[High Molecular Weight Urokinase Reference Standard (Control 901) of National Institute of Health Sciences].

A candidate for "High Molecular Weight Urokinase Reference Standard (HMW-UK RS, Control 901)" of the National Institute of Health Sciences (NIHS) was examined for evaluating the fibrinolytic activity. The urokinase activity of the candidate HMW-UK RS was determined by "two-stage method" as 800 Unit/Amp. against the present Urokinase Reference Standard (Control 881), through the collaborative study involving five laboratories. According to the gel-permeation chromatography of the candidate for the molecular determination, it was confirmed that more than 97% of the total urokinase activity was attributed to the higher molecular fraction, named HMW-UK. Thus it was authorized as the High Molecular Weight Urokinase Reference Standard of the NIHS.

[Carrier and prenatal diagnosis of Duchenne and Becker muscular dystrophy by PCR methods].

The PCR methods for carrier and prenatal diagnosis of Duchenne and Becker muscular dystrophy are described. When a deletion of the dystrophin gene is detected in the proband, the deletion analysis is informative for prenatal diagnosis, and the quantitative PCR analysis of the concerned exons is necessary for carrier diagnosis. When a deletion is not detected, RFLPs analysis should be considered. We analyzed seven polymorphic makers (pERT87-15 combined with digestions with BamHI or XmnI, pERT87-8 combined with digestion with TaqI, and four CA repeat markers at the 5' end and the 3' untranslated regions of the dystrophin gene) by PCR methods. Every woman examined showed a heterozygous finding for at least one of these markers, making it possible to make carrier and prenatal diagnosis. However, it is important to keep in mind that a third of the patients are due to new mutations and that about 10% recombination rate, between the 5' and 3' ends of the dystrophin gene, can exist.

[RAEB transformed into AML (M0) showing Ph1 chromosome and rearrangement of major cluster region].

A 78 year old female was found to have pancytopenia in February 1991. Bone marrow was normocellular with 11.7% blasts and showed dysmegakaryopoietic changes. A diagnosis of MDS (RAEB) was made and she was treated with transfusions and ubenimex. Leukemic transformation was noted in July. On Admission in October 1991, her laboratory examinations revealed the following: WBC 38,900/microliters with 93% blast, Hb 8.0 g/dl, Plt 2.1 x 10(4)/microliters, a hypercellular bone marrow with 74% blasts which were negative for myeloperoxidase (MPO) by light microscopy, but were positive by electron microscopy. Surface marker for CD13 was positive. These findings corresponded to M0 of the FAB subtype. Chromosome analysis revealed Ph1 chromosome with 46XX, t (9;22) (q34;q11) in 3 of 3 cells examined, Southern analysis showed the rearrangement of the break point cluster region (bcr). Reverse transcriptase polymerase chain reaction technique demonstrated the presence of major bcr/abl mRNA. She was treated with transfusions and methyl-prednisolone. Her blast counts declined and Ph1 chromosome was only positive in 1 of 12 metaphases examined. She died of pneumonia in December 1991. Eleven cases with MDS showing Ph1 chromosome have previously been reported. The observations indicate that Ph1 chromosome positive acute leukemias were heterogenous in nature.

Liver abscess after percutaneous ethanol injection (PEI) therapy for hepatocellular carcinoma. A case report.

We present a case of liver abscess after percutaneous ethanol injection (PEI) therapy for the treatment of recurrent hepatocellular carcinoma (HCC). The 56-year-old woman had a past history of cholecystoduodenostomy for cogenital dilatation of the bile duct, and pneumobilia was observed in the intrahepatic bile ducts prior to PEI. The abscess was successfully treated by percutaneous abscess drainage and antibiotic therapy. Klebsiella pneumonia, one of the most common causative organisms of biliary tract infection, was isolated from the abscess. Thus, biliary tract infection related to the previous biliary-enteric anastomosis operation may have been one of the causative factors in the liver abscess in this patient. The rare experience reported here suggests that a careful search for coexistent abscess at the time of PEI is important in HCC patients with biliary-enteric anastomosis, especially in those with pneumobilia.

Plasma androgens and sex hormone binding globulin in women with polycystic ovaries diagnosed by transvaginal ultrasound.

In order to investigate the androgen and sex hormone binding globulin (SHBG) levels in the polycystic ovary, we compared the total testosterone, free testosterone and SHBG levels in three patterns of polycystic ovary diagnosed with transvaginal ultrasonography (18 in group 1, fewer than 5 cysts identified; 16 in group 2, 5-10 cysts; and 27 in group 3, 10 or more). Free testosterone, but not total testosterone, significantly correlated with body mass index. Androgen levels were found to increase and SHBG levels to decrease with increases in the amount of ovarian cysts, and the evaluation of free testosterone was important in diagnosing the polycystic ovary syndrome.

Microvascular architecture of the filiform papillae in primates and insectivores.

The microvascular architecture of filiform papillae was investigated under a scanning electron microscope in man, Japanese monkeys, common squirrel monkeys, common marmosets, common tree shrews, large Japanese moles and dwarf shrews utilizing microvascular corrosion casts. Filiform papillae were circularly arranged in primates, and each of them was supplied by a hairpin capillary loop. These papillae sometimes were aggregated. The filiform papillae of Japanese monkeys exhibited markedly locational differences on the lingual dorsum and were supplied by circularly arranged capillary loops or by an intrapapillary capillary network. Small filiform papillae were located on an epithelial eminence in the lingual radix, each of them supplied by a low and simple hairpin capillary loop. The aggregated filiform papillae of common squirrel monkeys were less frequent without any locational differences. Low filiform papillae of common marmosets and tree shrews were simpler in form, being arranged in a circle and supplied by a simple hairpin capillary loop. The filiform papillae of insectivores were not arranged in a circle. The filiform papillae of dwarf shrews were supplied by an incomplete capillary ring without a loop. With respect to species differences, the circularly arranged capillary loops in man were most complicated and highly developed. Microvascular architecture of the filiform papillae of insectivores was much simpler, different from those observed in primates.

[A girl presenting clinical course and neuroimagings on MRI compatible with Pelizaeus-Merzbacher disease].

A 5-year-old girl who showed congenital nystagmus and mental and motor developmental delay, is described. Auditory brainstem responses (ABR) revealed wave I at normal latency, but all of the following waves were absent. In T2-weighted images, magnetic resonance imaging (MRI) demonstrated diffuse high intensity area of cerebral white matter, suggesting extensive dysmyelination or demyelination. She has not shown any deterioration through her clinical course. Subsequent MRI examinations did not demonstrate a progressive disorder. These findings suggest the possibility of Pelizaeus-Merzbacher (P-M) disease in this patient, which is a rare form of sudanophilic leukodystrophy, transmitted by an X-linked recessive mutant gene. It is reported that the proteolipid protein, one of the major proteins of myelin, was absent in classical type P-M disease, resulting in dysmyelination. Because chromosomal study showed the normal female karyotype and no family history of a similar disease was found in this case, it might be different from classical P-M disease. Since P-M disease may be heterogeneous, more detailed chromosomal analysis in each case of congenital hypomyelination will give a clue to clarify the pathogenesis of P-M disease and other disorders showing failure in myelination.

[A case of malignant fibrous histiocytoma in the thymic carcinoid].

The clinical case is 68 year old woman. The patient experienced dyspnea and back pain. Chest X-ray and CT showed a large well-defined mass from the left thorax expanding to the right. The tumor, which was derived from the left lobe of thymus, was removed. Cut surface of the resected tumor showed multilobar patterns and a yellow-white portion like a wedge in a part of the tumor. Histological examinations revealed that the multilobar portion was thymic carcinoid and a yellow-white portion was malignant fibrous histiocytoma. As far as we examined, literature pertaining to such double cancers has never been reported.

[Effect of thromboxane receptor antagonist, BAY u 3405 on the late phase pulmonary response in guinea pigs].

The effect of the specific thromboxane (Tx)A2 receptor antagonist, BAY u 3405 on the late asthmatic response (LAR) was investigated in guinea pigs which are sensitized actively with Ascaris suum antigen. Respiratory resistance (Rrs) of awake guinea pigs was measured by 30 Hz oscillation method for 6 hours after antigen challenge. %Rrs was calculated as the increase rate of Rrs to baseline Rrs (pre-challenge value). Ten mg/kg of BAY u 3405 or vehicle (0.5% methyl cellulose) was administered orally after the end of immediate asthmatic response, i.e., 2 hours after antigen challenge. %Rrs of the control group at 4 and 5 hours after antigen challenge were 101.4 +/- 27.5% and 77.5 +/- 19.9%, respectively. Those of the BAY group were 32.5 +/- 6.2% and 23.5 +/- 5.0%, respectively. %Rrs of the BAY group were significantly lower than those of the control group at both timings (p < 0.05). No significant difference was shown between the BAY group and the control group in the results of the bronchoalveolar lavage (BAL) 4 hours after challenge. Concerning the results of BAL at 6 hours after challenge, the total cell count and its segment, eosinophils count and lymphocytes count in the BAY group were 274.9 +/- 70.5, 62.5 +/- 13.1, 11.3 +/- 3.3 (x 10(5) cells), respectively, resulting in significantly smaller numbers (each p < 0.05) than those in the control group which were 491.9 +/- 55.1, 198.6 +/- 43.9, 32.1 +/- 7.3 (x 10(5) cells), respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Two different types of cell loss patterns of murine tumors and their corresponding histological findings and possible mechanisms of production of hypoxic cells.

Cell loss patterns of five tumors and their histological findings were investigated in WHT/Ht strain mice. The tumors investigated were squamous cell carcinoma H, squamous cell carcinoma NOS, squamous cell carcinoma TAK, rhabdomyosarcoma KAS, and fibrosarcoma YAS. The present study revealed that histological findings of tumor tissues are not necessarily corded structure presented by Thomlinson and Gray. The five were divided into two groups according to their histological findings, i.e. a corded structure (squamous carcinoma H and NOS) and a non-corded structure (squamous cell carcinoma TAK, rhabdomyosarcoma KAS and fibrosarcoma YAS). The cell loss patterns (125I-iododeoxyuridine retention curves) were also divided into two groups which corresponded to the histological structures. Two tumors with the corded structure (squamous cell carcinoma H and NOS) have a cell loss pattern with a constant shoulder portion (a migration time of tumor cells through a tumor cord from capillary to necrotic region). In these tumors, the cell loss occurred in the necrotic regions. In the other tumors with the non-corded structure, cell loss curves have no constant shoulder portion and cell loss might occur throughout the tumor. The origin of hypoxic cells in these two types of histology are presumably different. The diffusion-limited hypoxia, i.e. chronically hypoxic cells, may be the main cause for the tumors with corded structure. On the other hand, hypoxia as a result of temporary cessation of blood flow within the tumor vasculature, i.e. acutely hypoxic cells, may mainly occur in the tumor with non-corded structure.

Changes of DNA methylation in protooncogenes in the process of radiation-induced transformation of mouse m5S/1M cells in vitro.

To assess the importance of changes in DNA methylation in an X-ray-induced cellular transformation process, methylation patterns of five nuclear protooncogenes in fifteen transformant clones were studied and compared to that of the parental non-transformed cell line m5S/1M. All transformants examined revealed an alteration in DNA methylation in some of the genes, although these changes were variable among them. A comparison of cellular characteristics with corresponding DNA methylation changes in different clones suggested that the loss of contact inhibition and the gain of anchorage independency were associated with increases of methylation in many genes, whereas the acquisition of tumorigenicity was often accompanied by a decrease of methylation in the N-myc and c-myc genes. Resultant data indicate that the alteration of DNA methylation is closely related to transformation process, yet how this involvement occurs is complex and remains unclear.

Elevation of hepatic levels of metallothionein and zinc in mice bearing experimental tumors.

A tumor growth-dependent elevation in the hepatic levels of Zn and metallothionein (MT), without a change in the level of Cu, was found in mice and rats bearing solid tumors in the inguinal region. The levels of Zn and MT thus elevated gave a significant correlation (r = 0.95) between them. Nevertheless, when tumor-bearing mice and rats were fed a Zn-deficient diet, the hepatic levels of Zn and MT did not increase. In mice in which inflammation was induced at the same region, on the other hand, hepatic levels of Zn and MT increased transiently after the injection of turpentine or carrageenan even when they were fed the Zn-deficient diet. These results suggest that the elevation of MT and Zn levels can be a helpful marker for detecting malignancy.

Structure of serum transferrin in carbohydrate-deficient glycoprotein syndrome.

The structure of the defective transferrin in carbohydrate-deficient glycoprotein syndrome was characterized. Structurally abnormal sugar chains were not found in reversed phase chromatograms of pyridylaminated derivatives from the transferrin of two patients in different families. Electrospray ionization mass spectrometry of the whole transferrin molecules revealed an abnormal species that was smaller than normal tetrasialotransferrin by 2,200 daltons, just the size of the disialylated biantennary sugar chain. These data indicated that the disialotransferrin specifically found in this syndrome is missing either of two N-linked sugar chains, suggesting a metabolic error in the early steps of protein glycosylation.
