A double-blind comparison of the clinical tolerance of bacampicillin and pivampicillin. A preliminary report.

In this double-blind study, the clinical tolerance of high doses of bacampicillin hydrochloride (800 mg t. i. d.) and of pivampicillin base (700 mg t. i. d.) administered for ten days were compared in out-patients with acute urinary tract infections. The clinical response was comparable with that previously reported for those antibiotics. Upper gastrointestinal side-effects were more common after treatment with pivampicillin and were reported in 9/38 patients compared to 3/34 patients treated with bacampicillin. The duration of the symptoms of the side-effects was also more prolonged after pivampicillin than after bacampicillin.

Long-term fate of [14C]nicotine in the mouse: retention in the bronchi, melanin-containing tissues and urinary bladder wall.

N-methyl-14C and 2'-14C-labelled nicotine were used for whole-body autoradiographic distribution studies on C57BL- and NMRI-mice. Radioactivity was retained in the melanin-containing tissues, in the bronchial walls, and in the urinary bladder wall, up to 1 month after administration. The activity levels in the bronchi decreased faster if [2'(14)C] nicotine was used. Quantitative measurements of the retention of the 2 14C-labelled nicotine preparations confirmed the autoradiographic findings. It is proposed that nicotine is N-demthylated in the bronchial mucosa, the off-coming methyl group being incorporated into the cell constituents of the mucosa. Thin-layer chromatographic studies showed that no nicotine was present in the lungs after 24 h. In melanin, however, only unmetabolized nicotine was found from 4 h on. Some reactive nicotine metabolites may be responsible for the retention in the urinary bladder wall. Also in the full-term fetuses radioactivity accumulated in the pigmented eyes and in the respiratory tract. The accumulation and long-term retention of nicotine in the melanin-containing structures might accelerate the development of drug-induced or senile changes in these tissues. The retention in the urinary bladder wall persisted even after rinsing. This may indicate an accumulatory mechanism worth considering in the pathogenesis of urinary bladder cancer.

The post-stroke hemiplegic patient. 1. a method for evaluation of physical performance.

A system for evaluation of motor function, balance, some sensation qualities and joint function in hemiplegic patients is described in detail. The system applies a cumulative numerical score. A series of hemiplegic patients has been followed from within one week post-stroke and throughout one year. When initially nearly flaccid hemiparalysis prevails, the motor recovery, if any occur, follows a definable course. The findings in this study substantiate the validity of ontogenetic principles as applicable to the assessment of motor behaviour in hemiplegic patients, and foocus the importance of early therapeutic measures against contractures.

Female university students in Sweden: sex, contraception and STDs.

Female university students (n = 241) visiting the Student Health Center participated in a study concerning sexual behavior (study I). A randomized postal survey (study II) with similar questions was also performed to verify the findings in the first study (n = 137). The mean age was 23 years in both studies. Sexual experience was reported by 96% in study I and 87% in study II. Mean age at first intercourse and mean number of partners were similar; 17.6 years and 6 partners, respectively. Change of contraceptive type was frequent, and about one third had done so within the past two years. A common reason for adopting a new method was a change in social life. More than half the women were afraid of using oral contraceptives. Twenty-six percent of the students in study I and 29% in study II had had at least one sexually transmitted disease (STD). Forty-three percent claimed to have changed their sexual behavior as a result of AIDS publicity but condoms were used by less than 50% at the first intercourse with a new partner. Although this group of intelligent young people have access to information, many of them were risk-takers with respect to both unwanted pregnancies and STD. Misconceptions were frequent concerning the safety of OCs. It is clear that new approaches must be made to educate them about safe sexual practices.

Radiology in the Nordic Countries.

NEMT, Nordic Evaluation of Medical Technology, conducted a study in 1988-89 on the use and diffusion of diagnostic radiology technologies in Denmark, Finland, Iceland, Norway and Sweden, i.e. the Nordic Countries. The study analysed the responses to a questionnaire sent to all Nordic radiology departments. Our findings show a variation from about 500 to nearly 900 radiology examinations per 1000 inhabitants among the Nordic Countries. Some of the differences are explained by unique structural factors of the health care system in each country, even if they all provide comparable public health services. Other differences are explained by variations in medical practice, accessibility to new imaging modalities, and replacement policies. This paper summarizes the results of the study.

Results and experience with four years of development work on a filmless department in Mjölby health care centre.

Since the end of 1994, the radiology department in Mjölby has been running their activity without X-ray films. When the film developing machine at the department was discarded in April 1995, after six months of test-running of the new system, the film epoch was definitely abandoned. The practical experiences from working with digital images are all good. The background to this change lays in a four years development work, which has been supported financially by the Ostergötland County Council, Swedish National Board for Industrial and Technical Development (NUTEK), and a regional foundation for development of new ways to organise and perform work (Arbetslivsfonden).
