Electron-microscopic and immunohistochemical study of beta-2-microglobulin-related amyloidosis.

beta 2-Microglobulin (beta 2-MG)-related amyloidosis has been reported as a complication in long-term hemodialysis patients. We observed beta 2-MG amyloid deposits in synovial sheaths, bone cysts and gastric mucosa. They showed unique ultrastructural features, that is bundles or nodules consisting of curved or linear amyloid fibrils, associated with various cell reactions. The electron-microscopic histochemical study showed that they strongly stained with periodic acid-silver methenamine stain. A similar phenomenon was noticed in the spicules or bundles of amyloid fibrils in primary and secondary renal amyloidosis. With the cationic reagent toluidine blue 0, proteoglycan-like structures were observed around amyloid bundles and nodules, but not on each fibrils. Based on these results, we postulate that there is a close relationship between ultrastructural features and histochemical characteristics in beta 2-MG amyloid fibrils.

[Allergen skin tests to insects, chironomid, caddis fly and silkworm moth, in patients with nasal allergy].

The present study was undertaken to assess the importance of insects as allergens causing nasal allergy. We performed intradermal skin tests with extracts from three insects, chironomid, caddis fly, and silkworm moth, on patients with nasal allergy and on volunteers among medical students. Twenty-two (27.5%) of 80 nasal allergic patients showed positive reactions to chironomid allergen and 9 (11.3%) had positive reactions to caddis fly allergen and 18 (22.5%) to silkworm moth allergen. These positive rates were as frequent as those to Candida or ragweed. Medical students were divided into two groups, symptomatic and asymptomatic, according to the presence or the absence of nasal allergy. Three (11.5%) of 26 symptomatic group showed positive reactions to chironomid and one (3.8%) to caddis fly and 5 (19.2%) to silkworm moth. While even 4 (8.3%) of 48 asymptomatic group showed positive reactions to chironomid, 2 (4.8%) to caddis fly, and 6 (12.5%) to silkworm moth. These results suggested that these insects, chironomid, caddis fly, and silkworm moth, are relatively common than we expected and suspected as important allergens in nasal allergy.

Nasal allergy in medical students.

The incidence of nasal allergy in medical students was studied in our University from 1983 to 1987. Intradermal skin tests were performed using six allergens: house dust (HD), ragweed, Japanese cedar, orchard grass, candida and broncasma berna. 154 out of 471 students (32.7%) had symptoms indicative of nasal allergy. HD and Japanese cedar were the main allergens and their positive rates were 66.4% and 51.0%, respectively in symptomatic cases. However, 34.4% of asymptomatic students also showed positive reactions to HD and 19.6% to Japanese cedar. In nasal provocation tests, 53.8% of symptomatic students who reacted positively to HD skin test showed positive reactions, while even 34.8% of asymptomatic students also showed positive reactions. The same results were obtained for Japanese cedar. It seemed that some asymptomatic students who had positive reactions to skin tests have latent allergies. A long-term follow-up would be necessary for these cases.

[Mucormycosis in paranasal sinuses].

Paranasal sinus mucormycosis is a rare and often fatal condition that occasionally occurs in patients with debilitating disease. Five mucormycosis cases with paranasal sinus involvement are reported. Two also complicated diabetes mellitus. These diabetic patients rapidly developed meningitis after the onset of symptoms. One case was soon operated upon and necrotic masses with fungus were excised. Despite i.v. amphotericin B administration, two patients lost their consciousness in a few days, relapsed into coma and died. In these cases, the infection originated from the ethmoidal sinus and directly spread towards the orbit and brain. The remaining three cases with mucormycosis had operations after the initial diagnosis of sinusitis. All of the three patients are alive and well. The combination of an excisional operation and antifungal therapy resulted in favorable response. High resolution CT scanning is valuable both in planning treatment and monitoring the response to therapy.

[The desensitization therapy in children with nasal allergy to house dust].

A study about the desensitization therapy carried out on children with nasal allergy to house dust who were under 15 years old. The good effect was seemed in 35.7% of patients with the desensitization for 3 months, and 62.7% for 6 months, 90.0% for 12 months, 92.7% for 24 months. The patients with the desensitization for over 6 months had a considerable effect, as if they dropped out. In the questionnaire to patients who dropped out with no contact to us, the effect of this therapy was seemed on 83.3% in those who answered questions, as so we considered that the sensitization for children with nasal allergy was very important as the basic therapy. Refer to the efficacy of this therapy, it was seemed to be recommended to children who were about from 7 to 9 years old, or those who had allergic rhinitis for a long time. At the point of 6 months after starting HD desensitization, the patients who reacted intensively to the provocation test had not significantly the efficacy compared with those who reacted slightly, but other clinical data were not significantly concerned with the efficacy of this therapy.

Long-term observation of postoperative course of habitual tonsillitis.

Tonsillectomy and adenoidectomy are among the most frequently held operations in the ENT field. In our hospital, the number of cases of tonsillectomy has been decreasing recently as compared with 10 to 15 years ago. Especially the number of cases under 10 years old has shown a decrease. It is sometimes difficult to decide the indication for tonsillectomy even for habitual tonsillitis. In this paper, the post operative course of tonsillectomy with habitual tonsillitis was investigated during 1976 to 1986 using a questionnaire. Answers evaluable were obtained from 207 cases. The efficacy of the operation on sore throats, high temperatures and recurrent colds was very high, but a few cases complained that a foreign sensation of the throat had occurred or worsened after operation. The incidence of nasal allergy gradually increased after tonsillectomy, but further studies may be necessary to decide the exact relationship. Laboratory examinations, such as ASLO, CRP, white blood cell counts, immune complex, C3a and C5a, were not particularly helpful in deciding the indication of tonsillectomy for habitual tonsillitis. In general, it is thought that tonsillectomy is very effective for habitual tonsillitis, but we must be more careful in deciding such an indication.

Direct expression of urogastrone gene in Escherichia coli.

Human epidermal growth factor (urogastrone; UG) is a 53-amino acid polypeptide hormone. A 192-bp DNA fragment containing the coding sequence for methionyl UG (Met-UG) and the ribosome-binding site (RBS) was chemically synthesized and placed downstream from the promotor for the Escherichia coli outer-membrane lipoprotein gene (lpp) on a plasmid. E. coli cells harboring the plasmid directed the synthesis of Met-UG at 10(2)-10(3) molecules per cell. Next, the coding sequence for Met-UG was inserted in a runaway-replication plasmid and expressed under the control of the lpp promoter and the RBS derived from bacteriophage Mu cII gene. Upon heat induction, the cells harboring the recombinant plasmid synthesized 10(5) molecules of Met-UG per cell.

[PMU therapy of recurrent gastric cancer. A case report].

A 56-year-old woman with recurrent gastric cancer treated with PMU therapy, combined CDDP 75 mg/m2 i.v. MMC 10 mg/body i.v. and UFT 400mg/body/2 alpha/day p.o., was reported. She was admitted because of cervical lymph node swelling and abdominal tumor (para-aorta lymph node swelling). She was treated two times with this therapy and induced into complete remission. The serum CEA level, more than 500 ng/ml before the treatment, was reduced to 6.7 ng/ml after treatment. She has currently been free of disease for more than four weeks. We conclude that this PMU therapy is extremely effective for indurable gastric cancer.

Aspirin-induced asthma and nasal polyps.

Asthma, aspirin intolerance and nasal polyps form a triad of aspirin-induced asthma (AIA). Eighteen cases, 6 males and 12 females, who complained of asthma attacks and/or rhinorrhea after ingestion of non-steroidal anti-inflammatory drugs were encountered over several years. The mean age of onset was 32.1 for asthma and 25.4 for rhinitis. Asthma was found in all of the 18 cases and nasal polyps in 13 cases (72.2%). The polyps were recurrent and 7 patients had undergone polypectomies. Urticaria was seen in 44.4% and sinusitis diagnosed by X-ray in 81.8%. Sensitivity to at least one allergen was found in 7 out of 9 cases (77.8%) and 6 out of 11 cases (54.5%) gave results positive for RAST. Eosinophilia was seen in 14 out of 16 patients (87.5%). The pathogenesis of AIA is obscure but is probably related to inhibition of prostaglandin biosynthesis. We concluded that AIA is not a rare disease in Japan either, and the studies for eosinophilia may be useful for elucidation of the pathogenesis of AIA and nasal polyps.
