[Reconstruction methods of anterior chest wall resection--comparison between acrylic resins and myocutaneous flaps].

A total of 32 patients were treated by chest wall resection for primary and recurrent breast cancer or other malignant tumors of the chest wall. Reconstruction of the anterior chest wall was conducted using either acrylic resin plate or myocutaneous flap. Although acrylic resin plate proved to be satisfactory for maintaining stability of the chest wall and protecting important organs in the anterior mediastinum, some patients developed foreign body reactions including local infections, local retention of serous discharge and slight anterior chest pain during a prolonged observation. Reconstruction with myocutaneous flap achieved acceptable stability and caused no such foreign body reactions. We confirmed that myocutaneous flap was the best way to reconstruct the anterior chest wall after full thickness chest wall resection.

[Clinical analysis of high grade bladder cancer].

The prognosis and other clinical manifestation of 128 patients with high grade bladder tumor were analyzed. Thirty two percent of the total cases of bladder cancer were high grade bladder cancer and 83% of their tumors were invasive tumor at stage T2 and worse. Urinary cytologies were positive in 88% of these patients. The 5-year survival rate in these patients was 32% and those in T1, T2 T3 and T4 cases were 64.2%, 55.6%, 22.7% and 8.0% respectively. The patients treated with radical (total) cystectomy showed a much better survival rate than the cases treated with TUR or partial cystectomy. These results suggest that high grade bladder cancers tend to be invasive and the patients with high grade bladder cancer would have a poorer prognosis than the patients with other histological grade tumors. Thus, these patients should be treated more aggressively including radical cystectomy than the other cases of bladder cancers.

Prognosis of minute carcinoma of thyroid. Follow-up study of 49 patients.

Forty-nine patients with minute carcinoma of thyroid gland (clinically silent carcinoma less than 10 mm in diameter) underwent partial thyroidectomy and regional lymph node dissection. Nodal metastasis was found in 33 patients (67%). Histologically, no difference in the incidence of nodal metastasis was found between sclerosing carcinoma and nonsclerosing carcinoma. During the period of follow-up (from 3 to 18 years), neither recurrence of the carcinoma nor death was observed in 16 patients without nodal metastasis and 24 patients who had less than 4 metastatically involved nodes. In these 24 patients, most of the metastatic deposits were less than 5 mm in diameter and even the largest one did not exceed 16 mm. Recurrence without death was found in 2 of 4 patients who had many metastatically involved nodes, more than 8, or at least one large metastatic deposit measuring larger than 16 mm in diameter (moderately involved group), whereas recurrence followed by death occurred in 3 of 5 who had many involved nodes and at least one large deposit (extensively involved group). In both moderately and extensively involved groups, recurrence and death occurred in elder patients. The present study suggested that the prognosis of minute carcinoma depends on the degree of nodal involvement and age of the patient.

Values of CA 19-9 in the serum, pure pancreatic juice, and aspirated pancreatic material in the diagnosis of malignant pancreatic tumor.

The diagnostic accuracy of the measurement of CA 19-9 in the serum, pure pancreatic juice, and aspirated pancreatic fluid in the diagnosis of pancreatic tumors was assessed in 32 patients with malignant pancreatic tumors and 19 patients with pancreatitis. Pure pancreatic juice was collected from the pancreatic duct by endoscopic cannulation with a duodenofiberscope after intravenous administration of secretin. Pancreatic material was obtained by percutaneous fine-needle aspiration biopsy under ultrasonic guidance. Abnormally high CA 19-9 levels in the serum were significantly more frequent in patients with malignant pancreatic tumors than in those with pancreatitis: they were elevated in 71.9% of the patients with pancreatic tumors. High CA 19-9 levels were found primarily in patients with a tumor of the head of the pancreas, in those with a tumor greater than 3 cm in its greatest diameter, and in those with an unresectable cancer. Only 57.1% of seven patients with a tumor of less than 3 cm in its greatest diameter showed an increase in CA 19-9 level. The CA 19-9 levels in pure pancreatic juice were significantly higher in patients with pancreatic tumors than in patients with pancreatitis without pancreatic stone. However, it was not useful for differentiating pancreatic tumors from pancreatitis with pancreatolithiasis. The CA 19-9 level in pancreatic materials obtained by aspiration biopsy was significantly higher in patients with malignant pancreatic tumors than in those with pancreatitis. Eight patients (80%) with pancreatic tumors had values above 1000 U/ml, whereas all five patients with pancreatitis had values of less than 30 U/ml. Although CA 19-9 levels in pancreatic materials was useful only when cytologic examination did not provide any evidence of malignancy, the combination of the CA 19-9 assay and the cytologic study of specimens obtained by percutaneous fine-needle aspiration biopsy of the pancreas increased the diagnostic rate to 100%.

Cell-surface changes during in vitro differentiation of pluripotent embryonal carcinoma cells.

When aggregates of HM-1 embryonal carcinoma (EC) cells were exposed to 10(-6) M retinoic acid for 2 days and cultured in medium lacking retinoic acid, they differentiated to nerve cells, endoderm cells, and myoblasts. Cells 2 days after initial exposure to retinoic acid were not significantly different from the parental EC cells, as judged by cell-surface architecture and by reactivity to lectins. On the fourth day, the surface of the aggregates was covered with two kinds of cells distinguishable from the parental cells. The round cells with short villi seemed to be precursors to endoderm cells. Receptors for Dolichos biflorus agglutinin (DBA) newly appeared and receptors for peanut agglutinin (PNA) were still expressed on their surfaces. The other cells, which were round cells with a few processes, might be precursors to nerve cells. PNA receptors had disappeared from their surfaces, and DBA receptors were not expressed. On the sixth day of differentiation, possible precursors to myoblasts were detected; they were flat cells with smooth surfaces. These cells lacked cell-surface receptors for the two lectins, while the precursor cells and the myoblasts excreted intercellular fibers reacting with PNA. HM-1 cells synthesized much embryoglycan, the structure of which was similar to that of the glycan isolated from quasinullipotent F9 cells. The only difference was that the glycan from HM-1 cells lacked DBA binding sites. Synthesis of fucosylated embryoglycan mainly decreased between the second and fourth day of differentiation. As above, cell-surface changes occurred mainly between the second and fourth day. The period seems to be important in determining the fate of the cells, since endoderm cells were scarcely seen among differentiated cells which had been continuously exposed to 10(-6) M retinoic acid during the period.

Altered synthesis of glucosamine-labeled glycopeptides during postimplantation embryogenesis of the mouse.

Dissected mouse embryos were examined for the ability to synthesize [3H]glucosamine-labeled embryoglycan, which is the glycoprotein-bound large carbohydrate eluted near the excluded volume of a Sephadex G-50 column. Although the ratio of glucosamine-label incorporated into embryoglycan decreased progressively during embryogenesis, 11- and 12-day-old embryos still synthesized significant amounts of the glycan. Severe inhibition of the glycan synthesis was noticed after the 15th day. Therefore, synthesis of the core portion of embryoglycan was suppressed later than when fucosylation of the glycan was known to be suppressed.

Gastrin has no promoting effect on chemically induced colonic tumors in Wistar rats.

The effects of prolonged administration of tetragastrin from the beginning of intrarectal instillation of 1 ml of 0.25% N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) and after MNNG-treatment on the incidence and histology of colonic tumors were compared in inbred Wistar rats. In week 35 prolonged administration of testragastrin in depot form from the beginning of MNNG-treatment resulted in a significant reduction in the incidence of colonic tumors and a significant increase in the incidence of mucinous adenocarcinoma, unlike the well-differentiated adenocarcinoma produced in controls without gastrin. In contrast, prolonged administration of tetragastrin after MNNG-treatment had little or no influence on the incidence, size or histology of colonic tumors. Thus tetragastrin had no promoting effect on colonic tumors.

Chromoendoscopic observations on extension and development of fundal gastritis and intestinal metaplasia.

The development and extension of fundal gastritis and intestinal metaplasia were investigated by the endoscopic Congo red-methylene blue test in 44 healthy subjects, 106 patients with gastric ulcer, and 26 patients with gastric polyp. Follow-up chromoendoscopic examinations were made an average of 3.4 yr (range 1-10 yr) after the initial endoscopic assessment. Fundal gastritis and intestinal metaplasia developed or became more extensive with time in only 3 (6.8%) and 2 (4.5%) healthy subjects, respectively. In patients with gastric ulcer and in those with gastric polyp, fundal gastritis and intestinal metaplasia developed or extended significantly more frequently and more rapidly than in healthy subjects. Fundal gastritis and intestinal metaplasia developed or became more extensive, respectively, in 26 (24.5%) and 23 (21.7%) patients with gastric ulcer and in 10 (38.5%) and 10 (38.5%) patients with gastric polyp. Fundal gastritis and intestinal metaplasia extended with time mainly in an upward direction.

Histocytologic diagnosis of pancreatic cancer by percutaneous aspiration biopsy under ultrasonic guidance.

Percutaneous aspiration biopsy of the pancreas using a heparinized 22-gauge fine needle was performed under ultrasonic guidance in five patients with benign pancreatic diseases and in 18 patients with pancreatic cancer. Using a heparinized needle and syringe, it was possible to make good smears containing abundant tumor cells and to obtain small tissue specimens. Using egg albumin as binding material, a new cell-block technic was developed to conveniently obtain histologic specimens. In this way, a correct diagnosis was made cytologically in all 23 patients suspected of having a pancreatic malignancy. Histologic specimens were obtained in 22 (95.6%) our of 23 patients. A correct diagnosis was established histologically in all patients from whom histologic materials were obtained. This procedure thus has proved a very reliable method for diagnosing pancreatic cancer.

Effect of a chemically defined diet in liquid form on colon carcinogenesis in rats.

The effects of ad libitum feeding of a chemically defined diet in liquid form on the incidence and histology of colon cancer induced by 10 weekly sc injections of 7.4 mg/kg of azoxymethane [(AOM) CAS: 25843-45-2] were investigated in W-rats. The chemically defined diet was adjusted once every 24 hours from 4 weeks before injection of the carcinogen to the end of the experiment at week 40. Oral administration of the defined diet resulted in significant increase in the incidence of colon cancer at week 40. Histologic examination showed that unlike adenocarcinomas with high mucin-producing activity, which were common in rats on pellet diet, most of the adenocarcinomas that developed in rats fed on defined diet were highly or well differentiated, with a typical glandular pattern. Administration of the chemically defined diet also resulted in marked colon mucosal hypoplasia and reduced gastrin levels in the serum at weeks 4 and 40.

Characterization of a rat tRNA gene cluster: comparison of nucleotide sequences of the gene for tRNALeu newly found in repeating units.

In the rat, DNA carrying a cluster of the genes for tRNAAsp, tRNAGly, and tRNAGlu, aligned in that order, is repeated about 10 times. Seven DNA clones corresponding to the independent repeating units were isolated from a rat gene library. Nucleotide sequence analysis of these clones revealed the presence of a fourth tRNA gene, the gene for tRNALeu, in the cluster. The tRNALeu gene is located about 600 base pairs (bp) upstream from the tRNAAsp gene and its polarity differs from those of the other three tRNA genes. Among the repeating units, the nucleotide sequence of tRNALeu is conserved to a relatively high degree.

Minute carcinoma of the thyroid and its development to advanced carcinoma.

The relationship between the size of carcinomas and their histological characteristics such as encapsulation and sclerosis was studied using 157 cases of thyroid minute carcinomas with diameters less than 10 mm. When the diameter of the tumor was less than 1 mm, encapsulated carcinomas were not found, but they were presented in a group with a larger diameter. Sclerosing carcinomas were more frequently distributed in the group with a diameter less than 5 mm. With increasing tumor size, the carcinomas were reduced markedly in number and only two cases of sclerosing carcinoma were observed with a diameter more than 8 mm. It was concluded that thyroid minute carcinomas arise as nonencapsulated carcinoma, either sclerosing or non-sclerosing, followed by encapsulation at a later time and that some of the non-sclerosing carcinomas become apparent clinically, whereas most of the sclerosing carcinomas remain clinically silent throughout the entire life of the host.

An immuno-electron microscopic study on intracellular localization of thyroglobulin (TG) in the thyroid gland in Grave's disease.

Intracellular localization of thyroglobulin (TG) using a pre-embedding diffusion technique and an indirect localization sequence has been made in human thyroid obtained from 20 patients with treated Grave's disease. Both antibodies, anti-human TG-rabbit IgG F(ab')2 and anti-rabbit IgG F(ab')2-goat IgG F(ab')2 fragments easily penetrated the cytoplasm of follicular cells which were dissociated by RPMI-1640 solution containing collagenase, dispase, and deoxyribonuclease. With light microscopic observation of semithin sections positive immuno-reaction for TG was demonstrated as fine granular deposits in the cytoplasm of the dissociated cells. In electron microscopic studies, intracellular antigen was well circumscribed within certain cell organelles in all cases with the positive immuno-reaction for TG being observed in perinuclear space, rough endoplasmic reticulum, Golgi complexes, secretory granules, and reabsorbed colloid droplets. Content of positive immuno-reaction product differed somewhat from one case to another and from one follicle to another even in the same case. There was no immunoreaction product in nuclei, mitochondria, lysosomes, and lipofuscin-like granules.

Thyroid carcinoma in benign thyroid diseases. An analysis from minute carcinoma.

The relative frequency of thyroid carcinoma in benign thyroid diseases such as toxic diffuse goiter (toxic goiter), adenomatous goiter (goiter), adenoma, and chronic thyroiditis (thyroiditis) was studied using 3,219 surgically removed thyroid glands. Coexistence of carcinoma and benign diseases was found in 257 glands. Among them, 157 glands had minute carcinoma (diameter of cancer nodule is smaller than 10 mm) and 100 glands had advanced carcinoma (larger than 10.1 mm in diameter). The incidence of carcinoma including minute carcinoma was 29.4% in goiter (98/333), 21.0% in thyroiditis (11/53), 8.6% in adenoma (55/638), and 5.3% in toxic goiter (98/1852). Chi-square test also revealed that the rate of carcinoma not only advanced carcinoma but also minute carcinoma was higher in goiter than in the other diseases (P less than 0.01, respectively). Among the patients aged under 39, the incidence of advanced carcinoma and minute carcinoma in thyroiditis were the highest, respectively (83% and 100% in thyroiditis, 9.0% and 11.4% in goiter, 2. 6% and 1.9% in adenoma, and 0.7% and 3.4% in toxic goiter), however, among the patients aged over 40, they were secondary lower and the lowest, respectively (19% and 16.8% in goiter, 5.2% and 7.5% in adenoma, 4.2% and 6.7% in thyroiditis, and 1.4% and 7.5% in toxic goiter). We concluded that adenomatous goiter accompanies carcinoma more frequently than other benign thyroid diseases and the high incidence of carcinoma in chronic thyroiditis is probably due to a preoperative selection of the patients.
