Simplified method for total correction of interrupted aortic arch with ventricular septal defect in infancy.

Type A interrupted aortic arch with a ventricular septal defect (VSD) in a 3-month-old infant was successfully corrected with the aid of profound hypothermia, limited cardiopulmonary bypass, and total circulatory arrest. The aortic arch was reconstructed by side-to-side anastomosis of the ascending aorta and the main pulmonary artery and then creation of a tube from the anastomotic orifice to the patent ductus arteriosus (PDA) by using a superabundant flap of the anterior wall of the main pulmonary artery. The pulmonary arteriotomy and VSD then were closed. The operative field can be approached easily through a median sternotomy with minimum dissection. Cardiac catheterization and angiocardiography 2 months later demonstrated a satisfactory reconstruction of both the aortic arch and the pulmonary artery.

[Chemotherapy of peritonitis with particular reference to concentrations of sulbenicillin in human ascites (author's transl)].

Twenty-seven patients with peritonitis to whom a drain was applied were given sulbenicillin (SBPC), a broad-spectrum antibiotic, which has so little hepatic and renal toxicity that massive doses may be feasible, and examination was made as to its therapeutic effects and concentrations of the antibiotic in the ascites. Daily dosage of SBPC was 10g in two divided doses in most cases given by the intravenous infusion. Medication was continued for 3 approximately 15 days. The highest daily dosage was 20g and the largest total dosage reached 190g, but there was no adverse reaction except for one case of a slight anemia. Peritonitis complicated appendicitis, adnexitis, duodenal ulcer perforation, intestinal obstruction or trauma as its primary disease. No difference in the therapeutic effect existed among the primary diseases. The response to SBPC treatment was excellent in 8 of the 27 patients and good in 17. Two patients failed to respond to the therapy. When SBPC was given just before operation, the SBPC concentration in ascites obtained at operation was 112 microgram/ml in 2 cases. The SBPC concentrations in ascites were examined following intravenous infusion of 5g over an hour, and a peak concentration of 94.7 microgram/ml was obtained at the completion of infusion (an hour after the start of infusion), which gradually decreased thereafter. In the ascites excreted from the drain after operation, a high concentration of 12.7 approximately 90.2 microgram/ml (mean: 51.7 +/- 7.7 microgram/ml) was obtained on the day after the operation day, but the concentration was lower thereafter. The SBPC concentrations in ascites were compared as regards the sites of drainage (Winslow's foramen, ileocecum and Douglas' fold), but no particular difference was observed. The SBPC concentrations in ascites after operation were in inverse proportion to the alleviation of peritonitis. They were higher when the inflammation was severer.

Serum levels of proline imino-peptidase in normal adults and children.

Serum proline imino-peptidase activities in 98 normal individuals aged 1 month to 75 years are examined. Mean enzyme activity in sera of individuals ages less than 1 year is 66.6 U; 2 to 6 years; 46.2 U; 7 to 11 years, 52.8 U; 12 to 16 years, 48.8 U; 17 to 19 years, 34.1 U; 20 to 29 years, 28.8 U; 30 to 39 years, 30.5 U; 40 to 59 years, 35.6 U; and more than 60 years, 31.6 U. Serum enzyme activities were markedly elevated in infants aged less than 1 year, then decreased with age up to 20 years. Serum proline imino-peptidase activity in 44 normal adults aged 20 to 59 years ranged from 19.9 to 43.1 U and averaged 31.5 +/- 5.8 U. There is no statistical difference in serum activities in normal adults between males and females.

Chemical studies on tuberactinomycin. XV. Total synthesis of tuberactinomycin O.

Tuberactinomycin O, one of the four congeners of the antituberculous peptide tuberactinomycin, was totally synthesized. The beta-ureidodehydroalanine moiety was constructed from beta,beta-diethoxyalanine with excess urea in acidic medium after a cyclization reaction of a pentapeptide was finished. Cyclization was carried out by means of the 1-succinimidyl ester method. To the cyclic pentapeptide, beta-lysine was introduced as the branched moiety and then deprotected to afford tuberactinomycin O which was completely identified with the natural form of the antibiotic.

The revised structure of capreomycin.

The total structures of capreomycins IA and IB were determined mainly from the results of the comparative study on the NMR spectra of capreomycins and tuberactinomycins, resulting in a revision of the formerly proposed structure. The beta-lysine residue as a branched part was revealed to be linked with the beta-amino group of the alpha, beta-diaminopropionic acid residue in the cyclic pentapeptide moiety in a different manner than in tuberactinomycins.

Furildioxime method for nickel analysis in serum and urine by electrothermal atomic absorption spectrometry.

We describe a method of analysis for nickel in serum and urine. After the sample (1 ml) has been subjected to acid digestion, Ni(II) is extracted into methylisobutylketone as a furildioximate complex, and is measured by electrothermal atomic absorption spectrometry. The method is more sensitive and more convenient than previous methods, and is less subject to nickel contamination because the digestion and extraction steps are performed in a single tube. Analytical recovery of Ni(II) (10 or 20 microng/liter) added to 20 specimens of serum or urine averaged 101% (range, 94 to 107). The following values (mean +/- SD) were obtained for nickel concentrations in specimens of serum and urine from 19 healthy adults living in central Connecticut: serum = 3.1 +/- 1.6 microng/liter; urine = 2.7 +/- 1.6 microng/liter. The method provides a practical means for monitoring occupational exposures of workers to nickel compounds.

The chemical structure of capreomycin.

The chemical structure of capreomycin, antituberculous peptide antibiotic, was revised from the results of NMR-analysis in comparison with tuberactinomycins. Capreomycin IA and IB were concluded to possess the similar amino acid sequences in their cyclic peptide moieties to those of tuberactinomycins.
