Bone marrow-derived osteoclast-like cells from a patient with craniometaphyseal dysplasia lack expression of osteoclast-reactive vacuolar proton pump.

Craniometaphyseal dysplasia (CMD) is a rare craniotubular bone dysplasia transmitted in autosomal dominant or recessive form. This disease is characterized by cranial bone hyperostosis and deformity of the metaphyses of the long bones. Using osteoclast-like cells formed from patient bone marrow cells, we investigated the pathophysiology of CMD in a 3-yr-old patient. Untreated bone marrow cells from the patient differentiated into osteoclast-like cells in vitro. These cells were shown to have vitronectin beta-receptors using a specific monoclonal antibody, i.e., 23C6 (CD51), which reacts with osteoclasts in human bone biopsy samples. However, the number of these osteoclast-like cells formed from the patient's bone marrow was only 40% of the normal controls. 1,25-dihydroxyvitamin-D3, bovine 1-34 parathyroid hormone, recombinant human interleukin-1 beta, recombinant human interleukin-6, or recombinant human macrophage colony-stimulating factor significantly increased, while salmon calcitonin significantly inhibited, the number of osteoclast-like cells. However, these cells could not resorb sperm whale dentin slices and lacked the osteoclast-reactive vacuolar proton pump as evidenced by a monoclonal antibody (E11). Western blot analysis using a monoclonal antibody to pp60c-src (327) revealed that protooncogene c-src expression by the platelets of the CMD patient was comparable to the normal control. These data suggest that: (a) the hyperostosis and the metaphyseal long bone deformity in the present CMD patient might be explained by osteoclast dysfunction due to impaired expression of the osteoclast-reactive vacuolar proton pump; and (b) a protooncogene c-src was not associated with the pathogenesis of the present CMD patient.

Midline theta rhythm revisited.

The midline theta rhythm consists of runs of theta waves occurring in the midline, especially in the central and parietal vertex regions. Few reports regarding this uncommon EEG finding have been published. It has been given various names, e.g. theta discharges in the middle-line, theta spindles, etc. Out of the 4,236 patients who were administered EEG examinations during an investigation period of 9 months, the midline theta rhythm, with a frequency in the range of 4-7 Hz, voltage over 50 microV, duration over 3 seconds, and most prominent in the central and parietal vertex areas, was recorded in 35 (0.83%). The age distribution of these patients peaked in two groups, adults aged 21-40 years and elderly patients aged 61-77 years. The waveform of the theta rhythm observed in adults 40 years old and under was often sinusoidal and regular. In this group, the most common clinical disorder was epilepsy. Often, the appearance of the theta rhythm observed in elderly patients was triangular and irregular, and sometimes sharp. In this group, the common clinical symptoms were headaches and dizziness, characterized by diagnoses of organic brain diseases such as cerebrovascular disorders. This study re-examines the clinical significance of the midline theta rhythm, taking into consideration studies that have been published in the past.

Altered sensitivities to potential inhibitors of cholesterol biosynthesis in Niemann-Pick type C fibroblasts.

Cultured fibroblasts from patients with Niemann-Pick disease type C (NP-C) are characterized by the lysosomal accumulation of unesterified cholesterol and the inability of low-density lipoprotein (LDL) to stimulate cholesterol esterification, in addition to impaired LDL-mediated down-regulation of LDL receptor activity and cellular cholesterol synthesis. Although a defect in the transport of cholesterol from lysosomes to other intracellular membrane sites has been suggested, it is unclear how cells regulate the levels of cellular sterols and whether their membrane cholesterol requirements are satisfied or not. We studied the esterification of exogenously added cholesterol, total levels of cellular cholesterol and cholesteryl ester, cholesterol synthesis from a two-carbon precursor, and sensitivities to potential inhibitors of cholesterol biosynthesis in proliferating NP-C cells. We observed the following: (a) esterification of [3H]cholesterol was decreased but the total amount of cellular cholesteryl ester was not decreased; (b) synthesis of cholesterol from [3H]acetate was increased; and (c) cells were hypersensitive to cholecalciferol, an inhibitor of 3-hydroxy-3-methylglutaryl-CoA reductase, and were resistant to filipin, which binds to membrane sterols and presumably damages the membrane. The results indicate that NP-C cells depend on the cellular cholesterol synthetic pathway for their proliferation, but the plasma membrane sterols are presumably decreased. The altered sensitivities to potential inhibitors of cholesterol biosynthesis should be a useful marker for diagnosis and genetic studies.

Proliferation, migration and platelet release by megakaryocytes in long-term bone marrow culture in collagen gel.

The proliferation of megakaryocytes, their migration, and their platelet release processes were observed in long-term bone marrow culture in collagen gel. Megakaryocytes proliferated for more than 6 months, not only with myeloid cells but also with sinusoid-like capillaries. The megakaryocyte count decreased at 2 weeks of culture, increased to more than 400 at 6 weeks, and then decreased to about 100. Megakaryocyte colonies appeared after 2 weeks of culture; the number increased to more than 10 at 4 weeks of culture, and was maintained at that level. Morphologically, most fully mature megakaryocytes extended several long antennae-like processes, with periodic constrictions, demarcation membranes (DM), and platelet fields. Pro-platelets were released from these processes primarily by a "pinching off" mechanism, but some megakaryocytes released pro-platelets by the dissociation of DM, without extending long processes. Mature megakaryocytes migrated like ameba in collagen gel, and a few migrated to the abluminal side of the capillary or into the capillary; in both cases releasing pro-platelets into the capillary lumina. These observations were very similar to those noted in vivo.

Determination of the molecular-weight distribution of low-molecular-weight heparins using high-performance gel permeation chromatography.

The molecular-weight distribution of commercially available low-molecular-weight heparins (LMWHs) was estimated by high-performance gel permeation chromatography (HPGPC) using a LMWH molecular-weight calibrant, supplied by the National Institute for Biological Standards and Control for the calibration of columns. This calibrant is a mixture of many polysaccharides with a disaccharide unit prepared by a random depolymerization of unfractionated heparin, and the ends of the fragments have a UV absorption. The ratio of the refractive index to the UV absorption is a relative measure of molecular weight. Various molecular-weight parameters such as weight-, number- or z-average molecular-weights, polydispersity and relative distributions of high, middle and low molecular-weight components were estimated, indicating that LMWHs have a wide variation in molecular-weight distribution. This variation was ascribed to the difference in preparation methods from unfractionated heparins.

Effects of cholesterol on the miscibility of synthetic glucosamine diesters in lipid bilayers and the entrapment of superoxide dismutase into the positively charged liposomes.

Methyl-D-glucosamine-3,6-dilauroyl, dimyristoyl, dipalmitoyl or distearoyl esters were synthesized as positively charged lipids. They were incorporated into phosphatidylcholine liposomal membranes and the entrapment of superoxide dismutase (SOD) into the liposomes was attempted. The efficiency of the SOD-entrapment into the positively charged multilamellar vesicles (MLVs), comprising egg yolk phosphatidylcholine and synthetic glucosamine diesters, was enhanced by the addition of cholesterol to the membranes. A differential scanning calorimetric study showed that the miscibility (solubility) of glucosamine diesters in phosphatidylcholine-bilayers increased on the addition of cholesterol to the membranes. Cholesterol assisted in the mixing of phosphatidylcholines with positively charged glucosamine diesters and increased the positive charges on the liposomal membranes. This was confirmed by incremental increases in the zeta-potential of liposomal membranes with an increase in the cholesterol content. Entrapment of SOD thus became more efficient due to the enhanced electrostatic attraction between the positively charged membranes and the negatively charged SOD, and/or the electrostatic repulsive interactions between positively charged membranes; the latter interactions induced a thickening of the water layer in MLVs.

[Partition behavior of tropane alkaloids between organic solvents and water].

The partition behavior of tropane alkaloids (TrA) between organic solvent and water was studied under various conditions. The apparent partition coefficient, log P', increases with pH with a slope of +1 in the acidic to neutral pH range, then tends to approach to a constant value in the alkaline region. Thus the partition of TrA to the organic phase is based on the neutral form of TrA. From the pH dependency of the partition and pH titration of TrA, the intrinsic partition coefficients log P0 and pKa values for each TrA were estimated. Values of pKa and log P0 for scopolamine were smaller than those for atropine or homatropine. It was considered that the presence of the epoxide ring in scopolamine molecule reduces the log P0 and pKa because of its polar and electro-inductive effect. It was also expected from the results of pH-dependent partition behavior that in the low pH region some portion of TrA cations is transferred to the organic phase by forming an ion-pair complex with an anion, in the aqueous phase. The log P' of TrA was measured in the presence of various kinds of anions (halides and alkyl sulfonates, C5-C8) at about pH 2, where almost all TrA are present as a protonated form. The relations between the log P' and the logarithmic concentration of anions is linear at low concentrations of anions as expected from the ion-pair partition equation.(ABSTRACT TRUNCATED AT 250 WORDS)

Isolation of Aspergillus flavus MO-5 producing two types of intracellular alpha-D-xylosidases: purification and characterization of alpha-D-xylosidase I.

One strain (MO-5) of fungi producing alpha-D-xylosidase was isolated from sake koji by a method using glucose oxidase. MO-5 was identified as an Aspergillus flavus strain, produced no aflatoxin (B1, B2, G1, and G2). Two different types of alpha-D-xylosidases were detected in the cell-free extract. One of the enzymes (alpha-D-xylosidase I) was purified to an electrophoretically pure state by successive chromatography on Q-Sepharose, Phenyl Superose, PL-SAX, and TSK-gel G3000SWXL. The purified enzyme hydrolyzed p-nitrophenyl alpha-D-xylopyranoside (alpha-p-NPX), methyl alpha-D-xylopyranoside, isoprimeverose [alpha-D-xylopyranosyl-(1-->6)-D- glucopyranose], and xyloglucan oligosaccharide. The activity of this enzyme was highly specific for alpha-D-xylosidic linkages. The apparent Km and Vmax of the enzyme for alpha-p-NPX and isoprimeverose were 1.32 mM and 4.4 mumol/min/mg protein, and 4.00 mM and 58.8 mumol/min/mg protein, respectively. This enzyme had an apparent molecular weight of 100,000 by SDS-polyacrylamide gel electrophoresis and 400,000 by gel filtration chromatography (TSK-gel G3000SWXL). The enzyme showed the highest activity at pH 4.5 and 45 degrees C, and was stable from pH 4.5 to 6.0 and at temperatures up to 45 degrees C. The activity was inhibited by SDS and Hg2+, and slightly by Cu2+ and Fe3+. This enzyme showed transfer action at high concentrations of isoprimeverose and transfer products were detected, and it had transxylosylation activity on maltose from isoprimeverose as a donor, too.(ABSTRACT TRUNCATED AT 250 WORDS)

Purification and characterization of an intracellular alpha-D-xylosidase II from Aspergillus flavus MO-5.

Alpha-D-Xylosidase II activity from Aspergillus flavus MO-5 was increased roughly 5- to 10-fold by use of xylose instead of methyl alpha-D-xylopyranoside (alpha-MX) as a carbon source. The enzyme was purified to an electrophoretically pure state by successive chromatography on Q-Sepharose, Phenyl Superose, PL-SAX, and TSK-gel G3000SWXL. The purified enzyme hydrolyzed isoprimeverose [alpha-D-xylopyranosyl-(1-->6)-D-glucopyranose] and p-nitrophenyl alpha-D-xylopyranoside (alpha-p-NPX), but not alpha-MX or xyloglucan oligosaccharide. The apparent Km and Vmax of the enzyme for alpha-p-NPX and isoprimeverose were 0.97 mM and 28.0 mumol/min/mg protein, and 47.62 mM and 2.0 mumol/min/mg protein, respectively. This enzyme had an apparent molecular weight of 67,000 by SDS-polyacrylamide gel electrophoresis and 180,000 by gel filtration chromatography (TSK-gel G3000SWXL). The enzyme showed the highest activity at pH 6.0 and 40 degrees C, and was stable in the pH range from 6.0 to 7.0 and at the temperatures up to 40 degrees C. The activity was inhibited by Cu2+, Zn2+, Hg2+, p-CMB, SDS, Fe3+, and N-ethylmaleimide. This enzyme had nothing in common with alpha-D-xylosidase I and four alpha-D-xylosidases reported already.

[Study on the relationship between appearance of autoantibodies and chest X-ray findings of asbestos plant employees].

Antinuclear factor (ANA) and rheumatoid factor (RF) were determined in 220 employees of an asbestos plant. The relationship of the appearance of the two factors to sex, age, smoking habit, duration of asbestos exposure and asbestos exposure level was studied. In 207 employees who had chest roentgenograms, the relationship of these two factors to pulmonary lesions and pleural lesions was examined. Of the 220 employees, 33 (15%) had ANA and 7 employees (3.2%) had RF. ANA positive rate in the asbestos plant employees was significantly higher than in the control. Factors significantly correlated to their appearance could not be demonstrated. With the progress of pulmonary lesions the ANA positive rate significantly elevated, but ANA positive rate was not significantly correlated with pleural lesions. RF did not show any significant correlation with any of the factors.

A convenient and inexpensive chemo-radiosensitivity assay for lung cancer cells using Terasaki's microplate.

We devised a simple in vitro sensitivity test for lung cancer cells using Terasaki's microplate. We used the test to screen for sensitivity to various carcinostatic drugs and radiation, and to determine the optimum method of administration. This assay has been used in routine clinical examinations because about 40% of non-small cell carcinoma and 80% of small cell carcinoma of the lung can be subcultured. We describe here our patients who underwent treatment, various sensitivity tests and the preparation of an optimal course of treatment based upon the results of the sensitivity tests. Cells were placed in primary culture as previously described for short-term selective culture, and 2nd-3rd generation subcultured cells were transferred to individual wells of Terasaki's microplates for various sensitivity tests. After culture for 10 days, the effect was evaluated using 0.1% iodonitrotetrazolium (INT). This test permits a variety of sensitivity tests and various studies of clinical models of intensive treatment to be performed conveniently and reproducibly, because subcultured cancer cells are used. Another advantage is that these cells can be applied to basic investigations, including the preparation of monoclonal antibodies and chromosomes, DNA ploidy and oncogene studies.

What factors are involved in the knowledge necessary for the self-management of diabetic patients?

The aim of this study is to obtain data for improving a training program for patients with diabetes mellitus. One hundred eighty-seven patients with non-insulin dependent diabetes mellitus were tested with 20 questions about their knowledge for self-management of diabetes mellitus. Then to draw out factors in their personal backgrounds relating to their correct answers, multiple regression analyses were conducted. As a result, four factors showed significant differences in the following order: Educational careers > ages > duration of disease > socioeconomic strata. The results of the present study have shown for the first time, that these four factors closely concern patients to acquire the necessary knowledge for their self-management of the disease. In addition, this study has raised some fundamental problems regarding the training program for patients: how education should be given to patients.

Methanol stimulates the colony-formation rate in a human hepatoma cell line (HLE).

Effects of methanol on colony-formation of human hepatoma cells were investigated. Among five human hepatoma cell lines (Hep G2, HLE, HuH-6, HuH-7, and PLC/PRF/5), only HLE cells showed enhanced colony formation due to methanol. The effective concentrations of methanol were around 1%. The enhancement occurred in a greater degree when the cells were seeded in the culture medium containing methanol than when methanol was added 24h after the cells were seeded. Methanol itself, however, did not enhance the cell proliferation.

Rat stem cell factor and IL-6 preferentially support the proliferation of c-kit-positive murine hemopoietic cells rather than their differentiation.

We have investigated the effect of stem cell factor (SCF) alone and in combination with interleukin-3 (IL-3) or interleukin-6 (IL-6) on the proliferation and maintenance of primitive hemopoietic progenitor cells. Results from liquid preculture of either unfractionated bone marrow cells or lineage (Lin)-c-kit+ cells indicates that the combination of SCF + IL-3 results in the greatest expansion of total nucleated cell numbers; however, the combination of SCF + IL-6 results in the greatest expansion of colony-forming cells in culture (CFU-C) and in spleen (CFU-S). Morphologic examination confirmed the increase in immature cells after culture with SCF + IL-6 and, therefore, this combination is deemed superior for the expansion of primitive cells in liquid culture. Reconstitution assays using congenic mice (Ly5) revealed that cultured cells in the presence of SCF + IL-6 contained stem cells that were capable of reconstituting the hemopoiesis in lethally irradiated mice. Although it remains unclear whether SCF + IL-6 directly supports the self-renewal of stem cells, SCF + IL-6 is a powerful tool for manipulating primitive hemopoietic cells in vitro.

Follow-up examination schedule of postoperative HCC patients based on tumor volume doubling time.

The tumor volume doubling time of 27 recurrent hepatocellular carcinoma nodules measuring < or = 2.0 cm in diameter discovered after hepatectomy in 19 patients was analyzed by ultrasonography for 31 to 331 days. The tumor volume doubling time ranged from 39 to 420 days before specific treatment, with a mean of 112 days and a median of 80 days. It correlated with age and intrahepatic site of recurrence, which was sometimes the same lobe as that of the prior hepatocellular carcinoma, sometimes not. The tumor volume doubling time also tended to correlate with hepatitis C antibody status and initial tumor diameter. The tumor volume doubling time was, however, independent of sex, hepatitis B surface antigen status, number of recurrent tumors, initial serum alpha-fetoprotein level, interval between hepatectomy and recurrence detection, and associated liver disease. On the basis of the shortest tumor volume doubling time of the recurrent hepatocellular carcinoma nodules, we recommend that US and/or CT be performed at least every four months to screen for small recurrent carcinomas in postoperative patients.
