[Problems of surgical treatment of malignant brain tumours in the elderly].

A group of 69 patients aged 60-80 years with brain tumours were analysed, estimating the surgical risk at the time of qualification for operations. Surgical treatment was given to 36 patients limiting it to lobectomy of the part of the brain containing the greater part of the tumour. The peri-operational mortality was 8.3%. The decision on surgical treatment of old age patients with brain tumours depends on many factors, both medical and sociopsychological.

[Aneurysmal bone cyst of the skull and vertebral column treated operatively].

Aneurysmal bone cysts considered as a benign lesion are extremely rare in the skull or in the vertebral column. In the last 29-years were admitted to Department of Neurosurgery in Poznań 11 patients with aneurysmal bone cysts involving the skull in 4 and the spine in 7 cases. All but one patients were female, aged between 4 and 37 years. Location of skull cysts were: parietal in 1, occipital in 1, and parietotemporal in two cases. In patients with spinal affection the position of vertebral cysts were: cervical -2, dorsal -4 and lumbar -1 case. All patients suffered severe pain radiating to the adjacent area and showed local deformity of bone structure. Paraparesis or paraplegia were observed in cases with spinal involvement. In all patients, total surgical removal of the aneurysmal bone cysts was possible. Skull defect was covered with plastic materials. In the repair of the vertebral defect, pelvic bone grafts were used and fixed with wire suture. In follow-up ranging from 1 to 25 years there was no tendency for recurrence.

[Lumbar intervertebral disk herniation in children].

A consecutive series of 16 children from 9 to 18 years old, with lumbar intervertebral disc herniation who were treated surgically in Department of Neurosurgery Medical Academy in Poznań, between 1978-1992, was analysed. The clinical symptoms, X-ray assessment, type of operation and outcome are reviewed. In our analysis characteristic features of disc herniation in children are: the onset of pain due to violent injury in relation to lifting, bilateral neurological symptoms and radiculography imaging central intervertebral disc herniation.

[Brain tumor and imminent premature labor: a case report].

A pregnant woman with cerebral tumour of right hemisphere is reported. In the 30th week of pregnancy intracranial pressure was increased and imminent premature delivery were diagnosed. The patient was operated on for cerebral tumour and immediately after this cesarean section was performed. We conclude that in this case neurosurgical operation followed by cesarean section saved the life of the mother and child.

[Surgical approach to cranial base and craniofacial tumors].

Skull base tumour is rare and until recently was considered unresectable. The authors present a patient with skull base tumour of extra and intracranial compartment penetrating into parapharyngeal space and maxillary bone. Radiologic findings on CT, MRI, arteriography were studied. A surgical technique to expose the cranial base and intraoperative findings are described. Significant improvement occurred after incomplete tumour resection.

[Perimesencephalic non-aneurysmal subarachnoid hemorrhage. Case report].

A case of perimesencephalic non-aneurysmal subarachnoid haemorrhage is reported. The patient was a 54-year-old women, after spontaneous subarachnoid haemorrhage with negative cerebral angiography. Internal hydrocephalus due to haemorrhage developed and ventriculo-atrial shunt was inserted. Outcome is presented and the possible causes of bleeding are discussed.

[Post-traumatic cerebrospinal rhinorrhea managed surgically. Clinical analysis of cases].

47 patients was treated surgically for posttraumatic cerebrospinal fluid rhinorrhea between 1977 and 1996 in the Department of Neurosurgery in Poznań. Cerebrospinal fluid fistula developed after road accident, less often because of assault, fall etc. Frontobasal fracture usually caused rhinorrhea and was verified by plain X-rays, computed tomography and magnetic resonance. Indications to early surgery were rhinorrhea coexisting with intracranial haematoma or pneumocephalus, optic nerve lesion, the persistent or recurrent rhinorrhea and meningitis. The authors present surgical methods and postoperative complications. Good outcome was in 86%.

[Enzymatic activity in cerebrospinal fluid in the monitoring of the brain lesions following intracranial tumors].

After intracranial tumour surgery edema, ischaemic and haemorrhagic changes are developing in the brain. It causes leakage of certain intracellular components and enzymes into extracellular and cerebrospinal fluid. This study includes 46 patients operated on for cerebral tumours. Lumbar CSF was obtained in postoperative course and the activity of AST, ALT, LDH, alpha HBDH, CK, gamma GT and oxygen concentration were estimated. In the control group were 10 patients suffering from sciatic pain and CSF was obtained during radiculography. For diagnostic and prognostic purposes of biochemical markers statistical analyse methods were employed and comparisons with clinical factors were carried out. In conclusions we suggest that enzymes AST, alpha HBDH, CK and oxygen content give valuable additional information of postoperative brain damage.

[Conservative treatment of cerebral arteriovenous malformations. Follow-up study of 31 cases].

The clinical course of 31 patients with conservatively treated intracranial arteriovenous malformations (AVM's) was followed-up. The average follow-up period was 8.2 years, 75% of patients were followed up for more than 5 years. The initial symptom was intracranial bleeding in 21 patients (68%), seizure in 5 (16%), neurological deficit in 2 (6.5%), headache in 2 (6.5%) and intracranial hypertension in 1 (3%). The prognosis was more favourable for patients presenting with seizures than for patients presenting with haemorrhage. Patients in the haemorrhage group had 29% incidence of haemorrhage causing 14% mortality in this group. Of the 31 patient's 20 (64.5%) had a good clinical outcome, 4 (13%) had a fair outcome and 2 (6.5%) poor, 5 patients died, with one not due to AVM. There was no correlation between pregnancy and bleeding.

[Large intracranial tumors invading skull base].

Large intracranial tumours (diameter > 4.5 cm) invading skull base are not frequent. In Department of Neurosurgery in Poznań 22 patients were operated on between 1977-1966. There were 15 males and 7 females. Mean age was 34.78 +/- 15.31 years. The authors analyse the time between the onset of illness and diagnosis, symptoms and neurological condition. Tumour localization was verified by X-rays, computed tomography, cerebral angiography and in some cases magnetic resonance. There were 11 tumours in both fossae--anterior and middle, in 7 tumour site was limited to anterior and in 4--to middle fossa. Extracranial tumour penetration was observed as follows: in 10 cases into paranasal sinuses, 10--orbit, 8--nasopharyngeal cavity and in 4 cases into subtemporal fossa. The authors discuss surgical methods, histological appearances and postoperative complications. Mortality rate was 9% (2 patients died).

[Metastatic vertebral tumors diagnosed by percutaneous needle biopsy].

The purpose of this paper is to establish diagnostic value of percutaneous needle biopsy in patients with vertebral tumours treated in Department of Neurosurgery in Poznań between 1975 and 1996. Radiological examinations: X-rays, plain X-rays, CT and MR, demonstrated vertebral destruction and allowed needle biopsy performance. The most frequent destruction was observed in thoracic region (45% of cases), rarely--in sacral bone (5%). In cases with single neoplastic focus, only in vertebral body, histological diagnosis by needle biopsy was achieved in 70% of cases. However in patients with paravertebral tumour positive results of needle biopsy were higher--82% of cases.

[Transpedicular stabilization in spinal and vertebral column injuries in thoracic and lumbar spine segments].

The authors present their own experience in application of transpedicular internal stabilization of the thoracic and lumbar spine. Clinical analysis was carried out in a group of 12 patients after vertebral column and spinal trauma managed surgically in the Department of Neurosurgery in Poznań between 1.06.95 and 31.12.96. Age of patients ranged from 19 to 56 years (mean age 35.08 +/- 13.04 yrs.). The level of vertebral fracture was as follows: thoracic (2 cases), thoraco-lumbar (6 cases) and lumbar (4 cases). Three patients were completely paraplegic. All patients underwent posterior or posterolateral surgical approach. The fractured parts of bones, translocated into vertebral canal were removed and nervous structures were decompressed. Transpendicular stabilization was performed after the decompression. Improvement of neurological condition was observed in 8 patients.
