic lateral sclerosis (ALS) and controls were studied by electron microscopy, and their amino acid compositions were examined. On electron microscopy, the most conspicuous findings in ALS were (1) a marked increase in amorphous material separating collagen bundles, and (2) the smaller diameter of collagen fibrils. These were more marked with longer duration of illness. Amino acid analysis showed that the levels of hydroxyproline, hydroxylysine, and glycine were significantly low (P less than 0.001, P less than 0.01, and P less than 0.001, respectively) in ALS patients as compared with those of controls, and there was a significant negative correlation between the level of hydroxyproline and duration of illness in ALS patients (r = -0.88, P less than 0.01). In addition, the collagen content per dry weight (mg) of the tissues in ALS was significantly smaller (P less than 0.001) than in controls. These results indicate that the metabolism of skin collagen might be affected in the disease process of ALS.

B-B cell interaction involved in polyclonal B cell activation is restricted by I-A but not by I-E molecules.

We have previously demonstrated that the class II MHC restricted B-B cell interaction is involved in the polyclonal differentiation of unprimed murine B cells into IgM-producing cells induced by a T cell-derived lymphokine B151-TRF2 or bacterial LPS. The present study has addressed the question of whether I-A and/or I-E molecules function as restriction elements for the B-B cell interaction. The results revealed that (B10 x B10.BR)F1(H-2b/k) B cells could be separated into I-Ab- and 1-Ak-restricted subpopulations by their ability to bind to B10(H-2b) or B10.BR(H-2k) B cell monolayers, whereas an I-E-restricted F1 B cell population was not obtained. Moreover, B10-derived B cells isolated from (B10 + B10.BR) - (B10 x B10.BR)F1 but not from B10 - (B10 x B10.BR)F1 radiation-induced bone marrow chimeras acquired newly the ability to co-operate with mitomycin C-treated auxilary B cells expressing I-Ak but not I-Ek molecules. Thus, these results indicate that I-E molecules, unlike I-A molecules, do not serve as restriction elements for the B-B cell interaction, and that I-A and I-E molecules on B cells play functionally disparate roles in the activation of polyclonal B cells.

Iodine-131 uptake in a patient with thyroid cancer and rheumatoid arthritis during acupuncture treatment.

A patient with thyroid carcinoma had abnormal accumulation of I-131 in the areas of both feet and hands on whole body scan. The sites of abnormal accumulation of I-131 were similar to those on bone scintigraphy. The radiographic examination of the lesions showed characteristic findings of rheumatoid arthritis, and the presence of small gold needles for acupuncture treatment was demonstrated. There were no findings of bone metastases. Although the mechanism of accumulation of I-131 in this patient is unknown, interpreters of I-131 whole body scintigraphs should keep this case in mind when acupuncture treatment has been done. The authors can only speculate on a common blood flow mechanism for enhanced HMDP and I-131 uptake in this arthritic patient who had been treated by acupuncture.

Equilibrium radionuclide ventriculography in Duchenne's cardiomyopathy.

Myocardial dysfunction in 14 patients with Duchenne's muscular dystrophy (DMD) was evaluated by Tc-99m HSA multigated radionuclide ventriculography (MGRV). Follow-up studies were performed on eight patients for 5 years. Left ventricular ejection fraction and standard deviation of the phase angle were abnormal in all patients older than 15 years. Hypokinetic changes and phase delay were recognizable in most patients older than 17 years. Peak filling rate decreased in patients older than 17 years. MGRV was considered an effective method for evaluating serial changes in cardiomyopathy of patients with DMD.

Side reaction of pyrenylalanine-peptides containing NG-tosylarginine during detosylation with hydrogen fluoride.

Formation of by-products of pyrenylalanine-peptides was observed during the cleavage of a tosyl group in pyrenylalanine-peptides containing an Arg(Tos) residue with HF. NMR and fluorescence experiments showed that by-products were compounds in which a pyrenyl group was modified with a tosyl group(s). The side reaction was little suppressed by the addition of usual scavengers such as anisole, 1,2-ethanedithiol, thioanisole and p-cresol. Under the conditions used for the cleavage of N alpha-Boc group, i.e., trifluoroacetic acid or 4 M HCl in dioxane, the pyrenylalanine residue was stable. No side reaction was, furthermore, observed in the reductive cleavage with sodium in liquid ammonia. These results suggest that the side reaction may be due to an electrophilic attack of tosyl cations to the electron-rich pyrenyl group in the pyrenylalanine residue under the HF-mediated acidic conditions.

Effects of ultraviolet B irradiation on lenticular riboflavin metabolism and high-molecular-weight-protein aggregation.

We investigated the effects of ultraviolet B (UVB) irradiation on the formation of ester forms of riboflavin and of high-molecular-weight (HMW) protein aggregates and on lenticular riboflavin-binding capacity (LRBC). Esterification of riboflavin decreased as the duration of UV irradiation increased, suggesting the irradiation-induced denaturation of the apoenzyme of synthetases of ester forms of riboflavin. UVB irradiation of lens homogenate supplemented with riboflavin increased LRBC and the formation of HMW protein aggregates, while gamma-crystallin was decreased. These results are consistent with those of our earlier studies in which we obtained data suggesting that, upon exposure of rat lens homogenate to fluorescent light, photosensitized riboflavin may bring about cross-linking of lens protein. Our data demonstrate that the photosensitivity of lenticular riboflavin is increased by longer periods of UV irradiation.

Cardiac sports rehabilitation for patients with ischemic heart disease.

This paper summarizes data gathered from an 8 years sports cardiac rehabilitation program at Kyoto University and satellite hospital (Takeda Hospital). In total, 185 patients were rehabilitated under our original program with no serious cardiac accidents. Compliance was 58.2% after 6 months of this rehabilitation. In patients selected to evaluate the value of rehabilitation, exercise study with treadmill and 201Tl scintigraphic study showed improvement in exercise tolerance (in 58% of patients) and in perfusion on exercise (in 40%). Coronary angiographic study showed regression of arterial narrowing in 16% of the patients studied. We conclude that in addition to emotional and psychological support, our sports cardiac rehabilitation is safe and effective in improving exercise tolerance and cardiac perfusion.

T1 and T2 relaxation times on gadolinium-diethylenetriaminepentaacetic acid enhanced magnetic resonance images of brain tumors.

MRI of brain tumors was performed with CPMG pulse sequence, and T1 and T2 relaxation times were measured before and after the administration of 0.1 mmol/kg Gd-DTPA. T1 and T2 became significantly shorter than before with the T1 shortening being about twice the T2's. Three types of T2 shortening were classified by the difference in the intra-tumoral transport of Gd-DTPA in the time-course study after the administration. Transportal activity of Gd-DTPA seemed to be reflected by the physiologic viability of tumor suggested by pre-contrast T2.

[A case of recurrent lung cancer complicated acute interstitial pneumonia, just after injected peplomycin].

A 77-year-old man with recurrent lung cancer was administered peplomycin (PEP) 15 mg 1A only one time, from a link in the chain of chemotherapy. But just after it was occurred dyspnea, and heard crepitus by auscultation. And it was recognized of ground glass appearance and air-bronchogram in the entire lung field by chest X-ray photograph, so made a diagnosis of acute diffuse interstitial pneumonia by clinical. He died two days after in spite of emergency treatment, that is high dose steroid, O2 flow etc. We discussed the acute pulmonary side effect of peplomycin which is one of the carcinostatic antibiotic, according to this clinical progress.

Inhibitory effect of midaglizole on alpha adrenoceptor agonist-induced contractions of canine tracheal smooth muscle.

To determine why midaglizole is effective in some patients with severe asthma, we investigated the inhibitory effects of midaglizole, prazosin or yohimbine on the BHT 920-, phenylephrine-, or noradrenaline-induced contractions of canine tracheal smooth muscle. After pretreatment with atropine (10(-6) M) and propranolol (10(-6) M) and precontraction with serotonin (3 x 10(-7) M), the tracheal muscle showed contractile responses to the exogenous administration of both alpha 1 and alpha 2 adrenoceptor agonists. Every alpha antagonist inhibited these agonist-induced contractions. Inhibitory activity of midaglizole (10(-4) M) for the alpha agonists was BHT-920 greater than noradrenaline greater than or equal to phenylephrine, while that of prazosin (3 x 10(-6) M) was phenylephrine greater than noradrenaline greater than BHT-920. Moreover, yohimbine completely inhibited the contractions at the lower concentration of 3 x 10(-7) M than that of other two antagonists. Our findings demonstrate that midaglizole dose-dependently inhibits airway contractions induced by alpha adrenoceptor agonists.

[1H-NMR studies of plasma from patients with liver diseases].

We examined the 1H-NMR spectra of the lipid region of human plasma according to the method of Fossel et al. as the specific blood test for cancer. The study group included 20 patients with hepatocellular carcinoma (HCC), 10 patients with liver cirrhosis (LC) and 10 normal controls (NC). We measured the linewidths of methyl and methylene resonances of lipoprotein lipids at a proton resonance frequency of 400 MHz using a JNM GX400 FT NMR spectrometer. The mean linewidth values and the standard deviation of methyl were as follows: 41.3 +/- 7.2 Hz (NC), 34.8 +/- 6.9 Hz (LC), 29.6 +/- 5.7 Hz (HCC). There was thus a significant difference between NC and HCC. On the other hand, there was no significant difference between the three groups in the case of methylene. Although the spectrum in the HCC and LC groups had a blunt head but narrow body, that of the NC group had a pointed head but wide body, and the linewidth values thus showed wide dispersion. This difference in spectral patterns between the two groups was characteristic. Linewidth values, however, did not correlate well with the laboratory data.
